1.Effects of senior-friendly foods on health, nutritional status, and dietary intake among rural elderly women in Korea: a quasi-experimental study

Objectives:
We evaluated the impacts on health, nutritional status, and dietary intake of providing senior-friendly foods to community-dwelling elderly women in a rural area in Korea.
Methods:
A pretest–posttest nonequivalent control group design with repeated measures was conducted among 71 rural-dwelling elderly women. Changes in health indicators, nutritional status, and dietary intake were assessed at three time points: baseline, post-intervention, and two months after intervention.
Results:
Immediately after a three month intervention, significant differences were observed between the intervention and control groups in frailty score, Dysphagia Handicap Index, Mini Nutritional Assessment, social isolation, resilience, quality of life, and depression (P < 0.05). Significant group-by-time interaction effects were found for muscle mass, hemoglobin A1c, and energy, protein, and micronutrient intake, all of which showed significant improvements in the intervention group (P < 0.05).
Conclusion
Providing senior-friendly foods effectively improved physical and physiological health and emotional well-being among rural older adults. This intervention also contributed broadly to improved dietary intake. These findings provide empirical evidence to support the development of community-based integrated care models and tailored nutrition intervention programs for rural elderly populations in Korea.Trial Registration: Clinical Research Information Service Identifier: KCT0011666.
2.Artificial intelligence-based personalized oncological outcome prediction model for upper urinary tract urothelial carcinoma after radical nephroureterectomy: A development and multicenter validation

Purpose:
To develop and validate an artificial intelligence (AI)-based personalized outcome prediction model for upper-urinary tract urothelial carcinoma patients undergoing radical nephroureterectomy.
Materials and Methods:
Data from patients who underwent radical nephroureterectomy between 2010 and 2020 across three hospitals were retrospectively analyzed. A model was developed using one tertiary center’s data and externally validated with data from two other hospitals. An AI model using XGBoost as risk estimator and bootstrapped Weibull Accelerated Failure Time model for 10-year survival probability was employed. Hyperparameter tuning used Optuna method. Model efficacy was assessed using concordance index, average Brier score, D-calibration, and six-month interval time-dependent area under the curve (AUC).
Results:
Of 1,039 patients, 627 qualified after excluding 50 with neoadjuvant chemotherapy. Model development used 564 patients (507 training, 57 test) with 9:1 stratified random split, plus 63 for internal validation and 362 for external validation. Significant parameters included preoperative glomerular filtration rate (p<0.001), hydroureteronephrosis (p=0.013), pathological N stage (p<0.001), concurrent carcinoma in situ (p<0.001), disease progression (p<0.001), and survival rate (p<0.001). Disease-free survival (DFS) model’s concordance index: internal validation 0.789, external validations 0.734 and 0.771. Overall survival (OS) model’s concordance index: internal validation 0.819, external validations 0.780 and 0.771. Mean time-dependent AUC was 0.66–0.77 for DFS and 0.67–0.80 for OS during 10-year periods.
Conclusions
AI-based model effectively predicts disease-free and OS outcomes for upper-urinary tract urothelial carcinoma patients with post-radical nephroureterectomy, showcasing robust performance across multicenter settings.
3.Current status of hepatitis C treatment and its barriers in Jeonbuk, Republic of Korea

Objectives:
In alignment with the World Health Organization’s goal of eliminating hepatitis C, this study assessed the current treatment status and reasons for non-treatment among patients with hepatitis C in Jeonbuk State, Republic of Korea, to inform strategies for improving care engagement.
Methods:
Among 311 individuals diagnosed with hepatitis C and reported through the NationalNotifiable Infectious Disease Surveillance system between January 2023 and June 2024, 208 patients were surveyed after excluding those who had died or could not be contacted.Statistical analyses included the chi-square test, the Cochran-Armitage test for trend, and logistic regression.
Results:
Overall, 116 participants (55.8%) reported having received antiviral therapy. Among the 92 untreated individuals, the most common reason for non-treatment was the absence of symptoms (n = 23; 25.0%), followed by the burden of drug costs (n = 21; 22.8%).
Conclusion
These findings highlight suboptimal treatment uptake and key barriers that may hinder progress toward hepatitis C elimination. Expanding screening and strengthening linkage-to-care strategies, while addressing financial barriers, will be essential to achievingnational elimination targets.
4.Diagnostic performance evaluation of the PowerChek Respiratory Virus Panels 1, 2, 3, and 4 for the detection of respiratory viruses, including SARS-CoV-2

Background:
The accurate laboratory-based detection of respiratory viruses, including severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is essential for effective patient management and infection control in the post–coronavirus disease 2019 era. In this study, the performance of the PowerChek Respiratory Virus Panels 1–4 assay (Kogene Biotech) in detecting non-SARS-CoV-2 respiratory viruses and SARS-CoV-2 was compared with those of two established commercial assays.
Methods:
Residual clinical respiratory specimens collected between December 2023 and February 2024 were retested using the PowerChek assay. Of the 129 specimens retested, 80 had tested positive for 14 non-SARS-CoV-2 respiratory virus targets in the Allplex Respiratory Panels 1–3 assay (Seegene) and 49 had been tested using the Alinity m SARS-CoV-2 assay (Abbott; 20 positive and 29 negative for SARS-CoV-2). Agreement, Cohen’s kappa, and discordant results were assessed.
Results:
For the 14 non-SARS-CoV-2 respiratory virus targets, the virus-specific overall percent agreement (OPA) ranged from 95.0% to 100%, the positive percent agreement (PPA) ranged from 66.7% to 100%, and the negative percent agreement (NPA) ranged from 96.0% to 100%, with kappa values of 0.64–1.00. Lower agreements were observed for human coronavirus OC43 and influenza A virus. For SARS-CoV-2 detection, the OPA was 98.0% (PPA, 95.0%; NPA, 100%; kappa, 0.96).
Conclusion
The detection performance of the PowerChek assay was comparable to those of the established assays for most respiratory virus targets, with lower agreement observed for some targets. These features support its practical utility for the routine multiplex molecular detection of respiratory viruses, including SARS-CoV-2.

6.Acute health effects of accidental exposure to lithium hydroxide at a battery material production plant

Background:
Lithium hydroxide (LiOH) is increasingly used in electric vehicle battery production; however, its health effects in the work environment remain underreported. This study characterizes the acute health effects on workers following accidental occupational exposure to LiOH at a Korean lithium plant in March 2024.
Methods:
We analyzed health effects from two LiOH exposure accidents on March 6 and 9, 2024, involving 50–100 kg powder spills. Two datasets were examined: acute symptoms from 115 workers who visited hospitals immediately after exposure, and a symptom severity survey from 474 workers conducted 2 weeks post-exposure. Workers were stratified by distance from the leak source (<10 m, 10–20 m, >20 m) and respirator use. Univariate general linear modeling was applied to analyze the relationship between symptom occurrence and both the distance from the exposure source and respirator use.
Results:
Among workers visiting hospitals immediately, local irritation symptoms predominated: sore throat (58.3%), cough (28.7%), and skin dermatoses (17.4%). Systemic symptoms included headache (45.2%), nausea (18.3%), chest tightness (12.2%), and dizziness (9.6%). Two-week follow-up revealed overall symptom improvement; severe cough decreased from 22.2% to 10.1%. However, despite general improvement, a significant portion of workers reported persistent respiratory issues, including cough (28.7%) and sputum production (31.0%). Symptom severity showed a significant dose-response relationship with proximity to the source (p < 0.001). While respirator use offered initial protection (p = 0.021), this effect was not statistically significant after 2 weeks.
Conclusions
Occupational LiOH exposure caused acute irritation and systemic symptoms, demonstrating immediate tissue damage consistent with its alkalinity and systemic toxicity upon absorption. As lithium battery production expands globally, these results emphasize the necessity for developing specific occupational exposure limits and medical surveillance guidelines for lithium compounds.
7.Work-life balance and effort-reward imbalance, and their interaction, associated with presenteeism among Korean wage workers: Based on 6th Korean working conditions survey

Background:
This study investigated the relationship between work-life balance (WLB), effort-reward imbalance (ERI), and presenteeism among Korean wageworkers through two models used simultaneously.
Methods:
Data from the 6th Korean Working Conditions Survey for 28,669 Korean workers, including 13,513 men and 15,156 women, were analyzed. All analyses were performed with pre-designed weight. Multiple logistic regression analysis was used to analyze the association between WLB, ERI, and presenteeism among Korean workers. The participants were classified into the following four groups: group 1, status of both WLB and ERI is “balanced”; group 2, WLB and ERI status are “imbalanced” and “balanced”, respectively; group 3, WLB and ERI status are “balanced” and “imbalanced”, respectively; group 4: status of both WLB and ERI is “imbalanced.”
Results:
We found that WLB and ERI were associated with presenteeism in both men and women. Depending on their WLB and ERI status, women generally had a higher tendency of presenteeism than men. Multiple logistic regression shows that, in most models and groups, there is an increased odds ratio (OR) for presenteeism in both men and women compared to the reference value. Moreover, the OR in both men and women gradually increased in groups 2, 3, and 4 compared with group 1. When considering both WLB and ERI status simultaneously, ERI had a greater impact on presenteeism than WLB. Furthermore, it was found that a synergistic effect of presenteeism manifests when both WLB and ERI are in a state of imbalance simultaneously.
Conclusions
Using the two models simultaneously, we found an association between WLB, ERI, and presenteeism according to sex with a synergistic effect among Korean workers. Our research suggests that active interventions targeting WLB and ERI are necessary to reduce presenteeism, which ultimately leads to decreased productivity.
8.Clinical Outcomes of Trimalleolar Ankle Fractures in Patients Aged 65 Years and Older

Purpose:
To evaluate clinical and radiographic outcomes after the surgical treatment of trimalleolar ankle fractures in patients aged ≥65 years and to identify the prognostic factors for poor clinical outcomes.
Materials and Methods:
Sixty-three patients aged ≥65 years who underwent surgery for trimalleolar ankle fractures (2013~2022) were reviewed retrospectively. Pathologic fractures, open fractures, and follow-up <6 months were excluded. The candidate prognostic variables included the demographics, fracture classification (Lauge–Hansen and Danis–Weber), bone mineral density, body mass index, comorbidities, associated injuries, posterior malleolar fixation status, and syndesmotic injury. The reduction quality was graded using the Burwell–Charnley criteria. The outcomes were the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle–Hindfoot Score and final ambulation status. The continuous variables were compared using Student’s t-test or the Mann–Whitney U-test, and the categorical variables were analyzed using the chi-square test or Fisher’s exact test, where appropriate. Multivariable logistic regression was performed to identify the independent prognostic factors for poor clinical outcomes.
Results:
The mean age was 73.4 years, and the mean follow-up was 14.8 months; 39 patients were male, and 24 were female. Radiographic reduction was anatomic in 81.0% and fair in 19.0%. The mean time to union was 5 months, and the mean AOFAS score was 82.4. Final ambulation, limping gait, cane use, and wheelchair use were normal in 23.8%, 47.6%, 22.2%, and 6.3%, respectively. Infection and delayed union occurred in 6.3% and 9.5%, respectively, with no cases of nonunion or peri-implant fracture. Multivariable analysis showed that comorbidities and associated injuries were independently associated with poor clinical outcomes.
Conclusion
Surgical treatment generally achieved satisfactory radiographic reduction and union in patients aged ≥65 years, but functional recovery was variable, and many patients required walking aids. Comorbidities and concomitant injuries independently predicted poor outcomes, underscoring the importance of careful perioperative risk assessment and multidisciplinary postoperative management in this population.
9.Associations of fall experiences with cognitive function and activities of daily living disability among older adults: A cross-sectional study

This study examined the associations of fall experiences with cognitive function and activities of daily living (ADL) disability among older adults. Methods: Using data from the 2023 National Survey of Older Koreans, a total of 9,816 individuals aged 65 years or older were analyzed. Independent samples t-tests, ANOVA, chi-square tests, linear regression, and logistic regression analyses were conducted. Additionally, subgroup regression analyses were conducted to identify socially vulnerable groups according to age group, education level, and caregiving expenses. Results: The analysis showed that cognitive function tended to be lower in individuals with fall experience (β=-0.44, p=.026), and the odds of ADL disability were higher among individuals with fall experience (odds ratio [OR]=2.04, 95% confidence interval [95% CI]=1.63~2.54). In addition, subgroup analyses showed that cognitive function was lower among individuals with fall experience in the ≥85 years group (β=- 1.30, p=.020) and among those with education at elementary school or below (β=-0.88, p=.001). The odds of ADL disability among those with fall experience were higher among those aged 65~74 years (vs. 75~84; OR=2.80, 95% CI=1.99~3.94) and ≥85 years (vs. 75~84; OR=2.59, 95% CI=1.38~4.84), those with higher education (vs. lower; OR=4.95, 95% CI=1.19~20.60), and those with no caregiving expenses (vs. any; OR=2.06, 95% CI=1.63~2.60). Conclusion: These findings provide important foundational data for both policy development and academic research. They highlight the need for a multifaceted approach to fall prevention and underscore its importance in enhancing the quality of life for the older adults.
10.An Analysis of Judicial Precedents on Medical Practice in Criminal Litigation in Obstetrics and Gynecology

This study analyzed 22 criminal cases involving obstetricians and gynecologists to identify legal trends and propose measures to reduce the legal burden on medical professionals while maintaining stable healthcare services. The cases were retrieved from the Supreme Court of Korea Judicial Information Disclosure Portal using keywords such as “obstetrics,” “gynecology,” “expectant mother,” “fetus,” “neonate,” “delivery,” “uterus,” and “placenta.” The cases were classified into two categories:medical malpractice (16 cases, 72.7%) and abortion and bioethics (6 cases, 27.3%). Guilty verdicts were issued in 8 cases (36.4%), whereas 14 cases (63.6%) resulted in acquittal. The qualitative analysis showed that courts consistently protected physicians’ clinical discretion in unpredictable and unavoidable situations, such as amniotic fluid embolism and uterine atony, provided that standard medical protocols were followed. Procedural appropriateness, rather than the perfection of clinical outcomes, appeared to be the primary basis for judicial protection. Although the judiciary acknowledges the inherent limitations of medical practice and tends to protect physicians when established guidelines are followed, the high frequency of criminal prosecution remains a substantial burden on healthcare providers and may threaten the sustainability of obstetric care.
