1.Mechanism of Medicated Serum of Buyang Huanwutang in Inhibiting Endothelial-to-mesenchymal Transition of Human Pulmonary Artery Endothelial Cells Based on Wnt1/β-catenin Signaling Pathway

ObjectiveTo investigate the role of the Wnt1/β-catenin signaling pathway in the intervention of medicated serum of Buyang Huanwutang (BYHWT) in endothelial-to-mesenchymal transition (EndMT) of human pulmonary artery endothelial cells (HPAECs) as well as its related mechanisms. MethodMedicated serum of BYHWT was prepared by gavage to New Zealand rabbits with a dosage of 53.36 g·kg-1·d-1 after decocting the medicine as usual. In addition, the same volume of normal saline was used to prepare blank serum. The HPAECs were cultured in vitro, and then induced by the transforming growth factor-β1 (TGF-β1) to establish the EndMT model. Five groups were established: blank group (10% blank serum), model group (TGF-β1+10% blank serum), low-dose BYHWT group (TGF-β1+2.5% medicated serum+7.5% blank serum), medium-dose BYHWT group (TGF-β1+5% medicated serum+5% blank serum) and high-dose BYHWT group (TGF-β1+10% medicated serum). Through Western blot, the expressions of Wnt1, β-catenin, and glycogen synthase kinase-3β (GSK-3β) were detected. In order to further clarify the mechanism of the Wnt1/β-catenin signaling pathway in the intervention of the medicated serum of BYHWT in inhibiting EndMT, the overexpression of β-catenin was confirmed by polymerase chain reaction after plasmid of overexpression β-catenin was constructed and transfected into the HPAECs. The HPAECs were intervened by 10% medicated serum with the optimal effect in previous studies. Then, they were divided into another five groups: the blank group (10% blank serum), the model group (TGF-β1+10% blank serum), the BYHWT group (TGF-β1+10% medicated serum), the BYHWT+overexpression plasmid control group (TGF-β1+10% medicated serum+blank plasmid) and the BYHWT+β-catenin overexpression plasmid group (TGF-β1+10% medicated serum+β-catenin). Apart from that, cell proliferation ability was detected by the methyl thiazolyl tetrazolium (MTT) method and cell migration ability by scratch assay and Transwell assay together. Immunofluorescence was adopted to detect the expressions of platelet endothelial cell adhesion molecule (PECAM-1/CD31), vascular endothelial cadherin (VE-cadherin), fibroblast-specific protein 1 (FSP1), and α-smooth muscle actin (α-SMA). ResultIn comparison to the blank group, the expressions of Wnt1 and β-catenin were significantly increased (P<0.01) while the expression of GSK-3β significantly decreased (P<0.01) in the model group. In comparison to the model group, the expressions of Wnt1 and β-catenin were significantly decreased (P<0.01) while the expression of GSK-3β was significantly increased (P<0.01) in the high-dose BYHWT group. The expression of β-catenin was significantly decreased (P<0.01) while the expression of GSK-3β was significantly increased (P<0.01) in the medium-dose BYHWT group. There was no significant difference in these indexes of the low-dose BYHWT group. In comparison to the blank group, proliferation and migration abilities were remarkably increased (P<0.01) and the immunofluorescence intensities of CD31 and VE-cadherin were decreased, while those of FSP1 and α-SMA were increased in the model group. In comparison to the model group, proliferation and migration abilities were significantly decreased (P<0.01) and the immunofluorescence intensities of CD31 and VE-cadherin were increased, while those of FSP1 and α-SMA diminished in the BYHWT group. Beyond that, the change trend of those indexes in the BYHWT+β-catenin overexpression plasmid group was consistent with that in the model group. In comparison to the BYHWT+overexpression plasmid control group, proliferation and migration abilities were significantly increased (P<0.01) and the immunofluorescence intensities of CD31 and VE-cadherin were decreased, while those of FSP1 and α-SMA were increased in the BYHWT+β-catenin overexpression plasmid group. ConclusionMedicated serum of BYHWT can inhibit EndMT of HPAECs by the Wnt1/β-catenin signaling pathway.

A novel technology is proposed for non-contact and real-time detection of atrial fibrillation using millimeter-wave radar.A 60 GHz PCR millimeter wave radar is used to continuously detect the chest echo signal of the subject.After signal acquisition,I-Q signal is generated through I-Q demodulation,and the signal phase information is extracted using effective points phase trend evaluation for obtaining the signals from oscillations in the chest wall,from which the respiratory signals and cardiac signals are extracted through digital filtering for the analysis of cardiac movement.Whether the atrial fibrillation occurs or not is determined by the characteristics of atrial fibrillation wave in the time domain.The effective points phase trend evaluation for extracting more accurate signal phase information and the time-domain method for real-time atrial fibrillation detection are the innovations of the study.The experimental results show that the proposed method achieves a detection accuracy of 99.2%in clinic.

4.The influence of patellofemoral joint degeneration on outcome of Oxford medial unicompartmental knee arthroplasty

Objective:To explore the influence of lateral patellofemoral joint degeneration on the treatment of anteromedial osteoarthritis of knee joint by Oxford medial unicompartmental knee arthroplasty.Methods:The clinical data of 73 patients (73 knees) with knee osteoarthritis underwent unicompartmental knee arthroplasty at Department of Orthopaedic Surgery, Xuanwu Hospital, Capital Medical University from March 2016 to December 2017 were analysed respectively.There were 18 males and 55 females, aged (68.6±7.5) years(range: 53 to 89 years).The lateral patellofemoral joints of patients were evaluated by Ahlback grading system. Patients with Ahlback 0 andⅠ were in the non degenerative group (37 cases), and those with Ahlback Ⅱ and above were in the degenerative group (36 cases). Hospital for special surgery knee score(HSS) and the Western Ontario and McMaster Universities(WOMAC) osteoarthritis index, as well as the condition of kneeling, sit to stand movement, up stair and down stair were recorded. The data before and after operation were compared by paired sample t test, and the data between groups were compared by independent sample t test. χ 2 test was used for counting data. Pearson correlation analysis was used to compare the correlation between ahlback score, HSS and WOMAC osteoarthritis index. Results:The follow-up time was (35.1±6.6) months (range: 25 to 47 months).The knee function of the patients improved significantly after operation.The HSS score increased from 57.7±11.8 preoperative to 81.8±7.8 postoperative ( t=16.64, P=0.00) and WOMAC osteoarthritis index decreased from 48.9±13.4 preoperative to 15.6±8.8 postoperative ( t=20.48, P=0.00). There was no statistical difference in the change of HSS between the degenerative group and the non-degenerative group before and after surgery(27.5±12.2 vs. 22.5±12.3, t=-1.65, P=0.10) as well as the change of WOMAC osteoarthritis index(31.8±14.0 vs. 36.4±13.7, t=-1.35, P=0.18), but the lateral patellofemoral joint degeneration was related to inability to complete squats (χ 2=5.17, P=0.04) and sitting up (χ 2=7.22, P=0.01). Conclusion:The degeneration of lateral patellofemoral joint has no effect on the early functional recovery of patients with anteromedial knee osteoarthritis after Oxford medial unicompartmental knee arthroplasty.

6.Effects of Buyang Huanwu Decoction on Endothelial-mesenchymal Transformation of Lung Tissue in Idiopathic Pulmonary Fibrosis Model Rats and Mechanism Study

OBJECTIVE: To investigate the effects of Buyang huanwu decoction on endothelial-mesenchymal transformation (EndMT) of lung tissue in idiopathic pulmonary fibrosis (IPF) model rats, and to explore its potential mechanism. METHODS: Male SD rats were randomly divided into normal group, model gorup, dexamethasone group [0.405 mg/(kg·d)], Buyang huanwu decoction low-dose, medium-dose and high-dose groups [6.435, 12.87, 25.74 g/(kg·d), by raw material], with 8 rats in each group. Except for normal group, other groups were given endotracheal injection of bleomycin to induce IPF model. On the second day after modeling, normal group and model group were given water intrgastrically [10 mL/(kg·d)]; administration groups were given relevant medicine intragastrically, once a day, for consecutive 28 days. 24 h after last medication, the expression of endothelial cell markers [platelet endothelial cell adhesion molecule 1, vascular endothelial cell cadherin] and interstitial cell markers [α-smooth muscle actin, fibroblast specific protein 1] were detected by immunohistochemistry method. The expression of Notch4 and DLL4 in lung tissue of rats were detected by Western blotting assay. RESULTS: Compared with normal group, the expression of endothelial cell markers were decreased significantly in lung tissue of model group, while the expressipon of interstitial cell markers, Notch4 and DLL4 were increased significantly (P<0.01). Compared with model group, the expression of endothelial cell markers in lung tissue of rats were increased significantly in administration groups, while Buyang huanwu decoction low-dose group was significantly lower than dexamethasone group; the expression of interstitial cell markers, Notch4 and DLL4 were decreased significantly, while Buyang huanwu decoction low-dose group was significantly higher than dexamethasone group (P<0.05 or P<0.01). CONCLUSIONS: Buyang huanwu decoction can relieve IPF of model rats by intervening in EndMT, the mechanism of which may be associated with inhibiting DLL4/Notch4 singaling pathway.
7.Impact of Cardiac Contractility Modulation on Myocardial Remodeling in Rabbit Model of Chronic Heart Failure

Objective: To observe the impact of cardiac contractility modulation (CCM) on myocardial remodeling in rabbit model of chronic heart failure (CHF) with its possible mechanism. Methods: Rabbit HF model was established by ascending aortic root ligation; the animals were divided into 3 groups: Sham group, the animals received thoracotomy without aortic ligation, HF group and HF+CCM group, the HF animals received CCM treatment for 4 weeks. n=10 in each group. Cardiac function was measured by echocardiography at 12 and 16 weeks in each group respectively; myocardial tissue fibrosis and pathological changes were examined by Masson staining; plasma BNP level was assessed by ELISA; protein expressions of collagen I, collagen II, MMP2,MMP9, TIMP1 and galectin-3 in myocardial tissue were determined by Western blot analysis. Results: ① By echocardiography: with 12 weeks treatment, compared with Sham group, HF group and HF+CCM group had increased LVESD, LVEDD and decreased LVFS, LVEF, all P<0.05; with 16 weeks treatment, compared with HF group, HF+CCM group had improved LVESD, LVEDD, LVEF and LVFS, all P<0.05. ② Pathological changes:compared with Sham group, HF group showed increased collagen content in myocardial tissue, P<0.05; CCM treatment could partially decrease collagen accumulation, P<0.05. ③ After 12 weeks treatment, compared with Sham group, HF group and HF+CCM group presented elevated plasma BNP level, P<0.05; after 16 weeks treatment, compared with HF group, HF+CCM group presented reduced plasma BNP, while it was still higher than that in Sham group, P<0.05. ④ By Western blot analysis: compared with Sham group, HF group demonstrated increased protein expressions of collagen I, collagen II, MMP2, MMP9, TIMP1 and galectin-3 in myocardial tissue; the above indexes were much lower in HF+CCM group while still higher than those in Sham group, all P<0.05. Conclusion: CCM could improve myocardial remodeling in rabbit model of CHF which might be related to down-regulated protein expressions of collagen I, collagen III, MMP2, MMP9, TIMP1 and galectin3 in myocardial tissue.

Contrast induced nephropathy (CIN)is one of the main complications caused by intravenous iodinated contrast media injection,which can increase hospitalization rate and mortality of patients,and it has become the third main cause of acquired acute renal failure in hospital currently, but its fundamental pathogenesis is still not clear,which is considered to be related to inflammation and oxidative stress response etc.The present article made a review on pathogenesis of CIN and preventive effects of statins on CIN.
9. RESEARCH PROGRESS OF PATHOGENESIS MECHANISM OF SPINAL DEFORMITY IN NEUROFIBROMATOSIS TYPE 1

OBJECTIVE: To review the research progress of pathogenesis mechanism of spinal deformity in neurofibromatosis type 1 (NF1). METHODS: Recent literature concerning the pathogenesis mechanism of spinal deformity in NF1 was extensively reviewed, and current developments of the correction of spinal deformity and NF1 and the pathogenesis mechanism were summarized. RESULTS: The pathogenesis mechanism of spinal deformity in NF1 is not yet clearly known. Current theories include erosion and stress of neurofibromas, melatonin-related decreased contractility of paraspinal muscles, osteopenia and osteoporosis, sexual precocity and mesoderm dysplasia. CONCLUSIONS: The clinical manifestations of NF1 may cause the spinal deformities in patients with NF1. The research of pathogenesis mechanism of spinal deformity in NF1 will be conducive to further understanding, diagnosis and treatment of NF1-related spinal deformity.
10.Therapeutic effect and safety of atorvastatin sequential treatment on patients with acute coronary syn- drome undergoing PCI

Objective:To observe therapeutic effect and safety of six‐month atrovastatin sequential treatment on pa‐tients with acute coronary syndrome (ACS) undergoing PCI .Methods :A total of 280 ACS patients were consecu‐tively enrolled ,randomly and equally divided into sequential treatment group (received atorvastatin 80mg 2d before PCI operation ,then 40mg/d after operation ,20mg/d a month later ) and routine treatment group (no taking atorv‐astatin before PCI operation ,then 20mg/d after operation and as a maintenance dose ) .Both groups were observed for six months ,therapeutic effect and safety were compared between two groups .Results:Compared with before PCI ,there were significant rise in levels of creatine kinase isoenzyme (CK‐MB) and cardiac troponin I (cTnI) after PCI 18h~24h in two groups ,and those of sequential treatment group were significantly lower than those in routine treatment group [CK‐MB:(2.72 ± 0.52) μg/L vs .(6.04 ± 0.51) μg/L ,cTnI : (0.28 ± 0.13) μg/L vs .(0.42 ± 0.16) μg/L , P < 0.01 both ];Compared with routine treatment group , the high sensitive C reactive protein (hsCRP) level significantly reduced in sequential treatment group after PCI 2d ,and continued until post‐operation six months [ (2.34 ± 0.51) mg/L vs .(1.49 ± 0.78) mg/L ,P=0.001];the blood lipids levels (except level of high density lipoprotein cholesterol significantly rose ) in sequential treatment group were significant lower than those of routine treatment group , P<0.01 all;there were no significant difference in percentage of transaminase level high‐er than normal three times and incidence rates of major adverse cardiovascular events (MACE) between two groups , P>0.05 all .Conclusion:Preoperative 80mg atrovastatin combined postoperative 40mg atrovastatin treatment can significantly reduce inflammatory reaction ,improve blood lipids ;compared with routine dose ,it doesn't significant‐ly increase adverse drug reactions ,so it's safe and effective .
