1.Construction of diagnostic model of depression in insomnia patients based on polysomnography data

Objective To establish a diagnostic model for depression in insomnia patients by mining polysomnography (PSG) data of insomnia patients with machine learning algorithms,and to provide a scientific basis for the diagnosis of depression in insomnia patients. Methods According to the inclusion and exclusion criteria,2162 insomnia inpatients and outpatients who attended the Inner Mongolia Autonomous Region Mental Health Center from January to December 2023 and underwent polysomnographic monitoring were included,and depression was diagnosed using the International Statistical Classification of Diseases and Related Health Problems,10th version (ICD-10). The general condition and PSG data of the patients were collected. Six algorithms—logistic regression (LR),Support vector machines (SVM),Random forest (RF),Adaptive Boosting (AdaBoost),Extreme Gradient Boosting (XGBoost) and Naive Bayes (NB)—were used to build the diagnostic model of depression in insomnia patients after the patients' general condition and PSG data were gathered. Results Among the enrolled patients with insomnia,40.1% had comorbid depression. Among the six models,LR and RF exhibited the highest values of area under the curve (AUC) of receiver operating characteristic (ROC),at 0.825 and 0.823,respectively,indicating superior overall classification performance. Conclusion Logistic regression and random forest modeling have good diagnostic efficacy in the population of insomniacs with depression.
2.Training needs for clinical core competence of master in nursing specialist from the perspective of supervisors: a qualitative study

Objective:To explore the training needs for clinical core competence of master of nursing specialist (MNS) from the perspective of supervisors, providing reference for the development of future MNS clinical practice training programs.Methods:Using phenomenological research methods from qualitative research, purposive sampling was used to select 10 MNS supervisors from Jilin Province, Heilongjiang Province, Sichuan Province, and Zhejiang Province as research subjects for semi-structured interviews from May to July 2023. Colaizzi 7-step analysis method was used to extract themes.Results:Six themes were extracted, including the need to strengthen MNS ideological and political education, differences in clinical training needs and ability goals between fresh and non-fresh students, the need to enhance MNS clinical practice ability, clinical research should be a key training content, thinking ability training should be integrated throughout the entire clinical training process, and achievement transformation.Conclusions:Relevant training institutions should attach importance to the cultivation of MNS ideological and political education, specialized practical abilities, thinking abilities, clinical research, and achievement transformation abilities, distinguish the tendency of cultivating fresh and non-fresh students, and actively set up relevant courses to improve students' core competence and job competitiveness, and cultivate nursing expert talents that truly meet the needs of clinical development.
3.Development of a joint clinical practice teaching and assessment program based on core competency needs for master of nursing specialist postgraduates

Objective:To develop a joint clinical practice teaching and assessment program tailored to the clinical training needs of master of nursing specialist (MNS) postgraduates which focuses on core competency requirements.Methods:Totally 10 MNS postgraduate supervisors were selected by convenience sampling for semi-structured interviews between May and July 2023. Subsequently, a Delphi method was employed with 22 MNS postgraduate supervisors over two rounds of consultations from October to December 2023.Results:A total of 22 experts participated in the Delphi consultations, with an effective response rate of 100.00% (22/22) in both rounds. The expert authority coefficients were 0.822 and 0.833, respectively, for the two rounds. The Kendall's W for various levels of indicators ranged from 0.097 to 0.243 and 0.159 to 0.256, respectively ( P<0.01). The final training program included five primary indicators, 10 secondary indicators, and 26 tertiary indicators. Conclusions:The development process for the joint clinical practice teaching and assessment program for MNS postgraduates is scientific and reliable. The program can serve as a reference for the clinical practice training of MNS postgraduates.
4.Role of Sirtuins family proteins in ovarian aging

The decline of ovarian function due to aging is an irreversible natural physiological process, manifested by the progressive depletion of follicles and the decrease of oocyte quality in the ovary. Existing studies have shown that the Sirtuins family is involved in the development of ovarian aging through various pathways, such as regulation of nicotinamide adenine dinucleotide levels, oxidative stress, chromosome segregation and genomic stability. In this paper, we review the current roles of Sirtuins family proteins in ovarian aging and their molecular mechanisms.
5.Research on embryo development and clinical pregnancy outcomes in patients with ovarian hyperstimulation syndrome

Objective:To investigate the embryo development and clinical pregnancy outcomes in patients with ovarian hyperstimulation syndrome (OHSS) undergoing assisted reproductive technology (ART).Methods:A retrospective cohort study was conducted on data from 4 080 cycles of in vitro fertilization/intracytoplasmic sperm injection-embryo transfer treatments performed at the Department of Obstetrics and Gynecology, the First Affiliated Hospital of Anhui Medical University from August 2019 to August 2021. Patients were divided into OHSS group ( n=524) and non-OHSS group (control group, n=3 556) based on whether OHSS occurred, and the OHSS group was further divided into OHSS combined with polycystic ovary syndrome (PCOS) subgroup ( n=231) and OHSS combined with non-PCOS subgroup ( n=293) based on the presence of PCOS. General information, embryo developmental data and clinical outcomes were compared between the two groups. Results:1) Patients in the OHSS group [(30.7±3.6) years] were younger than those in control group [(31.5±4.8) years, P<0.001], and the number of retrieved oocytes (28.2±5.7), rates of high-quality embryos [52.7% (4 982/9 463)], blastocyst formation [54.0% (5 059/9 371)], biochemical pregnancy [75.0% (393/524)], clinical pregnancy [69.5% (364/524)], and live birth [58.0% (304/524)] were significantly higher in the OHSS group than in control group [12.5±6.7, 49.8% (14 042/28 204), 51.4% (14 279/27 797), 59.5% (2 115/3 556), 54.1% (1 924/3 556), 43.6% (1 550/3 556), respectively; all P<0.001]. 2) Patients in the OHSS combined with PCOS subgroup [(30.2±3.1) years] were younger than those in the OHSS combined with non-PCOS subgroup [(31.1±4.0) years, P=0.009], and the estradiol level [165.0 (101.0, 222.5) pmol/L] was higher than that in the OHSS combined with non-PCOS subgroup [141.0 (81.0,202.0) pmol/L, P=0.005]; rates of high-quality embryos [56.3% (2 413/4 284)], blastocyst formation [67.1% (2 846/4 239)], and high-quality blastocysts [57.7% (2 445/4 239)] were also significantly higher in the OHSS combined with PCOS subgroup than in the OHSS combined with non-PCOS subgroup [49.6% (2 569/5 179), 60.3% (3 092/5 132), 50.9% (2 614/5 132), respectively; all P<0.001]. Conclusion:There is a certain correlation between OHSS and female age. The occurrence of OHSS does not affect embryo development and does not increase adverse pregnancy outcomes in infertile patients. The presence of PCOS does not affect the pregnancy outcomes of OHSS patients. However, in ART, we still strive to avoid the occurrence of adverse events such as OHSS as much as possible.

7.Impact on analgesic efficacy and safety of esketamine as an adductor canal block adjuvant in knee arthroplasty

Objective:To explore the impact on analgesic efficacy and safety of esketamine as an adductor canal block adjuvant in knee arthroplasty.Methods:The study was a prospective, single center, randomized controlled trial. The subjects were patients who received knee arthroplasty in Zhangjiakou First hospital from October 2021 to October 2023. The patients were randomly divided into trial group and control group using a random number table method. The patients in the both groups received general anesthesia and postoperative patient-controlled intravenous analgesia (PCIA). The patients in the trial group were injected with 0.375% ropivacaine combined with esmketamine, while those in the control group were only injected with 0.375% ropivacaine. The postoperative vital signs, quadriceps femoris muscle strength, resting and dynamic visual analogue scale (VAS) pain score, use of PCIA, and incidence of postoperative adverse reactions in patients between the 2 groups were compared.Results:A total of 120 patients were included in this study, with 60 cases in each group. There were no significant differences in heart rate, mean arterial pressure, respiratory rate, and body temperature in patients between the trial group and the control group at 24 and 48 hours after operation (all P>0.05). There were no significant differences in quadriceps femoris muscle strength at 12, 24 and 48 hours after operation (all P>0.05). At rest, the VAS pain score at 12 hours after operation in the trial group was lower than that in the control group [(2.89±0.49) points vs. (3.36±0.23) points], the difference was statistically significant ( P<0.05). In the active state, the VAS scores at 12 and 24 hours after operation in the trial group were lower than those in the control group [(3.78±0.41) points vs. (4.11±0.45) points, (3.79±0.56) points vs. (4.19±0.69) points], the differences were statistically significant (all P<0.05). The number of times of PCIA compression 24 hours after operation in the experimental group was less than that in the control group [(5.35±1.03) times vs. (7.89±1.34) times], and the difference was statistically significant ( P<0.05), the sensory block time in the experimental group was longer than that in the control group [(488.26±57.75) min vs. (92.26±65.15) min], and the difference was statistically significant ( P<0.05). There were no significant differences in total incidence of adverse reactions including nausea, vomiting, urinary retention, dizziness, drowsiness, delirium and hallucination in patients between the 2 groups within 1 week after operation (all P>0.05). Conclusion:Esketamine as an adjuvant of ropivacaine for adductor block can alleviate the pain level of patients undergoing knee arthroplasty, and it has good safety.
