1.Analysis of the Capacities of Health Facilities in the Eastern Visayas Region based on Health Care Provider Network Service Delivery Guidelines.

OBJECTIVES
Timely access to appropriate levels of care is essential for improving maternal, newborn,
and child health outcomes. To address persistent service delivery fragmentation and strengthen referral systems, the Philippine Department of Health issued Administrative Order 2020-0019 to guide the design of Health Care Provider Networks (HCPNs) under the Universal Health Care Act of 2019. This study assessed the extent to which sixteen municipalities across four provinces in Eastern Visayas comply with the HCPN service delivery guidelines in the context of maternal and newborn care.
The study employed a descriptive cross-sectional mixed-methods design, utilizing structured facility checklists to assess compliance with HCPN standards. Qualitative data were gathered through key informant interviews and focus group discussions with purposively selected stakeholders—decision makers, health personnel, and mothers—to contextualize findings. A three-lever framework for integrated care (policy, operational, and cross- cutting) guided the analysis
RESULTSThe findings revealed significant gaps between the current capacities of study health facilities and the requirements outlined in the HCPN guidelines. Major gaps included (1) weak cooperative governance mechanisms to support network-wide coordination; (2) limited systematic linkages between facilities, including fragmented referral protocols and non-interoperable health information systems; (3) inadequate investments in infrastructure, health human resources, and medical commodities; and (4) absence of performance monitoring systems across HCPNs.

2.Genomic variant surveillance of SARS-CoV-2 positive specimens using a direct PCR product sequencing surveillance (DPPSS) method.

BACKGROUND AND OBJECTIVE
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) as the causative agent of COVID-19 has significantly challenged the public health landscape in late 2019. After almost 3 years of the first ever SARS-CoV-2 case, the World Health Organization (WHO) declared the end of this global health emergency in May 2023. Although, despite the subsequent drop of COVID-19 cases, the SARS-CoV-2 infection still exhibited multiple waves of infection, primarily attributed to the appearance of new variants. Five of these variants have been classified as Variants of Concern (VOC): Alpha, Beta, Gamma, Delta, and the most recent, Omicron. Therefore, the development of methods for the timely and accurate detection of viral variants remains fundamental, ensuring an ongoing and effective response to the disease. This study aims to evaluate the feasibility of the application of an in-house approach in genomic surveillance for the detection of SARS-CoV-2 variants using in silico designed primers.
METHODSThe primers used for the study were particularly designed based on conserved regions of certain genes in the virus, targeting distinct mutations found in known variants of SARS-CoV-2. Viral RNA extracts from nasopharyngeal samples (n=14) were subjected to quantitative and qualitative tests (Nanodrop and AGE). Selected samples were then analyzed by RT-PCR and amplicons were submitted for sequencing. Sequence alignment analysis was carried out to identify the prevailing COVID-19 variant present in the sample population.
RESULTSThe study findings demonstrated that the in-house method was able to successfully amplify conserved sequences (spike, envelope, membrane, ORF1ab) and enabled identification of the circulating SARS-CoV-2 variant among the samples. Majority of the samples were identified as Omicron variant. Three out of four designed primers effectively bound into the conserved sequence of target genes present in the sample, revealing the specific SARSCoV-2 variant. The detected mutations characterized for Omicron found in the identified lineages included K417N, S477N, and P681H which were also identified as mutations of interest. Furthermore, identification of the B.1.448 lineage which was not classified in any known variant also provided the potential of the developed in-house method in detecting unknown variants of COVID-19.
CONCLUSIONAmong the five VOCs, Omicron is the most prevalent and dominant variant. The in-house direct PCR product sequencing surveillance (DPPSS) method provided an alternative platform for SAR-CoV-2 variant analysis which is accessible and affordable than the conventional diagnostic surveillance methods and the whole genome sequencing. Further evaluation and improvements on the oligonucleotide primers may offer significant contribution to the development of a specific and direct PCRbased detection of new emerging COVID-19 variants.

3.A qualitative program evaluation study on the perceived impact of health and nutrition programs among beneficiaries of a civil society organization in the Philippines.

BACKGROUND
Undernutrition remains a public health concern in the Philippines despite multitude of government efforts using different nutrition specific and sensitive interventions. Hence, the role civil society organizations play is important in augmenting the health and nutrition programs in the country. Community feedback is important as they are the receiving end of these programs.
OBJECTIVEThe study aimed to know the perceived impact and community insights on the implemented health and nutrition programs in the four selected sites in the Philippines.
METHODSQualitative program evaluation was used, and 50 discussants were recruited through purposive sampling. Semi-structured interview guide for focus group discussion was utilized to determine the perceived impact of the programs among the discussants, and thematic analysis was used to generate codes and themes.
RESULTSThe implemented health and nutrition programs were found to be a source of hope for the family and community and beyond basic necessities. However, there remain barriers in successful acquisition of new information such as a lack of resources and environmental influences. There are also learned opportunities such as continued support from the Civil Society Organization (CSO), and impact capacity building that are not sponsor-driven.
CONCLUSIONOverall, the participants see the health and nutrition programs as helpful for their children as these programs foster proper child rearing. The programs also empower the parents and the community through capacity building. However, there is a need to revisit implementing rules and guidelines to further maximize the benefits and resources of the programs.

4.Facility assessment for the implementation of the Philippine package of essential noncommunicable disease interventions (PhilPEN) in primary health care centers in Metro Manila.

BACKGROUND AND OBJECTIVE
The Philippine Package of Essential Noncommunicable Disease Interventions (PhilPEN) was introduced by the Department of Health through AO 2012-0029. This is anchored to WHO PEN, a prioritized set of cost-effective interventions that can be carried out to provide an acceptable standard of care at the primary health care level, even in low-resource settings. The study aims to evaluate the availability and adequacy of primary health care facilities in providing the PhilPEN package of interventions using the WHO assessment tool.
METHODSA cross-sectional survey was conducted in 25 randomly selected primary health care facilities in Metro Manila. Data were collected through structured interviews with facility staff and direct observation using a standardized questionnaire aligned with PhilPEN and WHO PEN guidelines. The tool assessed PhilPEN inputs (infrastructure, human resources, basic tools and equipment, essential medicines, record-keeping, financing) and services (risk assessment and screening, early diagnosis and monitoring, treatment and follow-up, counseling, referral of patients).
RESULTSAll facilities met the basic standards for infrastructure, human resources, record keeping, and financing. However, only 40% had all essential medicines, and just 16% had complete tools, including urine ketone/protein test strips. Risk assessment and patient counseling were consistently implemented, but early diagnosis and follow-up services were inconsistent due to training and supply gaps.
CONCLUSIONPrimary health care centers in Metro Manila demonstrate partial readiness for PhilPEN implementation. Gaps in tools, medicines, and protocol availability should be addressed to optimize NCD service delivery.

This study aims to explore the perceptions of UST Occupational Therapy students regarding AI chatbots in the context of school-related activities. It will further focus on their concerns, utility, and perceived effects of AI on learning related to school activities
METHODSA qualitative descriptive design will be used and will utilize three focus group discussions, one for each year level (first, second, and third-year students), to gather extensive and accurate accounts of students’ perceptions. Thematic analysis, using manual coding and following Braun and Clarke’s six-phase analytic framework, will be employed for data analysis.
RESULTSThe study is expected to generate themes describing students’ perceived usefulness, concerns, and learning-related impacts of AI, providing insights that may support the responsible and informed integration of AI in occupational therapy education.

6.Can asthma go away in children? Pro: Yes, it can

Asthma is a chronic respiratory disease characterized by persistent airway inflammation and irreversible airway remodeling. Current treatment strategies primarily rely on symptom relievers and controllers, which do not fundamentally modify the underlying disease pathology, and thus asthma has long been regarded as being difficult to cure. However, long-term follow-up studies in various cohorts have reported sustained absence of symptoms, or clinical remission. In pediatric asthma, several birth cohort studies have demonstrated clinical remission rates approaching 50%, and even in disease cohorts applying more stringent diagnostic criteria, remission has been still observed in children. Higher remission rates have been reported in individuals with younger age at onset, male sex, nonatopic phenotypes without sensitization to inhalant allergens, preserved lung function, and lower bronchial hyperresponsiveness. Immunotherapy, which can modify the natural course of allergic diseases, has been shown to reduce bronchial hyperresponsiveness and increase remission rates in asthma. Recently, the development and widespread use of biologics have enabled remission to be considered as a therapeutic goal even in severe asthma, prompting efforts to standardize its definition. In the current era of diverse therapeutic options and improved clinical outcomes, establishing both more precise and practical evaluation and definition of asthma remission will be pivotal for achieving personalized and precision asthma management.
7.Cluster analysis of laryngomalacia in infants: insights into prognostic factors from a 10-year cohort

Purpose:
Laryngomalacia exhibits diverse morphological patterns, severities, and comorbidities. Defining clinical phenotypes could improve management and prognosis. This study aimed to identify and characterize phenotypes using cluster analysis and to evaluate prognostic factors.
Methods:
We retrospectively reviewed records of 195 children diagnosed with laryngomalacia between 2014 and 2023 using flexible laryngoscopy or bronchoscopy. Demographics, endoscopic findings, comorbidities, and outcomes up to 1 year of age were collected. Hierarchical cluster analysis was conducted using 10 clinical variables.
Results:
Four phenotypes emerged: cluster 1 (n= 75, 38.5%), Groningen Laryngomalacia Classification System (GLCS) type 1 dominant-mild; cluster 2 (n= 35, 17.9%), GLCS type 2 dominant-mild; cluster 3 (n= 40, 20.5%), severe with multiple comorbidities; and cluster 4 (n = 45, 23.1%), GLCS combined-type moderate. Distinct clinical courses were observed. Cluster 3 showed the highest rates of surgical intervention (32.5%, P < 0.001), pediatric intensive care unit admission (17.5%, P = 0.016), and Emergency Department (ED) visits (60.0%, P= 0.013) for respiratory problems during the first year. When stratified by comorbidities, children with multiple comorbidities, particularly those with major feeding problems had a higher risk of hospitalization (adjusted odds ratio [aOR], 2.65;95% confidence interval [CI], 1.11–6.33) and ED visits (aOR, 3.17; 95% CI, 1.39–7.23), even after adjusting for sex and severity.
Conclusion
Four clinically meaningful phenotypes of laryngomalacia were identified from the cluster analysis based on morphology, comorbidities, and disease severity. Children with multiple comorbidities accompanied by feeding problems had the greatest risk of hospitalization and ED visits for respiratory problems within the first year, even after adjusting for the severity of laryngomalacia.
8.Successful desensitization to contrast media in a patient with recurrent hypersensitivity to multiple iodinated contrast agents: A case report

Hypersensitivity reactions (HSRs) to iodinated contrast media (ICM) can range from mild cutaneous symptoms to life-threatening anaphylaxis. In patients with a history of ICM hypersensitivity, avoidance of the culprit agent is generally recommended. This case report describes a successful desensitization in a 56-year-old man with recurrent HSRs to multiple agents including ioversol, iohexol, iobitridol, and iopamidol. Intradermal testing was performed to identify potentially safe alternatives; however, all tested agents, including iohexol, ioversol, iobitridol, iopamidol, iodixanol, iomeprol, and iopromide, yielded positive results. Given the clinical necessity of transcatheter arterial chemoembolization, a 13-step rapid desensitization protocol with iodixanol was implemented. The procedure was completed without any breakthrough reactions. This case highlights desensitization as a feasible and effective strategy for patients with hypersensitivity to multiple ICM agents.
9.An adolescent case of post-ictal diffuse alveolar hemorrhage

Diffuse alveolar hemorrhage (DAH) is a rare but life-threatening condition characterized by bleeding into the alveolar spaces because of damage to the pulmonary microvasculature. While it is commonly associated with autoimmune or coagulation disorders, post-ictal DAH following a generalized tonic-clonic seizure is extremely rare, especially in pediatric populations. We report the case of a previously healthy 13-year-old boy with allergic rhinitis and buckwheat allergy who presented with his first generalized tonicclonic seizure. Following admission, he developed dyspnea and tachypnea (respiratory rate of 38/min); on hospital day 2, oxygen desaturation (SaO 2 88%) accompanied by hemoptysis. Laboratory tests showed no evidence of coagulopathy or autoimmune disease. Chest radiography and computed tomography revealed bilateral pulmonary infiltrates consistent with alveolar hemorrhage.Intravenous methylprednisolone (1 mg/kg for 3 days) was administered, resulting in rapid improvement of respiratory symptoms and imaging findings. This case highlights the importance of early clinical suspicion and imaging-based diagnosis of post-ictal DAH, and the necessity of prompt supportive management to achieve favorable outcomes.
10.Clinical experiences in the diagnosis and management of primaryimmunodeficiency disorders in adults

Primary antibody deficiency (PAD) is the most common form of primary immunodeficiency (PID) in adults, although the overall prevalence remains low. Recent studies have suggested a rising incidence due to changes in environmental factors and increased awareness. PID typically presents with recurrent upper and lower respiratory tract infections but may also be associated with allergic and autoimmune diseases, resulting in various clinical manifestations. This report presents three representative adult cases of PAD— X-linked agammaglobulinemia (XLA), common variable immunodeficiency (CVID), and IgG3 subclass deficiency (IgG3D) with bronchial asthma—offering insights into their diagnosis and long-term management. These cases emphasize the need to suspect XLA in patients with recurrent pneumonia and bronchiectasis, to recognize chronic severe urticaria as a potential clinical clue for CVID, and to evaluate IgG3D in asthma patients with frequent viral infections despite standard care. Through these examples, this clinical insight underscores the importance of early suspicion of PID, comprehensive immunological evaluation and individualized treatment strategies in improving outcomes for adult PID patients.
