1.Intraoperative Visual Evoked Potential Monitoring in Endoscopic Endonasal Surgery for Nonpituitary Adenoma Suprasellar Tumors

Background:
and Purpose Intraoperative visual evoked potential (VEP) monitoring has been studied mainly in pituitary adenoma, while its role in nonpituitary suprasellar tumors has remained unclear. This study evaluated the predictive usefulness of intraoperative VEP monitoring during endoscopic endonasal surgery (EES) and aimed to identify optimal alarm criteria for visual outcomes.
Methods:
We retrospectively analyzed a cohort of 87 patients who underwent EES with intraoperative VEP monitoring between April 2021 and September 2023. Visual outcomes were evaluated preoperatively and at short-term (≤3 months) and long-term (12 months) followups, with visual deterioration at these time points defined as worsening of either visual acuity or the visual field. Reductions in the VEP amplitude were quantified using both the maximum intraoperative decrease and the final amplitude after recovery. Receiver operating characteristic (ROC) curve analyses were performed to identify the optimal alarm thresholds, and the sensitivity, specificity, positive predictive value, and negative predictive value were calculated for short-term and long-term visual deteriorations.
Results:
Short-term and long-term visual deteriorations were detected in 12 (9.2%) and 5 (3.8%) of the 130 analyzed eyes, respectively. ROC curve analyses identified ≥40% and ≥30% reductions in the N75–P100 amplitude as optimal alarm criteria for short-term and long-term visual deteriorations, respectively. A 30% reduction without intraoperative recovery demonstrated markedly higher sensitivity than the conventional 50% alarm threshold for short-term (58.3% vs. 33.3%) and long-term (80.0% vs. 20.0%) outcomes, while maintaining acceptable specificity (82.2% and 80.8%, respectively).
Conclusions
A 30% reduction in amplitude represents a more-sensitive and clinically relevant alarm threshold than a 50% reduction for intraoperative VEP monitoring during EES for nonpituitary suprasellar tumors. Incorporating both the magnitude and recovery pattern of VEP amplitude changes may improve the accuracy of predictions of long-term visual deterioration. However, the potential for false positives warrants cautious interpretation, and further studies are needed to validate the impact of intraoperative VEP monitoring on visual outcomes.
2.Relationship of Sleep Quality and Cognitive Function in Patients with Mild Cognitive Impairment and Subjective Memory Impairment

Objectives:
To investigate the relationship between subjective sleep quality and cognitive function in patients with subjective memory impairment (SMI), a self-perceived cognitive decline without objective cognitive impairment, and amnestic mild cognitive impairment (aMCI).
Methods:
We enrolled 246 patients with memory impairment (126 with SMI and 120 with aMCI) who fulfilled the Korean version of the Pittsburgh Sleep Quality Index (PSQI-K), a standardized battery of neuropsychological tests, and mood questionnaires. Based on the PSQI-K cutoff point of 5, patients were classified as good sleepers (GS) or poor sleepers (PS).
Results:
There was no difference in the proportion of GS and PS between patients with SMI and aMCI [68 PS (54.0%) in SMI vs. 62 PS (51.7%) in aMCI, p>0.05]. Demographics did not differ between the SMI and aMCI groups. In both the SMI and aMCI groups, PS had worse sleep-wake parameters, such as sleep latency, total sleep time, and sleep efficiency, than GS and reported worse performance in all PSQI-K subcomponents. Neuropsychological data were not different between GS and PS, except for the Stroop word test in patients with aMCI. Depressive scores were worse in PS than in GS in both the SMI and aMCI groups.
Conclusions
We observed that cognitive function was not significantly different between GS and PS in both the SMI and aMCI groups, except in the Stroop word test in the aMCI group, while PS had more depressive mood than GS in both groups. This suggests that subjective sleep quality may depend on mood disturbances in patients with mild cognitive impairment.

4.Cognition, Olfaction and Uric Acid in Early de novo Parkinson's Disease

OBJECTIVE: Cognitive impairment is one of the nonmotor symptoms in Parkinson's disease (PD), and olfactory dysfunction is used as a marker to detect premotor stages of PD. Serum uric acid (sUA) levels have been found to be a risk factor for PD. Our objective in this study was to examine whether sUA levels are associated with cognitive changes and olfactory dysfunction in early de novo PD patients. METHODS: The study participants included 196 de novo PD patients. We assessed cognitive function by the Korean versions of the Mini-Mental State Examination and the Montreal Cognitive Assessment and assessed olfactory function by the Korean version of the Sniffin' Sticks test. RESULTS: The mean sUA level was 4.7 mg/dL and was significantly lower in women than in men. Cognitive scores were lower in women, suggesting that sUA levels were related to cognitive function. The olfactory functions were not related to sUA level but were clearly associated with cognitive scores. Olfactory threshold, odor discrimination, and odor identification were all significantly related to cognitive scores. CONCLUSIONS: We conclude that lower sUA levels were associated with cognitive impairment, not olfactory dysfunction, in de novo PD patients. This finding suggests that UA is neuroprotective as an antioxidant in the cognitive function of PD patients.

5.Sonographic Follow up Finding of Internal Jugular Vein Thrombosis Caused by Lymphoma.

Internal jugular vein (IJV) thrombosis is a form of deep vein thrombosis that has a potentially fatal outcome associated with its intracranial propagation. Computed tomography and ultrasonography are useful diagnostic tools. We report a case of IJV thrombosis associated with lymphoma that was detected by carotid ultrasonography and had disappeared in follow up ultrasonography.
