Purpose:
Endoscopic submucosal dissection (ESD) has become a standard minimally invasive treatment for selected patients with early gastric cancer (EGC). This study presents the first nationwide survey of patients with EGC treated with ESD in 2023, conducted by the Korean College of Helicobacter and Upper Gastrointestinal Research.
Materials and Methods:
Data were retrospectively collected from participating referral centers across Korea using a standardized case report form covering patient characteristics, tumor features, procedural details, histopathological findings, and clinical outcomes.Descriptive and comparative analyses were conducted to summarize nationwide ESD practice patterns and outcomes.
Results:
Data from 5,460 ESD cases from 5,250 patients across 27 institutions were analyzed. The mean age was 67.4 years, with 74.1% males. Multiple synchronous lesions were identified in 3.7%. Most lesions were located in the lower third of the stomach (64.0%), and differentiated-type adenocarcinomas accounted for 87.8%. The en bloc and complete resection rates were 99.2% and 91.4%, respectively. Curative resection was achieved in 80.5%, whereas local non-curative resection (L-NCR) and surgical non-curative resection (S-NCR) were identified in 2.8% and 16.7%, respectively. Additional surgery was performed more frequently in patients with S-NCR than in those with L-NCR (59.3% vs. 24.7%). The bleeding and perforation rates were 3.6% and 0.9%, respectively, and were mostly managed conservatively or endoscopically. The median length of hospitalization was 4.0 days.
Conclusions
This first nationwide survey provides a comprehensive overview of the current practice of EGC treatment using ESD in Korea, demonstrating high technical success and safety, and establishing a baseline dataset for future longitudinal research.
2.Predictors and patterns of early liver regeneration after major hepatectomy

Background:
Postoperative liver regeneration is essential for maintaining hepatic function. This study evaluated the rate, determinants, and volumetric patterns of early liver regeneration after hemihepatectomy.
Methods:
A retrospective review was conducted of 50 patients who underwent right or left hemihepatectomy between April 2019 and March 2025. Liver and spleen volumes (SV) were assessed preoperatively, at postoperative day (POD) 1 week, and at POD 3 months. Early liver regeneration rate (LRR) was defined as the percentage increase in remnant liver volume at POD 1 week relative to the preoperative future liver remnant (FLR), and patients were categorized into low (<50%) and high (≥50%) LRR groups. Clinical, biochemical, and volumetric variables were compared, and predictors of regeneration were identified using multivariable analyses. Regeneration patterns were also examined according to whether the FLR/standard liver volume (SLV) ratio was <50% or ≥50%.
Results:
FLR/SLV was the strongest independent predictor of rapid early liver regeneration (p<0.001). Remnants with FLR/SLV <50% exhibited rapid and sustained regeneration, whereas those with FLR/SLV ≥50% showed slower regrowth that plateaued after reaching approximately 90% of SLV. SV increased at POD 1 week in all patients; however, only the FLR/SLV ≥50% group showed a reduction by POD 3 months, whereas the <50% group maintained elevated volumes.
Conclusions
FLR/SLV reliably predicts early postoperative liver regeneration. Smaller remnants regenerate more rapidly, whereas persistent splenic enlargement suggests a sustained portal hemodynamic burden. Combined evaluation of FLR/SLV and SV may enhance perioperative risk assessment and surgical planning.
3.Synergistic Exacerbation of Allergic Inflammation by Combined Exposure to Air-Pollutants in a Murine Model of Allergic Rhinitis

Objectives:
Allergic rhinitis (AR) is a chronic inflammation of the nasal mucosa triggered by environmental allergens. Although its pathophysiology has been extensively investigated, the influence of environmental aggravating factors—particularly combined pollutant exposure—remains insufficiently characterized. This study aimed to assess the impact of coexposure to PM2.5, formaldehyde, and Zn on allergic inflammation in a murine AR model and to delineate the associated immunological and histopathological responses.
Methods:
Female BALB/c mice were sensitized with ovalbumin (OVA) and challenged intranasally to induce AR. On days 21–24, the mice were exposed to PM2.5, formaldehyde, and Zn, either individually or in combination with OVA. Allergic symptoms were evaluated through behavioral observation, while immune responses were assessed by analyzing nasal and bronchoalveolar lavage fluids (NALF and BALF), serum immunoglobulin levels, nasal histopathology, and cytokine profiles.
Results:
Combined exposure to PM2.5, formaldehyde, and Zn significantly intensified allergic inflammation compared with single exposures. Coexposure to PM2.5 and Zn led to synergistic increases in total and OVA-specific immunoglobulin E levels, eosinophilic infiltration, nasal rubbing, sneezing, and Th2/Th17 cytokine levels in NALF and BALF. Histological analysis demonstrated epithelial thickening and goblet cell hyperplasia after combined exposure. Other combinations, including PM2.5 with formaldehyde, also produced additive or modestly amplified inflammatory responses.
Conclusions
Coexposure to PM2.5, formaldehyde, and Zn aggravated allergic inflammation in an OVA-induced murine model, with PM2.5+Zn yielding the strongest synergistic effects. These findings emphasize the role of pollutant–pollutant interactions in allergic airway diseases and highlight the need for further research to clarify long-term health effects and relevance to human disease.
4.Physiological Measures of Reduced Listening Effort in Active Noise Canceling: Evidence From a Low-β Functional Network in an Event-Related Potential Study

Objectives:
. In this study, we investigated the neurophysiological impact of active noise cancellation (ANC) technology on auditory–cognitive processing and listening effort, addressing a critical gap in the literature on ANC efficacy. Although ANC is increasingly adopted in contemporary devices, its influence on underlying neurophysiological mechanisms remains underexplored. Given that current assessment approaches rely predominantly on subjective measures, we employed a multi-condition experimental design integrating subjective self-reports with electroencephalographic assessments, thereby providing a more comprehensive evaluation of ANC’s capacity to mitigate cognitive load.
Methods:
. Thirty participants underwent comprehensive audiological testing and cognitive event-related potential recordings during an auditory oddball paradigm to elucidate the efficacy of ANC functionality and its impact on cognitive processing, as indexed by the P3 component. Functional connectivity was quantified using the source-localized weighted phase-lag index, and graph-theory-based network analysis was applied to assess changes in neural efficiency under noisy conditions with and without ANC activation.
Results:
. The analyses revealed significant alterations in low-β–band functional architecture across the central executive, dorsal attention, salience, and default mode networks during effortless listening with ANC enabled, compared with the effortful listening condition. Subjective listening effort correlated with network efficiency in the left posterior middle frontal gyrus, dorsal anterior cingulate cortex, and inferior parietal lobule during ANC-aided effortless listening tasks.
Conclusion
. Our findings suggest that ANC activation significantly reduces neural processing demands across key cognitive networks during effortless listening. These results reinforce the role of ANC in alleviating cognitive load and enhancing neural efficiency in auditory–cognitive processing. Furthermore, the findings provide empirical support to inform evidence-based noise mitigation strategies in both clinical and ecological contexts.
5.Longitudinal analysis of serotypes, virulence factors, and MLST profiles of Pasteurella multocida from porcine pneumonia in South Korea (2016–2023)

Objective:
This study performed a longitudinal molecular characterization of P. multocidaisolates from pigs with respiratory lesions in South Korea (2016–2023), including subspecies identification, capsular serogrouping, virulence gene profiling, and multilocus sequence typing (MLST).
Methods:
A total of 1,358 pneumonic lung samples were collected from 960 pig farms between 2016 and 2023, yielding 169 P. multocida isolates. Subspecies were assigned by 16S rRNA gene sequencing. Capsular serogroups were determined using multiplex Virulence genes (pfhA, hgbB, tbpA, and toxA) were detected by PCR assays. MLST was conducted using the RIRDC scheme, and phylogenetic analysis of concatenated loci assessed clonal relationships.
Results:
Serogroup A predominated (109/169, 64.5%), followed by D (47/169, 27.8%) and B (3/169, 1.8%). Serogroups F and A/D were each detected once (0.6%), and 8 isolates were untypable (4.7%). Co-detection of hgbB and pfhA occurred in multiple isolates. Notably, two serogroup B isolates carried both genes, representing the first such finding in Korea and suggesting enhanced virulence potential. MLST identified 5 clonal complexes and 15 sequence types, including ST9 linked to serogroup B.
Conclusions
and Relevance: The emergence of serogroup B/ST9 isolates co-harboring hgbB and pfhA highlights evolving virulence patterns in Korean swine and supports continued molecular and genomic surveillance to guide control strategies and reduce health risks.
6.Actinobacillus pleuropneumoniaein Korea (2015-2025): serovar distribution, toxin gene profiles, antimicrobial resistance, and identification of an apxIICA-deficient serovar 15 profile

Objective:
To provide a decade-long molecular and phenotypic characterization of APP isolates from Korean pig farms, focusing on the serovar distribution, apx-based toxingene profiles, and antimicrobial susceptibility.
Methods:
Between 2015 and 2025, 1,215 pneumonic lung samples from 965 pig farms yielded 132 APP isolates. The species identity was confirmed by 16S rRNA sequencing. The serovars were determined using capsule polysaccharide (CPS) gene-based multiplex polymerase chain reaction (PCR). Toxin genes (apxIA–apxIVA) were profiled, and the antimicrobial susceptibility to 29 agents was assessed by broth microdilution according to the Clinical and Laboratory Standards Institute guidelines.
Results:
Serovar 1 was predominant (66.7%), followed by serovars 5 (17.4%) and 2 (6.8%).CPS multiplex PCR identified three isolates (2.3%) as serovar 15, which displayed heterogeneous toxin gene profiles, including apxIICA-deficient profiles. Most isolates exhibited classical repeats-in-toxin operon arrangements, suggesting ongoing diversification of toxin gene profiles. High resistance rates were observed for oxytetracycline (90.9%) and florfenicol (50.8%), and recurrent multidrug-resistant combinations were frequently detected.
Conclusions
and Relevance: Serovar 1 is dominant in Korea, but the emergence of atypical toxin gene profiles in serovar 15 may carry immunological implications. Persistent resistance to older drug classes underscores the necessity for long-term molecular surveillance, evaluation of vaccine coverage against evolving strains, and enhanced antimicrobial stewardship to strengthen the control efforts for porcine pleuropneumonia in Korea.
7.Correlation and Agreement Between Transcutaneous Oxygen Pressure and Toe Pressure in Patients with Diabetic Foot Ulcers

Background:
Adequate tissue oxygenation is a key determinant of diabetic foot ulcer (DFU) outcomes. Though transcutaneous oxygen pressure (TcPO2) is the gold standard for evaluating tissue oxygenation, its limited availability restricts routine clinical use. Consequently, toe pressure is frequently utilized as a practical surrogate; however, the direct correlation between these two modalities has yet to be rigorously investigated. This study aimed to assess the correlation and agreement between TcPO2 and toe pressure in patients with DFUs.
Methods:
A retrospective review was conducted on 837 DFU patients who received simultaneous TcPO2 and toe pressure assessments. The correlation between the two tests was analyzed using the Pearson correlation coefficient, and agreement was evaluated using Bland–Altman analysis. To aid interpretation, a scatterplot and Bland–Altman plot were generated.
Results:
TcPO2 and toe pressure demonstrated a strong correlation (R=0.66; 95% confidence interval, 0.62 to 0.70; P<0.001). Bland–Altman analysis showed a mean bias of 26.9 mmHg (standard deviation of differences, 28.8 mmHg; 95% limits of agreement, −28.9 to 82.6 mmHg) between toe pressure and TcPO2, reflecting limited agreement and increased variability at higher perfusion levels.
Conclusion
TcPO2 and toe pressure are strongly correlated. However, they are not interchangeable, particularly in DFU patients with high tissue perfusion.
8.Consensus of Korean Asthma Study Group on Definition of Clinical Remission in Severe Asthma: A Modified Delphi Study

Background:
Asthma remission has recently emerged as an aspirational treatment goal, yet its definition remains inconsistent across studies and expert groups. The absence of a standardized framework hampers its application in clinical practice and research, particularly in Korea where biologics use is rapidly increasing. This study aimed to establish a consensus definition of clinical remission in severe asthma among Korean experts.
Methods:
A two-round modified Delphi survey, followed by a focused third round, was conducted among 28 board-certified pulmonologists from the Korean Academy of Tuberculosis and Respiratory Diseases (KATRD). The questionnaire consisted of six domains and 27 items. Responses were analyzed using agreement rates, interquartile ranges, and content validity ratios to determine consensus levels.
Results:
Consensus was reached on defining clinical remission as a composite of no exacerbations, no systemic corticosteroid use, sustained symptom control (Asthma Control Test ≥20 on at least three occasions over 12 months), and stabilization and optimization of pulmonary function while on maintenance treatment. Experts agreed that pulmonary function should be assessed based on clinical judgment rather than absolute thresholds. Complete remission was additionally defined as fulfilling all clinical remission criteria with normalization of type 2 inflammation (blood eosinophils <300/μL and fractional exhaled nitric oxide <25 ppb).
Conclusion
This Delphi consensus provides a regionally relevant and pragmatic framework for defining remission in severe asthma. These statements may help guide clinical practice, inform guideline development, and support future research on remission as a treatment goal.
9.Age- and disability-based trends in potentially preventable hospitalizations: evidence from nationwide claims data in Korea

OBJECTIVES:
Individuals with disabilities are at greater risk of hospitalization than the general population. We examined 10-year trends in potentially preventable hospitalizations (PPH) in Korea, comparing individuals with and without disabilities and assessing age-specific patterns.
METHODS:
Using National Health Information Database claims data (2010–2019), we established a fixed cohort of newly registered individuals with disabilities and control subjects statistically matched (1:1.5) at baseline. Annual PPH rates among patients with each condition were calculated and age- and sex-standardized according to Organization for Economic Cooperation and Development Health Care Quality Indicators definitions. Trends and annual percent changes (APCs) were analyzed by disability status and age group (non-older: 30–64; older adults: ≥65 years).
RESULTS:
Between 2010 and 2019, PPH rates declined significantly in both groups. Among individuals with disabilities, the steepest decline was observed for hypertension (APC, −15.7%; 95% confidence interval [CI], −17.7 to −13.7), whereas congestive heart failure showed the largest reduction among individuals without disabilities (APC, −7.8%; 95% CI, −10.8 to −4.7). Declines were generally greater among non-older adults aged 30–64 years, regardless of disability status. The disparity between disability and non-disability groups narrowed over the decade, primarily due to larger improvements among non-older adults. Older adults with disabilities consistently exhibited the highest PPH rates for most conditions, whereas younger individuals with disabilities had the highest rates for diabetes.
CONCLUSIONS
PPH rates declined over the decade among both individuals with and without disabilities, particularly for hypertension and among non-older adults. However, older adults with disabilities remain at elevated risk, underscoring the need for targeted strategies to improve access to community-based primary care.
