1.Data mining and analysis for ADE signals of three third-generation tetracycline antibiotics

OBJECTIVE To excavate the adverse drug event (ADE) signals of three third-generation tetracycline antibiotics (tigecycline, omadacycline, eravacycline) based on FDA adverse event reporting system (FAERS), and to provide reference for the safe use of them. METHODS The ADE reports of tigecycline, omadacycline and eravacycline from the first quarter of 2005 to the second quarter of 2023 were retrieved from FAERS database. The ADE signals of 3 kinds of drugs were mined by the method of reporting odds ratio method and the proportional reporting ratio method. RESULTS Totally 2 538 ADE reports with tigecycline, omadacycline and eravacycline as the primary suspected drugs were obtained, including 2 135 tigecycline ADE reports, 349 omadacycline ADE reports and 54 eravacycline ADE reports. A total of 131 ADE positive signals of tigecycline were mined, involving 19 system organ classes (SOCs), mainly concentrated in investigations, hepatobiliary system, blood and lymphatic system, and gastrointestinal system, etc; the preferred terminologies (PT) with intense signal were hypofibrinogenaemia and blood fibrinogen decreased. Fourteen ADE signals were not mentioned in the drug instruction, such as renal failure, acute kidney injury and hemorrhage. Totally 24 ADE positive signals of omadacycline were mined, involving 6 SOCs, mainly concentrated in the gastrointestinal system and various examinations; the PTs with intense signals were tooth discoloration, jet-like vomiting and loose feces, etc. ADE signals were not mentioned in the drug instructions, included lip swelling, gastroesophageal reflux disease, eosinophilia, skin discoloration, feces softening, and night sweats. Five ADE positive signals of eravacycline were mined, involving 4 SOCs, mainly concentrated in various examinations, gastrointestinal system, etc. The most intense ADE signals were blood fibrinogen decreased and hypofibrinogenaemia. CONCLUSIONS ADE of the gastrointestinal system are mostly identified in the three third-generation tetracycline antibiotics, especially pancreatitis caused by tigecycline and gastroesophageal reflux disease caused by oral administration of omadacycline. The liver function, renal function (for tigecycline) and coagulation function (for tigecycline, eravacycline) should be monitored biyiliang@hotmail.com regularly during medication, so as to prevent the occurrence of serious ADE.

3.Establishment of a candidate reference method for angiotensin Ⅱ in human plasma

Objective:To establish a candidate reference method for the determination of angiotensin Ⅱ in human plasma by isotope dilution liquid chromatography tandem mass spectrometry (ID-LC-MS/MS) and to evaluate its performance.Methods:Using [ 13C 6- 15N]-angiotensin Ⅱ as the internal standard, the plasma was accurately weighed by gravimetric method and mixed with a certain amount of internal standard. At the same time, enzyme inhibitor was added. After zinc sulfate solution protein precipitation and reversed-phase solid-phase extraction plate treatment, it was analyzed by liquid chromatography tandem mass spectrometry. The multi reaction ion monitoring mode(MRM)was selected by mass spectrometry to detect specific ion fragments of angiotensin Ⅱ and internal standard. The linearity, sensitivity, precision, recovery rate and uncertainty of the performance of the established method were evaluated according to ISO15193. Results:Angiotensin Ⅱ had good linearity in the range of 10-1 000 pg/g ( r=0.999 5), the lower limit of quantification was 7.68 pg/g, the analytical recoveries were 97.14% to 102.85%, intra-batch imprecision≤3.21%, inter-batch imprecision≤2.96%, and total imprecision≤3.67%. Conclusion:A method for the determination of plasma angiotensin Ⅱ was established by ID-LC-MS/MS. The method is accurate and reliable, and is expected to be a reference method for the determination of plasma angiotensin Ⅱ.
4.Molecular biological characteristics of the 2019 novel Coronavirus in Shijiazhuang

Objective:To analyze the molecular epidemiological characteristics of the Corona virus disease 2019 (COVID-19) cases in Shijiazhuang, which can reveal the origin of the outbreak and provide a scientific basis for COVID-19 prevention and control.Methods:From January 2 to January 8, 2021, a total of 404 samples from 170 COVID-19 cases were collected from the Shijiazhuang Fifth Hospital. The consensus sequence of 2019 novel Coronavirus(2019-nCoV) was obtained through multiplex polymerase chain reaction-based sequencing. The sequences of 170 COVID-19 cases were analyzed by the PANGOLIN, and the data were statistically analyzed by T-test.Results:Among the 404 COVID-19 samples, a total of 356 samples obtained high quality genome sequences (>95%,100×sequencing depth). The whole genome sequences of 170 COVID-19 cases were obtained by eliminating repeated samples. All 170 sequences were recognized as lineage B1.1 using PANGOLIN. The number of single nucleotide polymorphism arrange from 18-22 and most of the single nucleotide polymorphism were synonymous variants. All of 170 genomes could be classified into 48 sub-groups and most of the genomes were classified into 2 sub-groups (66 and 31, respectively).Conclusions:All cases in this study are likely originated from one imported case. The viruses have spread in the community for a long time and have mutated during the community transmission.
5.Pharmacodynamics study of Panax notoginseng saponins zein lipoprotein nanoparticles for cerebral ischemia reperfusion injury in rats

PLZ-NPs (PNS-lipid-zein nanoparticles) prepared by co-assembly of Panax notoginseng saponins, lecithin, β-sitosterol and zein were applied for in vitro cell experiment and oral gavage to study the protective effect of cerebral ischemia-reperfusion rats.PLZ-NPs were characterized by Malvin-particle size analyzer and transmission electron microscope (TEM), respectively. The toxicity of PLZ-NPs and free carrier were evaluated by MTT, and the uptake of nanoparticles in Caco-2 cells was analyzed by laser confocal and flow cytometry. The cerebral ischemia reperfusion rat model was established by MCAO method and then be given samples by gavage for 3 days. The brain tissues were taken to stain by 2, 3, 5-triphenyltetrazole chloride (TTC) and the biochemical indicators of MDA, inflammatory cytokines IL-1β and TNF-α, apoptosis-related proteins Bax and Bcl-2 from the harvested brain tissues were detected to evaluate the protective effect of PNS in PLZ-NPs on cerebral ischemia reperfusion. The particle size, PDI, and zeta potential of formed PLZ-NPs were (116.4 ± 0.81) nm, 0.048 and -(31.5 ± 0.31) mV, respectively. The results of MTT showed that the zein lipoprotein carrier was non-toxic to Caco-2 cells. The results of laser confocal and flow cytometry showed that FITC uptake of nanoparticles could be significantly improved in Caco-2 cells.The uptake from the nanoparticles at 4h was 1.76 times of that of the free FITC group.Compared with the model group, the TTC staining images of free drug PNS group and PLZ-NPs group showed certain reduction in the white infarct area.The contents of MDA, IL-1β, TNF-α and Bax were significantly decreased, while the content of Bcl-2 was significantly increased. Furthermore, all parameters of PLZ-NPs group showed better results than those of PNS group, and there was a significant difference (P < 0.05). All results indicated that the prepared PLZ-NPs had good stability and biological safety, and could significantly increase the uptake in intestinal epithelial cells, and effectively protect against the damage caused by cerebral ischemia reperfusion in rats.
6.Impact of WeChat-assisted follow-up on the self-administer medication capability of stroke survivors

Objective To explore the impact of WeChat-assisted follow-up on self-administer medication capability of stroke survivors.Methods Seventy patients were recruited from a tertiary hospital and randomly divided into intervention group (n=35) and control group (n=35).Patients in intervention group were given WeChat-assisted follow-up including health education program and behavior guidance,and patients in control group received regular discharge care.The effects of WeChat-assisted follow-up and regular care on self-administer medication ability,cognitive and behavior level were assessed.Results After three months' follow-up,the scores of self-administer medication capability (38 (32,42)),experience (25 (20,28) and the integral level (87 (71,92)) in intervention group were significantly higher than those in control group (capability 25(16,38),experience 21 (8.75,27),total score 68 (38,87)) (Z=-2.511,-2.033,-2.209,P<0.05).The self-administer medication experience,cognitive,capability and total scores increased significantly in intervention group after intervention(Z=-4.525,-4.610,-3.806,-4.718,all P< 0.01),while only the cognitive level increased slightly in control group (18 (10,24) vs 13 (11,18),Z =-1.794,P=0.073).Conclusions Follow-up intervention based on WeChat platform can improve the stroke patients' self-administration medication capability,however,the long-term effects on cognitive level and further reform need to be strengthened.
7.Influencing factors of macrosomia in second term singleton pregnancy

Objective To explore the characteristics of neonatal birth weight (BW) in two consecutive term singleton pregnant women and to investigate the influencing factors of macrosomia in the second birth.Methods In this case-control study,medical records of 1 920 singleton full-term pregnant women who gave birth twice in Peking University First Hospital from January 2005 to December 2017 were reviewed.All subjects were divided into two groups according to neonatal BW at the second birth regardless of the first one:macrosomia group (n=122) and non-macrosomia group (n=l 798).Then,those two groups were further divided into four sub-groups based on the neonatal BW in the first birth:macrosomia at both deliveries (n=27),macrosomia at the second time and non-macrosomia at the first time (n=95),non-macrosomia at the second time and macrosomia at the first time (n=90) and non-macrosomia at both deliveries (n=l 708).The differences of delivery interval,and the maternal age,pre-pregnancy body mass index (BMI),weight gain during pregnancy,area under the curve of oral glucose tolerance test results,weight retention,the incidence of diabetes in pregnancy (including gestational diabetes mellitus and diabetes mellitus complicated with pregnancy),incidence of hypertensive disorders during pregnancy and cesarean section rate at the second pregnancy between the groups and sub-groups were compared with t-test,Chi-square test and logistic regression analysis.Results (1) The total average neonatal BW in the second pregnancy was higher than that in the first [(3 443 ± 378) vs (3 403 ± 396) g,t=-4.119,P<0.001].However,the proportion of macrosomia in each group was similar [6.4% (122/ 1 920) vs 6.1% (117/1 920),x2=3.237,P=0.198].The pre-pregnancy BMI,weight gain during pregnancy and proportion of previous macrosomia in the macrosomia group were significantly higher than those in non-macrosomia group [(23.6±3.4) vs (22.7±3.1) kg/m2,t=-2.882,P=0.004;(13.4±5.0) vs (12.4± 4.1) kg,t=-2.522,P=0.037;22.1% (27/122) vs 5.0% (90/1 798),x2=58.554,P<0.001].Logistic regression analysis showed that previous macrosomia (OR=4.979,95%CI:3.052-8.122,P<0.001),pre-pregnancy BMI (OR=1.084,95%CI:1.023-1.149,P=0.001) and weight gain during pregnancy (OR=1.077,95%CI:1.031-1.125,P=0.007) were influencing factors for macrosomia in the index delivery.(2) The pre-pregnancy BMI in the subgroup of macrosomia at both deliveries was significantly higher comparing to the subgroup of non-macrosomia at the second time and macrosomia at the first time [(25.8±4.3) vs (23.9±2.9) kg/m2,t=-2.600,P=0.011].Women in the subgroup of macrosomia at the second time and non-macrosomia at the first time had higher weight gain during second pregnancy than the subgroup of non-macrosomia at both deliveries [(13.5 ± 4.2) vs (12.5 ±4.1) kg,t=-2.404,P=0.016].Conclusions For two consecutive term singleton pregnancies,the average neonatal BW in the second time is slightly higher than that in the first,but the incidence of macrosomia is similar.Pre-pregnancy BMI,weight gain during pregnancy and macrosomia in the first birth are influencing factors for macrosomia in the second pregnancy.More attention should be paid to pre-pregnancy BMI reduction in women with history of macrosomia.For women without a history of macrosomia,weight management should be emphasized during the second pregnancy.
8. Validation and optimization of the indicator system of risk assessment for mechanical cuts

Objective:
To validation and optimization the indicator system of risk assessment for mechanical cuts.
Methods:
The risk assessment index system of mechanical cutting injury established earlier was used to assess the risk of mechanical cutting injury in 40 cases of mechanical cutting injury registered from January 2015 to December 2017 and 40 similar positions without accidents in the same period. The multiple stepwise regression analysis was used to screen the indicator system, and to adjust the weight coefficient of each index. The total coincidence rate and Kappa value were compared between before and after optimization respectively.
Results:
The new index system has 3 first-class indicators, 10 second-class indicators and 14 three-class indicators, fewer than the old index system which has 3 first-class indicators, 10 second-class indicators, 34 three-class indicators. There three indicators have revamped in the first-class. The total of coincidence rates of the new and old indicator systems were 67.50% and 90.00%, the difference was statistically significant (

Mbile Health has been recommended by World Health Organization, and mostly actualized by smartphone application (APP). Home patients with stroke accept APP well, and the acceptance of APP relates with the factors of patients, APP and society. Acceptance of APP may be improved by the improvement of technical support, user experience and usability.

The stroke patients at home have many problems, such as knowledge, medication, monitoring and rehabilitation, which results in poor rehabilitation effect, and the patients need home care urgently. Smartphone applications can provide disease knowledge, health monitoring, drugs reminder, rehabilitation guide and system management, as well as social activities and emergency treatment strategy supplying, which can provide a new home care method for stroke patients. However, there are also many deficiencies, such as simple function design, lack of medical institutions participation in the design, lack of authoritative quality evaluation tool, limitations in users and potential safety hazard.
