1.Visual analysis of acupuncture and moxibustion treatment for cognitive impairment after stroke based on CiteSpace

Objective:To identify the characteristics of research on scalp acupuncture in the treatment of post-stroke cognitive impairment (PSCI).Methods:This was a bibliometric analysis. We selected relevant studies from both Chinese databases (China National Knowledge Infrastructure, Chinese Science and Technology Periodical Database, and WanFang Data Knowledge Service Platform) and English databases (Web of Science, PubMed, and Scopus) from their inception to December 2023. The CiteSpace software was used to perform visual-based analyses of publication trends, collaborative networks among countries and institutions, author collaboration networks, and research hotspots.Results:We included 730 Chinese studies and 112 English-language studies. From 2004 to 2023, the number of publications on scalp acupuncture for PSCI showed an increasing trend. All Chinese studies were published in China, mainly by universities, and the institutional collaboration density was low (0.002 9). For English-language studies, China had the most publications (103), followed by the USA (7) and Germany (3). Chinese universities were the main publishing institutions, and the density of inter-institutional collaboration (0.183 3) was higher than that for Chinese literature. The density of the author collaboration network was 0.005 0 for Chinese studies and 0.023 8 for English-language studies. In Chinese studies, the most frequent keywords with a centrality greater than 0.1 were cognitive impairment (248 times), stroke (240 times), needling (162 times), cerebral infarction (82 times), and electroacupuncture (80 times). In English-language studies, these were stroke (40 times), cognitive impairment (32 times), acupuncture (15 times), dementia (15 times), ischemic stroke (10 times), and mechanism (7 times). For Chinese studies, the top 3 clusters focused on clinical studies of the effects of scalp acupuncture on PSCI. In English-language studies, the top 4 clusters mainly involved research on the effects and assessment methods of scalp acupuncture for PSCI. The timeline map of keywords in Chinese studies showed that before 2015, the high-frequency keywords were stroke, cognitive impairment, needling, electroacupuncture, scalp acupuncture, body acupuncture, rehabilitation, moxibustion, and clinical research. From 2015 onwards, keywords such as rat, apoptosis, inflammatory factors, mechanism research, signaling pathways, mice, and animal experiments appeared. The timeline map of keywords in English-language studies showed that before 2013, there were few accumulated keywords. After 2013, high-frequency keywords included "multi-infarction rat""computer-based cognitive training""apoptosis""electroacupuncture""mild cognitive impairment""randomized controlled trial""scalp acupuncture""post-stroke cognitive impairment""stroke rehabilitation""systematic review" and "hippocampal CA1 region". In Chinese studies, keywords with high burst intensity included cerebral infarction, acupuncture therapy, vascular, baihui (GV-20), review, learning and memory, and scalp acupuncture. In English-language studies, these included "multi-infarction rat""mild cognitive impairment""electroacupuncture""post-stroke cognitive impairment"and"systematic review".Conclusions:Research in this field has grown rapidly in recent years. Chinese studies focus mainly on the clinical effects of scalp acupuncture for PSCI, while English-language studies focus more on related mechanisms. Furthermore, both Chinese and English-language studies show good consistency in the focus of experimental research, which mainly focuses on the mechanisms of electroacupuncture in the treatment of PSCI.
2.Application of a novel drainage catheter in treatment of acute high-tension breast milk cyst during lactation

Objective To develop a novel drainage catheter based on the anatomical structure of the mammary duct and establish a non-invasive treatment protocol for patients with acute high-tension breast milk cyst during lactation.Methods Ten primiparous patients with acute high-tension breast milk cyst during lactation,admitted to the postpartum clinics of three hospitals,were enrolled as study subjects.A novel drainage catheter was prepared using an epidural anesthesia catheter,and intubation was performed to unblock the obstructed mammary ducts.For cases with intubation failure,puncture as-piration was employed.Local cold compresses were applied to the breasts after treatment.Results Out of 10 patients,6 were successfully unblocked(successful group),and 4 failed(failure group).The duration of illness in the successful group was(6.00±0.82)days,which was shorter than that in the failure group[(9.50±1.80)days](P<0.05).The insertion depth of the catheter in the successful group was(6.42±0.67)cm,which was greater than that in the failure group[(4.00±0.71)cm](P<0.05).There was no statistically significant difference in cyst diameter between the two groups(P>0.05).For patients in whom unblocking failed,puncture aspiration was performed,and 3 of them experienced recurrence after the initial puncture.Ultrasound-guided re-puncture aspiration and weaning were performed.One patient with evident inflammatory symptoms was administered systemic antibiotic therapy.After treatment,7 patients resumed lactation,and follow-up results indicated that all 10 patients were cured.Conclusion Acute high-tension breast milk cyst during lactation is rela-tively rare in clinical practice.The novel drainage catheter can achieve non-invasive unblocking of distally obstructed mammary ducts in the lactiferous sinus.The comprehensive treatment protocol can improve the overall cure rate and result in a high rate of resumed lactation post-treatment.
3.Analysis of the Current Situation of the Construction of National Regional Medical Centers for Children in China

Objective Examine and analyze the current situation of the construction of national regional medical centers for children in China,in order to provide reference for promoting the high-quality development of national regional medical centers for children and other categories.Methods Refer to literature and official websites of relevant national departments and hospitals to obtain the required data and establish an analysis database for children's national regional medical centers.Using PEST analysis method,conduct research from four perspectives:politics,economy,society,and technology.Results The development trend of national regional medical centers for children is good,with a favorable policy environment,increasing demand for children's medical and health services,and high social expectations.However,there are issues such as targeted support policies that need to be improved,varying levels of economic security,regional cultural conflicts,and differences in technological resource levels.Conclusion It is suggested to improve the targeted policy support system,establish a sound compensation mechanism,actively integrate into the overall development of regional society,and integrate the advantages of various technological resources.

Osteoporotic proximal humeral fracture (OPHF) is one of the common osteoporotic fractures in the aged, with an incidence only lower than vertebral compression fracture, hip fracture, and distal radius fracture. OPHF, secondary to osteoporosis and characterized by poor bone quality, comminuted fracture pattern, slow healing, and severely impaired shoulder joint function, poses a big challenge to the current clinical diagnosis and treatment. In the field of diagnosis, treatment, and rehabilitation of OPHF, traditional Chinese and Western medicine have accumulated rich experience and evidence from evidence-based medicine and achieved favorable outcomes. However, there is still a lack of guidance from a relevant consensus as to how to integrate the advantages of the two medical systems and achieve the integrated diagnosis and treatment. To promote the diagnosis and treatment of OPHF with integrated traditional Chinese and Western medicine, relevant experts from Orthopedic Expert Committee of Geriatric Branch of Chinese Association of Gerontology and Geriatrics, Youth Osteoporosis Group of Orthopedic Branch of Chinese Medical Association, Osteoporosis Group of Orthopedic Surgeon Branch of Chinese Medical Doctor Association, and Osteoporosis Committee of Shanghai Association of Integrated Traditional Chinese and Western Medicine have been organized to formulate Expert consensus on the diagnosis and treatment of osteoporotic proximal humeral fracture with integrated traditional Chinese and Western medicine ( version 2024) by searching related literatures and based on the evidences from evidence-based medicine. This consensus consists of 13 recommendations about the diagnosis, treatment and rehabilitation of OPHF with integrated traditional Chinese medicine and Western medicine, aimed at standardizing, systematizing, and personalizing the diagnosis and treatment of OPHF with integrated traditional Chinse and Western medicine to improve the patients ′ function.
7.Mechanism of the inhibitory effect of galangin on hepatoma cells

The mortality rate of hepatocellular carcinoma (HCC) remains high, and although there have been several treatment methods, HCC patients still have poor treatment outcome and prognosis in clinical practice. Therefore, it is necessary to find a new drug for the treatment of HCC to improve patients’ survival rate. This article introduces a new antitumor drug, galangin, which can exert an antitumor effect by inhibiting cell proliferation, promoting apoptosis, inducing autophagy, and inhibiting metastasis. In addition, galangin can also inhibit angiogenesis in liver cancer, reverse multidrug resistance, and enhance the synergistic effect between drugs. Therefore, galangin is believed to have a promising future in clinical practice, and it is expected that more studies will focus on the anti-hepatoma cell mechanism of galangin to provide a scientific basis for the clinical translation of galangin.

Objective To develop the clinical practice guidelines for the management of medication adherence to highly active antiretroviral therapy (HAART) in China. Methods The development methods included qualitative interview of 31 stakeholders, questionnaire survey of 423 PLHIV, adaptation of 30 clinical practice guidelines related to AIDS care, and overviews of reviews of 44 systematic reviews/Meta-analysis. Results 10 clinical practice guidelines and 10 systematic reviews/Meta-analysis were included. The clinical practice guidelines for the management of HAART were formed. Conclusions The formed clinical practice guidelines showed better applicability and higher general quality. It is recommended to use the guidelines in AIDS care.

Bonetumorisonekindofmalignanttumorwhichthreatslifeamongchildrenandadoles-cent.Osteosarcoma (OS ) and Ewing sarcoma (ES ) have been paid much attention by medical community because of their complex occurrence,development,metastasis mechanism and poor prognosis.The research showed that activation of protooncogene and mutation of suppressor gene,absence of apoptosis signal and hypox-ia-inducible factor (HIF)all participated in the occurrence,development and metastasis of OS and ES.It is suggested that clarified pathomechanism of bone tumors could provide more effective therapeutic strategies and lower the mortality of patients.
10.Predictive effect of microRNA ratio in osteosarcoma

Objective To explore the roles of biomarkers of the ratios of microRNA (miRNA)in osteosarcoma.Methods The blood samples from 20 patients with osteosarcoma and 30 healthy people with similar age were choosed.The miRNA expression profiles were filtered by gene chips of high flux.The results were verified by qRT-PCR.The miRNA expression ratio was analysed by receiver operating characteristic (ROC)curve aiming to find the most predictive potential combination-type markers.Results Two hundred and fifty-eight miRNAs were found to be significantly deregulated in blood samples from 30 patients with osteo-sarcoma as compared with their matching normal blood samples (F=5.564,P<0.05).Four miRNAs were choosed from blood samples of22 patients with osteosarcoma and normal blood samples.MiR-181b,miR-199b-5p and miR-451 were significantly higher than that in normal blood sample (F=6.283,P<0.05),while miR-124 showed low expression (F=7.201,P<0.05).Furthermore,the ratio of miR-199b-5p/miR-124 showed a high sensitivity (96%)and specificity (97%).Conclusion The abnormal expression of miRNA in osteosar-coma makes it have a predictive effect.The ratio of miR-199b-5p/miR-124 could be served as combination-type biomarkers in the diagnosis of osteosarcoma.
