Objective To explore the risk factors for puncture site complications after radiofrequen-cy catheter ablation(RFCA)in elderly patients with tachyarrhythmias.Methods A retrospective study was conducted on 290 elderly tachyarrhythmic patients undergoing RFCA in our depart-ment from January 2017 to June 2023.According to the occurrence of postoperative complications during hospitalization,they were divided into a complication group(13 cases)and a non-complica-tion group(277 cases).The basic clinical data were compared between the two groups.Multivari-ate logistic regression analysis was used to identify the independent influencing factors for periop-erative puncture site complications in RFCA,and ROC curves were plotted to determine the pre-dictive value of BMI on puncture site complications.Results Puncture site complications were more common in atrial arrhythmia radiofrequency ablation,followed by supraventricular arrhyth-mia,and but rare in ventricular arrhythmia,with statistical difference in the incidence(P=0.014).The complication group had significantly higher BMI and larger proportion of atrial ar-rhythmias when compared with the non-complication group(P＜0.05).There were no statistical differences between the two groups in terms of age,proportions of ventricular arrhythmia,su-praventricular arrhythmia,hypertension and type 2 diabetes(P＞0.05).Multivariate logistic re-gression analysis showed that higher BMI was a risk factor for puncture site complications in eld-erly tachyarrhythmic patients after RFCA(OR=1.256,95％CI:1.032-1.528,P=0.023).ROC curve analysis indicated that the AUC value of BMI in predicting puncture site complications was 0.702(95％CI:0.586-0.817,P=0.018),with a sensitivity of 91.7％,a specificity of 49.6％,and a Yoden index of 0.413.Conclusion RFCA for atrial arrhythmias is closely associated with punc-ture site complications,and BMI has a predictive value for the complications of puncture site.

Objectives:To investigate whether circumferential pulmonary vein isolation(CPVI)combined with additional ablation reduces the recurrence rate of atrial fibrillation(AF)compared with CPVI alone. Methods:A literature search was conducted using the databases CNKI,Wanfang Data,VIP,Chinese Biomedical Literature Database,PubMed,Embase,the Cochrane Library.Randomized controlled studies that met the standards in recent ten years were screened,and the relevant literature contents were meta-analyzed by Review Manager 5.3 software.The comparison results of CPVI alone with CPVI combined with the additional ablation strategies in the recurrence of AF were summarized. Results:A total of 21 literatures were included with 3 357 patients,of whom 1 580 underwent CPVI alone and 1 777 underwent CPVI combined with additional site ablation.The follow-up time was more than 1 year.Meta-analysis showed that there was no significant difference between CPVI alone and CPVI combined ablation strategies in reducing AF recurrence(OR=0.86,95%CI:0.73-1.01,P=0.06).Subgroup analysis based on additional ablation site,AF type,left atrial diameter,and AF diagnosis time showed that only in the small left atrial diameter subgroup,CPVI combined with additional ablation further reduced the recurrence rate of AF(OR=0.68,95%CI:0.47-0.98,P=0.04). Conclusions:The ablation strategy of CPVI combined with additional sites could not reduce the recurrence rate of AF compared with CPVI alone.

Objective To investigate the concentration of homocysteine (Hcy) and related factors of hyperhomocyste-inemia (hHcy,Hcy≥15μmol/L) among non-hypertensive people aged between 40-70 in Tianjin. Methods Non-hyperten-sive community residents aged 40-70 years were enrolled randomly from May 2011 to December 2012 in six districts in Tian-jin. Plasma Hcy was assessed by enzyme cycling method. Factors related to hHcy were analyzed in multivariate logistic re-gression models. Results Our study included 874 participants (mean age is 57 ± 6 years, 25.5%of all are males) and the con-centration of Hcy was 12.0 μmol/L. The OR (odds ratio)(95%CI; P)for hHcy were 1.048(1.015,1.083; P=0.004), 4.191 (2.359,7.448;P<0.001), 1.280(0.896,1.829;P=0.175), 0.460(0.259,0.816;P=0.008)respectively for age, male, smoking, exercise, and the odd ratio for hHcy were 0.290(0.179, 0.469;P<0.001), 0.168(0.092,0.309;P<0.001)for consumption of vegetable and fruits 250-500 g/d and>500g/d, compared with<250 g/d. Conclusion Male and age were adverse factors for hHcy, consumption of vegetable and fruits, and exercise were positive factors.
