Purpose: Atherosclerotic plaques in peripheral arterial disease (PAD) include fatty, mixed, and calcified types. Plaque burden is significantly associated with restenosis, reintervention, and amputation-free survival. Rotational and aspirational atherectomy (RAA) may effectively remove such plaques. This study aimed to evaluate long-term outcomes of RAA for infrainguinal PAD. Methods: Patients with infrainguinal lesions underwent revascularization using the Jetstream Atherectomy System (Boston Scientific). This 60-month extension assessed primary patency rate (PPR) and clinically driven target lesion revascularization (CD-TLR). Kaplan-Meier curves were used for survival analysis; P < 0.05 was considered statistically significant. Results: A total of 150 patients (mean age, 70.9 years; male, 86.0%; 65.4% with diabetes) were enrolled. The mean lesion length was 15.8 cm, with 74.0% occlusions and 47.3% severe calcification. Lesions were sclerotic (72.4%), thrombosclerotic (13.4%), thrombotic (9.4%), or in-stent (4.7%). A drug-coated balloon (DCB) was used in 85.5% of cases. PPR at 1, 3, and 5 years was 84.1%, 68.1%, and 58.5%, respectively. CD-TLR rates were 93.0%, 81.5%, and 67.4%, respectively. The benefit of DCB was sustained through 3 years but attenuated thereafter, highlighting the need for extended follow-up in infrainguinal interventions. Conclusion RAA demonstrated durable 5-year patency and safety outcomes. Device type, DCB use, lesion morphology, and calcium grade did not significantly influence long-term results. Lesion complexity remains the primary predictor of clinical outcome. Despite the complexity of infrainguinal lesions, the use of RAA demonstrated sustained patency through 3 years, with lesion complexity (particularly TASC classification) emerging as the most critical predictor of long-term success.

Background: The nipple is a potential source of pathogens because its lactiferous ducts act as direct conduits from the nipple–areolar complex to the breast parenchyma. Our previous studies identified breast microbiota as a factor in postoperative complications following immediate breast reconstruction using silicone implants and acellular dermal matrix. This study aimed to investigate the correlation between preoperative nipple swab microbiota and the incidence of surgical site infections (SSIs) after autologous breast reconstruction. Methods: We conducted a retrospective chart review of patients who underwent autologous breast reconstruction following total mastectomy. Preoperative nipple swab cultures were obtained. Patient demographics, surgical characteristics, and complication rates were compared between culture-positive and culture-negative groups. Microbiological data, including antibiotic‑resistance profiles, were collected. Results: Among 39 reconstructed breasts, 18 (46.9%) had positive preoperative nipple cultures. The mean duration of drain placement was significantly longer in the culture‑positive group (14.39±3.96 days) than in the culture‑negative group (12.14±2.76 days, P=0.045). Methicillin‑susceptible Staphylococcus epidermidis accounted for 55.0% of isolates. Of the four SSIs observed, three occurred in patients with positive preoperative cultures. Conclusions Although pathogen strains differed between preoperative and postoperative settings, obtaining preoperative nipple microflora cultures and determining antibiotic‑resistance profiles can guide immediate antibiotic selection for SSIs and enhance postoperative management.

Materials and Methods: A retrospective chart review was conducted for patients treated with the customized TMJ arthrocentesis guide to address hemarthrosis. Inclusion criteria included adults older than 18 years with condylar fractures not indicated for open reduction and internal fixation. The guide was fabricated using computed tomography or cone-beam computed tomography scans and 3D printing to control the degree and depth of the needle through the guide to aspirate hemarthrosis. Results: Six joints from five patients (average age 52.4 years) were treated using the guide. Post-procedural magnetic resonance imaging confirmed complete resolution of hemarthrosis in all cases. A representative case demonstrated successful outcomes with normal healing and no complications at 8 months post-procedure. Conclusion The customized TMJ injection guide, using ear-based reference points, significantly enhances the accuracy and safety of TMJ arthrocentesis. This innovation is expected to increase the accessibility and effectiveness of TMJ-related treatments for clinicians without specialized TMJ expertise.

Purpose: This study aimed to comprehensively analyze the thematic trends and paradigm shifts in sports medicine research by applying keyword network analysis and text mining techniques to papers published in Korean academic journals over the past decade (2014−2024). Through this, the study sought to identify the overall research flow in the field and provide foundational insights for setting future research directions in Korean sports medicine. Methods: Research articles related to sports medicine published between 2014 and 2024 were collected from the RISS (Research Information Sharing Service) using the keywords “sports medicine,” “athletic training,” and “sports rehabilitation.” Data were processed and analyzed with the big data analysis program TextoM (The IMC Inc.). Text mining and term frequency−inverse document frequency (TF-IDF) analyses were performed, followed by semantic network and CONCOR (CONvergent of iterated CORrelation) analyses using Ucinet 6.0 (Analytic Technologies) and NetDraw to examine relational structures among keywords. Results: The most frequent keywords included “training” (2,133), “effect” (1,536), “research” (1,428), “athlete” (1,311), and “measurement” (889). In TF-IDF analysis, the highest values were observed for “disorder,” “group,” “athlete,” “training,” and “muscle strength.” Six major research clusters were identified: (1) sports medicine/rehabilitation, (2) golf and taekwondo, (3) soccer, (4) program development, (5) psychology, and (6) weight training. Conclusion The findings indicate a paradigm shift in Korean sports medicine research from treatment- and rehabilitation-centered studies toward prevention, performance enhancement, and multidisciplinary topics. These results provide a theoretical foundation for future integrative and specialized research designs in sports medicine.

In the case of skeletal class III edentulous patients, the stability of dentures can be achieved by using a crossbite by considering the residual ridge relationship when fabricating complete dentures. Forming the anterior teeth in a normal occlusal relationship in a skeletal class III ridge relationship may reduce the stability of the denture by increasing the anterior cantilever. However, when patients use complete dentures, not only functional aspects but also aesthetic aspects are important. The aesthetics of complete dentures depends on how the anterior artificial teeth are arranged, and cases of complete denture fabrication using normal occlusion or edge-to-edge bite in edentulous patients with skeletal class III ridge relationships have been reported. In this case, complete dentures were fabricated for an edentulous patient with a skeletal class III edentulous patients by forming anterior edgeto-edge bite considering neutral zone in maxillary anterior teeth, and good aesthetic and functional results were obtained.

Previously, we reported the concept of a cloud-based telemedicine platform for patients with intracerebral hemorrhage (ICH) at local emergency rooms in rural and medically underserved areas in Gangwon state by combining artificial intelligence and remote consultation with a neurosurgeon. Developing a telemedicine ICH treatment protocol exclusively for doctors with less ICH expertise working in emergency rooms should be part of establishing this system. Difficulties arise in providing appropriate early treatment for ICH in rural and underserved areas before the patient is transferred to a nearby hub hospital with stroke specialists. This has been an unmet medical need for decade. The available reporting ICH guidelines are realistically possible in university hospitals with a well-equipped infrastructure. However, it is very difficult for doctors inexperienced with ICH treatment to appropriately select and deliver ICH treatment based on the guidelines. To address these issues, we developed an ICH telemedicine protocol. Neurosurgeons from four university hospitals in Gangwon state first wrote the guidelines, and professors with extensive ICH expertise across the country revised them. Guidelines and recommendations for ICH management were described as simply as possible to allow more doctors to use them easily. We hope that our effort in developing the telemedicine protocols will ultimately improve the quality of ICH treatment in local emergency rooms in rural and underserved areas in Gangwon state.
