Background:
The clinical features of pediatric rhabdomyolysis differ from those of the adults with rhabdomyolysis; however, multicenter studies are lacking. This study aimed to investigate the characteristics of pediatric rhabdomyolysis and reveal the risk factors for acute kidney injury (AKI) in such cases.
Methods:
This retrospective study analyzed the medical records of children and adolescents diagnosed with rhabdomyolysis at 23 hospitals in South Korea between January 2007 and December 2016.
Results:
Among 880 patients, those aged 3 to 5 years old composed the largest subgroup (19.4%), and all age subgroups were predominantly male. The incidence of AKI was 11.3%. Neurological disorders (53%) and infection (44%) were the most common underlying disorder and cause of rhabdomyolysis, respectively. The median age at diagnosis in the AKI subgroup was older than that in the non-AKI subgroup (12.2 years vs. 8.0 years). There were no significant differences in body mass index, myalgia, dark-colored urine, or the number of causal factors between the two AKI-status subgroups. The multivariate logistic regression model indicated that the following factors were independently associated with AKI: multiorgan failure, presence of an underlying disorder, strong positive urine occult blood, increased aspartate aminotransferase and uric acid levels, and reduced calcium levels.
Conclusions
Our study revealed characteristic clinical and laboratory features of rhabdomyolysis in a Korean pediatric population and highlighted the risk factors for AKI in these cases. Our findings will contribute to a greater understanding of pediatric rhabdomyolysis and may enable early intervention against rhabdomyolysis-induced AKI.

The General Rules for the Study of Primary Liver Cancer was published in June 2001 as the first edition. Since then, the 5th edition of the General Rules for the Study of Primary Liver Cancer was published by the 17th Committee of the Korean Liver Cancer Association based on the most recent data. The 5th edition of the General Rules for the Study of Primary Liver Cancer ranged over numerous topics such as anatomy, medical assessment of the patients, staging of hepatocellular carcinoma, description of the image findings, summary of hepatic resection, description of the surgical specimens, liver transplantation, reporting the pathological findings, pathological examinations of liver specimen, non-surgical treatment, radiotherapy, and assessment of tumor response after non-surgical treatment of hepatocellular carcinoma. The 5th General Rules for the Study of Primary Liver Cancer will not only become the basis of academic development for liver cancer studies in Korea, but also serve as the primary form of national liver cancer data accumulation based on standardized rules.

3.Hip Screening in Infants Presenting with Asymmetricity of Lower Extremity.

PURPOSE: This study aimed to investigate the clinical features of congenital postural deformities and lower extremity asymmetry with respect to the presence of developmental dysplasia of the hip (DDH) in infants with a discrepancy of the limb length referred for suspected DDH. MATERIALS AND METHODS: We retrospectively reviewed the medical records and radiographs of 150 infants who visited Korea University Anam Hospital Orthopedic Clinic for suspected DDH between March 2013 and March 2015. RESULTS: There were greater numbers of infants with a shorter lower extremity on the left side (n=86, 57.3%) than the right. Plagiocephaly was present in 62 infants and trunk curvature in 124 infants (82.7%). Pelvic tilting—indirectly assessed by a skewed direction of the external genitalia in female infants—was present in 62 infants (63.3%). None of the 139 infants with normal physical examination of the hip were diagnosed with DDH. Of those 11 infants with abnormal findings from the hip physical examination, a total of 6 infants were diagnosed with DDH. CONCLUSION: Regardless of the associated findings of congenital postural deformation, all infants diagnosed with DDH had abnormal findings from the physical examination of the hip joint. Thus, we conclude that the hip examination is important as the primary clinical screening in aiding the diagnosis of DDH.

4.Efficiency of Intraoperative Recurrent Laryngeal Nerve Monitoring Using Electromyography Tube in Reoperative Thyroid Surgery.

BACKGROUND AND OBJECTIVES: ObjectivesZZRecurrent laryngeal nerve (RLN) injury is a potentially debilitating complication of thyroid surgery. In re-operative thyroid surgery, the risk of vocal fold paralysis increases significantly. This study evaluated the efficiency of intraoperative RLN monitoring using an electromyography (EMG) tube in an re-operative thyroid surgery and the prediction of postoperative neural function from the relationship between the intraoperative neuromonitoring response and postoperative vocal fold function. SUBJECTS AND METHOD: Sixty-nine patients undergoing reoperative thyroid surgery were divided into two groups: 37 patients underwent reoperative thyroid surgery with intraoperative neuromonitoring (EMG group) and 32 patients underwent reoperative thyroid surgery without intraoperative neuromonitoring (no EMG group). The prevalence of transient and permanent vocal fold paralysis was evaluated in each group. In addition, the sensitivity, specificity, and negative and positive predictive values of intraoperative neuromonitoring were evaluated. RESULTS: Transient and permanent vocal fold paralysis occurred in 8.1% (3/37) and 2.7% (1/37) of the EMG group and 12.5% (4/32) and 6.3% (2/32) of the no EMG group, respectively. The negative and positive predictive values of intraoperative neuromonitoring using an EMG tube in predicting postoperative vocal fold function were 100% and 57.1%, respectively. CONCLUSION: Although the differences were not significant, intraoperative neuromonitoring using the EMG tube resulted in shorter operating time, and less transient and permanent vocal fold paralysis during reoperative thyroid surgery. Intraoperative neuromonitoring using an EMG tube for reoperative thyroid surgery may be useful for preserving the postoperative vocal fold function.

5.Comparison of Conventional Excision via a Sublabial Approach and Transnasal Marsupialization for the Treatment of Nasolabial Cysts: A Prospective Randomized Study.

OBJECTIVES: Surgical excision via a sublabial approach is considered the standard treatment for nasolabial cysts. Although transnasal marsupialization has been proposed as an alternative method, no prospective study has compared the effectiveness of these techniques. We thus compared the surgical procedure, operating time, postoperative pain, complications, and recurrence rate between the two surgical methods. METHODS: Twenty patients diagnosed with nasolabial cysts were allocated randomly into two groups according to the surgical technique. In the sublabial approach group, the cysts were excised completely using a sublabial approach, while in the transnasal marsupialization group, the cysts were marsupialized transnasally under the guidance of nasal endoscopes. The pure operating time was measured and postoperative pain was evaluated using a visual analog scale. Complications after the procedure were assessed and recurrence was determined according to the clinical symptoms and postoperative radiologic findings. RESULTS: The transnasal marsupialization group had significantly shorter operating times, less postoperative pain, lower complication rates, and shorter duration of side effects than the sublabial approach group. No recurrence occurred in either group after a 1-yr follow-up period. CONCLUSION: Although both methods are effective for treating nasolabial cysts, the transnasal marsupialization of nasolabial cysts has many benefits over the conventional sublabial approach. Therefore, we propose that transnasal marsupialization be the treatment of choice for nasolabial cysts.

6.Blood Pressure Response during the Exercise Treadmill Test and the Risk of Future Hypertension and Cardiovascular Disease.

BACKGROUND AND OBJECTIVES: Many studies had established the risk factors for cardiovascular disease. The Duke treadmill score has gained widespread acceptance for making the prognosis and diagnosis for patients with cardiac disease. Recently, the changes in blood pressure during and after exercise have also been studied to predict the prognosis of cardiac disease. We examined the relationship between the incidence of hypertension or cardiovascular disease and the changes of blood pressure during a routine exercise treadmill test. SUBJECTS AND METHODS: 256 men were screened, and they performed exercise treadmill tests from March to May, 2000. Those subjects with histories of hypertension and ischemic heart disease or who were newly diagnosed with ischemic heart disease were excluded. 109 subjects were selected for the final analysis. The follow up period was 78 months. Review of medical records and telephone interviews were used for follow up. We defined clinical events as new onset hypertension, ischemic heart disease, congestive heart failure, cerebrovascular accident, diabetes and atrial fibrillation. The peak systolic blood pressure of 182.5mmHg had the highest specificity and sensitivity on the receiver operating characteristic (ROC) curve of the systolic blood pressure for prediction of clinical events. We defined a hypertensive response as a peak systolic blood pressure over 180 mmHg. RESULTS: 43 (39.4%) of the subjects had a hypertensive response on their exercise treadmill test. The mean exercise capacity was higher in the hypertensive response group. No significant differences were found between the hypertensive and non-hypertensive response groups, in terms of age, gender, body weight, height, body mass index and resting blood pressure. 18 (41.8%) of the hypertensive response subjects had clinical events, while only 11 (16.6%) of the non-hypertensive response subjects had clinical events. The hypertensive response group had more clinical events (p=0.006). 14 (32.5%) of the hypertensive response subjects had hypertension, while only 10 (15.1%) of the non-hypertensive response group had hypertension. The hypertensive response group had more hypertension (p=0.044). On the multivariate analysis, the hypertensive response on the exercise treadmill test was an independent risk factor for hypertension and clinical events (odds ratio=3.990, 95% confidence interval; 1.473-10.808, p=0.006). CONSLUSION: These results indicate that the exercise blood pressure response seems to be a risk factor for hypertension and clinical events. Careful medical care and close follow up may be needed for subjects with a hypertensive blood pressure response on the exercise treadmill test. Further study is needed to understand the significance of an exaggerated blood pressure response on the exercise treadmill test.

7.The Expression of Thymidine Phosphorylase in Cancer-infiltrating Inflammatory Cells in Stomach Cancer.

Thymidine phosphorylase (TP) has shown to be up-regulated in several cancers and to play a role in angiogenesis and invasion. Most studies regarding TP have focused on cancer cells. Recently, evidences suggest that TP in cancer-infiltrating inflammatory cells (CIICs) also affect the cancer cell behavior. To evaluate the significance of TP expression of CIICs in gastric cancer, we assessed TP expression of cancer cells and CIICs separately using immunohistochemical assay on 116 paraffin-embedded tissue samples from stomach cancer patients and investigated their clinical significance. When subjects were divided into 4 groups according to the TP expression: cancer/matrix (+/+), C/M (+/-), C/M (-/+), and C/M (-/-), intratumoral microvessel density scores were higher in the C/M (+/-) group than in the C/M (-/-) group (p=0.02). For lymph node metastasis and survival, there were no significant differences among the 4 groups. However, there were significant differences in survival (p=0.035) and LN metastasis (p=0.023) between the two groups divided by TP expression of CIICs alone irrespective of TP expression of cancer cells. Taken together, this study suggested the TP expression in CIICs could affect lymph node metastasis and patients' survival in gastric cancer.

8.A Study for the Acoustic Characteristics of PB Word List according to Frequency.

BACKGROUND AND OBJECTIVES: The Korean phonetically balanced (PB) word list (Ham's Korean PB Word List) is currently the most commonly used word list for speech discrimination test. The aim of this study was to investigate the frequency characteristics of the Ham's Korean PB word list and to compare frequency characteristics of the Ham's Korean PB word list with that of the 21st Century Sejong words, which represent characteristics of the contemporary spoken Korean language. SUBJECTS AND METHOD: The 300 syllables most commonly used were collected from the words in 21st Century Sejong project. The 21st Century Sejong words were weighted according to the frequency of usage. The 21st Century Sejong words and words from the Ham's Korean PB word list were analyzed using an acoustic analysis program that has the MATLAB function. RESULTS: Syllables with frequencies above 2,000 Hz comprise 34% of the 21st Century Sejong words and 16% of the Ham's Korean PB word list. The Ham's Korean PB word list does not represent the high frequency characteristics of the contemporary spoken Korean language. The 21st Century Sejong words have more 'eu', 'i' and open syllables than the Ham's Korean PB word list. CONCLUSION: The Ham's Korean PB word list does not reflect the high frequency characteristics of the contemporary spoken Korean language included in the 21st Century Sejong words. Therefore, a new PB word list that incorporates a greater number of syllables with high frequency characteristics is required.

BACKGROUND: This study was to evaluate a nationwide nosocomial infection rate and antimicrobial resistance in intensive care units(ICUs) in Korea. METHODS: The study was carried out at 16 university-affiliated teaching hospitals from July through October 2004. We performed a prospective multicenter study to investigate nosocomial infection rates, device-associated infection rated, and causative pathogens and their antimicrobial resistance. RESULTS: The urinary tract was the most commonly involved site. Nosocomial infection rate was 12.48 in medical. ICU (MICU), 9.59 in medical surgical ICU (MSICU), 14.76 in surgical ICU (MSICU), and 11.60 in other lCU. Device-associated infection rates were as follow: 1) rates of urinary catheter-associated urinary tract infection were 4.26 in MICU, 3.17 in SICU, 4.88 in MSICU, and 5.87 in other ICU; 2) rates of central line-associated bloodstream infection were 3.24 in MICU, 1.56 in SlCU, 2.36 in MSICU, and 1.78 in other ICU; 3) rates of ventilator-associated pneumonia were 3.61 in MlCU, 13.05 in SICU, 1.68 in MSICU, and 4.84 in other lCU. Staphylococcus aureus was the most frequently identified microorganism in this study; 93% of S. aurues were resistant to methicillin; 17% of Pseudomonas aeruginosa isolated were resistant to imipenem; 11% of Enterococcus faecium and 18% of Enterococcus faecalis showed resistance to vancomycin. Over a half of Acinetobacter spp, Stenotrophomonas maltophilia, Klebsiella pneumoniae, and Escherichia coli showed resistant to fluoroquinolone. Conclusion: This study shows the seriousness of antimicrobial resistance and the importance of infection control in the lCU in Korea. This study should provide a theoretical strategy to enforce the infection control.

10.Clinical Analysis of Spontaneous Pneumomediastinum.

BACKGROUND: Spontaneous pneumomediastinum (SPM) is a relatively rare and benign condition that generally occurs in young adults without any precipitating factor or disease. The purpose of this study was to assess whether more uncomforting diagnostic procedures are necessary and to establish standards in the diagnosis and treatment of spontaneous pneumomediastinum. MATERIAL AND METHOD: A retrospective study was done on 18 patients from the hospitals of Hanyang University Seoul Hospital and Hanyang University Guri Hospital between February, 1997 and June, 2004. All patients had presence of mediastinal air without a pneumothorax and no evidence of trauma or barotrauma. RESULT: Among the 18 patients, the majority were male patients with only two female patients. Their mean age was 20.95 years old with standard deviation of 14.3 years. The most common complaints were chest pain, dyspnea, and coughing. Evaluation included simple chest roentgenogram in all patients, 10 patients had a chest tomographic scan, 10 patients had an esophagoscopic exam, 6 patients had a bronchofiberoscopic exam, and 3 patients had an esophagogram done. The mean hospital stay was 10.9 days. All patients were treated conservatively and in a follow-up of 1~8 years only one recurrence was found. CONCLUSION: SPM is caused by alveolar rupture in the pulmonary interstitium leading to dissection of air towards the hilum and mediastinum. Although SPM is a self-limiting condition, evaluation should include chest roentgenogram and chest tomographic scans to rule out any other secondary condition. More aggressive evaluation seems unnecessary.
