1.Diffuse large B-cell lymphoma with pulmonary cryptococcosis: report of 1 case and review of literature

Objective:To improve the understanding of patients with diffuse large B-cell lymphoma (DLBCL) with pulmonary cryptococcosis.Methods:The clinical data of 1 DLBCL patient with pulmonary cryptococcosis in the Central Hospital of Fengxian District of Shanghai in May 2023 were retrospectively analyzed, and the relevant literatures were reviewed.Results:This 75-year-old female patient was asymptomatic after 2 cycles of R-CHOP chemotherapy. The high-resolution CT of lung showed that lung nodules were progressively enlarged. Antibacterial treatment was ineffective. Pulmonary cryptococcosis was confirmed by bronchoalveolar lavage fluid (BALF) targeted high-throughput sequencing (tNGS) and cryptococcus capsular antigen (CrAg) detection. The central nervous system was not involved. And the long-term adequate-dose fluconazole was prescribed for 6 months, and the treatment against lymphoma was given synchronously. The lung nodule lesions reduced after antifungal therapy for 1 month. The lung nodules disappeared after the follow-up of 6 months after completion of final chemotherapy. The evaluation of lymphoma indicated complete remission.Conclusions:Pulmonary cryptococcosis occurs insidiously and shows no specific symptoms; its imaging manifestations are variable and routine anti-infection is ineffective. Immunochemotherapy for lymphoma patients is a high-risk factor for cryptococcal infection. tNGS and CrAg testing for BALF are effective methods of the confirmed diagnosis. The early and long-term adequate-dose antifungal treatment is the key to preventing the recurrence or progression.
2.Effect Evaluation of Pharmaceutical Care Network Europe Classification System on Drug-related Problems in Respiration Department

OBJECTIVE:To evaluate the effect and role of Pharmaceutical Care Network Europe (PCNE) classification system (8.0 edition) on drug-related problems (DRPs) in respiration department.METHODS:Clinical pharmacists provided pharmaceutical care for DRPs in a patient with acute exacerbation of chronic obstructive pulmonary disease of respiration department by using PCNE classification system (8.0 edition).Type,cause,intervention measure,intervention acceptability and state of DRPs were analyzed.RESULTS:PCNE classification system (8.0 edition) mainly included 5 aspects as problems,reasons,plan intervention,the acceptance of intervention plan,the situation of DRPs.Clinical pharmacists confirmed the type of DRPs,with the help of the system and solved two items of DRPs.Physicians accepted intervention and fully implemented it.CONCLUSIONS:Clinical pharmacists can provide standardardized pharmaceutical care systematically,discover and solve DRPs in time through PCNE classification system (8.0 edition) so as to guarantee safe,effective and reasonable drug use.
3.Comments on the Prescriptions of Analgesic Drugs for the Patients with Cancer Pain Using PCNE Classifi-cation System

Objective: To analyze the prescriptions of analgesic drugs for the patients with cancer pain, and promote the rational use of narcotic and analgesic prescriptions. Methods: Using the PCNE classification system, the list of all drug-related problems (DRPs) from 115 patients admitted to the standardized treatment of cancer pain from January to December 2017 was analyzed. Re-sults: In our hospital from January to December 2017, there were 52 cases of prescribed DRPs for narcotic and analgesic drugs in the standardized treatment of cancer pain with the incidence of 45. 22% . DRPs mainly focused on treatment safety with the incidence of 72. 31% . The drug choice caused the highest incidence of DRPs accounting for 63. 24% , followed by dose selection accounting for 20. 59% . Conclusion: There are still unreasonable prescriptions of narcotic analgesia in our hospital. According to the PCNE classifi-cation system, it is necessary to strengthen the communication with clinicians, carry out education for the patients with cancer pain, and further educate pharmacy staff on the knowledge of analgesic drugs. Through the above measures, the regulation of prescription medication for narcotic analgesics can be intensified.
4.Blood Vessel and Intraplaque Hemorrhage Simultaneous Imaging Sequence in Evaluation of the Integrity of Willis Circle

Purpose To investigate the value of simultaneousnon-contrast angiographyand intra-plaquehemorrhage (SNAP) in assessing Willis circle integrity of the brain using three-dimensional time of flight (TOF) MRA as a reference. Materials and Methods According to the inclusion criteria, a total of 62 patients with stroke symptoms within 3 months were collected. All patients underwent head SNAP and TOF on Philips 3.0T MR scanner. TOF and SNAP were treated respectively at the Philips workstation using maximum and minimum intensity projections to generate non-enhanced MRA images which were, afterwards, interpreted using blind reading to determine the presence or absence of individual blood vessels in the circle of Willis. In order to assess the consistency of the interpretation, all images were interpreted again 2 weeks later to avoid memory bias. Finally, The consistency of the two interpretations was analyzed, and the ability of SNAP and TOF in evaluating the integrity of Willis circle was compared. In addition, for 15 patients with VISTA images, the plaque of Willis circle vascular wall was evaluated using SNAP and VISTA images to compare their evaluation ability. Results The results of the first and second interpretations of SNAP and TOF were highly consistent. The arteries with exactly the same interpretation included left anterior cerebral artery A1 (LA1), right anterior cerebral artery A1 (RA1), left posterior cerebral artery P1 (LP1) and right posterior cerebral artery P1 (RP1) (Kappa=1.000). The results in terms of left posterior communicating artery (LPCoA) (Kappa=0.926 and 0.924, respectively), right posterior communicating artery (RPCoA) (Kappa=0.931 and 0.732, respectively) and integrity (Kappa=0.815 and 0.816, respectively) were relatively consistent, while there was a relatively low consistency in terms of anterior communicating artery (ACoA) (Kappa=0.640 and 0.675, respectively). In evaluating the constituent vessels of the circle of Willis and its integrity, SNAP and TOF showed good consistency. Vessels with identical SNAP and TOF interpretations include LA1, RA1, LP1, and RP1 (Kappa=1.000). The interpretation consistency of the two imaging techniques on LPCoA (Kappa=0.852 and 0.848), RPCoA (Kappa=0.796 and 0.796, respectively), and integrity (Kappa=0.701 and 0.742, respectively) was relatively high. While the consistency of SNAP and TOF in terms of ACoA was slightly lower (Kappa=0.680 and 0.714, respectively). In assessing the plaque of Willis circle vascular wall, the evaluation results of SNAP and VISTA were consistent. Conclusion As a non-enhanced angiography technique, the magnetic resonance SNAP sequence can well evaluate the integrity of Willis circle, as well as the presence or absence of plaque on Willis circle vascular wall.
5.Analysis of the Utilization of Antibiotics in TypeⅠIncision Surgery of Our Hospital before and after Special Rectification

OBJECTIVE:To evaluate the effect of clinical antibiotics use special rectification in our hospital. METHODS:100 discharged medical records of typeⅠincision surgery were randomly sampled from our hospital during May in 2010 to Apr. in 2011,May in 2011 to Apr. in 2012,May in 2012 to Apr. in 2013,May in 2013 to Apr. in 2014,totaling 400 records. And then evaluation indicators were analyzed statistically,such as antibiotics use intensity,perioperative DDDs of antibiotics in typeⅠinci-sion surgery,DUI,types of antibiotics during perioperative period,medication time,etc. RESULTS:Since the implementation of clinical antibiotics use special rectification in May 2011,the utilization ratio of antibiotics in typeⅠincision surgery of our hospital decreased from 96% to 33%;DUI decreased from 1.44 to 0.79;while reasonable rate of drug selection increased from 19.8% to 100%,and that of medication time increased from 43.8% to 100%. CONCLUSIONS:Rational medication evaluation indicators in typeⅠincision surgery of our hospital have been improved after the implementation of clinical antibiotics use special rectification.
6.Effect of UK combined with tirofiban in young patients with ST segement elevation acute myocardial infarction

Objective To study the thrombolysis effect and safety of UK combined with tirofiban in young patients with ST segement elevation acute myocardial infarction.Methods 76 young patients with ST segement elva-tion acute myocardial infarction were selected as research objects,and they were divided into UK group(control group, n =34)and UK combined with tirofiban group(observation group,n =42)by random number table,then the recanali-zation rate of infarct -related artery and incidence of complications were compared.Results The recanalization rate of infarct -related artery in the observation group was 97.6%,which was 91.2% in the control group,there was no significant difference between the two groups(χ2 =1.564,P >0.05);The vascular infarction related to blood flowⅡlevel of the patients was 90.5% in the observation group,which was significantly higher than 70.6% in the control group(χ2 =4.945,P <0.05);The death,complications incidences had not significant differences between the two groups(χ2 =1.252,2.837,2.837,all P >0.05).Conclusion The thrombolysis effect of UK has a high recanaliza-tion rate of infarct -related artery in young patients with ST segement elevation acute myocardial infarction,and the thrombolysis effect of UK combined with tirofiban is better,and it does not increase the incidence of complications.
7.Reliability and validity of Chinese-version of the chronic pain acceptance questionnaire-8

Objective To evaluate reliability and validity the Chinese-version of chronic pain acceptance questionnaire-8(CPAQ-8).Methods The reliability and validity of CPAQ-8 was tested in 223 old patients with chronic pain.Result The Chinese-version of CPAQ-8 was (23.79±-5.23),AE was (12.84±4.96),PW was(10.97 ±2.90).Cronbach' s of CPAQ-8 was 0.853,and Guttmann Split-Half Coefficient was 0.843.Two factors were extracted by factor analysis through exploratory factor analysis,whose cumulative contribution was 58.217%.While confirmatory factor analysis showed probability level of the modified model was P=0.322,with indexes as follows: x2/df =1.124,GFI =0.979,AGFI =0,956,TLI =0.069,RMSEA =0.024,NFI =0.960,TLI =0.992,CFI =0.995.Conclusion Reliability and validity of CPAQ-8 were both well.

Objective To investigate work stress state of nursing home staff and to analyze the influential factors affecting work stress.Methods Stratified random sampling method was adopted to investigate 180 nursing assistants by self-designed questionnaire,Work Stress Scale(WSS),Simplified Coping Style Questionnaire and internal-external locus of control scale investigation.Results Totally 174 valid questionnaires were collected.Variance analysis revealed there were significant differences of age and other factors.Stepwise multiple regression analysis showed five variables entered the equation,including workexpenence education level number of the cared ages work scheduline coping stule and explain 56.7% of the total variable.Conclusion Nurse managers should pay attention to and improve achievement sense and mental adjustment ability of nursing assistants,to build a well-organized support system to reduce the workload.
9.Imaging of superior sinus venosus atrial septal defect by multi-slice CT

Objective To evaluate the effectiveness of MSCT in the diagnosis of superior sinus venosus atrial septal defect.Methods The MSCT features of superior sinus venosus atrial septal defect in twenty cases were evaluated retrospectively.The following data were recorded:the size and location of sinus venosus atrial septal defect,the anatomy of pulmonary vein,including number of anomalously draining pulmonary veins and their site of drainage,and associated anomalies.Results In all patients,the superior sinus venosus atrial septal defect locates in the extraseptal wall,which normally separates the right upper pulmonary vein from superior vena cava(SVC).And anomalous connection of right upper pulmonary venous and SVC was identified in all the patients.The mean value of the defect diameter was ( 17.1±5.8) mm.Left superior vena cava was identified in 3 patients.In an elderly patient,left anterior descending branch of coronary artery presented significant stenosis.And in another elderly patient with large atrial septal defect,severe pulmonary hypertension was identified by cardiac catheterization.MSCT findings of superior sinus venosus atrial septal defect in 6 cases were finally confirmed by surgical operation.Conclusions Contrastenhanced MSCT was a useful technique for the diagnosis of superior sinus venosus atrial septal defect,which accurately displayed the anatomical characteristics of the associated malformations for preoperative evaluation.
10.Diabetes mellitus ulcers treatment with Bletilla striata polysaccharide.

The aim of this study was to evaluate the efficacy of Bletilla striata polysaccharide on diabetes mellitus ulcers. Diabetes mellitus animal model was established by single ip injection of streptozotocin (STZ, 50 mg x kg(-1)) with the criteria of blood glucose > or = 16.7 mmol x L(-1) after 72 h. 4 weeks after STZ injection, each animal received two full thickness incisional wounds (1.8 cm in diameter). The wounds then were divided into B. striata polysaccharide group and PBS group. Wound closure rate, fibroblast (FB) infiltration, hydroxyproline (OHP) content and myeloperoxidase (MPO) levels were examined on day 3, 7, 14, 21 post wound. The treatment of B. striata polysaccharide significantly facilitated diabetes mellitus ulcers healing compared to PBS group. Histological analysis showed that B. striata polysaccharide markedly increased inflammatory cell infiltration in wound area. The herb also strongly evaluation of FB, OHP demonstrated a significantly increased in B. striata polysaccharide group. B. striata polysaccharide group promoted wound closure by means of enhanced inflammatory cell infiltration and re-epithelialization, and the promotion of FB and OHP levels.
