Objectives: Tegoprazan is an alternative to proton pump inhibitors (PPIs). This study evaluated the occurrence of upper gastrointestinal (GI) and cardiovascular (CV) complications in patients with ischemic heart disease undergoing antiplatelet therapy after percutaneous coronary intervention (PCI) who were treated with tegoprazan or PPIs. Methods: Data from 604 patients who received antiplatelet therapy with tegoprazan or PPIs for >6 months after PCI between March 2019 and November 2023 were retrospectively analyzed. The primary GI endpoints were symptomatic gastroduodenal ulcers and upper GI bleeding, while the primary CV endpoints comprised major adverse cardiac events (MACEs), nonfatal myocardial infarction (MI), target vessel revascularization, and death from CV-related causes. Results: Among the 604 patients, 265 received tegoprazan and 339 received PPIs. During a mean follow-up of 17 months, seven patients experienced a GI event (0.4% with tegoprazan vs. 1.8% with PPIs; p=0.112) and 12 experienced MACEs (1.1% with tegoprazan vs. 2.7% with PPIs; p=0.183). Subgroup analysis indicated that target vessel revascularization occurred in six patients, with event rates of 0.8% (n=2) for tegoprazan and 1.2% (n=4) for PPIs (p=0.598). Tegoprazan was associated with similar rates of nonfatal MI (0.8% vs. 2.4%; p=0.125) and death from CV-related causes (0.4% vs. 0%; p=0.258) as PPIs. Conclusions There were no significant differences in GI- or CV-related complications between patients treated with tegoprazan and those treated with PPIs.

Background: Inferior orbital blowout fractures (BOF) cause ocular symptoms like diplopia and extraocular muscle limitation, influenced by inferior rectus muscle (IRM) status. This study evaluated the relationship between preoperative IRM status on computed tomography (CT) and ocular symptom recovery time. Methods: This retrospective study analyzed 127 patients with inferior BOF and ocular symptoms (2012–2024). Inclusion criteria included age 18–80 years and preoperative CT availability. IRM status was assessed for bone interruption, herniation extent, and swelling (short/ long axis ratio < 0.54 or ≥ 0.54). Recovery times were analyzed using t-tests and Kaplan-Meier survival analysis (p< 0.05). Results: Of 127 patients (mean age, 35 years; 67.7% male), 66 (52.0%) recovered from ocular symptoms within 7 days, 53 (41.7%) within 30–90 days, and eight (6.3%) had diplopia that remained as a permanent sequela at the last follow-up. Diplopia recovery time was longer in IRM–bone contact (16.9± 5.5 days, n= 35) and bone-pierced IRM (29.4± 10.3 days, n= 15) groups versus no-contact (5.1± 4.2 days, n= 77; p= 0.02, t-test). Herniation and swelling showed no significant correlation (p> 0.05). Conclusion Preoperative CT-based assessment of the IRM–bone interface predicts diplopia recovery time, with bone-pierced IRM linked to prolonged recovery. Patients with bone-pierced IRM may require closer follow-up and careful consideration of timely surgical management.

Scalp angiosarcoma is a rare, aggressive malignancy with poor prognosis, particularly when multifocal. We report a 62-year-old woman with multifocal scalp angiosarcoma, a 3.5× 3.5 cm ulcerated vertex lesion within an approximately 10× 10 cm clinically abnormal vertex field and three 1× 1 cm temporal satellite lesions (two in the right temporal region and one in the left temporal region). Initial wide excision with frozen biopsy-guided margins and reconstruction using a rotation flap and split-thickness skin grafts was followed by adjuvant radiotherapy. At 7 months after completion of radiotherapy, radiation-induced necrosis with bone exposure developed in the frontal and occipital regions, with multiple biopsies negative for recurrence. Double free flap reconstruction (radial forearm free flap and adipofascial anterolateral thigh flap using a piggyback anastomosis) was performed 11 months after radiotherapy using a single recipient vessel pair. Venous congestion on postoperative day 1 was successfully revised with vein graft interposition, resulting in complete flap survival. The patient was disease-free with stable flap coverage at the latest follow-up, more than 2.5 years after radiotherapy. This case demonstrates that double free flap reconstruction using a piggyback technique can be a safe and effective reconstructive option for extensive irradiated scalp defects in multifocal angiosarcoma.

Purpose: The aim of this study is to compare the detection ability of quality of life (QoL) changes and responsiveness of the KOrean QUality of life in Stomach cancer patients Study group (KOQUSS)-40 and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ). Materials and Methods: A multicenter prospective observational study was conducted to evaluate QoL changes after various gastrectomies between January 2021 and April 2022. Participants were instructed to complete the KOQUSS-40 and EORTC QLQ-C30/STO22 preoperatively and at 1, 3, 6, and 12 months postoperatively. QoL changes over time and QoL responsiveness were assessed for each questionnaire. Results: Data from 491 patients who underwent curative gastrectomy for gastric cancer at 22 institutions were analyzed. The summary scores of the KOQUSS-40 and EORTC QLQ-STO22 showed significant differences between the total and proximal gastrectomy groups (p=0.044 and p=0.038, respectively), but no difference was observed for the EORTC QLQ-C30. Dysphagia on the KOQUSS-40 was significantly different between the total and proximal gastrectomy groups (p=0.031); however, dysphagia on the EORTC QLQ-STO22 did not differ. The responsiveness of the KOQUSS-40 was similar to that of the EORTC QLQ in patients who experienced ≥ 10% body weight loss, but approximately 10% less in patients receiving adjuvant chemotherapy than the EORTC QLQ. Conclusion KOQUSS-40 has several advantages over EORTC QLQ-C30/STO22 when comparing QoL between the total and proximal gastrectomy groups. The findings provide information for researchers investigating the QoL of patients who have undergone curative gastrectomy for gastric cancer.

Although aspartate aminotransferase (AST) is a serum marker of hepatocellular damage in chronic hepatitis, it is difficult to interpret very high AST levels with concurrent low alanine aminotransferase (ALT) levels. Macro-AST is an immunoglobulin-AST complex that can present as aberrant high enzymatic activity without significant inflammation in the liver. Two patients with chronic hepatitis B presented with disproportionate AST elevations. Their plasma samples were precipitated with polyethylene glycol (PEG) and stored at 4°C for macro-AST determinations. In Case 1, PEG precipitation showed 100% removal of AST activity, and refrigerated storage resulted in a ~70% decline over seven days, confirming macro-AST. In Case 2, both tests showed minimal changes, suggesting that macro-AST was unlikely. The AST levels normalized after antiviral therapy, suggesting immune-active hepatitis as the probable cause, but the other contributing factors could not be completely excluded. The abrupt decrease in AST activity after PEG precipitation and during refrigeration storage suggests that relatively high AST values compared to ALT might be attributed to the presence of macro-AST.These non-invasive methods for detecting macroenzymes might help the patient avoid unnecessary further work-ups.

Purpose: This study aimed to identify factors associated with normal cognitive reversion and progression to dementia in older adults with mild cognitive impairment (MCI) residing in the community and to develop a nomogram. Methods: This longitudinal study used secondary data from the Korea Longitudinal Study of Ageing Panel Data (2006–2018). The study included 1,262 participants aged 60 or older, with initial Mini-Mental State Examination scores ranging from 18 to 23. Data were analyzed using the Rao-Scott chi-square test, panel binary logistic regression, and receiver operating characteristic (ROC) curve in Stata ver. 17.0 (Stata Corp.). Results: The rate of reversion from MCI to normal cognition was 37.0% after 2 years and 32.9% after 12 years. The rate of progression to dementia was 18.0% after 2 years and 30.2% after 12 years. In the nomogram for reversion to normal cognition, the most significant influences were grip strength, depression, number of meetings, age, and regular exercise, with an area under the ROC curve (AUC) of .66. In contrast, in the nomogram for progression to dementia, the most significant influences were age, grip strength, instrumental activities of daily living, number of social meetings attended, depression, and regular exercise, with an AUC of .69. Conclusion These nomograms can serve as an effective intervention tool for preventing dementia in the field of community health care since they can serve as a visual technique for presenting information on risk to individuals with MCI.

Background: Barriers to treatment with intravenous thrombolysis (IVT) for patients with acute ischemic stroke (AIS) in South Korea remain incompletely characterized. We analyze a nationwide prospective cohort to determine patient-level features associated with delayed presentation and non-treatment of potential IVT-eligible patients. Methods: We identified consecutive patients with AIS from 01/2011 to 08/2023 from a multicenter and prospective acute stroke registry in Korea. Patients were defined as IVT candidates if they presented within 4.5 hours from the last known well, had no lab evidence of coagulopathy, and had National Institute of Health Stroke Scale (NIHSS) ≥ 4. Multivariable generalized linear mixed regression models were used to investigate the associations between their characteristics and the IVT candidates or the use of IVT among the candidates. Results: Among 84,103 AIS patients, 41.0% were female, with a mean age of 69 ± 13 years and presentation NIHSS of 4 [interquartile range, 1–8]. Out of these patients, 13,757 (16.4%) were eligible for IVT, of whom 8,179 (59.5%) received IVT. Female sex (adjusted risk ratio [RR], 0.90; 95% confidence interval [CI], 0.86–0.94) and lower years of education (adjusted RR, 0.90; 95% CI, 0.84–0.97 for 0–3 years, compared to ≥ 13 years) were associated with a decreased likelihood of presenting as eligible for IVT after AIS; meanwhile, young age (adjusted RR, 1.12; 95% CI, 1.01–1.24 for ≤ 44 years, compared to 75–84 years) was associated with an increased likelihood of being an IVT candidate. Among those who were eligible for IVT, only age was significantly associated with the use of IVT (adjusted RR, 1.09; 95% CI, 1.03–1.16 for age 65–74 and adjusted RR, 0.83; 95% CI, 0.76–0.90 for ≥ 85 years, respectively). Conclusion Most patients with AIS present outside IVT eligibility in South Korea, and only 60% of eligible patients were ultimately treated. We identified increased age, female sex and lower education as key features on which to focus interventions for improving IVT utilization.

Background: Prevention of contrast-induced nephropathy (CIN) is crucial in acute myocardial infarction (AMI) patients undergoing coronary interventions. Previous studies suggest that high-dose statins may aid in CIN prevention, yet comparative studies among different statin types using cystatin C (cysC) as a biomarker for CIN are absent. This study evaluated the effectiveness of high-dose rosuvastatin versus atorvastatin in preventing cysC-based CIN (cysC-CIN) in AMI patients. Methods: This multicenter registry included 431 patients (rosuvastatin 20 mg: n = 231, atorvastatin 40 mg: n = 200). The primary endpoint was cysC-CIN incidence within 48 hours post contrast; the secondary endpoints were creatinine-based CIN (cr-CIN) incidence within 72 hours post contrast and post 30 days adverse events. Results: The incidences of cysC-CIN (12.1% vs. 7.5%, P = 0.103) and cr-CIN (6.2% vs. 3.5%, P = 0.103) were higher in the atorvastatin group without significant statistical differences.Multivariable regression analysis, which was adjusted for CIN risk factors and the variables with univariate association, showed no increased odds ratio (OR) (OR, 2.185; 95% confidence interval [CI], 0.899, 5.315; P = 0.085) for cysC-CIN in the atorvastatin group compared to the rosuvastatin group. However, statin-naïve atorvastatin subgroup had significantly increased odds of cysC-CIN compared to the rosuvastatin group (OR, 2.977; 95% CI, 1.057, 8.378; P = 0.039). At post 30 days renal, cardiovascular, and mortality event rates were both low and similar between the two groups. Conclusion No significant difference in cysC-CIN incidence was found between the highdose rosuvastatin and atorvastatin groups in AMI patients and cysC was more sensitive to the early detection of CIN than creatinine.
