1.Improvement effect of epicatechin on liver injury in mice induced by acetaminophen and its mechanism

Objective:To discuss the improvement effect of epicatechin(EC)on acetaminophen(APAP)-induced liver injury in the mice,and to clarify its possible mechanism.Methods:A total of 60 C57BL/6J mice were randomly divided into blank control group,APAP model group,low dose of EC group(10 mg·kg-1),middle dose of EC group(20 mg·kg-1)and high dose of EC group(40 mg·kg?1),with 12 mice in each group.Except for blank control group,the mice in the other groups were given intraperitoneal injection of APAP(200 mg·kg?1)to establish the liver injury models.At 1 h before APAP injection,the mice in low,middle and high doses of EC groups were intraperitoneally injected with 10,20 and 40 mg·kg-1 EC,respectively.A total of 36 nuclear factor E2-related factor 2(Nrf2)deficient mice(Nrf2-/-mice)were randomly divided into control group,APAP group and APAP+EC group,with 12 mice in each group.After modeling 24 h,the mice were sacrificed,and the blood and liver tissue of the mice were collected for subsequent detection.HE staining was used to observe the pathomorphology of the liver tissue in the mice in various groups;kit assay was used to detect the activities of aspartate aminotransferase(AST)and alanine aminotransferase(ALT)in serum of the mice in various groups and the myeloperoxidase(MPO)activity and the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),malondialdehyde(MDA),adenosine triphosphate(ATP),glutathione(GSH)and ferrous ion(Fe2?)in liver tissue of the mice in various groups;Western blotting method was used to detect the expression levels of nuclear factor-κB(NF-κB)and Nrf2 signaling pathway-related proteins in liver tissue of the mice in various groups.Results:The HE staining results showed that compared with APAP model group,the APAP-induced liver pathology injury in the mice in different doses of EC groups was significantly improved.Compared with blank control group,the levels of ALT and AST in serum of the mice in APAP model group were significantly increased(P<0.01).Compared with APAP model group,the activities of ALT and AST in serum of the mice in low,middle and high doses of EC groups were significantly decreased(P<0.05 or P<0.01).Compared with blank control group,the MPO activity and the levels of TNF-α and IL-1β in liver tissue of the mice in APAP model group were significantly increased(P<0.01).Compared with APAP model group,the MPO activity and the levels of TNF-α and IL-1β in liver tissue of the mice in low,middle and high doses of EC groups were decreased(P<0.05 or P<0.01).Compared with blank control group,the levels of MDA and Fe2? in liver tissue of the mice in APAP model group were significantly increased(P<0.05),and the levels of ATP and GSH were significantly decreased(P<0.05).Compared with APAP model group,the levels of MDA and Fe2? in liver tissue of the mice in low,middle and high doses of EC groups were significantly increased(P<0.05 or P<0.01),and the levels of ATP and GSH were significantly increased(P<0.01).Compared with blank control group,the expression levels of amino acid exchange transporter(xCT)and glutathione peroxidase 4(GPX4)proteins in liver tissue of the mice in APAP model group were significantly decreased(P<0.05);compared with APAP model group,the expression levels of xCT and GPX4 proteins in liver tissue of the mice in low,middle and high doses of EC groups were significantly increased(P<0.01).Compared with blank control group,the expression levels of nuclear factor-κB(NF-κB)p-p65 and phosphorylated NF-κB inhibitor α(p-IκBα)proteins in liver tissue of the mice in APAP model group were significantly increased(P<0.05);compared with APAP model group,the expression levels of NF-κB p-p65 and p-IκBα proteins in liver tissue of the mice in low,middle and high doses of EC groups were significantly decreased(P<0.01).Compared with blank control group,the expression levels of Nrf2 and heme oxygenase-1(HO-1)proteins in liver tissue of the mice in APAP model group were significantly increased(P<0.05);compared with APAP model group,the expression levels of Nrf2 and HO-1 proteins in liver tissue of the mice in low,middle and high doses of EC groups were significantly increased(P<0.01).Compared with control group,the ALT level in serum and the levels of MDA and Fe2+in liver tissue of the Nrf2-/-mice in APAP group were significantly increased(P<0.01),and the levels of ATP and GSH in liver tissue were significantly decreased(P<0.01).Conclusion:EC can improve APAP-induced liver injury in the mice,and its mechanism may be related to the inhibition of ferroptosis by activating the Nrf2/GPX4 signaling pathway.
2.Summary of Huang Shuling's Experience in Treating Diabetic Foot Ulcers

Diabetic foot ulcers(DFU)are the primary manifestations of diabetic foot,and the diabetic foot is one of the complications of diabetes mellitus.DFU present the characteristics of high morbidity,high disability rate and high recurrence rate.Clinically,DFU can be classified into the category of finger-toe gangrene in traditional Chinese medicine(TCM).In the view of Professor Huang Shuling,DFU develop from diabetes after a long time,and are caused by insufficiency of kidney yin together with phlegm and blood stasis blocking collaterals,which results in the internal retention of damp-heat and toxins under the conditions of insufficiency of healthy qi and attack of external pathogens.According to the progression of DFU,DFU can be differentiated as five syndromes,namely heat-toxin and blood stasis accumulation syndrome,heat retention impairing yin syndrome,spleen deficiency and phlegm-stasis syndrome,qi and blood deficiency complicated by blood-stasis obstructing collaterals syndrome,and liver-kidney yin deficiency complicated by blood-stasis obstruction syndrome.Following the therapeutic principle of supporting healthy qi to eliminate pathogens,Professor Huang formulated the therapies for DFU,i.e.,clearing heat and removing toxins and activating blood,clearing heat and nourishing yin and activating blood,invigorating spleen and resolving phlegm and unblocking collaterals,tonifying qi-blood and removing blood stasis,and nourishing liver-kidney and unblocking blood stasis.Moreover,three-level prevention and treatment strategies for DFU were put forward by Professor Huang.The strategies include early diagnosis and early treatment of high-risk diabetic foot,delaying the progression of peripheral neuropathy and vascular disease of diabetes mellitus and treating tinea pedis positively,and effectively treating DFU to reduce amputation rate.The preventive and therapeutic strategies for DFU cover the therapies of integrated Chinese and western medicine,and therapies of simultaneous internal and external treatment.In treating DFU with TCM therapy,Professor Huang addressed the importance of combined use of Zizhuye Lotion for wet compressing on the wound continuously based on oral administration of Chinese medicine,which achieves a stronger therapeutic effect.

Cardiac remodeling in microgravity exerts effects on astronauts'health and space mission efficiency.This paper analyzes the etiology,pathogenesis,prevention,and treatment of cardiac remodeling in microgravity based on the Trinity View of"Physique-Qi-Spirit"of traditional Chinese medicine.The microgravity environment disrupts the heaven-earth qi interaction law described in"Suwen Yin Yang Ying Xiang Da Lun":"Earth qi ascends to form clouds through heavenly evaporation,while heaven qi descends as rain".This disruption causes upward reversal of human qi and cephalad fluid shift,consequently influencing the dynamic balance of the physique-qi-spirit system and inducing cardiac remodeling.The core pathogenesis initiates with qi upward reversal at the initial stage,advances through spiritual disturbance during progression,and culminates in physical impairment at the terminal stage.We therefore propose a holistic"physique-qi-spirit"co-regulation strategy implemented via three therapeutic principles:regulating qi,stabilizing spirit,and restoring physique.This approach aims to restore the dynamic balance of the"physique-qi-spirit"system in microgravity,apply the concept of"preventive treatment of diseases",holistically prevent or delay cardiac remodeling,and provide Traditional Chinese Medicine solutions for safeguarding astronauts'cardiovascular health.
4.The impact of prominent cortical vein sign on the clinical prognosis of patients with acute anterior circulation ischemic stroke after endovascular recanalization

Objective To explore the effect of prominent cortical vein(PCV)sign on the clinical prognosis of patients undergoing endovascular recanalization for acute anterior circulation cerebral infarction.Methods A total of 102 patients with acute anterior circulation large-vessel occlusion in Zhongshan Municipal Hospital of Tradiontional Chinese Medicine from January 1,2020 to December 21,2022,who received endovascular recanalization,were enrolled in this study.The clinical data and radiographic materials were collected.According to whether PCV sign was present or not,the patients were divided into PCV group and non-PCV group.The clinical data were compared between the two groups.Logistic regression analysis was used to analyze the correlation between the postoperative 90-day mRS score and the occurrence of PCV.Results Compared with non-PCV group,in PCV group the incidence of atrial fibrillation was even higher,and the good rate of collateral compensation was even lower,and the postoperative(7±2)-day NIHSS score was even lower;the differences in above indexes between the two groups were statistically significant(all P<0.05).Ninety days after treatment,the proportion of patients having modified Rankin Scale(mRS)score ≤2 points in the non-PCV group was 76.3%,which was higher than 58.6%in the PCV group,but the difference was not statistically significant(P=0.074).Logistic regression analysis indicated that a positive correlation existed between the presence of PCV sign and atrial fibrillation(OR=9.679,95%CI=1.264-74.364)and a negative correlation existed between the presence of PCV sign and good collateral compensation(OR=0.015,95%CI=0.033-0.331).No relationship existed between the postoperative 90-day good prognosis and the presence of preoperative PCV sign(OR=2.078,95%CI=0.680-6.348),but patient's age,preoperative intravenous thrombolysis were negatively correlated with the preoperative Alberta Stroke Program Early CT(ASPECT)score(P<0.005).Conclusion In patients with anterior circulation acute ischemic stroke(AIS),the occurrence of PCV sign is associated with the impaired collateral compensation and atrial fibrillation,but its impact on the clinical outcomes of patients after receiving reperfusion therapy remains uncertain.PCV sign may be used as a potential evaluation indicator for inadequate cerebrovascular collateral compensation.
5.The application analysis of dyadicillness management theory in chemotherapy-stage ovarian cancer patients and their spouses

Objective:To explore the effects of an intervention strategy based on dyadic illness management theory on dyadic coping level, supportive care needs, and patients' disease symptoms with chemotherapy-stage ovarian cancer patients and their spouses, and to provide reference for improving their disease coping ability and quality of life.Methods:This study used the method of randomized controlled trial. Seventy pairs of ovarian cancer patients and their spouses who received regular chemotherapy and their spouses from Obstetrics and Gynecology Hospital, Fudan University from July 2022 to June 2023 were selected using convenience sampling method. They were assigned to the control group (36 pairs) and the observation group (36 pairs) by the random number table method. Participants in the control group received routine nursing care and the observation group received the intervention strategy of dyadic illness management theory. Data were collected and compared the differences in dyadic coping levels, care needs, and disease symptoms between two groups of patients and their spouses before intervention (before chemotherapy) and after intervention (after the end of the chemotherapy cycle). SPSS 20.0 software was used for analysis, and t-tests, χ2 tests, and Wilcoxon rank sum tests were used to compare the two groups. Results:Finally, 36 pairs were included in the control group and 34 pairs in the observation group. The age of the control group patients was (52.03 ± 9.44) years old, while that of the observation group patients was (53.41 ± 10.14) years old. After the intervention, the total score of dyadic coping level in the patients and their spouses of the observation group were (113.50 ± 8.03), (114.62 ± 10.59) points, respectively, which were higher than (106.64 ± 10.06), (108.78 ± 10.89) points of the control group with significant differences ( t=-3.14, -2.27, both P<0.05); the total score of care needs in the patients and their spouses of the observation group were (89.65 ± 8.29), (95.12 ± 7.25) points, respectively, which were lower than (100.25 ± 10.82), (110.11 ± 7.58) points of the control group with significant differences ( t=4.58, 8.45, both P<0.001); the total score of disease symptoms in the patients of the observation group was (20.09 ± 4.70) points, which was lower than (31.53 ± 6.08) points of the control group with significant differences ( t=8.77, P<0.001). Conclusions:The model of dyadic illness management can improve dyadic coping level and supportive care needs of chemotherapy-stage ovarian cancer patients and their spouses, and alleviate patients′ disease symptoms.
6.Different reaction patterns and influencing factors among family caregivers of patients with advanced gynecological malignancies: a latent profile analysis

Objective:To explore the different reaction patterns among family caregivers of patients with advanced gynecological malignancies, and to analyze the influencing factors of different profiles.Methods:A cross-sectional study was conducted with 210 family caregivers of patients with advanced gynecological malignancies from Obstetrics and Gynecology Hospital in Shanghai from January 2022 to December 2022. Data were collected by Caregiver Reaction Assessment, Mishel's Uncertainty in Illness Scale-Family Member Form and Perceived Social Support Scale. Latent profile analysis was used to explore the different reaction patterns among family caregivers of patients with advanced gynecological malignancies. The influencing factors of caregiver reaction were identified by multivariate Logistic regression.Results:Totally 208 questionnaires were effectively collected. The family caregivers among patients with advanced gynecological malignancies included 163 males and 45 females, aged (53.89 ± 12.61) years old. The reaction characteristics of 208 family caregivers among patients with advanced gynecological malignancies were divided into three categories: low burden and high benefit group (24.5%, 51/208), moderate burden and benefit group (30.8%, 64/208), and high burden and low benefit group (44.7%, 93/208). Compared to the low burden and high benefit group, caregivers with lower levels of social support were more likely to be classified as moderate burden and benefit group, high burden and low benefit group ( OR = 0.563, 0.407, both P<0.01). Caregivers with moderate burden and benefit group, high burden and low benefit group had higher levels of disease uncertainty ( OR = 1.328, 2.064, both P<0.01). The caregiver′s age, monthly family income, education level, and co-caregivers were also influencing factors regarding to care reaction among family caregivers of patients with advanced gynecological malignancies ( OR values were 0.207-6.422, all P<0.05). Conclusions:The care reaction of family caregivers among patients with advanced gynecological malignancies has obvious categorical features. Healthcare professionals should implement targeted nursing interventions according to their reaction characteristics, so as to reduce the care burden of family caregivers and improve the quality of care for patients with advanced gynecological malignancies.
7.Protective effect of Shenbining granule on renal tissue of IgA nephropathy rats based on mitochondrial quality control system

OBJECTIVE To explore the renal protective mechanism of Shenbining granules on IgA nephropathy (IgAN) rats based on mitochondrial quality control system. METHODS IgAN rat model was established by the method of “bovine serum albumin+carbon tetrachloride+lipopolysaccharide”. The model rats were randomly divided into model group, prednisone acetate group (6.25 mg/kg), Shenbining equal-dose group (4.1 g/kg) and Shenbining high-dose group (20.5 g/kg). The normal rats were taken as the normal control group, with 12 rats in each group. Rats were given corresponding drugs or distilled water intragastrically in each group, once a day, for 4 consecutive weeks. After the last medication, the 24 h total urinary protein (24 h- UTP) and erythrocyte count in urine were determined, and the levels of serum creatinine (Scr), blood urea nitrogen (BUN), albumin (ALB) and alanine transaminase (ALT) were also detected. The histopathological changes in the kidneys and changes in IgA deposition in the mesangial area of the kidney were observed. mRNA and protein expression levels of PTEN-induced putative kinase 1 (PINK1), E3 ubiquitin ligase(Parkin), microtubule-associated protein 1 light chain-3 (LC3), dynamin-related protein 1 (Drp1) and mitofusin 2 (Mfn2) were detected in the kidney tissues of rats. RESULTS Compared with model group, 24 h-UTP, urinary erythrocyte count, ALT, BUN and Scr levels, LC3-Ⅱ/LC3-Ⅰ mRNA ratio, mRNA and protein expressions of Drp1 were reduced significantly in prednisone acetate group, Shenbining equal-dose group and Shenbining high-dose group (P<0.05); ALB level, LC3-Ⅱ/LC3-Ⅰ protein ratio, mRNA and protein expressions of PINK1, Parkin and Mfn2 were increased significantly (P<0.05); the pathological morphology of kidney tissue in rats was significantly improved, and IgA deposition was significantly reduced. CONCLUSIONS Shenbining granule may reduce renal pathological injury in IgAN rats and protect renal function by activating the PINK1/Parkin pathway, enhancing mitochondrial autophagy, and correcting mitochondrial kinetic disorders.
8.Current status and progress of TACE and HAIC in the treatment of hepatocellular carcinoma

Primary hepatocellular carcinoma(HCC)is a common malignant tumor in the world,and in China it is the fourth most common malignant tumor and the second cause of cancer mortality.In China,most HCC patients are already in the advanced stage when the clinical diagnosis of HCC is confirmed,and it is impossible to adopt a radical treatment for the patient.At present,transarterial chemoembolization(TACE)has become the mainstream therapeutic option for unresectable HCC,and hepatic arterial perfusion chemotherapy(HAIC),as a new therapeutic option,has attracted extensive attention.This article reviews the research progress in TACE combined with HAIC for the treatment of HCC,and obtains the following conclusions:TACE combined with HAIC has better efficacy than TACE along in patients with unresectable HCC of BCLC stage C,massive HCC,and portal venous invasion,but there is no significant difference in the therapeutic efficacy between TACE combined with HAIC and TACE alone for patients with HCC of BCLC stage A/B.It is expected that this review will provide effective recommendations for treating HCC patients in clinical practice,further standardize TACE treatment and later-stage HAIC treatment,and provide the basis for the design of relevant clinical studies.
9.Effect of laparoscopic radical resection based on membrane anatomy vs traditional laparoscopic procedure for the treatment of right colonic cancer

Objective:To compare the clinical efficacy of laparoscopic right colon cancer radical resection(CME+D3,60 cases) based on membrane anatomy and traditional laparoscopic right colon cancer radical resection(CME/D3, 120 cases).Methods:A retrospective cohort study was used to collect the consecutively admitted cases undergoing laparoscopic radical right colon cancer resection at the Department of Colorectal Surgery, Shanxi Hospital from Jan 2018 to Jun 2020.The postoperative data , postoperative complications, follow-up outcomes were compared between the two groups.Results:Compared with the CME/D3 group, that the based on membrane anatomy(CME+D3) group had a decreased intraoperative blood loss [(79.3±16.7) ml vs. (103.6±20.8) ml, t=-3.894, P<0.001],and more lymph node harvest [(27.0±5.1) vs. (25.1±6.2), t=2.138, P=0.034]; The sectional grading of surgical specimens was also better than that in the traditional operation group(χ 2=4.146, P=0.042); while the operation time was slightly longer than that of the traditional operation group [(161.1±18.4) minutes vs.(142.6±19.5) minutes, t=-6.166, P<0.001]. There was no significant difference in the length of specimen resection, postoperative exhaust time, hospital stay, incidence of postoperative complications, proportion of unplanned reoperation within 30 days, re-admission within 30 days, and mortality within 30 days ( all P>0.05). The median follow-up time was 36.7 (5.3-48.7) months, and there was no significant difference in overall survival rates between the two groups ( P>0.05); The difference in disease-free survival rates between the two groups is significant(χ 2=4.246, P=0.039). Conclusions:Compared with the traditional laparoscopic right colon cancer radical surgery (CME/D3),laparoscopic radical resection of right colon cancer (CME+D3) based on membrane anatomy reduces intraoperative bleeding, improves the number of lymph node dissection and specimen quality, and has higher 1-year and 3-year disease-free survival rate .
10.Localized delivery of nanomedicine and antibodies for combating COVID-19.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has been a major health burden in the world. So far, many strategies have been investigated to control the spread of COVID-19, including social distancing, disinfection protocols, vaccines, and antiviral treatments. Despite the significant achievement, due to the constantly emerging new variants, COVID-19 is still a great challenge to the global healthcare system. It is an urgent demand for the development of new therapeutics and technologies for containing the wild spread of SARS-CoV-2. Inhaled administration is useful for the treatment of lung and respiratory diseases, and enables the drugs to reach the site of action directly with benefits of decreased dose, improved safety, and enhanced patient compliance. Nanotechnology has been extensively applied in the prevention and treatment of COVID-19. In this review, the inhaled nanomedicines and antibodies, as well as intranasal nanodrugs, for the prevention and treatment of COVID-19 are summarized.
