1.Diagnosis of bipolar disorder and differential diagnosis from other conditions: a narrative review

Bipolar disorder (BD) is characterized by alternating episodes of mania or hypomania and depression, resulting in substantial psychosocial impairment. Despite its clinical importance, BD is frequently underrecognized or misdiagnosed as major depressive disorder (MDD) or other psychiatric conditions. This review aims to summarize current diagnostic criteria, key clinical differentiating features, and validated assessment tools in order to enhance diagnostic accuracy and facilitate earlier recognition in clinical practice.Current concepts: The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR), and the International Classification of Diseases, 11th Revision, define mania and hypomania by abnormal mood elevation accompanied by increased activity or energy. Bipolar I disorder requires the presence of at least one manic episode, whereas bipolar II disorder requires both hypomanic and major depressive episodes. Cyclothymic disorder and broader bipolar spectrum concepts emphasize subthreshold or atypical presentations that may be overlooked by categorical diagnostic systems. Structured and self-report instruments, including the structured clinical interview for DSM-5-TR, mood disorder questionnaire, hypomania checklist-32, bipolar spectrum diagnostic scale, and Young mania rating scale, play an important role in standardized assessment and longitudinal monitoring.Discussion and conclusion: Accurate diagnosis of BD requires integration of structured diagnostic interviews, standardized rating scales, collateral information, and longitudinal clinical observation. The differential diagnosis from MDD, borderline personality disorder, attention-deficit/hyperactivity disorder, and schizophrenia depends on careful evaluation of episode duration, illness course, and associated functional impairment. Future diagnostic approaches may benefit from dimensional and personalized models incorporating biomarkers, neuroimaging, and digital phenotyping.
2.Antibacterial effect of nano-silver coating on implant abutment screw

Purpose:
. Interfacial microleakage between dental implant and its abutment has been reported as a source of bacteria which can cause peri-implantitis and persistent inflammation, and abutment screw is on the pathway of this contamination. The aim of this study was to evaluate the antibacterial effect of nano-silver coating on the abutment screw against bacteria proliferation.
Materials and methods:
. Titanium abutment screws were coated with silver in nano particle level by sputtering method. The coated screws were examined with SEM and their surface composition were analyzed with EDS. Leakage of metal ions/particle was examined with inductively coupled plasma optical emission spectrometry. To evaluate antibacterial effect, abutment screws with/without coating were contaminated with P. gingivalis, S. sanguinis, T. denticola, T. forsythia for 48 hours. Then their antibacterial effect was evaluated with real-time PCR by comparing the total genomic DNA concentration and 16S rRNA derived cDNA concentration, and effect was confirmed with fluorescent staining technique which enables segregation between live and dead cells.
Results:
. The coating was in even layer with nano sized silver particles (mean particle size 28 nm in diameter) and showed 0.53 µg leakage for 24 hours per screw. The antibacterial effect of the coated group on the tested bacteria resulted in a decrease in the Viable/Total ratio of approximately 86% for P. gingivalis, 89% for S. sanguinis, 61% for T. denticola, and 97% for T. forsythia. The results were statistically significant (P < .05). The microscopic images confirmed anti-bacterial effect of the coated abutment screw.
Conclusion
. Within the limit of this study, nano-silver coating on the abutment screw may damage the viability of P. gingivalis, S. sanguinis, T. denticola, T. forsythia. Silver coating on the titanium abutment screw can be considered as an effective method to give antibacterial effect.
3.3D Printed Patient-Specific Resorbable Bone Scaffolds for Alveolar Bone Regeneration

BACKGROUND:
Alveolar bone loss following trauma, periodontal disease, congenital anomalies, or tooth extraction poses a major challenge for oral rehabilitation, especially in implant dentistry. Traditional grafting techniques using autografts, allografts, or xenografts provide limited predictability due to issues of resorption, donor morbidity, and immune incompatibility. Advances in three-dimensional (3D) printing now enable preparation of patient-specific biodegradable scaffolds designed using 3D imaging, like cone beam computed tomography (CBCT) and computer-aided design (CAD), allowing precise replication of defect geometry and tailored biological performance.
METHODS:
This review article summarizes the most recent preclinical and clinical studies investigating biodegradable 3D printed scaffolds for alveolar bone regeneration. Studies were searched in the PubMed database, Scopus, and Google Scholar with the most relevant keywords related to 3D printed scaffolds focusing on alveolar bone regeneration.
RESULTS:
Polymers such as PLA, PCL, and PLGA offer mechanical stability and printability but require bioactive modification. Ceramics, including hydroxyapatite and tricalcium phosphate, provide osteoconductivity yet are brittle.Hydrogels such as gelatin and alginate support cellular viability but lack structural strength. Composite scaffolds integrating polymers with ceramics or bioactive agents demonstrated superior osteogenic potential. Clinical applications included alveolar ridge preservation, guided bone regeneration, cleft repair, and implant site reconstruction. Emerging strategies utilizing bioinks with stem cells and growth factors further enhanced the biological properties of patient-specific 3D printed scaffolds for clinical purposes.
CONCLUSION
3D printed patient-specific biodegradable scaffolds represent a promising alternative to conventional grafting, offering precise defect reconstruction, improved biological integration, and translational potential in maxillofacial surgery. Continued optimization of material printing combinations and vascularization strategies will be critical to achieving long-term clinical success.
4.Muscle Loss Driven by Extracellular Signal-Regulated Kinase Suppression via β-Adrenergic Activation in High-Normal Catecholamine Status

Background:
Catecholamines play a crucial role in muscle biogenesis, but their persistent elevation is linked to muscle wasting, which is poorly understood. This study aimed to investigate the association between catecholamine levels and age-related muscle loss.
Methods:
This retrospective study evaluated the plasma levels of two catecholamines, metanephrine and normetanephrine, and the clinical characteristics of 830 patients with adrenal incidentaloma on computed tomography (CT). Cross-sectional CT data at the L3 lumbar vertebrae were used to measure muscle areas. In vitro studies on C2C12 myotubes were conducted to examine β-adrenergic receptor signaling pathways and their role in myogenesis.
Results:
Men had significantly higher mean metanephrine levels of 0.17 nmol/L and normetanephrine levels of 0.63 nmol/L than women (P<0.05). Total abdominal muscle area was negatively correlated with catecholamine levels in both men and women, with the strongest negative correlation between normetanephrine levels and total abdominal muscle area in men (r=–0.31, P<0.001). Similarly, the strongest negative correlation between visceral fat area and metanephrine was observed in men (r=–0.25, P=0.004). Clenbuterol, a β-adrenergic receptor agonist, inhibited myogenesis, including myotube formation by extracellular signal-regulated kinase (ERK) suppression in C2C12 myoblasts. Conversely, β-blockers increased myogenesis via increasing ERK phosphorylation in C2C12 cells. These findings suggest that β-adrenergic modulation influences skeletal muscle differentiation, with ERK phosphorylation.
Conclusion
Catecholamine levels are associated with age, sex, muscle mass, and fat mass. Monitoring catecholamine levels, particularly in older men and in individuals with reduced muscle mass, may help manage age-related muscle loss and lead to individualized treatment strategies.

Objective:
This study aimed to develop a reliable and valid Mixed State Severity Index (MSSI) to assess mood instability in patients with mood disorders and determine cutoff scores.
Methods:
Twenty-one items were selected based on Koukopoulos’ criteria for mixed depressive episode, historically referred to as agitated depression, and Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision mixed features criteria. The MSSI was administered to 242 patients (major depressive disorder [n=92], bipolar disorder [BD] I [n=78], and BD II [n=72]) and 726 controls.
Results:
The MSSI demonstrated high internal consistency (α=0.78–0.90). Exploratory factor analysis revealed a stable four-factor structure. Based on receiver operating characteristic analysis, optimal cutoff scores were identified to distinguish mood disorder groups from controls, ranging from 19.5 to 27.5 depending on diagnosis.
Conclusion
The MSSI is a reliable and valid instrument for assessing the severity of mixed features in patients with mood disorders. The established cutoff scores enhance its clinical utility, providing robust diagnosis and treatment planning support.

Background:
Recombinant human follicle-stimulating hormone (rhFSH) is commonly used to treat female infertility, but its short half-life necessitates multiple doses. Even corifollitropin alfa, with an extended half-life, requires supplementary injections of rhFSH after 7 days. This study aimed to develop and evaluate a long-acting follicle-stimulating hormone (FSH) formulation using anti-serum albumin Fab-associated (SAFA) technology to avoid additional injections and enhance ovarian function.
Methods:
SAFA-FSH was synthesized using a Chinese hamster ovary expression system. Its biological efficacy was confirmed through assays measuring its ability to stimulate cyclic adenosine monophosphate (cAMP) production, estradiol synthesis, and the expression of human cytochrome P450 family 19 subfamily A member 1 (hCYP19α1) and human steroidogenic acute regulatory protein (hSTAR) in human ovarian granulosa (KGN) cells. To evaluate the effects of SAFA-FSH, we compared its impact on serum estradiol levels and ovarian weight increase with that of rhFSH in Sprague-Dawley (SD) rats using the modified Steelman-Pohley test.
Results:
The results indicated that SAFA-FSH induces cAMP synthesis in KGN cells and upregulates the expression of hCYP19α1 and hSTAR in a dose-dependent manner. Female SD rats, aged 21 days, receiving daily subcutaneous human chorionic gonadotropin injections for 5 days exhibited a significant increase in serum estradiol levels and ovarian weight when administered SAFA-FSH on the first day or when given nine injections of rhFSH over 5 days. Notably, the group receiving SAFA-FSH on the first and third days demonstrated an even greater rise in serum estradiol levels and ovarian weight.
Conclusion
These findings suggest that SAFA-FSH presents a promising alternative to current rhFSH treatments for female infertility. However, further research is essential to thoroughly assess its safety and efficacy in clinical contexts.

In South Korea, the Acute Stroke Quality Assessment Program (ASQAP) has contributed to improving the quality of stroke care and practice behavior in healthcare institutions. While the mortality of hemorrhagic stroke remains high, there are only a few assessment indices associated with hemorrhagic stroke. Considering the need to develop assessment indices to improve the actual quality of care in the field of acute stroke treatment, this study aims to investigate the current status of human resources and practices related to the treatment of patients with acute stroke through a nationwide survey.
Methods:
For the healthcare institutions included in the Ninth ASQAP of the Health Insurance Review and Assessment Service (HIRA), data from January 2022 to December 2022 were collected through a survey on the current status and practice of healthcare providers related to the treatment of patients with acute stroke. The questionnaire consisted of 19 items, including six items on healthcare providers involved in stroke care and 10 items on the care of patients with acute stroke.
Results:
In the treatment of patients with hemorrhagic stroke among patients with acute stroke, neurosurgeons were the most common providers. The contribution of neurosurgeons in the treatment of ischemic stroke has also been found to be equivalent to that of neurologists. However, a number of institutions were found to be devoid of healthcare providers who perform definitive treatments, such as intra-arterial thrombectomy for patients with ischemic stroke or cerebral aneurysm clipping for subarachnoid hemorrhage. The intensity of the workload of healthcare providers involved in the care of patients with acute stroke, especially those involved in definitive treatment, was also found to be quite high.
Conclusion
Currently, there are almost no assessment indices specific to hemorrhagic stroke in the ASQAP for acute stroke. Furthermore, it does not reflect the reality of the healthcare providers and practices that provide definitive treatment for acute stroke. The findings of this study suggest the need for the development of appropriate assessment indices that reflect the realities of acute stroke care.
