1.Major clinical research advances in gynecologic cancer in 2023:a tumultuous year for endometrial cancer

In the 2023 series, we summarized the major clinical research advances in gynecologic oncology based on communications at the conference of Asian Society of Gynecologic Oncology Review Course. The review consisted of 1) Endometrial cancer: immune checkpoint inhibitor, antibody drug conjugates (ADCs), selective inhibitor of nuclear export, CDK4/6 inhibitors WEE1 inhibitor, poly (ADP-ribose) polymerase (PARP) inhibitors. 2) Cervical cancer: surgery in low-risk early-stage cervical cancer, therapy for locally advanced stage and advanced, metastatic, or recurrent setting; and 3) Ovarian cancer: immunotherapy, triplet therapies using immune checkpoint inhibitors along with antiangiogenic agents and PARP inhibitors, and ADCs. In 2023, the field of endometrial cancer treatment witnessed a landmark year, marked by several practice-changing outcomes with immune checkpoint inhibitors and the reliable efficacy of PARP inhibitors and ADCs.
2.Glue-Sniffing Neuropathy: A Case Report

Glue, a volatile substance that is illegal as a recreational drug, has been popularly used for decades, and it can serve as a stepping stone for harder drugs. Chronic exposure can lead to substantial damage to several organs, including central and peripheral nervous systems. Glue-sniffing neuropathy has been reported since the 1960s, but with a lower frequency in recent years. We report a 45-year-old man who sniffed glue and presented with symmetrical distal motor weakness and paresthesia. Based on the patient’s inhalation history and initial electrodiagnostic study, we considered toxic neuropathy and demyelinating polyneuropathy in the differential diagnosis. He became chair-bound with repeating glue sniffing, and a following nerve conduction study showed the progression of motor-dominant polyneuropathies with markedly reduced amplitudes. An incomplete response to steroid therapy and recovery with inhalant cessation confirmed the diagnosis of glue-sniffing neuropathy. We conclude that glue, a neurotoxic volatile inhalant, produced glue-sniffing neuropathy with characteristic clinical and electrodiagnostic features.
3.Discordance Between Spine-Hip and Paretic-Nonparetic Hip Bone Mineral Density in Hemiplegic Stroke Patients: A Multicenter Retrospective Study

Objective:
To identify the prevalence and factors associated with T-score discordance between the spine and hip, as well as between the paretic and non-paretic hips in hemiplegic stroke patients, this study investigated bone mineral density (BMD) patterns. Bone loss predominantly affects the paretic hip after a stroke, and typical clinical assessments using dual-energy X-ray absorptiometry (DXA) that scan the lumbar spine (LS) and a single hip may overlook an osteoporosis diagnosis. This oversight could potentially lead to suboptimal treatment for stroke patients.
Methods:
This study was a multicenter retrospective analysis of 540 patients admitted for stroke rehabilitation between October 2014 and February 2022, who underwent DXA of LS and bilateral hips.
Results:
The prevalence rates of concordance, low LS discordance, and low hip discordance between the LS and hips were 48.2%, 12.2%, and 39.6%, respectively. The discordance rate between bilateral hips was 17.0%. The paretic side had significantly lower total hip T-scores than the non-paretic side (p<0.001). Notably low paretic hip discordance was more prevalent during the chronic phase. DXA scans of the LS and both hips revealed a 0.7%–0.9% higher major discordance compared to LS and single hip DXA scans. The multivariate analysis revealed a significant correlation between a low paretic hip discordance and cognitive impairment (adjusted odds ratio 0.071, 95% confidence interval 0.931–1.003, p<0.05).
Conclusion
Since stroke survivors are at high risk for hip fractures, comprehensive BMD assessments, which include LS and bilateral hips, should be considered for post-stroke osteoporosis care to enhance diagnostic accuracy and timely treatment.

6.Differences Between Positive and Negative Perceptions of Stress Based on Patterns of Coping Strategies: Through Mobile Application in College Students

Background:
Stress management is an important component of management of mental health. Stress processing can be understood as the appraisal of and coping with stress. This study aimed to compare positive and negative perceptions of stress according to patterns of coping strategies.
Methods:
The study recruited 116 college students who completed the Perceived Stress Scale and Coping Inventory for Stressful Situations using a tablet PC or smartphone application. K-means cluster analysis was used to categorize the subjects into three clusters.
Results:
Positive perceptions were highest in Cluster 2 (i.e., task and avoidance dominant) at 3.18±0.66 followed by 2.30±0.65 in Cluster 1 (passive coping) and lowest in Cluster 3 (emotion dominant) at 1.75±0.63. Negative perceptions were highest in Cluster 3 at 2.45±0.66 followed by Cluster 1 at 1.66±0.65 and Cluster 2 at 1.07±0.68.
Conclusion
Differences in coping patterns can reduce the negative effects of stress. Thus, assessing the perception and management of stress can be useful and help people change their coping styles to more adaptive ones. Toward this end, a mobile application can be an appropriate tool.

8.Establishing Reference Values for a New Computerized Cognitive Function Test Program for Children

Objective:
To establish reference values for the computerized cognitive test and evaluate cognitive function improvements across different age groups, we introduce the computerized Cognitive Function Test program (eCFT), specifically designed for children. We aimed to establish eCFT reference values and assess cognitive function improvements across different age groups.
Methods:
We included children aged 3–6 years with confirmed normal cognition based on the Korean Developmental Screening Test for Infants and Children and Kaufman Assessment Battery for Children-II. The eCFT consists of 8 subtests for visual perception, attention, memory, and executive function.
Results:
A total of 66 participants (36 males and 30 females) with an average age of 4.4 years participated. The age 6 group consistently outperformed both age group 3 and 4 in terms of correct responses. With regard to the completed stage, the “selective auditory stimulus” test findings were 2.0 and 3.9 for the age 3 and age 6 groups, respectively (p<0.05). The “trail-making” test findings were 1.7, 2.1, 2.6, and 2.8, respectively (between ages 3 and 6, p<0.01; between ages 4 and 6, p<0.05); moreover, the age 5 group surpassed the age 3 group (2.6 and 1.7, respectively, p<0.05).
Conclusion
The eCFT is an easily accessible tool to evaluate cognitive function in young children. We introduce reference values with a cutoff range for preschool-aged children, enabling early intervention for those with cognitive impairment. Given its accessibility and relatively short evaluation time, the eCFT has potential for clinical use.
9.Prevalence and confounders of chronic endometritis diagnosed using CD138 in patients with recurrent implantation failure

Objective:
This retrospective study aimed to investigate the prevalence of chronic endometritis, diagnosed using CD138 immunohistochemistry, among infertile women and to assess the association between chronic endometritis and recurrent implantation failure (RIF).
Methods:
In total, 266 patients who underwent hysteroscopy due to infertility between 2019 and 2020 were included in the analysis. Of these, 136 patients with RIF and 130 non-RIF patients were included in the study. CD138 immunohistochemistry test results, blood biomarkers (including natural killer cells, white blood cells, and the lymphocyte-to-neutrophil ratio), and data on pregnancy outcomes were obtained. If the CD138 test yielded a positive result, the patients received antibiotic treatment.
Results:
The overall proportion of CD138-positive patients was 32.7% (87/266). The CD138 positivity rate was not related to the number of cycles with implantation failure. In the RIF patient group, no significant associations were found between CD138 positivity and peripheral blood markers. The clinical pregnancy rates were similar between infertile women treated with antibiotics for chronic endometritis and those without chronic endometritis.
Conclusion
To improve the pregnancy rate in infertile patients, it may be helpful to combine CD138 testing with other laboratory tests and administer antibiotic treatment if the result is positive.
10.Recent Research Trends in Stem Cells Using CRISPR/Cas-Based Genome Editing Methods

The clustered regularly interspaced short palindromic repeats (CRISPR) system, a rapidly advancing genome editing technology, allows DNA alterations into the genome of organisms. Gene editing using the CRISPR system enables more precise and diverse editing, such as single nucleotide conversion, precise knock-in of target sequences or genes, chromosomal rearrangement, or gene disruption by simple cutting. Moreover, CRISPR systems comprising transcriptional activators/repressors can be used for epigenetic regulation without DNA damage. Stem cell DNA engineering based on gene editing tools has enormous potential to provide clues regarding the pathogenesis of diseases and to study the mechanisms and treatments of incurable diseases. Here, we review the latest trends in stem cell research using various CRISPR/Cas technologies and discuss their future prospects in treating various diseases.
