1.Safety and efficacy of mitoxantrone liposome combined chemotherapy in the treatment of mixed phenotype acute leukemia

Objective:To evaluate the safety and efficacy of mitoxantrone liposome (MIT-LIP) combined chemotherapy in treating mixed phenotype acute leukemia (MPAL) .Methods:December 2021 to November 2024, MPAL patients who underwent the MAED (MIT-LIP + cytarabine + etoposide + dexamethasone) regimen were retrospectively analyzed. Data on clinical characteristics, adverse reactions, therapeutic outcomes, and long-term prognoses were collected.Results:A total of 7 MPAL patients who received MAED regimen were admitted. Among them, two patients were initially diagnosed with T-ALL or B-ALL, respectively, and transformed into AML after treatment. Three patients were initially diagnosed as MPAL (B/myeloid), one as MPAL (T/myeloid), and one with MPAL (myeloid/plasmacytoid dendritic cell). Among the 7 patients, there were 3 males and 4 females, 1 chromosome abnormalities and 6 gene abnormalities, including 1 case with BCR∷ABL fusion gene. The median age was 38 years (range: 16–58 years). There was no clear related drug allergy and organ toxicity during MAED regimen, and the main adverse effect was hematological toxicity. After induced chemotherapy, all patients achieved complete remission (CR), 2 maintained MRD-negative CR and 1 maintained MRD-positive CR. The other 4 patients underwent allogeneic hematopoietic stem cell transplantation, 2 maintained MRD-negative CR, and 2 relapsed. The current median follow-up time was 12 months, the overall survival (OS) rate was 100%, the relapse-free survival (RFS) rate was 60%, and the median OS time and median RFS time were not reached.Conclusion:The MAED regimen demonstrates high safety and a favorable CR rate in MPAL treatment.
2.Perioperative oxygen concentration and postoperative organ damage in pediatric neurosurgery:a retrospective cohort study

Objective To investigate the correlation between perioperative oxygen concentration and postoperative organ injury and other adverse reactions in children undergoing neurosurgery.Methods This study is a single-center retrospective cohort study,including 512 children aged≤14 years and operating duration≥2 hours who were transferred after surgery to postanesthesia care unit(PACU),Department of Anesthesiology,Beijing Tiantan Hospital,Capital Medical University from October 2018 to September 2023.His electronic medical record system was used to review and collect the patient's medical records during hospitalization.Including baseline data,anesthetic use,duration of anesthesia,intraoperative oxygen concentration,postoperative diagnosis,and surgical method;blood pressure,heart rate,oxygen saturation,pain score and special conditions during anesthesia recovery;infection during postoperative hospitalization(including wound infection,intracranial infection),respiratory complications[confirmed by chest radiograph/chest computed tomography(CT)report],changes in renal function(first postoperative serum creatinine value-last preoperative serum creatinine value),and incidence of delirium during recovery period during anesthesia were recorded.The area under oxygen concentration-time curve area under curve AUCFiO2=the product of intraoperative oxygen concentration(%)and duration of anesthesia(min)was calculated,and then 512 children were divided into 3 groups according to the AUCFiO2 tripartite(group 1:AUCFiO2<8 720%min,group 2:8 720%·min≤AUCFiO2≤13 800%·min,group 3:AUCFiO2>13 800%·min)were included in the data analysis.Results The incidence of emergency delirium(ED)during postoperative recovery was 24%,26.9%and 19.4%,and the incidence of acute kidney injury(AKI)was 0.6%,2.3%and 0.6%,respectively.There was no significant difference between the three groups(P>0.05).The incidence of postoperative pulmonary complications in the three groups was 20.6%,20.8%and 31.5%,respectively,and there was significant statistical difference among the three groups(P<0.05).There were no significant differences in nausea,vomiting,hypoxemia,and PACU residence time among the three groups during anesthesia recovery room(P>0.05),and there were significant differences in intracranial infection,wound infection and hospital stay during hospitalization(P<0.05).Conclusion Although oxygen concentration AUCFiO2 has no correlation with the occurrence of postoperative AKI and ED,the longer the exposure time of conventional oxygen administration strategy,the higher the incidence of postoperative pulmonary complications and postoperative infection,and the longer the hospitalization time of children.
3.Relationship of physical fitness index with depressive, anxiety and stress symptoms among college students

Objective:
To investigate the association between the physical fitness index (PFI) and symptoms of depressive, anxiety and stress symptoms among college students, providing a reference for mental health interventions.
Methods:
From June to September 2025, combined convenience and cluster random sampling approach was used to administer questionnaire surveys and perform physical fitness tests on 2 712 college students from Zhejiang Chinese Medical University. The Depression Anxiety Stress Scales-21 Items (DASS-21) was used to assess mental health status. Chi square test and multivariate Logistic regression analysis were used to determine the associations between the PFI and the PFI component indicators with depressive, anxiety and stress symptoms.
Results:
The prevalence of depressive, anxiety and stress among college students were 24.26%, 33.22% and 13.68%, respectively. Statistically significant differences in the prevalence of these symptoms were detected across groups differing in sleep quality, physical activity, weekly breakfast frequency, and history of low back or neck pain ( χ 2=9.33-151.83, all P <0.05). After adjusting for confounding factors, Logistic regression revealed that the moderate and high PFI groups had significantly reduced risks of depressive and anxiety compared to the low PFI group ( OR =0.73, 0.63; 0.61, 0.72, all P <0.05). Poor speed (50 m run) and lower body strength (standing long jump) emerged as common risk factors affecting anxiety and depressive symptoms in both male and female college students (all P <0.05). Increased muscle strength (sit up for 1 min) in female students reduced the risk of depressive ( OR =0.81), anxiety ( OR =0.85), and stress symptoms ( OR =0.79) (all P <0.05). Enhanced lung capacity in male students decreased the risk of depressive ( OR =0.84) and anxiety symptoms ( OR =0.85) (both P <0.05).
Conclusions
The PFI is negatively correlated with depressive and anxiety symptoms among college students with notable gender differences. Insufficient speed and lower body explosive power represent common risk factors for mental health among male and female college students.

5.Application of a novel drainage catheter in treatment of acute high-tension breast milk cyst during lactation

Objective To develop a novel drainage catheter based on the anatomical structure of the mammary duct and establish a non-invasive treatment protocol for patients with acute high-tension breast milk cyst during lactation.Methods Ten primiparous patients with acute high-tension breast milk cyst during lactation,admitted to the postpartum clinics of three hospitals,were enrolled as study subjects.A novel drainage catheter was prepared using an epidural anesthesia catheter,and intubation was performed to unblock the obstructed mammary ducts.For cases with intubation failure,puncture as-piration was employed.Local cold compresses were applied to the breasts after treatment.Results Out of 10 patients,6 were successfully unblocked(successful group),and 4 failed(failure group).The duration of illness in the successful group was(6.00±0.82)days,which was shorter than that in the failure group[(9.50±1.80)days](P<0.05).The insertion depth of the catheter in the successful group was(6.42±0.67)cm,which was greater than that in the failure group[(4.00±0.71)cm](P<0.05).There was no statistically significant difference in cyst diameter between the two groups(P>0.05).For patients in whom unblocking failed,puncture aspiration was performed,and 3 of them experienced recurrence after the initial puncture.Ultrasound-guided re-puncture aspiration and weaning were performed.One patient with evident inflammatory symptoms was administered systemic antibiotic therapy.After treatment,7 patients resumed lactation,and follow-up results indicated that all 10 patients were cured.Conclusion Acute high-tension breast milk cyst during lactation is rela-tively rare in clinical practice.The novel drainage catheter can achieve non-invasive unblocking of distally obstructed mammary ducts in the lactiferous sinus.The comprehensive treatment protocol can improve the overall cure rate and result in a high rate of resumed lactation post-treatment.
6.Arthroscopic Total Internal Suture Combined With Platelet-rich Plasma for the Treatment of Lateral Meniscal Popliteal Hiatus Area Injuries

Objective To investigate the short-term efficacy of arthroscopic total internal suture combined with platelet-rich plasma(PRP)in the treatment of lateral meniscal popliteal hiatus area injuries.Methods Forty-eight patients diagnosed with lateral meniscal popliteal hiatus area injuries in our hospital from January 2020 to December 2022 were selected and divided into Fast-Fix total internal suture group(FF group,n=23)and Fast-Fix total internal suture combined with PRP group(PRP group,n=25)according to treatment methods.The positive rate of McMurray test at 6 and 12 months after surgery,the preoperative and postoperative visual analogue scale(VAS)of pain,the International Knee Documentation Committee(IKDC)scores,the Lysholm Knee Function Scores,and the Knee Society Scores(KSS)were compared between the two groups.Results No adverse events such as vascular and nerve injury,fever and infection occurred in both groups.There was no significant difference between the two groups in the positive rates of McMurray test at 6 and 12 months(χ2=0.880,P=0.348;Fisher's exact test,P=0.479).In the PRP group,25 cases were followed up for11-14 months,with an average of(12.2±0.8)months;in the FF group,23 cases were followed up for11-14 months,with an average of(12.8±0.8)months.The VAS scores at 6 months and 12 months after surgery were significantly lower than those before surgery in both groups(all P=0.000).The VAS scores of the PRP group at 6 and 12 months after surgery were significantly lower than those of the FF group(P<0.05).Both groups showed significant improvements in IKDC,Lysholm,and KSS scores at 6 and 12 months after surgery compared to preoperative levels(all P=0.000).The IKDC,Lysholm,and KSS scores of the PRP group at 6 and 12 months after surgery were significantly higher than those of the FF group(P<0.05).Conclusion Compared to Fast-Fix total internal suture alone,PRP joint cavity adjuvant therapy based on Fast-Fix total internal suture surgery could reduce the postoperative pain of patients,and promote the functional recovery of the knee joint,which is more recommended in clinical practice.
7.Development of a prediction model based on decision tree for acute kidney injury in critically ill children and its predictive value

Objective:To establish and validate a prediction model based on least absolute shrinkage and selection operator(LASSO)regression and classification and regression tree(CART)algorithm for acute kidney injury(AKI)in PICU.Methods:The prospective derivation cohort consisted of 350 critically ill children admitted to the PICU of Children′s Hospital of Soochow University from September 2020 to January 2021.The external data set consisting of 866 critically ill children admitted to the PICU of Children′s Hospital of Soochow University from February 2021 to February 2022 was employed for the external validation.Clinical data was obtained from the electronic medical record system,including demographic characteristics,laboratory data and the pediatric risk of mortality Ⅲ(PRISM Ⅲ)score.The variables associated with AKI were identified using LASSO regression.Subsequently,a decision tree prediction model was built using the CART algorithm.The predictive value of decision tree prediction model was evaluated using the receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis.Results:Among the 350 children in the derivation cohort,107(30.6%)developed AKI during the PICU stay;and of 866 children in the external validation cohort,165(19.1%)developed AKI during the PICU stay.The LASSO regression screened 16 candidate variables for further analysis,and the decision tree model ultimately identified 4 variables more closely associated with AKI,including fold change in serum creatinine from baseline,urine volume,PRISM Ⅲ,and C-reactive protein.The decision tree model exhibited high accuracy with AUC of 0.92,0.88,and 0.86 in the training,internal validation,and external validation cohorts,respectively.The model demonstrated good calibration and clinical applicability based on the calibration curve and decision curve analysis.Conclusion:The decision tree model based on the 4 identified clinical indicators,including fold change in serum creatinine from baseline,urine volume,PRISM Ⅲ,and C-reactive protein,is effective for the early prediction of AKI.
