A unified consensus statement on medication-related osteonecrosis of the jaw (MRONJ) has not yet been established among the Asian member countries or regions of the Asian Federation of Osteoporosis Societies (AFOS). This study aimed to develop a consensus on MRONJ in patients with osteoporosis across these countries and regions. In this study, the term “Asia-Pacific” refers specifically to the Asian member countries and regions of AFOS. A structured survey consisting of nine MRONJ-related questions was distributed across 10 countries and regions to assess the level of agreement and summarize regional perspectives. In addition, a manual literature review and voting were conducted to evaluate the current evidence on MRONJ. The key aspects of MRONJ, including definition, staging, diagnosis, pathogenesis, risk factors, management, and prevention, were generally consistent among the AFOS countries and regions. The annual incidence and incidence rate of MRONJ associated with low-dose antiresorptive therapy in patients with osteoporosis ranged from 0.025% to 0.136% and 21 to 283 cases per 100,000 person-years, respectively. However, evidence regarding the benefits of drug discontinuation before dental surgery, such as tooth extraction, remains insufficient. Large-scale, multinational studies across AFOS countries and regions are warranted to determine the incidence of MRONJ better and evaluate the impact of antiresorptive drug discontinuation before dental procedures. These findings may contribute to the devel opment of effective evidence-based strategies for preventing MRONJ in patients with osteoporosis.
2.A pediatric case of reversible dilated cardiomyopathy caused by autoimmune hypothyroidism

Dilated cardiomyopathy is the most common form of cardiomyopathy in children and often leads to irreversible myocardial dysfunction. To improve the prognosis, it is critical to identify rare but reversible causes. We report a 12-year-old girl referred to the emergency department for dyspnea on exertion and generalized edema that lasted for 1 month. She gained 20 kg over the past 3 years, which had gone uninvestigated. Transthoracic echocardiography demonstrated left ventricular dysfunction with pericardial effusion. Thyroid function tests revealed concentrations of free T4 and thyroid-stimulating hormone of 0.12 ng/dL (reference value, 0.93-1.7) and 86.9 µIU/mL (0.3-4.2), respectively, with anti-thyroid peroxidase and anti-thyroglobulin antibodies elevated, confirming the presence of autoimmune hypothyroidism. We initiated heart failure management alongside levothyroxine replacement therapy. After 5 months of treatment, a follow-up echocardiography showed improved left ventricular and thyroid function.
3.Introduction to Neurodevelopmental Disorders

Neurodevelopmental disorders (NDDs) comprise a heterogeneous group of conditions characterized by impairments in cognition, language, motor function, behavior, and social communication that originate during the developmental period. Advances in neuroscience, genomics, and digital health have substantially transformed the current understanding of the mechanisms, diagnosis, and management of NDDs. This review summarizes the contemporary conceptual framework of NDDs, highlights advances in etiological diagnosis—particularly those related to genetic and genomic technologies—and discusses emerging therapeutic approaches, including digital therapeutics. Emphasis is placed on the importance of early identification, domain-based assessment, and precision medicine approaches to optimize long-term outcomes.
4.Clinical Features and Treatment Response in Chronic Recurrent Erythema Multiforme: Difference Based on the Etiology Related to Herpes Simplex Virus

Background:
Erythema multiforme (EM) is typically a self-limited, acute hypersensitivity reaction. However, a subset of patients experiences chronic, recurrent episodes, for which clinical features and treatment strategies differ depending on the underlying etiology, especially in herpes simplex virus (HSV)-associated cases.
Objective:
To investigate the clinical and phenotypic features of chronic recurrent EM and assess treatment responses, with a focus on differences based on HSV association.
Methods:
This retrospective study included pathology-confirmed cases of suspected EM from 2010 to 2023. Forty patients with chronic EM (≥3 recurrences or persistent disease for ≥12 months) were included. Clinical, histopathologic, and serologic data were analysed.Patients were stratified into herpes simplex virus-associated erythema multiforme (HAEM) and non-HAEM groups. Clustering analysis was performed to identify clinical phenotypes.Treatment responses to antivirals and immunomodulators were evaluated.
Results:
Of the 40 patients, 24 (60%) were classified as HAEM. HAEM patients showed more mucosal involvement, smaller targetoid lesions, and acral predominance, while nonHAEM patients had larger, coalescing lesions with more trunk involvement. Cluster analysis supported HSV as the major discriminating factor. Antiviral agents were effective in 87.5% of HAEM cases but ineffective in 76.9% of non-HAEM patients. Immunosuppressants such as cyclosporine and mycophenolate mofetil showed variable responses. Baricitinib induced complete remission in all 3 refractory cases.
Conclusion
HSV association defines a distinct clinical subtype of chronic recurrent EM, with differences in lesion morphology, distribution, and treatment response. Recognizing these patterns may guide targeted therapeutic strategies, including the potential use of Janus kinase inhibitors in refractory cases.
5.Diagnostic Implications of the Endocrine Society and American Academy of Pediatrics Guidelines on blood pressure: associations with sex, blood pressure components, and metabolic syndrome in Korean Adolescents

Purpose:
Pediatric hypertension and metabolic syndrome (MS) are increasing in parallel with childhood obesity. Variations in diagnostic thresholds between the Endocrine Society (ES) and American Academy of Pediatrics (AAP) guidelines may affect detection and intervention timing.
Methods:
We analyzed data from 1,035 Korean adolescents aged 13–17 years drawn from the Korea National Health and Nutrition Examination Survey (2014–2016). Blood pressure (BP) abnormalities and MS were assessed according to ES and AAP guidelines. Differences by sex, age, and body mass index (BMI) category were examined.
Results:
The ES guidelines identified significantly more cases of diastolic BP (DBP) abnormalities than the AAP guidelines. For example, ES identified DBP abnormalities in 15.2% in 16-year-old males versus 8.1% identified by AAP. This difference was especially prominent for prehypertensive categories. ES percentile-based thresholds were more sensitive to subtle diastolic elevations, while AAP uses fixed cutoffs that may underestimate early risk. MS prevalence exceeded 30% in multiple age groups among adolescents with BMI≥85th percentile. However, MS prevalence did not significantly differ between the 2 guidelines in any age or sex subgroup.
Conclusion
Awareness of hypertensive status is essential in the era of increasing childhood and adolescent obesity. The ES guidelines might be more suitable for cardiometabolic screening in Korean adolescents than the AAP guidelines because they effectively detect hypertensive status, including DBP abnormalities.
6.Defect Size-Based Comparative Analysis of Treatment Modalities for Esophagojejunal Anastomotic Leakage Following Gastrectomy

Purpose:
Esophagojejunal anastomotic leakage (EJAL) represents a severe postoperative complication following total or proximal gastrectomy. Treatment strategies include conservative management, endoscopic interventions, and surgery; however, comparative data remain limited. This study aimed to compare clinical outcomes of different strategies to identify the optimal approach based on anastomotic defect size.
Materials and Methods:
This retrospective study reviewed 100 patients diagnosed with EJAL between January 2015 and October 2024. Patients were categorized into four groups:conservative management, endoscopic vacuum-assisted closure (E-VAC), other endoscopic treatments, and surgery. The primary outcomes were leakage duration and length of hospital stay after EJAL diagnosis, whereas the secondary outcome was time to C-reactive protein normalization. Subgroup analyses were performed according to defect size.
Results:
Among the 100 patients, 76 were male and 24 were female, with a mean age of 65.7 years. Conservative treatment was the most common modality (53%), followed by other endoscopic treatments (19%), E-VAC (14%), and surgery (14%). In patients with a defect size <1 cm, conservative treatment was associated with significantly shorter leakage duration (P=0.035) and earlier resumption of diet (P=0.029) compared with endoscopic treatment.Among those with defects ≥2 cm, E-VAC demonstrated the most favorable median outcomes across all variables; however, statistical significance was not achieved because of the small sample size.
Conclusions
Conservative treatment appears to be the most effective treatment strategy for EJAL with anastomotic defects <1 cm. For larger defects (≥2 cm), E-VAC may offer clinical benefit, although further studies are needed to confirm its efficacy. These findings highlight the importance of individualized treatment selection based on defect size.
7.Association between Fibrovascular Translucency of Pterygium and Corneal Curvature and Aberrations after Adjustment for Horizontal Invasion Length

Purpose:
This study aims to determine whether the translucency-based clinical grade of pterygium shows independent relationships with anterior and posterior corneal curvature and anterior wavefront aberrations, with adjustment for horizontal invasion length (HIL) measured by anterior segment (AS) swept-source optical coherence tomography (SS-OCT).
Methods:
This study involved 53 eyes from 51 patients diagnosed with primary nasal pterygium. The clinical grade of the pterygium was evaluated using T grade based on the fibrovascular translucency. AS SS-OCT (Anterion, Heidelberg Engineering) provided HIL (in millimeters), thickness (in micrometers), anterior/posterior keratometry at the 3.0-mm ring, and anterior corneal wavefront metrics within a 6.0-mm zone. The device was also used to evaluate the magnitudes for the individual first-to fourth-order aberrations of the anterior corneal fields. Also, the fourth- to seventh-order root mean square (RMS), RMS lower-order aberration (LoA), and RMS higher-order aberration (HoA) values were collected. For statistical analysis, Spearman rank correlation test and Pearson correlation test were performed, along with partial correlation test adjusted for HIL.
Results:
The mean age of subjects was 59.3 ± 11.4 years, and 51.0% were female. T grade correlated positively with HIL. HIL showed strong associations with anterior simulated keratometry (Sim K) average, anterior Sim K steep, anterior Sim K flat, and anterior corneal astigmatism—but not with posterior corneal parameters. HIL also showed significant correlations with multiple anterior corneal wavefront parameters. After adjustment for HIL, T grade remained independently correlated with anterior Sim K flat, RMS LoA, and defocus; no significant relationships were observed with other Zernike terms or posterior metrics. Additionally, T grade was positively correlated with pterygium thickness.
Conclusions
T grade adds independent, complementary information (associations with flat keratometry, LoA RMS, and defocus) even after controlling for HIL. Incorporating T grade into preoperative assessment may improve estimation of optical burden and surgical prognosis.
8.Reproducibility of Optical Coherence Tomography Radiomics after Cataract Surgery: Effects of Media Opacity, Signal Strength, and Region of Interest Selection

Purpose:
To determine how cataract-related image-quality improvement and region of interest (ROI) definition affect the reproducibility of optical coherence tomography (OCT) radiomics features.
Methods:
This retrospective study included 50 eyes from 46 patients who underwent cataract surgery. Preoperative and postoperative OCT images were acquired using a standardized protocol. Radiomics features (n = 93) were extracted from two ROI configurations: retina ROI (Rr; retina only) and full ROI (Rf; retina, choroid, vitreous). Preoperative versus postoperative differences were tested with the Wilcoxon signed-rank test and Cohen’s d; repeatability was assessed with the intraclass correlation coefficient (ICC). Agreement between ROIs was evaluated with Wilcoxon statistics, Spearman correlation, the concordance correlation coefficient (CCC) and Bland-Altman plots. The influence of signal strength was analyzed with paired t-tests and Spearman correlation.
Results:
Postoperative signal strength significantly improved from 28.24 ± 3.16 to 31.40 ± 3.06 dB (p < 0.001). Significant preoperative/ postoperative changes occurred in 49 / 93 features (52.7%) within Rr and in 81 / 93 (87.1%) within Rf. ICC showed good-to-excellent repeatability in 51.6% of Rr features and 87.1% of Rf features, yet 29.0% of Rr features still demonstrated poor reliability. Across ROI methods, 94.6% of feature pairs differed significantly between Rr and Rf, and no feature achieved CCC ≥0.85. Only two descriptors—first-order statistics 10th percentile and GLDM (gray level dependence matrix) gray level variance—showed no significant change with surgery and minimal dependence on signal strength.
Conclusions
Radiomics features in OCT respond variably to media opacity, signal-to-noise ratio, and ROI definition: most are susceptible to these factors, yet a small subset remains stable across them. Limiting analysis to an anatomically precise retina- bounded ROI and enrolling eyes with comparable cataract status and signal-strength scores may therefore improve reproducibility. Implementing these standardizations will help future OCT-radiomics studies identify the most reliable biomarkers for clinical use.
9.β-Catenin and AMPK/AKT/FOXO Signaling Mediate Doxorubicin-Induced Senescence and Lipid Accumulation in C2C12 Myoblasts

Skeletal muscle atrophy is a major complication associated with aging, chronic disease, and chemotherapy. Doxorubicin (Dox), a widely used anticancer agent, accelerates muscle wasting; however, the underlying cellular mechanisms remain poorly understood. In this study, we examined the effects of Dox on myogenic differentiation, senescence, and lipid metabolism using C2C12 myoblasts. Dox exposure impaired myotube formation without causing overt cytotoxicity. Mechanistically, Dox disrupted myogenic differentiation by inhibiting protein kinase B/mammalian target of rapamycin (AKT/mTOR) signaling, thereby de-repressing forkhead box O1/3 (FOXO1/3) and upregulating the muscle-specific ubiquitin ligases muscle atrophy F-box (MAFbx) and muscle RING finger 1 (MuRF1), which promote proteolysis. Dox also decreased glycogen synthase kinase 3β (GSK3β) phosphorylation while paradoxically increasing total and phosphorylated β-catenin, indicating dysregulated Wnt/β-catenin signaling. These alterations were accompanied by a senescence-like phenotype, characterized by elevated senescence-associated β-galactosidase (SA-β-gal) activity, increased phosphorylated histone variant γH2AX, and activation of the p53–p21 axis. Notably, cellular senescence coincided with excessive lipid accumulation in myotubes. Dox reduced phosphorylation of adenosine monophosphate-activated protein kinase (AMPK) and acetyl-CoA carboxylase (ACC) while enhancing expression of key lipogenic regulators, thereby creating a metabolic environment favoring lipid storage. Collectively, these findings demonstrate that Dox not only suppresses myogenic differentiation but also induces premature senescence and metabolic reprogramming toward lipid accumulation. Targeting these pathways through AMPK activation, FOXO inhibition, or senolytic interventions may offer therapeutic strategies to preserve skeletal muscle integrity in patients undergoing chemotherapy.
10.Long-term Immunogenicity of the 13-valent Pneumococcal Conjugate Vaccine during Adjuvant Chemotherapy in Patients with Gastric and Colorectal Cancer: A 5-Year Follow-up of a Randomized Controlled Trial

Purpose:
Current guidelines recommend vaccination at least 2 weeks before chemotherapy initiation to optimize the immune response despite limited evidence. Our previous study indicated no differences in short-term immune response for the 13-valent pneumococcal conjugate vaccine (PCV13) according to the vaccination timing. This study aims to investigate the long-term efficacy of PCV13 and clinical factors associated with the respective antibody response.
Materials and Methods:
Patients with gastric or colorectal cancer who received adjuvant chemotherapy were enrolled and divided into two groups: vaccinated 2 weeks before chemotherapy (arm A) and vaccinated concurrently with chemotherapy (arm B). Serum samples were collected before vaccination and in one month, 3 years, and 5 years. Immune responses were measured using enzyme-linked immunosorbent assay and multiplex opsonophagocytosis assay.
Results:
Including 63 patients, both groups showed an initial increase in the geometric mean titers of opsonophagocytic activity and the geometric mean concentrations of serotype-specific IgG levels after one month, followed by a decline at 3 and 5 years, particularly for serotypes 1, 14, 18C, and 19A. Despite the decline, global protection was maintained for 5 years, although global response decreased. The two arms did not show significant differences in immunogenicity nor in factors such as vaccination timing, age, cancer type, or chemotherapy regimen.
Conclusion
Vaccination timing is not a significant factor for the immunogenicity of PCV13 in cancer patients undergoing adjuvant chemotherapy. Global protection against pneumococcal infection was sustained for > 5 years, and global response remained in over half of patients.
