1.Analysis of risk factors influencing the effectiveness of conservative treatment for orbital complications of acute sinusitis

Objective:To identify the risk factors affecting the effectiveness of conservative treatment for acute sinusitis with orbital complications and provide evidence for recognition of high-risk patients.Methods:A retrospective analysis was conducted on 86 patients with acute sinusitis and orbital complications treated at Qingdao University Affiliated Hospital from 2017 to 2024 (61 males, 25 females; median age: 6.5 years). Patients were divided into an ineffective conservative treatment group and an effective conservative treatment group based on the outcomes after 48-72 hours of conservative treatment. Clinical characteristics were collected, and variables were screened using Lasso regression. Multivariate logistic regression was used to analyze the risk factors for poor conservative treatment outcomes, and a nomogram model was constructed to validate its predictive performance.Results:The overall success rate of conservative treatment was 50% (43/86). Multivariate logistic analysis showed that type Ⅲ orbital complications ( OR=0.17, P=0.017) and the presence of bone destruction ( OR=0.29, P=0.046) were independent risk factors for poor conservative treatment outcomes. The area under the ROC curve (AUC) of the combined predictive model was 0.762, with a sensitivity of 76.7% and a specificity of 69.8%. Conclusion:Patients with acute sinusitis and orbital complications, especially those with subperiosteal abscesses (type Ⅲ) and concurrent bone destruction, have a lower effectiveness rate of conservative treatment. These patients should be closely monitored for clinical signs and considered for prompt surgical intervention when necessary.
2.Establishment of single-chain antibody library targeting canine NT-proCNP,and screening and immune activity detection of a selected single-chain antibody

The amino-terminal pro-C-type natriuretic peptide(NT-proCNP)is a diagnostic inflam-matory marker clinically used for diagnosing bacterial infections.This study aims to establish a phage display library of single-chain variable fragment(scFv)antibodies against canine NT-proC-NP and to screen for scFvs with high binding affinity to NT-proCNP.Initially,NT-proCNP was prepared using prokaryotic expression system and was used to immunize New Zealand White rab-bits.Upon achieving the desired serum titer,total RNA was extracted from the splenocytes of rab-bits and reverse transcribed into cDNA.Using this cDNA as a template,degenerate primers were employed to amplify the genes of the rabbit antibody light chain variable region(VL)and heavy chain variable region(VH).The VL and VH regions were spliced together to form a complete scFv fragment via overlap extension PCR.The scFv was then ligated into the phagemid pComb3XSS and electroporated into competent E.coli TG1 cells to construct a rabbit-derived anti-NT-proCNP scFv immunological library.This library underwent four rounds of enrichment and screening to isolate specific single-chain antibodies.The selected antibody was subsequently ex-pressed in a soluble form within a prokaryotic system,and its immunological activity was evalua-ted.Using phage display technology,this study successfully identified a single-chain antibody scFv-1-CNP with strong antigen-binding activity and genetic sequence characteristics of scFvs,providing a research direction for further exploration of scFv applications in the detection of NT-proCNP.

5.The expression of TFF3 in chronic sinusitis with nasal polyps and its regulation on MUC5AC expression

OBJECTIVE To explore the expression of trefoil factor family peptide(TFF3)in chronic rhinosinusitis with nasal polyps(CRSwNP)and its regulation on mucin 5AC(MUC5AC)expression.METHODS The nasal polyp tissues of 16 patients in the CRSwNP group and the nasal mucosal tissues of 16 patients in the control group were selected,and the expressions of TFF3 and MUC5AC were detected by quantitative real-time polymerase chain reaction(RT-qPCR)and Western blot,and the correlation between them was analyzed.The human nasal epithelial cell(HNEpC)line with TFF3 knockdown was constructed,and the expression of MUC5AC in KD-TFF3 HNEpC was detected by RT-qPCR and immunofluorescence.RESULTS The expression level of TFF3 in nasal polyps was significantly lower than that of control group,and the expression level of MUC5AC was increased,and the expression level of both was negatively correlated(r=-0.556,P<0.05).The expression of MUC5AC in KD-TFF3 HNEpC was significantly higher than that in control group.CONCLUSION The expression of TFF3 decreases in CRSwNP and negatively regulates the expression of MUC5AC.This study provides a new idea for the treatment of abnormal hypersecretion of mucous in chronic nasal inflammatory diseases represented by CRSwNP.
6.Herbal Textual Research on Farfarae Flos in Famous Classical Formulas

By consulting ancient and modern literature, the herbal textual research of Farfarae Flos has been conducted to verify the name, origin, producing area, quality evaluation, harvesting and processing methods, so as to provide reference for the development and utilization of the famous classical formulas containing Farfarae Flos. According to the research, the results showed that Farfarae Flos was first described as a medicinal material by the name of Kuandonghua in Shennong Bencaojing(《神农本草经》), and the name was used and justified by later generations. The main origin was the folwer buds of Tussilago farfara, in addition, the flower buds of Petasites japonicus were used as medicine in ancient times. The ancient harvesting time of Farfarae Flos was mostly in the twelfth month of the lunar calendar, and the modern harvesting time is in December or before the ground freeze when the flower buds have not been excavated. Hebei, Gansu, Shaanxi are the authentic producing areas with the good quality products. Since modern times, its quality is summarized as big, fat, purple-red color, no pedicel is better. Processing method from soaking with licorice water in the Northern and Southern dynasties to stir-frying with honey water followed by micro-fire in the Ming dynasty, and gradually evolved to the modern mainstream processing method of honey processing. Based on the research results, it is suggested that the dried flower buds of T. farfara, a Compositae plant, should be selected for the development of famous classical formulas containing Farfarae Flos, and the corresponding processed products should be selected according to the specific processing requirements of the formulas, and raw products are recommended for medicinal use without indicating processing requirements.
7.Clinical analysis of 8 cases of spontaneous cerebrospinal fluid rhinorrhea with aspiration pneumonia

Objective:To investigate the clinical characteristics, treatment, and efficacy of spontaneous cerebrospinal fluid rhinorrhea (CFR) combined with aspiration pneumonia.Methods:In this case series study, a total of 8 patients diagnosed with spontaneous CFR combined with aspiration pneumonia were admitted to the Department of Otorhinolaryngology Head and Neck Surgery at Yuhuangding Hospital Affiliated with Qingdao University from March 2020 to March 2022. There were 3 males and 5 females, with ages ranging from 45 to 57 years. Endoscopic repair of CFR was performed for all patients. Retrospective analysis was conducted on the symptoms, chest CT findings, the leak location of CFR, and laboratory virological test results. The healing of the operative cavity and the recovery of pulmonary inflammation were followed up.Results:All the 8 patients had obvious postnasal cerebrospinal fluid drip with different degrees of dry cough, which was aggravated when lying flat and sleeping. Bone defects of CFR were predominantly located in the sphenoid sinus, lamina cribrosa and ethmoid roof. Chest CT scans upon admission showed multiple flocculent ground glass opacities and inflammatory changes in both lungs. Tests for TORCH (TOX, RV, CMV, HSV), EB (Epstein-Barr) virus nucleic acid, nine respiratory pathogens, 2019-nCoV oropharyngeal swab, and serum immunoglobulin(Ig)M/IgG were all negative. Seven patients were cured after endoscopic repair of CFR. CFR occurred again in one patient due to head trauma, and no recurrence was observed for 3 months after reoperation. Postoperative chest CT scans confirmed the resolution of aspiration pneumonia in all 8 patients.Conclusions:Aspiration pneumonia may arise from the aspiration of cerebrospinal fluid in patients with spontaneous CFR. Early diagnosis and timely repair of CFR contribute to the recovery of aspiration pneumonia.

9.Bioactive ingredients in activated platelet-rich plasma derived from cord blood and adult blood via Luminex technology: a comparative study

【Objective】 To compare the bioactive ingredients in activated platelet-rich plasma (PRP) from cord blood and adult blood, explore its reasonable indicators reflecting the inflammatory regulation ability, in order to guide the preparation. 【Methods】 PRP was prepared and activated from 63 healthy adults (31 males, 32 females) and 61 neonates (30 males, 31 females), and 20 cytokines were measured using Luminex technology for assessing the age- and sex-based bioactive differences of PRP. High-sensitivity C-reactive protein(hs-CRP), procalcitonin and MMPs/TIMPs from each sample were measured for their correlations with the 10 inflammation-related cytokines. 【Results】 The activated cord blood PRP released 10 growth factors and chemokines more than the adult blood PRP, whereas IGF-1, HGF and 8 pro-inflammatory cytokines lower than the latter. Most cytokines of adult PRP were more in females than in males (P<0.05), except for IGF-1 and HGF, which showed no difference by gender. Compared with hs-CRP and PCT, MMPs/TIMPs ratio was more closely related with the inflammation-related cytokines, which can reflect the inflammatory regulation of PRP. 【Conclusion】 Due to the lower immunocompetence and no age or gender disturbance, cord blood PRP has the rational MMPs/TIMPs ratio and more cytokines which promote the inflammation and wound healing.
10.Herbal Textual Research on Anemarrhenae Rhizoma in Famous Classical Formulas

Through reviewing ancient and modern literature, the textual research of Anemarrhenae Rhizoma(AR) has been conducted to verify the name, origin, changes in production areas, quality evaluation, harvesting and processing methods, so as to provide reference for the development and utilization of the famous classical formulas containing AR. Through the herbal textual research, AR was first published in Shennong Bencaojing, and has been used as the proper name for this herb for generations, and the mainstream source of AR used for generations is the rhizome of Anemarrhena asphodeloides. The high-quality production areas that have been revered throughout the ages are Hebei, Shanxi, Shaanxi, Inner Mongolia and Fangshan district of Beijing, etc. In recent times, AR produced in Yixian county of Hebei province(Xiling Zhimu), is better known and is regarded as a very good source. At present, cultivated AR is mainly produced in Yixian county and Anguo of Hebei province, Bozhou of Anhui province and other places. The medicinal parts of AR in ancient and modern times are all rhizomes, and the quality is better if it has thick flesh, hard wood, yellow outer color and white section color. The harvesting time recorded in ancient medical books is usually in lunar February and August, with exposure to dryness, while modern harvesting is spring and autumn. The processing methods of the past dynasties were mainly to remove the hair when using, avoid iron when cutting, process with wine or salt water, while the two main specifications in modern times are raw and salted products. Based on the systematic research, it is recommended that the dried rhizome of A. asphodeloides in the famous classical formulas be used for AR. If the original formula specifies processing requirements, it should be operated according to the requirements, if the processing requirements are not indicated, the raw products can be used as medicine.
