1.Study on the fingerprint and active ingredients contents of Denghong buyang huanwu granules

OBJECTIVE To study the fingerprint of Denghong buyang huanwu granules (DBHG), screen the quality markers, and establish the method for content determination of active ingredients. METHODS HPLC method was adopted. The fingerprints of 10 batches of DBHG (S1-S10) were established by using the Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine (2012 edition), and similarity evaluation was also performed. Traditional Chinese medicine pieces attribution analysis, cluster analysis (CA), principal component analysis (PCA) and orthogonal partial least squares discriminant analysis (OPLS-DA) were conducted for common peaks, and quality biomarkers were screened based on variable importance projection (VIP) values>1. The contents of 10 batches of samples were determined by the same HPLC method, such as salidroside, tyrosol, calycosin-7-O-beta-D-glucoside, scutellarin and calycosin. RESULTS A total of 25 common peaks were obtained in the fingerprints for 10 batches of samples and 6 common peaks were identified, i.e. salidroside, tyrosol, paeoniflorin, calycosin-7-O-beta-D-glucoside, scutellarin, calycosin. Their similarities were greater than 0.9, and 10 common peaks of them were unique components of Erigeron breviscapus. DBHG could be clustered into 2 categories by using CA and PCA; S4-S5, and S7 could be clustered into one category and other samples could be clustered into one category. The corresponding components of peaks 16 (scutellarin), 12, 15 (calycosin-7-O-beta-D-glucoside), 13 (paeoniflorin), and 14 were quality markers. The average contents of salidroside, tyrosol, calycosin-7-O-beta-D-glucoside, scutellarin and calycosin were 1.64, 0.45, 0.31, 0.73, 0.15 mg/g in 10 batches of samples. CONCLUSIONS HPLC fingerprint for DBHG and a method for determining the contents of five active ingredients including salidroside are successfully established. Five quality markers have been screened. It can be used for the quality control of the preparation.
2.Prokaryotic expression and purification of the transcription factor TaNAC14 in wheat (Triticum aestivum).

The transcription factors (TFs) in the NAC family are involved in regulating multiple biological processes, playing an important role in plant growth, development, and stress adaptation. Our previous studies have demonstrated that TaNAC14, a member of the NAC family in wheat (Triticum aestivum L.), positively regulates root growth and development and enhances the drought tolerance of wheat seedlings. In this study, we analyzed the physicochemical properties and structure and verified the subcellular localization and transcriptional activation activity of TaNAC14. The prokaryotic expression vector pET21a-HMT-TaNAC14 was constructed and transformed into Escherichia coli BL21 CodonPlus (DE3)-RIPL. The conditions for inducing the expression of the recombinant protein HMT-TaNAC14 were optimized. The solubility of the recombinant protein was analyzed, and the protein was purified by affinity chromatography on a Ni-nitrilotriacetic acid column. The results indicated that TaNAC14 had a conserved domain of the NAM family. It was located in the nucleus and had transcriptional activation activity. The optimal conditions for expression of the recombinant protein in E. coli were induction with 0.2mmol/L IPTG for 4 h. The recombinant protein mainly existed in the soluble form, and the target protein was obtained after purification. This study lays a foundation for the identification of target genes regulated by TaNAC14.

3.Incidence and mortality of lung cancer in countries with different human development index

ObjectiveTo compare the annual and age trends of the age-standard incidence rate (ASIR) and the age-standard mortality rate (ASMR) of lung cancer in countries with different human development index (HDI) from 1990 to 2019. MethodsThe data were collected from the global burden of disease study and GLOBOCAN 2020. The average annual percentage change (AAPC) and age trends of ASIR and ASMR in lung cancer were analyzed by the Joinpoint regression model, and the comparison between the four groups was analyzed by Kruskale-Wallis analysis. ResultsIn 2020, the incidence and mortality of lung cancer gradually increased with age and HDI grade. From 1990 to 2019, the global ASIR and ASMR of lung cancer decreased, and the ASIR of lung cancer among male decreased, while the ASIR of lung cancer among female increased. The results showed that ASIR of lung cancer in female residents in countries with very high HDI increased significantly from 1996 to 2011, resulting in an overall upward trend in female ASIR, while the other groups showed a downward trend. It was found that ASIR and ASMR of lung cancer in China and India were on the rise, while ASIR and ASMR of lung cancer in Russia and the United States were on the decline. ConclusionAlthough very high/high HDI countries face a higher burden of lung cancer occurrence and death, the accumulation of lung cancer burden is completed in the transitioning period. Therefore, lung cancer prevention measures in countries in transition are critical for global lung cancer control.
4.Association of AluYb8 insertion in the MUTYH gene with the risk of decreased left ventricular diastolic function in elderly diabetic patients

Objective:To explore the relationship between AluYb8 insertion in the MUTYH gene and the risk of decreased left ventricular diastolic function in the elderly.Methods:In the retrospective analysis, 498 elderly patients with decreased left ventricular diastolic function(the disease group)and 155 people without left ventricular diastolic function(the control group)were recruited.Polymerase chain reaction was employed to analyze the genotype distribution of AluYb8 insertion in MUTYH gene.Cardiac function was measured by high-resolution color Doppler ultrasound.Results:The frequencies of the A/A, A/P and P/P genotypes were 30.1%(150/498), 48.4%(241/498)and 21.5%(107/498)in patients with decreased left ventricular diastolic function, and 27.7%(43/155), 54.8%(85/155)and 17.5%(27/155)in the control group, respectively.There were no significant differences in genotype( χ2=2.162, P=0.339)and allele frequency( χ2=1.342, P=0.794)between the two groups.Further analysis after stratification revealed that there were statistically significant differences in genotype( χ2=7.173, P=0.028)and allele frequency( χ2=8.352, P=0.015). Multivariate Logistic regression analysis showed that, in elderly patients with diabetes, P-allele carriers had a higher risk of decreased left ventricular diastolic function than non-carriers( OR=3.450, 95% CI: 1.148-10.372, P=0.027). Conclusions:AluYb8 insertion in the MUTYH gene may be associated with the risk of decreased left ventricular diastolic function in the elderly with diabetes.
5.HIV infection among voluntary blood donors in Shijiazhuang, 2011-2021

【Objective】 To study the characteristics of HIV infection among voluntary blood donors in Shijiazhuang from 2011 to 2021, so as to provide reference for decision making in the control of HIV for blood centers. 【Methods】 The confirmatory results of HIV reactive samples in initial screening among voluntary blood donors from 2011 to 2021 in our center were statistical analyzed. 【Results】 A total of 2 008 299 samples from 1 667 315 blood donors were detected, among which 3 217 samples were HIV reactive and 234 were confirmed positive, with the positive rate at 11.65/100 000 and the prevalence of 14.03/100 000. The prevalence in men was higher than that in women (16.52/100 000 vs 1.39/100 000), in first-time blood donors higher than that in repeated donors (17.27/100 000 vs 8.12/100 000), in whole blood donors higher than that in plateletpheresis donors (12.01/100 000 vs 8.41/100 000), and the differences were statistically significant (P<0.05). The male homosexual transmission was the main routes of transmission, accounting for 62.39% (146/234). And 72% of double-reagent reactive samples were confirmed positive. Four samples were screened in the serological window period and 6 samples were from HIV positive confirmed donors. 【Conclusion】 The HIV prevalence among voluntary blood donors in Shijiazhuang was low. A certain percentage of repeated blood donors got newly infected. NAT could shorten the detection window period of HIV. Since some confirmed positive samples had not been detected out by NAT, publicity and education should be strengthened to reduce the probability of infected or high-risk groups to participate in blood donation.
6.Analysis of seven cases of multiple myeloma with initial manifestation of bleeding

A retrospective analysis of 7 patients of multiple myeloma (MM) with initial manifestation of bleeding and coagulation abnormalities were performed. Clinical manifestations, laboratory and imaging examinations were collected. The activity of coagulation factors was measured before the treatment. Single factor X deficiency was seen in one patient. Two cases had factor Ⅶ deficiency, while four other patients had multiple factor deficiency. The time from onset of symptoms to diagnosis ranged from 2 to 10 months. After anti-MM treatment started and plasma or coagulation factors were transfused, the prolonged coagulation time returned to normal from 28-84 days. Most of these patients presented large, deep and multiple sites of hematoma, which caused concerns of bone marrow puncture and may direct to other differential diagnoses. This is helpful to improve physicians′ understanding of the special clinical characteristics in MM patients.
7.Effects of specialized nursing team model in children with severe asthma

Objective:To explore the effect of the specialized nursing team model in children with severe asthma.Methods:A total of 80 children with severe asthma admitted to the Respiratory Intensive Care Unit, Tangshan Maternal and Child Health Hospital of Hebei Province from October 2018 to October 2019 were selected and divided into control group and observation group according to the random number table, with 40 cases in each group. Children in control group were given routine nursing care, and children in intervention group were treated with the specialized nursing team model, and they were all treated until discharge. We compared the efficacy, disease perception with the Brief Illness Perception Questionnaire (BIPQ) , medical phobia with the Children's Medical phobia Questionnaire, and quality of life one month after discharge with the Pediatric Asthma Quality of Life Questionnaire (PAQLQ) .Results:On the day of discharge, the total effective rate of clinical treatment of children in observation group was significantly higher than that of control group; and the scores of BIPQ and Children's Medical phobia Questionnaire of observation group were lower than those of control group, the differences were statistically significant ( P<0.05) . One month after discharge, the scores of symptom dimension and the total score of the PAQLQ of observation group were higher than those of control group, and the differences were statistically significant ( P<0.05) . Conclusions:The specialized nursing team model has a significant effect in the nursing care of severe asthma, which is conducive to improving the clinical efficacy of children, reducing children's disease perception and medical phobia, and improving children's quality of life.
8.Identification and clinical significance of corynebacterium from abscess in granulomatous mastitis

Objective:To make the early clinical antibiotic regimen by finding out the infection of corynebacterium in the pus of patients with granulomatous mastitis in the early stage.Methods:A total of 42 patients who were diagnosed and treated in the Breast Center of Guangdong Maternal and Child Health Hospital from Jun. 2016 to Mar. 2017 with complete follow-up data were retrospectively analyzed. PCR method was used to detect corynebacterium in the patients’ pus. Patients in the positive group were treated with antibiotics alone, antibiotics + hormones and hormones alone, while patients in the negative group were treated with antibiotics + hormones and hormones alone. The postoperative recurrence rate and cure rate of different groups of patients were observed.Results:The antibiotic regimen for granulomatous mastitis in patients with corynebacterium infection included a combination of short-acting levofloxacin and azithromycin and long-acting anti-mycobacterium drugs. Among the 42 patients in the subgroup, 21 patients were confirmed positive for corynebacterium by PCR detection of pus, and the postoperative recurrence rate was 23.5%. There was a statistically significant difference between the antibiotic group, the antibiotic + hormone group and the hormone group in treatment of granulomatous mastitis infected with corynebacterium ( χ2=5.494, P=0.036) . PCR detection shouwed corynebacterium negative in 21 cases, and postoperative recurrence rate of 16.7%. No statistically significant difference in efficacy was found between antibiotic + hormone group and hormone only group for GM patients without bacterial infection ( χ2=1.129, P=0.719) . Conclusion:Early detection of corynebacterium infection in GM patients is significant for clinical guidance of the application of lipophilic antibiotics.
9.Effects of self-management intervention under self-efficacy theory on 7-14 years old children with asthma

Objective To explore the effects of self-management intervention under the self-efficacy theory on 7-14 years old children with asthma. Methods In this prospective study, totally 201 pediatric patients with asthma treated in Tangshan Maternal & Child Health Care Hospital from April 2016 to April 2018 were selected and divided into managed (n=42) and control (n=42) groups according to the random number table. Patients in the control group received conventional care according to the recommendations under Global Initiative for Asthma (GINA), while patients in the managed group received self-management intervention under the self-efficacy theory on this basis for 6 months. The frequency of asthma and emergency treatment during the intervention period was calculated, and the control of asthma and pulmonary function within 1 year after intervention was evaluated. Exercise of Self-care Agency Scale (ESCA) and Chronic Disease Resilience Scale for Children (CDRSC) were used to assess the self-care agency and psychological resilience of the two groups before and after intervention. Results Compared with the control group, the average frequency of asthma and emergency treatment was reduced in the managed group during the intervention period. The proportion of patients whose asthma was partially or fully controlled within 1 year after intervention increased, and there was statistically significant difference between the two groups (P< 0.05). Daytime and nighttime symptom scores and pulmonary function were ameliorated in both groups after intervention. The improvement effects were better in the managed group than in the control group, and there was statistically significant difference between the two groups (P<0.05). The scores of self-concept, self-care responsibility, self-care skill and health knowledge in ESCA and the scores of personalities, coping style and interpersonal relationship in CDRSC increased in both groups after intervention. The scores of the managed group were higher than those of the control group, and there was statistically significant difference between the two groups (P<0.05). Conclusions Self-management intervention under the self-efficacy theory can effectively improve the self-care agency and psychological resilience in 7-14 years old children with asthma, thus reducing the incidence of asthma and emergency treatment and improving the recovery of pulmonary function.

Objective:
To investigate the effect of hypertensive disorder complicating pregnancy (HDCP) on the mortality and early complications of premature infants.
Methods:
The general clinical data of preterm infants with gestational age 24-36+ 6 weeks were collected from the cooperative units in the task group from January 1, 2013 to December 31, 2014.According to the severity of HDCP, the infants were divided into 4 groups: HDCP group, preeclampsia group, eclampsia group and non HDCP group, the mortality and major complications of preterm infants were compared, and the influencing factors were analyzed.
