1.Application and prospects of mobile health applications in the health management of organ transplant recipients

With the rapid development of mobile internet technology, mobile health application (mHealth APP) are increasingly widely used in the field of health management and have been proven to play an important role in the management of chronic diseases. Solid organ transplant recipients face complex health management needs after surgery, including postoperative follow-up, medication management, prevention and treatment of complications and comorbidities, and lifestyle adjustment. mHealth APP can provide solid organ transplant recipients with convenient self-management tools. Although some progress has been made in this field, there are still many challenges, such as insufficient user experience, technological dependence, and data security risks. Therefore, this article discusses the development process, main functions and current application status of mHealth APP, and analyzes its advantages in improving the self-management ability of solid organ transplant recipients, promoting doctor-patient communication and reducing the incidence of complications. At the same time, based on the practical experience of author’s team in developing the “TransMate” mHealth APP, we propose the directions that mHealth APPs should focus on in the future, in order to provide more effective support and services for the health management of solid organ transplant recipients.

A 20-year-old male patient presented to the Department of Dermatology of Peking Union Medical College Hospital with complaints of an 8-year history of facial scarring, swelling of the lower limbs, and a 4-year history of scalp thickening. Physical examination showed thickening furrowing wrinkling of the skin on the face and behind the ears, ciliary body hirsutism, blepharoptosis, and cutis verticis gyrate. Both lower limbs were swollen, especially the knees and ankles. The skin of the palms and soles of the feet was keratinized and thickened. Objective To evaluate platelet aggregation after 14 days of treatment with ticagrelor alone or aspirin plus ticagrelor in patients with stable coronary artery disease.Methods Patients with stable coronary artery disease receiving dual antiplatelet therapy with aspirin 100 mg/d plus clopidogrel 75 mg/d were randomly assigned to ticagrelor monotherapy group or ticagrelor plus aspirin group after washout of ticagrelor monotherapy.Platelet aggregation rate was measured after 7 days and 14 days.Results 57 patients(32 in Ticagrelor monotherapy group and 25 in Ticagrelor+Aspirin group)completed the follow-up.The mean age was(61.16±7.87)years.The platelet aggregation rates induced by arachidonic acid(AA),adenosine diphosphate(ADP),and collagen-induced were similar at baseline and randomization in both groups.After 14 days of treatment,the ADP-induced platelet aggregation rates in both groups were significantly lower than the baseline,but there was no statistical difference between the two groups[(21.50±7.86)%vs.(21.92±10.21)%,P=0.864].The ADP-induced platelet aggregation rates in both groups decreased compared to randomization,but the Ticagrelor monotherapy group was significantly higher than the Ticagrelor+Aspirin group[(48.22±24.01)%vs.(8.67±5.96)%,P<0.001].In the monotherapy group,5 patients had a significant increase in the AA-induced platelet aggregation after the discontinuation of aspirin.After 14 days of continued Ticagrelor monotherapy,the AA-induced platelet aggregation rate decreased significantly again(<30%).Conclusions Ticagrelor monotherapy exerts a strong inhibitory effect on ADP-induced platelet aggregation and a mild inhibitory effect on the AA-induced platelet aggregation.In addition to its strong inhibitory effect on ADP-induced platelet aggregation,the mild inhibitory effect of Ticagrelor on the AA-induced pathway may have some clinical significance for the early de-escalation of dual antiplatelet therapy to monotherapy in patients.

5.Effect of electroacupuncture combined with intradermal needling on simple obesity and serum intestinal lymphatic function-related factors.

OBJECTIVE:
To assess the efficacy of the combined treatment with electroacupuncture (EA) and intradermal needling on simple obesity and explore its underlying effect mechanism.
METHODS:
A total number of 116 patients with simple obesity were randomized into an observation group (58 cases, 3 cases dropped off and 2 cases removed) and a control group (58 cases, 4 cases dropped off and 1 cases removed). Patients in the control group received EA at Zhongwan (CV 12), Quchi (LI 11), Zusanli (ST 36), Pishu (BL 20), Weishu (BL 21), etc., for 30 min each time. On the base of the intervention as the control group, the patients in the observation group received the intradermal needling at Tianshu (ST 25), Daheng (SP 15), Zusanli (ST 36), Shangjuxu (ST 37), Quchi (LI 11), Pishu (BL 20) and Weishu (BL 21). In each group, the intervention was given once every two days, 3 times a week, consecutively for 3 months. Before and after treatment, the obesity indexes (body mass [BW], body mass index [BMI], body fat percentage [F%], adiposity [A] and waist circumference [WC]), the serum intestinal lymphatic function-related factors (vascular endothelial growth factor C [VEGF-C], delta-like ligand 4 [DLL4], adrenomedullin [ADM]), blood lipid (total cholesterol [TC], triglyceride [TG] and low density lipoprotein-cholesterol [LDL-C]), and fasting plasma glucose (FPG), fasting insulin (FINS) and insulin resistance index (HOMA-IR) were observed in the patients of both groups; and the efficacy was assessed.
RESULTS:
The effective rate was 88.7% (47/53) in the observation group, higher than 71.7% (38/53) in the control group (P<0.05). After treatment, except FPG in the control group, BW, BMI, F%, A, WC, and the concentrations of serum VEGF-C, DLL4 and ADM, as well as TC, TG, LDL-C, FBG, FINS and HOMA-IR were all reduced compared with those before treatment in both groups (P<0.05). The reduction ranges of BW, BMI, F%, A, WC, and the concentrations of serum VEGF-C, DLL4 and ADM, and TC, LDL-C, FINS and HOMA-IR in the observation group were all larger than those in the control group (P<0.05).
CONCLUSION
Electroacupuncture combined with intradermal needling can reduce body weight and lipid, and improve insulin resistance in treatment of simple obesity, which is achieved probably through inhibiting lymphangiogenesis and promoting lymphatic endothelial permeability.

In the nucleus, chromatin is folded into hierarchical architecture that is tightly linked to various nuclear functions. However, the underlying molecular mechanisms that confer these archi-tectures remain incompletely understood. Here, we investigated the functional roles of H3 lysine 9 dimethylation (H3K9me2), one of the abundant histone modifications, in three-dimensional (3D) genome organization. Unlike in mouse embryonic stem cells, inhibition of methyltransferases G9a and GLP in differentiated cells eliminated H3K9me2 predominantly at A-type (active) genomic compartments, and the level of residual H3K9me2 modifications was strongly associated with B-type (inactive) genomic compartments. Furthermore, chemical inhibition of G9a/GLP in mouse hepatocytes led to decreased chromatin-nuclear lamina interactions mainly at G9a/GLP-sensitive regions, increased degree of genomic compartmentalization, and up-regulation of hundreds of genes that were associated with alterations of the 3D chromatin. Collectively, our data demonstrated essential roles of H3K9me2 in 3D genome organization.
8.Effect of Nd:YAG laser treatment for early stage capsular block syndrome

?AIM:To evaluate the clinical outcomes of Nd:YAG laser capsulotomy in the treatment of early stage capsular block syndrome ( CBS) .?METHODS:Eighteen patients (21 eyes) with early stage capsular block syndrome were treated using Nd:YAG laser by anterior capsulotomy only or combined with posterior capsulotomy from January 2010 to July 2015 in Anyang Eye Hospital. Uncorrected distance visual acuity, intraocular pressure, spherical equivalent, depth of anterior chamber were observed preoperatively and 2wk postoperatively.?RESULTS:Seventeen eyes simply underwent peripheral anterior capsulotomy with Nd:YAG laser. Four eyes were combined with posterior capsulotomy. Compared with preoperative, uncorrected distance visual acuity improved, intraocular pressure returned to normal, degree of myopia reduced, depth of anterior chamber had deepened.? CONCLUSION: Nd: YAG laser capsulotomy is an effective treatment for early stage capsular block syndrome.
10.The Alberta score combined with Infant Language Assessment Scale used in rehabilitation for children with cerebral palsy

Objective To observe the Alberta combined with Infant Language Assessment Scale used in rehabilitation for children with cerebral palsy.Methods From Jun.2012 to Jun.2013,64 cases of cerebral palsy in Children's Hospital Affiliated to Suzhou University were selected according to the different interventions and were randomly divided into an observation group and a control group,32 patients in each group.The control group underwent conventional rehabilitation training included exercise therapy (mainly Bobath therapy,Ueda therapy) and application Infants Language Assessment Scale for treatment,and the observation group on the basis of the method above,set the action in accordance with Alberta infant motor scale(AIMS) assessment to develop rehabilitation programs.Adhere to 3 hours a day of repeated intensive training.Efficacy in children after treatment was compared,and forceful move and fine motor movements and the changes in development quotient(DQ) before and after treatment were compared between 2 groups.Results The total efficiency of the observation group and the control group was 90.6% and 71.9%,respectively,and there was statistically significant differences between 2 groups (x2 =6.317,P < 0.05).After treatment,the DQ of big movement and fine motor in observation group (47.92 ± 7.15,42.55 ± 8.1 3) were significantly higher than before treatment (36.18 ± 8.23,33.71 ± 10.16) and the control group (38.13 ± 8.21,36.58 ± 8.06),the differences were statistically significant(t =6.235,5.452,6.137,5.243,all P < 0.05).Conclusions The combination of the rehabilitation for children with cerebral palsy in infants language Alberta combined score rating scale,help to improve rehabilitation results and motor function in children,which is of recommendation and application.
