Objective: To explore the consistency between self perceived body size and body mass index (BMI) evaluation among medical students, and analyze their association with depressive symptoms, so as to provide scientific evidence for psychological intervention targeting medical students. Methods: From September to November 2022, a convenient cluster sampling method was employed to select 1 850 college students from two universities in Hefei, Anhui Province, and Nanjing, Jiangsu Province. The Patient Health Questionnaire-9 (PHQ-9) was used to assess their depressive symptoms. Chi-square test was applied to compare the rates of depressive symptoms among medical students with different demographic characteristics. Kappa consistency test was performed to evaluate the consistency between self-perceived body size and BMI evaluation. Generalized linear models with binomial distribution and Logit link were constructed to analyze the associations of self-perceived body size and consistency between self-rated and BMI evaluation with depressive symptoms, as well as gender differences in these associations. Results: A total of 40.7 % of medical students overestimated or underestimated their body size. Specifically, the underestimation rate was 20.5% in males and 4.9 % in females, while the overestimation rate was 12.7% in males and 40.5% in females, the distribution of consistency in the misclassification rate of body self-perception between males and females was statistically significant ( χ 2=221.54, P ＜0.01). The overall consistency between self perceived body size and BMI evaluation was low for both males and females, with Kappa values of 0.518 and 0.281 respectively (both P <0.01). After adjusting for confounding variables, generalized linear model analyses showed that medical students with self perceived obesity and body size overestimation had higher risks of depressive symptoms ( OR =1.62, 1.26, both P ＜0.05). Stratified analyses by gender revealed that girls with self perceived overweight and obesity had higher risks of depressive symptoms in female students ( OR =1.43, 1.66, both P <0.05), whereas no statistically significant associations among boys ( OR =0.84, 1.86, both P >0.05). Girls with body size overestimation had a higher risk of depressive symptoms ( OR=1.20, P ＜ 0.05), with no significant association in boys ( OR=1.00, P >0.05). Conclusions Differences exist related between self perceived body size and BMI evaluation among medical students. Self perceived obesity and body size overestimation are related factors to depressive symptoms.

Objective:To investigate the rates of low disease activity and clinical remission in patients with systemic lupus erythematosus(SLE)in a real-world setting,and to analyze the related factors of low disease activity and clinical remission.Methods:One thousand patients with SLE were enrolled from 11 teaching hospitals.Demographic,clinical and laboratory data,as well as treatment regimes were collec-ted by self-completed questionnaire.The rates of low disease activity and remission were calculated based on the lupus low disease activity state(LLDAS)and definitions of remission in SLE(DORIS).Charac-teristics of patients with LLDAS and DORIS were analyzed.Multivariate Logistic regression analysis was used to evaluate the related factors of LLDAS and DORIS remission.Results:20.7％of patients met the criteria of LLDAS,while 10.4％of patients achieved remission defined by DORIS.Patients who met LLDAS or DORIS remission had significantly higher proportion of patients with high income and longer disease duration,compared with non-remission group.Moreover,the rates of anemia,creatinine eleva-tion,increased erythrocyte sedimentation rate(ESR)and hypoalbuminemia was significantly lower in the LLDAS or DORIS group than in the non-remission group.Patients who received hydroxychloroquine for more than 12 months or immunosuppressant therapy for no less than 6 months earned higher rates of LLDAS and DORIS remission.The results of Logistic regression analysis showed that increased ESR,positive anti-dsDNA antibodies,low level of complement(C3 and C4),proteinuria,low household in-come were negatively related with LLDAS and DORIS remission.However,hydroxychloroquine usage for longer than 12 months were positively related with LLDAS and DORIS remission.Conclusion:LLDAS and DORIS remission of SLE patients remain to be improved.Treatment-to-target strategy and standar-dized application of hydroxychloroquine and immunosuppressants in SLE are recommended.

Objective To analyze the current situation of healthcare-associated infection (HCAI) management of fever clinics among different levels of medical institutions in Wuhan, and to provide a scientific basis for improving hospital infection management. Methods In January 2023, a network questionnaire survey was conducted on medical institutions with fever clinics in Wuhan. Results A total of 72 medical institutions were investigated, of which 58.33% had CT, and 48.61% had fever clinics for children. The total qualified rate of HCAI management was 78.28%. The qualified rates of four primary indicators, including hospital management, diagnosis and treatment environment protection, training and education, and implementation of infection control measures, were 82.27%, 71.49%, 75.93%, and 81.31%, respectively. There were statistical differences among different levels of medical institutions (all P<0.01). Among the 13 secondary indicators, the qualified rates of 7 indicators were more than 80%, with the highest being medical item management (93.06%), medical waste disposal (89.72%), and personnel management (83.33%), and the lowest being facilities and equipment (66.32%), and patients and accompanying personnel education (66.67%). Among the 65 tertiary indicators, 30 had a pass rate great than 80%. Conclusion Wuhan actively promotes the construction of fever clinics in medical institutions, and the overall situation of HCAI management is good. However, there are still some problems to varying degrees, especially in the layout procedures, hand hygiene, and staff training of fever clinics in secondary and lower medical institutions, which should be further strengthened.

Objective:To validate and compare the value of the Status Epilepticus in Pediatric Severity Score (STEPSS) versus PEDSS in assessing the short-term prognosis of children with status epilepticus (SE).Methods:Clinical data of 152 children with SE hospitalized at the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2022 were retrospectively analyzed.According to the STEPSS and PEDSS scores, children with SE were scored and their prognosis was predicted.Receiver operating characteristic (ROC) curves of the 2 scales in assessing the short-term prognosis of SE in children were plotted, and the area under the curve (AUC), optimal cut-off, sensitivity and specificity were calculated, thus validating and comparing the value of the STEPSS versus PEDSS in assessing the short-term prognosis of children with SE.Results:Of the 152 children with SE, 90 were male and 62 were female, with the age of (5.8±3.9) years (1 month to 15 years). There were 112 cases with good prognosis and 40 cases with poor prognosis, involving 13 deaths.The AUC of STEPSS and PEDSS scores in predicting the death in children with SE were 0.908(95% CI: 0.848-0.967) and 0.887(95% CI: 0.831-0.942), respectively, both with the optimal cut-off value of 4.The sensitivity of STEPSS and PEDSS scores in predicting the death in children with SE were 0.740 and 0.846, respectively, and the specificity were 0.745 and 0.835, respectively.There was no significant difference in predicting the death in children with SE between the 2 scales ( P>0.05). In predicting adverse outcomes, the AUC of the STEPSS and PEDSS scores were 0.869(95% CI: 0.800-0.937) and 0.926(95% CI: 0.873-0.979), respectively, both with the optimal cut-off value of 3.The sensitivity of STEPSS and PEDSS scores in predicting adverse outcomes in children with SE were 0.827 and 0.900, respectively, and the specificity were 0.732 and 0.866, respectively.There was significant difference in predicting the adverse outcomes in children with SE between the 2 scales ( P<0.05). Conclusions:Compared with the STEPSS, the PEDSS has a higher application in predicting the short-term treatment outcome of children with SE, which can be used as a routine method to assess the prognosis of children with SE.

Objective To observe the effects of ginkgolides drops on anginal attacks and peripheral blood levels of microRNA-29 a(miR-29a)and intercellular adhesion molecule-1(ICAM-1)in patients with stable angina pectoris.Methods 78 patients with stable angina pectoris patients,who had visited the cardiology department of our hospital during the period of January 2022 to December 2023,were selected.The patients were divided in a Chinese medicine group or a conventional group by using the random number table method,39 in each group.The conventional group received conventional Western medicine treatment,while the Chinese medicine received ginkgo ketone ester drip pills on the basis of conventional Western medicine therapies.Occurrence of angina attack,treadmill electrocardiogram test indexes,blood rheology,6-minute walking distance,TCM symptom scores and changes in peripheral blood miR-29a and ICAM-1 were observed in the two groups.The rate of nitroglycerin discontinuation or reduction and the incidence of adverse reactions were counted.Results Before treatment,there was no statistically significant difference in occurrence of angina attacks between the two groups(P＞0.05).After treatment,the number of attacks per week,weekly glyceryl trinitrate uses,and duration of each attack decreased in both groups,with a greater decline in the Chinese medicine group than in the conventional group(P＜0.05).Before treatment,there was no statistically significant difference in the indicators of treadmill electrocardiogram test between the two groups(P＞0.05).After treatment,induction positive time,exercise termination time,and maximum ST depression time increased in both groups,with longer induction positive time and maximum ST depression time in the Chinese medi-cine group than in the conventional group,while exercise termination time,as compared with the baseline,did not differ statistically(P＞0.05).Maximum ST depression amplitude and ST recovery time decreased in both groups,with a greater decrease in the Chinese medicine group than in the conventional group(P＜0.05).Before treatment,there were no statistically significant differences in miR-29a,ICAM-1,and rheology between the two groups(P＞0.05).After treatment,ICAM-1 and whole blood high/low shear viscosity,FIB,platelet aggregation rate,and plasma viscosity decreased in both groups,with a bigger drop in the Chinese medicine group than in the conventional group(P＜0.05).As compared with the baseline,miR-29a did not differ statistically between the two groups(P＞0.05).Before treatment,there were no statistically significant differences in 6min walking distance and Chinese medicine symptom scores between the two groups.After treatment,6 min walking distance increased in both groups,with a greater increase in the Chinese medicine group than in the conventional group(P＜0.05);Chinese medicine symptom scores decreased in both groups,with a greater decrease in the Chinese medicine group than in the conven-tional group(P＜0.05).The glyceryl trinitrate reduction rate was higher in the Chinese medicine group(79.49%,31/39 cases)than in the conventional group(56.41%,22/39 cases),and the incidence of adverse reactions did not differ statistically between the Chinese medicine group and the conventional group(7.69%vs.5.13%,3/39 vs.2/39 cases;P＞0.05).Conclusions Ginkgolides drops for stable angina improve the degree of myocardial ischaemia and blood rheology,reduce angina attacks,lower ICAM-1 levels,and increase the rate of nitroglycerin discontinua-tion or reduction.

Objective To analyze the characteristics and rules of 65 cases of medical compensation for neonatal brain damage,and put forward countermeasures and suggestions to provide reference for improving obstetric diagnosis and treatment.Methods Using neonatal brain damage as the key word,the judgment documents published on China Judgments Online from June 2021 to June 2023 were retrieved.the data database was established with Excel,and the basic situation of the case,disability grade and appraisal opinions were statistically analyzed.Results A total of 79 cases of relevant judgment documents were retrieved,and 65 cases were included in the judgment documents,of which 90.77%occurred in the last 10 years(2011-2020).38.46%,15.38%and 46.16%occurred on Saturday to Sunday,Friday and Monday to Thursday,respectively.The proportion of medical institutions being judged by the court to bear 50%or more of the responsibility was 49.23%,and the proportion of less than 50%was 50.77%;The proportions of compensation amount of 1 million yuan and above,500,000-990,000 yuan,100,000-490,000 yuan and less than 90,000 yuan were 18.46%,12.31%,49.23%and 20.00%respectively.The main causes of responsibility were improper medical and,nursing techniques,negligence of medical staff,out-of-range diagnosis and treatment,defects in meidcal record keeping,insufficient risk notification,and other factors,which were 29.23%,20%,7.69%,9.23%,13.85%and 20%,respectively.The incidence rates in tertiary hospitals,secondary hospitals,primary hospitals or township hospitals and community hospitals were 52.31%,41.54%and 6.15%,respectively.The proportion of first degree disability,second degree disability,third degree and above disability and unknown grade of newborn were 18.46%,18.46%,21.54%and 41.54%,respectively.Conclusion The disability level of newborn brain damage compensation cases is relatively serious,which highlights the problems such as unreasonable allocation of obstetric medical resources,"obstetric weekend effect",insufficient implementation of obstetric diagnosis and treatment norms,and weak legal awareness of medical staff.Therefore,it is necessary to strengthen the prevention and treatment of neonatal brain damage in view of the above problems,reduce the occurrence of incidents,and help establish a harmonious doctor-patient relationship.

Objective To monitor the antifungal resistance of clinical isolates of Candida spp.in the East China region.Methods MALDI-TOF MS or molecular methods were used to re-identify the strains collected from January 2018 to December 2022.Antifungal susceptibility testing was performed using the broth microdilution method.The susceptibility test results were interpreted according to the breakpoints of 2022 Clinical and Laboratory Standards Institute(CLSI)documents M27 M44s-Ed3 and M57s-Ed4.Results A total of 3 026 strains of Candida were collected,65.33％of which were isolated from sterile body sites,mainly from blood(38.86％)and pleural effusion/ascites(10.21％).The predominant species of Candida were Candida albicans(44.51％),followed by Candida parapsilosis complex(19.46％),Candida tropicalis(13.98％),Candida glabrata(10.34％),and other Candida species(0.79％).Candida albicans showed overall high susceptibility rates to the 10 antifungal drugs tested(the lowest rate being 93.62％).Only 2.97％of the strains showed dose-dependent susceptibility(SDD)to fluconazole.Candida parapsilosis complex had a SDD rate of 2.61％and a resistance rate of 9.42％to fluconazole,and susceptibility rates above 90％to other drugs.Candida glabrata had a SDD rate of 92.01％and a resistance rate of 7.99％to fluconazole,resistance rates of 32.27％and 48.24％to posaconazole and voriconazole non-wild-type strains(NWT),respectively,and susceptibility rates above 90％to other drugs.Candida tropicalis had resistance rates of 29.55％and 26.24％to fluconazole and voriconazole,respectively,resistance rates of 76.60％and 21.99％to posaconazole and echinocandins non-wild-type strains(NWT),and a resistance rate of 2.36％to echinocandins.Conclusions The prevalence and species distribution of Candida spp.in the East China region are consistent with previous domestic and international reports.Candida glabrata exhibits certain degree of resistance to fluconazole,while Candida tropicalis demonstrates higher resistance to triazole drugs.Additionally,echinocandins resistance has emerged in Candida albicans,Candida glabrata,Candida tropicalis,and Candida parapsilosis.

To sum up integrative nursing experience of a case with chronic refractory wound formation caused by drug extravasation.The essentials of integrative nursing are:structured nursing intervention of"assessment-management-treatment"based on the Triangle of Wound Assessment;determination of the timing for integrative nursing according to the theory of TCM sores and ulcers;implementation of copper board scraping method to promote circulation of qi and blood;use of Huo-long Comprehensive Moxibustion Therapy to promote muscle regeneration.With the help of the cooperation of the multidisciplinary specialist nursing team,the wound was completely healed after 59 days of integrated traditional Chinese and Western medicine nursing interventions.

Objective:To investigate the role of dopamine receptors in the central amygdala (CeA) in reduction of the anxiety level by propofol in a mouse model of post-traumatic stress disorder (PTSD).Methods:Fifty-six SPF healthy adult male C57BL/6 mice, aged 10 weeks, weighing 20-25 g, were divided into 7 groups ( n=8 each) using a random number table method: control group (C group), PTSD group (P group), PTSD+ propofol group (PP group), PTSD+ fat emulsion group (PF group), PTSD+ propofol+ normal saline group (PPN group), PTSD+ propofol+ dopamine receptor D1 (DRD1) antagonist group (PP+ DRD1-Ant group), and PTSD+ propofol+ DRD2 antagonist group (PP+ DRD2-Ant group). The PTSD model was developed by continuous plantar electric shock for 3 days. Propofol 120 mg/kg was intraperitoneally injected after successful establishment of the model in PP group, and the equal volume of fat emulsion was intraperitoneally injected in PF group. In PPN group, PP+ DRD1-Ant group and PP+ DRD2-Ant group, the equal volume of normal saline, DRD1 antagonist hydrochloride and DRD2 antagonist eticlopride hydrochloride were injected in bilateral CeA regions, respectively, 30 min later the efficacy of drugs reached the peak, and then propofol 120 mg/kg was intraperitoneally injected. The anxiety levels were measured at 4 h (T 1) and day 3 after propofol injection (T 2) by the open field test and elevated cross maze test. Results:Compared with C group, the time spent entering the open and central areas was significantly shortened at T 1, 2, and the number of entering the open and central areas was decreased at T 1, 2 in P group ( P<0.001). Compared with P group, the time spent entering the open and central areas was significantly prolonged at T 1, the number of entering the open and central areas was increased at T 1 ( P<0.001), and no significant change was found at T 2 in PP group ( P>0.05), and no significant change was found in the aforementioned parameters at T 1, 2 in PF group ( P>0.05). Compared with PPN group, the time spent entering the open and central areas was significantly shortened at T 1, and the number of entering the open and central areas was decreased at T 1 in PP+ DRD2-Ant group ( P<0.001), and no significant change was found at T 1 in PP+ DRD1-Ant group ( P>0.05). Conclusions:Activation of DRD2 in the CeA is involved in the process by which propofol reduces the anxiety level of mice with PTSD.

Objective: The aim of this study was to establish a comprehensive concise health index (CHI) for evaluating adolescents, so as to provide a basis for determining the overall health status of adolescents in China. Methods: On the basis of a literature review and consensus among core researchers, adolescent CHI indicators in the following five dimensions were assessed:physical growth, physical fitness, common diseases, mental health and behavioral health. A total of 24 experts used an analysis hierarcgy process (AHP) to calculate the indicators subjective weights. In addition, from October to December of 2021, two regions, A and B were selected to conduct empirical research, and the CRITIC method was used to calculate the objective weights of the indicators. Finally, the weight coefficients were determined through the AHP-CRITIC combination weight method, and comprehensive evaluation was performed with the TOPSIS method. Results: Across academic period and genders, the combined weighted coefficients of the health indicators were as follows:BMI, 0.081-0.095; waist circumference, 0.070-0.081; relative grip strength, 0.101-0.108; myopia, 0.110-0.128; dental caries, 0.055-0.070; psychological symptoms, 0.240-0.262; physical exercise, 0.085-0.115; screen time, 0.097-0.111; and sleep duration, 0.086-0.103. The health index of middle school students in city A (0.626±0.065) was significantly higher than that in city B(0.613±0.066)( t=6.34, P <0.01). Conclusion The comprehensive adolescent CHI evaluation method has good consistency and application value, and may serve as a reference for adolescent health monitoring.
