Objective To investigate the feasibility of granzyme B(GB)promoter-controlled ferritin heavy chain(FTH1)reporter gene expression for monitoring the activation status of chimeric antigen receptor T cells(CAR-T)by magnetic resonance imaging(MRI).Methods Cytotoxic T lymphocytes(CTLs)were screened by Ficoll density gradient centrifugation and flow sorting.The GB promoter and FTH1 gene were ligated together with disialoganglioside 2(GD2)CAR,and lentiviral vectors were transfected into CTLs to construct GD2-CAR-T/pGB-FTH1 cells.GD2-CAR-T/pCMV-FTH1,GD2-CAR-T,and T cells served as control cells.CytoTox96@non-radioactive cytotoxicity was used to detect the killing effect of each group of cells after co-culture with human neuroblastoma cells(SK-N-SH).ELISA was employed to detect the coincubation factor as well as the amount of GB secretion.Western blotting,Prussian blue staining and cellular MRI were applied to detect the expression of the FTH1 gene after co-culture.Results CTLs were successfully obtained,and then GD2-CAR-T/pGB-FTH1,GD2-CAR-T/pCMV-FTH1 and GD2-CAR-T cells were constructed.The killing effect,co-incubation factor and GB secretion of the above 3 groups of cells were significantly higher than those of the T cells,and the level of GB expression was highest at day 1,and then decreased in order at day 3 and day 7 after co-culturing with SK-N-SH cells.The relative expression of FTH1 and iron content of the GD2-CAR-T/pGB-FTH1 cells showed the same trend as GB expression,and the MRI signals were gradually increased.There were no significant differences in the relative expression of FTH1,iron content and MRI signals in the GD2-CAR-T/pCMV-FTH1 cells at all time points.No FTH1 expression or iron aggregation was observed in the GD2-CAR-T and T cells groups.Conclusion MRI based on the FTH1 reporter gene driven by the granzyme B promoter can reflect the GB expression level and tumor killing effect of CAR-T cells,which provides a potential real-time visual means to monitor the cell activation status for CAR-T therapy.

Objective To study the anesthetic effects of cervical plexus C4 transverse process block one-point method under ultrasound guidance in internal fixation of clavicle fracture.Methods Eighty patients with clavicle fracture admitted to Suqian Traditional Chinese Medicine Hospital from October 2019 to October 2021 were randomly assigned(1∶1)to control group,or experimental group according to radom number table.The control group was anesthetized by cervical plexus C4 transverse process block one-point method with blind exploration,while the experimental group was anesthetized by cervical plexus C4 transverse process block one-point method under ultrasound guidance.The completion time of nerve block and surgical time were compared between the 2 groups.The hemodynamics(heart rate[HR],mean artery pressure[MVP]),and analgesic effects(visual analogue scale[VAS])before anesthesia(T0),during surgical skin incision(T1),fracture reduction(T2),and during surgical skin suture(T3)were also compared between the 2 groups.The anesthetic efficacy and anesthesia-related adverse reactions were statistically analyzed.Results There were no significant differences in surgical time between the 2 groups(P>0.05).The completion time of nerve block in the experimental group was significantly shorter than that in the control group(P<0.05).There were significant differences in the levels of HR and MAP at different time points of the 2 groups,when comparisons were made between the 2 groups(P<0.05).There were no significant differences in the levels of HR or MAP,when comparisons were made between the 2 groups at T0(P>0.05).The levels of HR and MAP in the experimental group at T1,T2,and T3 were significantly lower than those in the control group(P<0.05).The levels of HR and MAP at T1,T2,and T3 in the control group were significantly higher than those at T0(P<0.05).However,there were no significant differences in the levels of HR or MAP,when comparisons were made at different time points in the experience group(P>0.05).There were significant differences in VAS scores,when comparisons were made between different groups,at different time points,at different time points between groups(P<0.05).There were no significant differences in VAS scores,when comparisons were made between the 2 groups at T0(P>0.05).The VAS scores of the experimental group at T1,T2,and T3 were significantly lower than those of the control group(P<0.05),and the VAS scores of the 2 groups at T1,T2,and T3 were significant differences lower than those at T0(P<0.05).The effective anesthetic rate of the experimental group was 95.00%and that of the control group was 82.50%,with statistical significance,when comparisons were made between the 2 groups(P<0.05).There were no singificant differences in the incidences of phrenic nerve block,recurrent laryngeal nerve block,or Horner syndrome,when comparisons were made between the 2 groups(P>0.05).The incidence of vascular injury in the experience group was significantly lower than that in the control group(P<0.05).Conclusion The application of cervical plexus C4 transverse process block one-point method under ultrasound guidance in clavicular fracture internal fixation can shorten the completion time of nerve block,maintain hemodynamic stability,improve analgesic effect,with good anesthetic effects.Furthermore,it can reduce the incidence of adverse reactions of vascular injury.

Objective: To introduce the application of free-style perforator flap based on aesthetic units to repair facial defect after tumor resection. Methods: By following the concept of free-style perforator flap and the principle of facial aesthetic unit, the design of a free-style perforator flap allowed over any nearby cutaneous vessel chosen purely on the characteristics of its Doppler signal. Conventional knowledge of anatomical landmarks and possible vascular variations were less relevant. A greater freedom in flap selection was gained to recover defect in different forms such as rotation flap, advanced flap and propeller flap, which were all based on free-style perforators. The flap size ranged from 1.5 cm×1.0 cm to 12.0 cm×6.0 cm with the perforator diameter of 0.3-3.0 mm in pedicle, and some of the pedicles are "perforator clusters" . Results: A total of 72 cases underwent surgery, and 68 cases survived completely with satisfactory appearance. 1 case healed two weeks later through dressing due to undesired healing, which result ed from high tension secondary postoperative blooding. 3 cases healed in a delay due to congestion and gained acupuncture treat. Conclusions The free-style perforator flap, which depended on Doppler-discerned perforator and facial aesthetic unit, represents safe, reliable and versatile for repairing facial defect after extended resection, and it not only offers a greater freedom in flap selection but also provides good aesthetic result.

Objective To analyze the characteristics of atherosclerosis in elderly patients with isolated systolic hypertension (ISH) combined with low diastolic blood pressure and risk factors related to low diastolic blood pressure (DBP).Methods A total of 447 elderly patients with untreated ISH were divided into the low DBP (DBP＜70 mmHg, 1 mmHg=0.133 kPa) group (n=206) and the non-low DBP (DBP＞ 70 mmHg) group (n=241) according the DBP level.Clinical data including brachial-ankle pulse wave velocity (baPWV), carotid artery ultrasonography, blood lipids, blood glucose, plasma homocysteine were collected and determined.baPWV, carotid intimal-medial thickness (IMT) and the incidence of carotid plaques were compared between the two groups.Risk factors for low DBP were analyzed.Results There were no significant difference in systolic pressure levels between the low DBP group and the non-low DBP group [(160.8±10.1) mmHg vs.(161.1-±-10.9) mmHg, P＞0.05].Pulse pressure, baPWV levels, IMT and the incidence of carotid artery plaque were higher in the low DBP group than in the non-low DBP group [(98.4 ±11.7) vs.(77.4±12.0) mmHg, (1 785.3±175.0) vs.(1 436.5±114.4), (1.2±0.2) mm vs.(1.1±0.3)mm and 78.2％ (161/206) vs.67.2％ (162/241), each P＜0.05].Stepwise logistic analysis showed that age, gender (female) and diabetes were independent risk factors for low DBP (OR=1.62, 2.07, 1.38, all P＜0.05).Conclusions Atherosclerosis is more serious in elderly patents with ISH combined with low diastolic blood pressure than those without low DBP.Age, gender (female) and diabetes are independent risk factors for low DBP.

With a survey through questionnaire among the students of the School of Economy and Management in Guangzhou University of Traditional Chinese Medicine in terms of students' attitude towards their majors,students' employment expectation,student's employment psychology as well as the employment channels,the essay analyzes the different employment trends among the students of the three majors in the school and offers some useful suggestions to enhance its work in employment guidance.
