1.Experience of Using Charcoal-Processed Traditional Chinese Medicine in the Treatment of Gynecological Hemorrhagic Disorders

Charcoal-processed traditional Chinese herbal medicine has various therapeutic effects, including astringing, hemostasis, anti-diarrhea, clearing heat, tonifying, and warming the interior. This paper summarizes the clinical application features, compatible experiences, dosages, and precautions for over 20 types of charcoal-processed herbal medicine in the treatment of gynecological bleeding disorders caused by dysfunctions such as dysfunctional uterine bleeding, endometriosis, uterine incision pseudocavity, and vaginal bleeding resulting from threatened miscarriage. The charcoal-processed herbal medicine include Huangqin (Scutellaria Baicalensis) Charcoal, Dahuang (Rheum Palmatum) Charcoal, Cebai (Platycladus Orientalis) Charcoal, Diyu (Sanguisorba Officinalis) Charcoal, Daji (Cirsium Setosum) Charcoal, Xiaoji (Cirsium Japonicum) Charcoal, Shengdi (Rehmannia Glutinosa) Charcoal, Aiye (Artemisia Argyi) Charcoal, Paojiang (Zingiber Officinale) Charcoal, Xuduan (Dipsacus Asper) Charcoal, Duzhong (Eucommia Ulmoides) Charcoal, Qiancao (Rubia Cordifolia) Charcoal, Puhuang (Typha Angustifolia) Charcoal, Shanzha (Crataegus Pinnatifida) Charcoal, Jingjie (Schizonepeta Tenuifolia) Charcoal, Xueyu (Carthamus Tinctorius) Charcoal, Zonglyu (Areca Catechu) Charcoal, Wumei (Prunus Mume) Charcoal, Shudahuang (Rheum Officinale) Charcoal, Lianfang (Nymphaea Alba) Charcoal, Mianmaguanzhong (Clematis Armandii) Charcoal, and Oujie (Nelumbo Nucifera) Charcoal.
2.Value of pulmonary transit time by contrast-enhanced echocardiography in evaluating cardiac dysfunction in patients with ST-elevation myocardial infarction after PCI

Objective:To explore the value of contrast-enhaoced echocardiography for measuring pulmonary transit time(PTT)in assessing heart failure after percutaneous coronary intervention(PCI)in acute ST-segment elevation myocardial infarction(STEMI)patients.Methods:From September 2023 to September 2024,120 patients with STEMI undergoing PCI at Hunan Provincial People's Hospital were prospectively selected and divided into a heart failure group( n=42)and a non-heart failure group( n=78)according to the guidelines. The differences in general clinical data,laboratory parameters,and echocardiographic parameters between the two groups were compared. The diagnostic efficacies of PTT,normalized PTT(nPTT),and N-terminal pro-B-type natriuretic peptide(NT-proBNP)were analyzed. Consistency between them and New York Heart Association(NYHA)heart function classification was tested. Results:Compared to the non-heart failure group,the NT-proBNP,PTT,and nPTT values in the heart failure group were significantly increased(all P<0.05). The area under the curve(AUC)of nPTT was 0.944,better than that of PTT and NT-proBNP(AUC=0.871,0.887). After K-means clustering reclassified patients into four levels based on nPTT values,nPTT classification showed moderate consistency with NYHA classification(Kappa=0.580, P<0.001),and nPTT differed significantly across NYHA classifications( P<0.05). Conclusions:PTT,as an echocardiographic index for assessing cardiac function,has similar diagnostic efficacy to NT-proBNP,the nPTT is even better. It shows moderate consistency with the NYHA classification and holds potential for differentiating overlapping NYHA grades. Importantly,it offers a fresh objective way to evaluate cardiac dysfunction after PCI in STEMI patients.

4.Clinical Experience of You Zhaoling in Treating Polycystic Ovarian Syndrome

This article summarized Professor You Zhaoling's clinical experience in the treatment of polycystic ovary syndrome(PCOS).It is believed that kidney deficiency and blood stasis are the basic etiology and pathogenesis,with liver and spleen disorders.Clinical treatment is based on tonifying the kidney and replenishing essence,promoting blood circulation and resolving stasis,supplemented by soothing the liver,strengthening the spleen,and resolving dampness;treatment should also be based on the patients'stage of illness,age,and body type;basic body temperature and B-ultrasound should be skillfully uses to monitor ovulation,and four methods of regulating bubbles were creatively created;an original hypothetical menstrual stimulation method for PCOS patients with menstrual disorders was created.Professor You's clinical experience has demonstrated the characteristics of TCM treatment,providing a new approach for the TCM treatment of PCOS.

Rosuvastatin(RVS)is an excellent drug with anti-inflammatory and lipid-lowering properties in the aca-demic and medical fields.However,this drug faces a series of challenges when used to treat atherosclerosis caused by hyperhomocysteinemia(HHcy),including high oral dosage,poor targeting,and long-term toxic side effects.In this study,we applied nanotechnology to construct a biomimetic nano-delivery system,macrophage membrane(M?m)-coated RVS-loaded Prussian blue(PB)nanoparticles(MPR NPs),for improving the bioavailability and targeting capacity of RVS,specifically to the plaque lesions associated with HHcy-induced atherosclerosis.In vitro assays demonstrated that MPR NPs effectively inhibited the Toll-like receptor 4(TLR4)/hypoxia-inducible factor-1α(HIF-1α)/nucleotide-binding and oligomerization domain(NOD)-like receptor thermal protein domain associated protein 3(NLRP3)signaling pathways,reducing pyroptosis and inflammatory response in macrophages.Additionally,MPR NPs reversed the abnormal distribution of adenosine triphosphate(ATP)-binding cassette transporter A1(ABCA1)/ATP binding cassette transporter G1(ABCA1)/ATP binding cassette transporter G1(ABCG1)caused by HIF-1α,promoting cholesterol efflux and reducing lipid deposition.In vivo studies using apolipoprotein E knockout(ApoE-/-)mice confirmed the strong efficacy of MPR NPs in treating atherosclerosis with favorable bio-security,and the mechanism behind this efficacy is believed to involve the regulation of serum metabolism and the remodeling of gut microbes.These findings suggest that the synthesis of MPR NPs provides a promising nanosystem for the targeted therapy of HHcy-induced atherosclerosis.
6.Application of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip replacement

Objective:To explore the application effect of multidisciplinary combined rehabilitation in patients with sarcopenia after total hip arthroplasty (THA) .Methods:A total of 114 patients with sarcopenia who underwent THA treatment in the Department of Joint Surgery of the First Affiliated Hospital of Zhengzhou University from May to December 2020 were selected as the research objects by the convenient sampling method. The patients were divided into the control group and the observation group by the random number table method, with 57 cases in each group. The control group received routine rehabilitation nursing, while the observation group received multidisciplinary combined rehabilitation intervention. Muscle related indexes were compared between the two groups before and after intervention. The short-form Fugl-Meyer motor function score and Harris score were used to compare the motor function of the affected limb and the recovery of hip joint function in the two groups after intervention.Results:After intervention, the whole body muscle mass, limb skeletal muscle mass, affected limb skeletal muscle mass and limb skeletal muscle mass index of patients in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . The Fugl-Meyer motor function of the affected limb in the observation group was better than that in the control group, and the difference was statistically significant ( P<0.01) . The total score of simplified Fugl-Meyer motor function score and Harris score of the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.01) . Conclusions:Multidisciplinary combined rehabilitation can effectively prevent postoperative muscle atrophy and muscle mass loss in THA patients, which is beneficial to the recovery of the motor function of the affected limb and hip joint function.
7. Analysis of the autoantibodies characteristics of 77 anti-soluble liver antigen positive patients with liver diseases

Objective:
To understand the characteristics and clinical significance of anti-soluble liver antigen antibody (anti-SLA) in patients with liver diseases.
Methods:
Serum samples from seventy-seven patients with anti-SLA were collected from Beijing You'An Hospital during the period between January 2010 and December 2018. Anti-SLA, anti-liver cytosol type 1 antibody (anti-LC1), anti-glycoprotein 210 antibody(anti-gp210) and anti-nuclear body protein sp100 antibody(anti-sp100) were detected by immunoblotting; indirect immunofluorescence assay used for detecting anti-nuclear antibody (ANA), anti-mitochondrial antibody (AMA), anti-smooth muscle antibody (SMA), and anti-liver kidney microsome antibody (anti-LKM). One-way analysis of variance was used to compare the ages of different anti-SLA groups. The non-parametric rank sum test was used to compare the liver function indexes and immunoglobulins in different intensity groups of anti-SLA.
8. Efficacy and safety of recombinant human thrombopoietin in adult patients with primary immune thrombocytopenia during the perioperative period

Objective:
To evaluate the efficacy and safety of recombinant human thrombopoietin (rhTPO) treatment for primary immune thrombocytopenia (ITP) patients during the perioperative period.
Methods:
Adult ITP patients who were refractory to first-line glucocorticoid therapy and underwent selective surgery were enrolled to be treated with rhTPO at the dosage of 1.5×104U/d subcutaneously during the perioperative period. rhTPO treatment would not be terminated until one of the following conditions occurred: ①Platelet counts met the requirement of surgery; ②Platelet counts were ≥100×109/L; ③Completed the 14 days of therapy. End points of the study were surgery rate, rhTPO therapy response rate, rescue therapy rate and adverse responses.
Results:
42 patients were enrolled from Jan. 1, 2016 to Jun. 30, 2018. 14 were male and 28 were female. The median age was 60 (25-73) years old. There were no newly diagnosed patients. 5 patients were persistent and 37 were chronic. 27 patients completed selective surgery. The surgery rate was 64.3% (27/42) . Among them, 13 patients were under local anesthesia and 14 under general anesthesia. Of 42 cases receiving rhTPO therapy. 31 patients achieved responses, The overall response rate was of 73.8%. Among them, 24 patients achieved CR. The CR ratio was 77.4% (24/31) . 7 achieved response. The response ratio was 22.6% (7/31) . 11 patients did not respond to rhTPO therapy. The non-response rate was 26.2% (11/42) . The median time to reach CR was 7 (3-16) days. The median time to reach the peak of platelet counts were 10 (3-21) days. rhTPO was used for a median of 7 (3-14) days. The median platelet counts of patients undergoing surgery before rhTPO therapy, before surgery and at day 7 after surgery were 33 (20-89) ×109/L, 125 (78-245) ×109/L and 72 (30-250) ×109/L, respectively. The median peak of platelet counts was 149 (101-466) ×109/L. No infection, bleeding, thromboembolism and therapy-related adverse responses occurred in the patients.
Conclusion
rhTPO for ITP patients during the perioperative period is safe and effective.
9.Predictive and prognostic value of monitoring lymphocyte subsets in peripheral blood before and after chemotherapy in patients with metastatic breast cancer

Objective:To detect the proportion of lymphocyte subsets in peripheral blood of the ad-vanced breast cancer patients before and after chemotherapy with docetaxel and thiotepa,as well as the association between the proportion of peripheral blood lymphocyte subsets with the response rate and prog-nosis.Methods:The proportions of lymphocyte subsets (CD3 +T cell,CD3 +/CD4 +T cell,CD3 +/CD8 +T cell,CD3 -/CD16 +56 +NK cell,CD3 +/CD16 +56 +T cell,CD19 +B cell,CD4 +/CD25 +T cell,CD8 +/CD28 -T cell,CD8 +/CD28 +T cell)in the peripheral blood of 86 patients were analyzed with flowcytometry before and after chemotherapy.The result was analyzed in combination with clinico-pathological data.Results:The proportion of regulatory T cells (Treg)after chemotherapy in the disease control patients decreased significantly compared with that of the progressive patients (P=0.034).The difference of the proportions of Treg before and after chemotherapy affected significantly the overall survi-val (OS).The OS of the patients with decreased proportion of Treg was significantly longer than that of the patients with increased proportion of Treg,which was 23.5 and 9.4 months respectively (P<0.05). Conclusion:The patients with decreased proportion of Treg after chemotherapy had higher response rate and better survival benefit.
10.Correlation study between illness uncertainty and coping style in recurrent or metastatic breast cancer ;patients

metastatic breast cancer , and analyze the correlation between illness uncertainty and coping style . Methods A total of 98 patients with recurrent or metastatic breast cancer were investigated using Chinese version of Mishel′s uncertainty in illness scale ( MUIS ) and medical coping mode questionnaire ( MCMQ ) . Results The average score of illness uncertainty in patients with recurrent or metastatic breast cancer was (62.50 ±10.89), and the level of illness uncertainty was intermediate .The score of coping style in patients with recurrent or metastatic breast cancer were higher than domestic norm .The scores of avoidance in coping style between two groups was significantly different (P<0.01).Partial correlation analysis indicated that the total score of illness uncertainty was negatively correlated with avoidance in coping style (P<0.05), positively correlated with resignation in coping style (P<0.01) and not correlated with confrontation in coping style (P>0.05).Conclusions Coping style of patients with recurrent or metastatic breast cancer is correlated with their illness uncertainty .Nurses should evaluate illness uncertainty in patients with recurrent or metastatic breast cancer and help them to take effective coping style so as to decrease illness uncertainty and improve therapeutic adherence and confidence in these patients .
