1.Clinical and epidemiological characteristics of human bocavirus in hospitalized children with acute lower respiratory tract infection at a hospital in Shanghai from 2021 to 2023

ObjectiveTo investigate the epidemiological and clinical characteristics of human bocavirus (HBoV) in hospitalized children with acute lower respiratory tract infection (ALRTI) at a single-center children’s hospital in Shanghai, thereby providing evidence for the diagnosis, treatment, and prevention of HBoV infection. MethodsA retrospective study was conducted on 19 537 hospitalized children with ALRTI at Shanghai Children’s Hospital from January 2021 to December 2023. Multiplex polymerase chain reaction (PCR) combined with capillary electrophoresis was used to detect HBoV and 12 other common respiratory viruses /atypical pathogens. The positive detection rate, demographic characteristics (sex, age), temporal distribution (year, season) of HBoV, as well as the clinical characteristics of severe and non-severe pneumonia were analyzed. ResultsThe overall HBoV-positive rate was 2.57% (503/19 537), with 59.44% (299/503) being single infections and 40.56% (204/503) being co-infections. The positive detection rate was significantly higher in boys than that in girls (2.78% vs 2.33%, χ²=3.88, P=0.049). The highest infection rate was observed in toddlers, followed by infants (χ²=379.57, P<0.001). The positive rate peaked in 2021 and reached its lowest point in 2023 (χ²=45.49, P<0.001), with epidemics mainly prevalent in summer and autumn. The main clinical symptoms were cough (90.06%, 453/503), fever (75.94%, 382/503), and wheezing (39.96%, 201/503). Children with severe pneumonia showed a higher incidence of wheezing compared with the non-severe group (P<0.001), while underlying diseases and co-infections had no significant association with disease severity (P>0.05). ConclusionHBoV was an important pathogen of ALRTI in children, predominantly affecting infants and toddlers, with higher susceptibility in boys and seasonal peaks in autumn and summer. The main clinical manifestations included cough, fever, and wheezing, with wheezing being more prevalent in children with severe pneumonia.
2.Association of polychlorinated biphenyl exposure with platelet parameters across different glycemic states: The moderating role of a healthy lifestyle

Background Platelet parameters are important indicators of cardiovascular risk, and environmental pollutants such as polychlorinated biphenyls (PCBs) may impair platelet function through oxidative stress. Objective To investigate the differential effects of single and mixed exposure to PCBs on platelet parameters among individuals with normal glucose tolerance (NGT), impaired fasting glucose (IFG), and type 2 diabetes mellitus (T2DM), and to evaluate the potential modifying role of a healthy lifestyle. Methods This study included 2249 participants (including 707 with NGT, 759 with IFG, and 783 with T2DM). Plasma PCB concentrations were measured using triple quadrupole gaschromatography-tandem mass spectrometry. Generalized linear regression was used to assess the associations between individual PCB congeners and platelet parameters. Quantile g-computation (QGC) and Bayesian kernel machine regression (BKMR) models were used to evaluate the overall effects of PCBs mixture exposure on platelet parameters across different glycemic states, as well as its interaction with healthy lifestyle score (HLS). Results Generalized linear regression analyses showed significant differences in the effects of PCBs on platelet parameters across different glycemic states (P<0.05). After adjusting for confounders, PCBs mixture exposure was significantly associated with lower platelet counts (PLT) in individuals with NGT (b=−10.60, 95%CI: −16.48, −4.71) and IFG (b=−12.91, 95%CI: −18.90, −6.92), whereas no significant association was observed in individuals with T2DM (P=0.051). Mean platelet volume (MPV) and platelet-large cell ratio (P-LCR) increased significantly with higher PCBs exposure levels across all three groups (P<0.05). BKMR analysis showed a positive association between PCBs mixture exposure and P-LCR, with the strongest association observed in the NGT group. Furthermore, a significant interaction was observed between HLS and PCBs mixture exposure, and a higher HLS attenuated the effects of PCBs on P-LCR. Conclusion Glycemic glycemic states may modify the effects of PCBs on platelets. Individuals with NGT appear more sensitive to PCBs exposure, whereas the T2DM state may attenuate this effect. Moreover, healthy lifestyles, including not smoking, moderate alcohol consumption, maintaining moderate-to-high physical activity, a healthy diet, and an appropriate body mass index (BMI), may mitigate the adverse effects of most PCBs on platelet parameters.
3.Status and correlations of psychological distress, job satisfaction, and sleep quality among prehospital emergency medical personnel in Guangzhou

However, in-depth research on the interactions among these factors is lacking. Objective To assess the status of psychological distress, job satisfaction, and sleep quality of PEMP in Guangzhou and to explore the mediating role of sleep quality in the relationship between psychological distress and job satisfaction. Methods From February to May 2025, 1085 PEMP from "120" emergency network hospitals in Guangzhou were selected using convenience sampling. Data were collected via the General Information Questionnaire, Kessler Psychological Distress Scale, Minnesota Satisfaction Questionnaire, and Pittsburgh Sleep Quality Index. Statistical analyses were performed using SPSS 25.0, and The mediation model of sleep quality in linking psychological distress and job satisfaction was constructed using AMOS 28.0. The bias-corrected Bootstrap method was employed to assessed the significance of the mediating effect. Results A total of 1063 valid responses were received (97.97% valid response rate). The mean scores were: psychological distress (27.99±10.75), job satisfaction (69.45±15.84), and sleep quality (9.82±4.47). Significant differences in the three scores were found across gender, age, monthly night shift frequency, and hospital grade (P<0.05). Higher job satisfaction was linked to lower psychological distress and better sleep quality and its dimensions, while psychological distress directly correlated with poorer sleep quality (P<0.01). Sleep quality partially mediated the relationship between psychological distress and job satisfaction, with a mediating effect of −0.195, accounting for 43.62% of the total effect. Conclusion The participants report moderate psychological distress, moderate-to-high job satisfaction, and poor sleep quality. Psychological distress directly affects job satisfaction and indirectly through its impact on sleep quality. Interventions aimed at improving sleep health and mental health are essential to improve personnel well-being and work efficiency.

6.Current awareness and optimization recommendations regarding the pneumonia with unknown etiology surveillance system among healthcare professionals in Shanghai

ObjectiveTo investigate the current awareness and optimization recommendations on pneumonia with unknown etiology (PUE) surveillance among Shanghai healthcare professionals, and to provide evidence for the improvement of the surveillance system. MethodsIn December 2024, healthcare professional participants from diverse medical and health institutions were randomly selected within each stratum using a stratified cluster sampling method in all 16 districts of Shanghai for questionnaire surveys on their knowledge and suggestions for optimization of the PUE surveillance system. Descriptive statistics analyses, multivariate logistic regressions, and content analyses were used for data analyses. ResultsA total of 385 survey subjects were involved in the study, including 119 professional staffs from disease prevention and control centers and 266 from medical institutions. Those who understood the case definition for PUE correctly accounted for 67.01% (258/385). Multivariate regression model analyses showed that female gender (aOR=2.22, 95%CI: 1.36‒3.66), professionals from disease control institutions (aOR=2.02, 95%CI: 1.12‒3.72), and those who self-assessed as being familiar with the monitoring system (aOR=2.18, 95%CI:1.23‒3.89) had better understanding of the definition. Overall, 89.61% (345/385) of the survey subjects acknowledged the necessity of the surveillance system. Small number of cases meeting the case definition (54.46%, 214/379), insufficient diagnostic awareness (39.84%, 151/379), and lack of incentives for case reporting (29.29%, 111/379) were major problems for the current surveillance system, with variations observed across different types of institutions. Suggested segments for optimizing the surveillance system included monitoring purpose, case definition, form of monitoring and reporting procedure, organization mechanism, and epidemiological investigation and response. ConclusionThe necessity of PUE surveillance is commonly recognized among healthcare workers. However, the existing system falls short in terms of mastery of case definition and overall surveillance adaptability. There is an urgent need for optimization and improvement to enhance the capacity for preventing and controlling emerging and sudden respiratory infectious diseases.
7.Influencing factors of work-related musculoskeletal disorders in the three body parts among workers of a power grid enterprise

Objective To investigate the prevalence and influencing factors of work-related musculoskeletal disorders (WMSDs) in neck, shoulders and lower back among power grid workers. Methods A total of 371 workers from a power grid enterprise in Guangdong Province were selected as the research subjects using the convenience sampling method. A cross-sectional survey of WMSDs in the neck, shoulders and waist of the workers was conducted using the modified Musculoskeletal Disorders Questionnaire. Results The overall prevalence of WMSDs among the power grid workers was 62.8%, with prevalence of 37.2%, 33.7% and 32.1% for WMSDs in neck, shoulders and lower back, respectively. Binary logistic regression analysis showed that power grid workers who did not maintain a good sitting posture or were unable to maintain good sitting posture over prolonged periods had a higher risk of neck WMSDs than those who maintained a good sitting posture (both P<0.01). A higher frequency of head-down posture during work was associated with an increased risk of neck WMSDs (both P<0.05). Female workers had a higher risk of shoulder WMSDs than male workers (P<0.05). A higher frequency of early awakening with inability to fall back asleep, as well as a higher frequency of head-down posture during work, were both associated with an increased risk of shoulder WMSDs (all P<0.05). Workers with a sedentary habit had a higher risk of shoulder WMSDs than those without (P<0.01). Workers with insufficient rest time had a higher risk of shoulder WMSDs than those with sufficient rest time (P<0.01). Both maintenance workers and managers had a higher risk of lower back WMSDs than workers in other occupational categories (all P<0.05). Workers who experienced nocturnal awakening with difficulty falling back asleep at frequency of several times per year, ≥1 time /month, ≥1 time /week, had higher risk of lower back WMSDs than those who never experienced such problem (all P<0.05). Workers with a sedentary habit and those uncertain about having a sedentary habit both had a higher risk of lower back WMSDs than those without (both P<0.05). Greater forward neck flexion during work was associated with an increased risk of lower back WMSDs (both P<0.05). Workers who occasionally tilted their necks to the side while working had a higher risk of lower back WMSDs than those who did not (P<0.05). Conclusion WMSDs are highly prevalent among power grid workers, particularly affecting the neck, shoulder, and waist. Influencing factors include individual characteristics, behavioral habits, work organization, work posture, and workload. Maintaining proper posture and adequate rest may reduce the risk of WMSDs.
8.Effect of scalp acupuncture combined with computer-assisted training on memory impairment after stroke

Objective To observe the effect of scalp acupuncture points including Shenting(GV24),Benshen(GB13)and Sishen-cong(EX-HN1)combined with computer-assisted training on memory impairment after stroke.Methods From May,2023 to December,2024,62 patients with post-stroke memory dysfunction who received rehabilita-tion treatment in Beijing Bo'ai Hospital were selected and divided into control group(n=31)and observation group(n=31)randomly.Both groups received conventional treatment and computer-assisted training,while the observation group received additional scalp acupuncture treatment,for four weeks.They were evaluated with Montreal Cognitive Assessment(MoCA),auditory memory span,and modified Barthel Index(MBI)before and after treatment.Results No adverse reaction occured during treatment.After treatment,the total score of MoCA and the memory dimen-sion score,and auditory memory span score improved in both groups(|t|>3.838,P<0.001),and the d-value was more in the observation group than in the control group(|t|>2.160,P<0.05);the score of MBI improved in both groups(|t|>7.471,P<0.001),however,there was no significant difference between two groups(P>0.05).Conclusion Computer-assisted training could significantly improve the cognitive function of patients with post-stroke memory dysfunction,especially the memory function,and is more effective while combining with scalp acupunc-ture.
9.Correlations between post-doctoral talent cultivation and high-quality development of public hospitals

Objective To explore the role of postdoctoral talent cultivation in high-quality development of public hospi-tals.Methods The data of 165 postdoctoral fellows were analyzed from Zhongshan Ophthalmology Center,Sun Yat-sen Univer-sity over the past decade.The analysis involved examining the entire management process from recruitment to completion,inclu-ding the improvement of relevant systems,evaluation standards,implementation of hierarchical management,mentor-student in-teractions,and academic exchanges,as well as studying the quality enhancement of the postdoctoral talent pool,research output,and employment analysis.Results The standardized management of whole postdoctoral cultivation process and the continuous improvement of systems significantly enhanced the achievements of postdoctoral fellows in research,publications,and commer-cialization of the scientific and research findings.For instance,from 2018 to 2022,the number of National Science Foundation-funded programs that the postdoctoral fellows obtained accounted for 26.7%of the total number,while the number of provincial and ministerial funded programs accounted for 73.3%.In 2020,the proportion of postdoctoral researchers who obtained the Na-tional Natural Science Foundation of China's Youth Program was 80%of the total.In 2021,the proportion of postdoctoral fellows who published papers with an impact factor of above 10 as the first author or co-first author was 45.83%.Conclusion The training of high-level medical postdoctoral talents is crucial for high-quality development of public hospitals,improving interdisci-plinary adaptability of postdoctoral fellows and collaborative mentorship awareness.
10.Distribution of van genes and sequence typing of Enterococcus faecium based on NCBI database

Objective To analyze the global distribution characteristics,sequence typing(ST),and the distribution of van genes in Enterococcus faecium globally to provide reference for the prevention,control and clinical treatment of glycopeptide-resistant strains in-fection.Methods This study used Aspera software to download all Enterococcus faecium genome sequence data from NCBI in batches up to December 7,2023.The nucleotide sequences and strain metadata of all genomes were then extracted from the downloaded GBK files using a Perl script.The sequence data of van resistance genes were obtained from the NCBI Pathogen Antimicrobial Resistance Gene Database and analyzed using BLASTN software to determine their distribution across genomes.For the ST analysis,allele se-quence files for seven housekeeping genes of Enterococcus faecium were downloaded from pubMLST website,and all of the strains were sequence typed using a self-developed ST_tool.Results A total of 2 781 Enterococcus faecium genomes were obtained,with isolation dates ranging from 1920 to 2023,showing an increasing peak of isolation rates observed in 2019.In terms of geographic distribution,China,the United States,and New Zealand accounted for 15.61%(434 strains),12.87%(358 strains),and 5.57%(155 strains)of the isolates,respectively,and ranked in the top three.A total of 771 strains originated from human samples,primarily isolated from u-rine(168 strains),feces(154 strains),and rectal swabs(69 strains).In terms of ST,2 781 Enterococcus faecium strains were clas-sified into 359 STs,with ST16(188 strains,7.29%),ST6(178 strains,6.90%),and ST179(150 strains,5.82%)being the pre-dominant types.On the national level,the dominant STs in the United States,China,and New Zealand were ST6(10.71%),ST16(9.59%),and ST108(67.10%),respectively.Additionally,282 Enterococcus faecium strains carried glycopeptide resistance genes,mainly vanA(207 strains)and vanB(72 strains),with ST108(100 strains)and ST6(43 strains)being the predominant types asso-ciated with vanA and vanB,respectively.Conclusion There are significant regional differences in the distribution of Enterococcus fae-cium globally.The vanA and vanB glycopeptide resistance genes are prevalent among resistant strains and exhibit a trend toward multi-drug resistance evolution,highlighting the need for enhanced clinical monitoring and control measures.
