Multiple myeloma (MM), a hematological malignancy, is characterized by malignant plasma cell proliferation in the bone marrow. Recent treatment advances have significantly improved patient outcomes associated with MM.In this study, we aimed to develop comprehensive, evidence-based guidelines for the diagnosis, prognosis, and treat‑ ment of MM. We identified 12 key clinical questions essential for MM management, guiding the extensive literature review and meta-analysis of the study. Our guidelines provide evidence-based recommendations by integrating patient preferences with survey data. These recommendations include current and emerging diagnostic tools, thera‑ peutic agents, and treatment strategies. By prioritizing a patient-centered approach and rigorous data analysis, these guidelines were developed to enhance MM management, both in Korea and globally.
2.The Value of Magnetic Resonance Imaging-Driven Apparent Diffusion Coefficient to Predict Pathologic Response of Neoadjuvant Chemotherapy in Muscle-Invasive Bladder Cancer: A Single-Center Retrospective Study

Purpose:
This study aimed to evaluate the predictive value of apparent diffusion coefficient (ADC) values from magnetic resonance imaging (MRI) in assessing pathologic response to neoadjuvant chemotherapy (NAC) in patients with muscle-invasive bladder cancer (MIBC) and the optimal cutoff value of ADC for predicting NAC response.
Materials and Methods:
Among 582 patients who underwent radical cystectomy between December 2011 and December 2024, we included 146 patients who received preoperative NAC and pretreatment MRI. Pathologic response was determined based on the surgical pathology results: patients with pathologic stage >pT2 and/or presence of nodal involvement (N+) were classified as nonresponders, while those with ≤pT2 and no nodal involvement (N0) were classified as responders. MRI-derived ADC values were compared between the 2 groups. The receiver operating characteristic (ROC) curve was used to determine the optimal cutoff value for predicting NAC response. Multivariate logistic regression analysis using preoperative parameter was performed and Kaplan-Meier survival analysis were utilized to assess overall survival (OS) and cancer-specific survival (CSS) according to NAC response.
Results:
Among the 146 patients, 90 (61.6%) were classified as responders, 56 (38.4%) were nonresponders. The mean ADC value was significantly higher in responders (1,100.35±249.12 mm2/sec) than in nonresponders (763.95±118.97 mm2/sec, p<0.001). ROC analysis demonstrated an area under the curve of 0.69, with an optimal cutoff value of 845.0 mm2/sec. Multivariate logistic regression identified ADC value as an independent predictor of NAC response (odds ratio, 1.025; 95% confidence interval, 1.006–1.043; p=0.008). During median follow-up 30.3 months, Kaplan-Meier analysis revealed that responders had significantly better 5-year OS (80.8% vs. 45.9%) and 5-year CSS (92.6% vs. 54.5%), with log-rank test p<0.001.
Conclusion
MRI-derived ADC values are a reliable imaging biomarker for predicting pathologic response to NAC in MIBC. Further validation in large-scale prospective studies is required to confirm these findings.
3.Is ChatGPT a “Fire of Prometheus” for Non-Native English-Speaking Researchers in Academic Writing?

Large language models (LLMs) such as ChatGPT have garnered considerable interest for their potential to aid non-native English-speaking researchers. These models can function as personal, round-the-clock English tutors, akin to how Prometheus in Greek mythology bestowed fire upon humans for their advancement. LLMs can be particularly helpful for non-native researchers in writing the Introduction and Discussion sections of manuscripts, where they often encounter challenges.However, using LLMs to generate text for research manuscripts entails concerns such as hallucination, plagiarism, and privacy issues; to mitigate these risks, authors should verify the accuracy of generated content, employ text similarity detectors, and avoid inputting sensitive information into their prompts. Consequently, it may be more prudent to utilize LLMs for editing and refining text rather than generating large portions of text. Journal policies concerning the use of LLMs vary, but transparency in disclosing artificial intelligence tool usage is emphasized. This paper aims to summarize how LLMs can lower the barrier to academic writing in English, enabling researchers to concentrate on domain-specific research, provided they are used responsibly and cautiously.

5.Diameter of the Solid Component in Subsolid Nodules on Low-Dose Unenhanced Chest Computed Tomography: Measurement Accuracy for the Prediction of Invasive Component in Lung Adenocarcinoma.

OBJECTIVE: To determine if measurement of the diameter of the solid component in subsolid nodules (SSNs) on low-dose unenhanced chest computed tomography (CT) is as accurate as on standard-dose enhanced CT in prediction of pathological size of invasive component of lung adenocarcinoma. MATERIALS AND METHODS: From February 2012 to October 2015, 114 SSNs were identified in 105 patients that underwent low-dose unenhanced and standard-dose enhanced CT pre-operatively. Three radiologists independently measured the largest diameter of the solid component. Intraclass correlation coefficients (ICCs) were used to assess inter-reader agreement. We estimated measurement differences between the size of solid component and that of invasive component. We measured diagnostic accuracy of the prediction of invasive adenocarcinoma using a size criterion of a solid component ≥ 6 mm, and compared them using a generalized linear mixed model. RESULTS: Inter-reader agreement was excellent (ICC, 0.84.0.89). The mean ± standard deviation of absolute measurement differences between the solid component and invasive component was 4 ± 4 mm in low-dose unenhanced CT and 5 ± 4 mm in standard-dose enhanced CT. Diagnostic accuracy was 81.3% (95% confidence interval, 76.7.85.3%) in low-dose unenhanced CT and 76.6% (71.8.81.0%) in standard-dose enhanced CT, with no statistically significant difference (p = 0.130). CONCLUSION: Measurement of the diameter of the solid component of SSNs on low-dose unenhanced chest CT was as accurate as on standard-dose enhanced CT for predicting the invasive component. Thus, low-dose unenhanced CT may be used safely in the evaluation of patients with SSNs.

6.Multimodality Imaging in Patients with Secondary Hypertension: With a Focus on Appropriate Imaging Approaches Depending on the Etiologies.

Although the causes of hypertension are usually unknown, about 10% of the cases occur secondary to specific etiologies, which are often treatable. Common categories of secondary hypertension include renal parenchymal disease, renovascular stenosis, vascular and endocrinologic disorders. For diseases involving the renal parenchyma and adrenal glands, ultrasonography (US), computed tomography (CT) or magnetic resonance (MR) imaging is recommended. For renovascular stenosis and vascular disorders, Doppler US, conventional or noninvasive (CT or MR) angiography is an appropriate modality. Nuclear imaging can be useful in the differential diagnosis of endocrine causes. Radiologists should understand the role of each imaging modality and its typical findings in various causes of secondary hypertension. This article focuses on appropriate imaging approaches in accordance with the categorized etiologies leading to hypertension.
