Objective: To analyze near work illuminance patterns on school days and weekends among third and fourth grade primary school students by using objectively monitored data, and to examine the associations of the duration of near work under different illuminance levels and time weighted average near work illuminance with screening myopia. Methods: From April to June 2024, students from grades 3 and 4 in two primary schools in Jiading District, Shanghai were selected as study subjects by using stratified random cluster sampling method. Continuous objective monitoring of near work behavior was conducted for 7 days while information from uncorrected visual acuity and refractive assessment was collected, and an online questionnaire survey was administered to their parents. Valid samples comprised 372 students on school days and 292 on weekends. Welch s t test was used to compare differences in the duration of near work under different illuminance levels between the screening myopia and non-myopia groups. Multivariable binary Logistic regression analysis was employed to assess the association between the duration of near work under different illuminance levels and screening myopia. Restricted cubic spline (RCS) model was used to explore the nonlinear association between time weighted average near work illuminance with 60 cm and screening myopia, and the critical thresholds identified by the RCS model were used to categorize illuminance levels for subsequent Logistic regression analyses. Results: Objective monitoring of near work behavior showed that the duration of near work in the low illuminance range of [0,125)lx was significantly longer on weekends[(4.12±2.21)h] than on school days [(2.42±1.22)h]. Multivariate Logistic regression analysis revealed that only on school days, each additional hour of near work duration in the [0,125)lx ( OR =1.56, 95% CI =1.23-1.98) and ≥1 000 lx range ( OR =5.92, 95% CI =1.48-23.73) were significantly associated with higher odds of screening myopia(both P < 0.05). RCS plot showed that on school days, time weighted average near work illuminance with 60 cm had a non-linear association with screening myopia ( P non-linear <0.05). On weekends, no significant association was found between near work illuminance indicators and screening myopia (all P >0.05). Conclusions Primary school students engage in near work for considerable durations in insufficient lighting environments on both school days and weekends. Increased duration of near work in overly dim or bright environments on school days may increase the risk of screening myopia.

From August to December 2023, 22 primary caregivers of children with post-infectious bronchiolitis obliterans in the First Affiliated Hospital of Guangzhou Medical University were selected by purposive sampling method as study subjects for semi-structured interviews. The information was analyzed using the Colaizzi 7-step analysis.Results:A total of three themes and nine sub-themes were distilled, including information (concern for information on pulmonary rehabilitation exercise showing dynamic changes with the course of the disease, access to information on disease diagnosis and treatment and pulmonary rehabilitation exercise management through non-professional channels, biased perceptions of pulmonary rehabilitation exercise, and lack of a unified guide to information on pulmonary rehabilitation exercise) ; motivation (physiological and psychological barriers in the management and implementation of pulmonary rehabilitation exercises, lack of comprehensiveness and continuity of professional instruction in pulmonary rehabilitation exercises, and insufficient communication and collaborative support among family members) ; and behavioral skills (insufficient mastery of respiratory training methods with complex procedures, and lack of the ability to effectively plan a pulmonary rehabilitation exercise program) .Conclusions:Caregivers of children with post-infectious bronchiolitis obliterans face certain dilemmas in managing pulmonary rehabilitation exercises. Objective:To investigate the feasibility of oblique overlap anastomosis plus single flap valvuloplasty (OSF) for reconstruction after laparoscopic proximal gastrectomy.Methods:The C-shaped seromuscular flap (2.5 × 3.5 cm), which was 2 cm from the top of the remnant stomach, was extracorporeally created on the anterior wall of the remnant stomach. The stomach was opened approximately 1.0 cm above the lower edge of the mucosal flap. Four supporting stitches were sutured around the hole and the right wall of the esophageal stump was incised with a support suture to prevent the linear stapler from entering the submucosa. Liner stapler was inclined to the left side of the esophagus at an angle of about 30 degrees to the longitudinal axis of the esophagus for oblique anastomosis between the dorsal side of the esophagus and the anterior stomach and then esophagogastrostomy was performed with the length of anastomosis was 4 cm. Entry hole was suture with 3 stitches and then the linear stapler was used for closing the entry hole. Finally, the seromuscular flap was closed using barbed sutures.Results:Clinical data of 11 patients with Siewert type II esophagogastric junction adenocarcinoma who underwent radical gastrectomy and reconstruction by OSF between January 2022 and May 2023 were retrospectively collected. There were 7 males and 4 females. The average age was (69.9±7.8) years, the BMI was (21.7±7.2) kg/m 2 and the tumor size was (2.1±0.6) cm. OSF reconstruction was successfully completed in all 11 patients. The median operative time was 275 (270-428) minutes, the time for OSF reconstruction was 112 (80-140) minutes, and the blood loss was 50 (20-400) ml. The pathological stage was 0-I in 7 cases and II-III in 4 cases. The patients were fed on the 4th day (4-7 days) and discharged from hospital on the 7th day (6-9 days) after surgery. No patient had gastroesophageal reflux symptoms of grade B and above, and no patient took anti-reflux medicine. Conclusions:OSF is a safe and feasible treatment for Siewert type II esophagogastric junction adenocarcinoma.

ObjectiveTo analyze the status of and family-related factors associated with extracurricular screen time (ST) among the primary and middle school students in a district of Shanghai, and to provide a scientific basis for the development of targeted intervention measures. MethodsA total of 819 students and their parents from one primary school and one middle school were selected by stratified cluster sampling method from November to December 2022 in a district of Shanghai. A questionnaire survey was conducted to investigate the total extracurricular ST and entertainment ST during school days, weekends and summer and winter vacation, as well as the underlying family-related factors. Taking daily total extracurricular ST>2 h as excessive extracurricular ST and daily extracurricular entertainment ST>1 h as excessive extracurricular entertainment ST. ResultsAmong the 795 valid questionnaires, the proportions of excessive extracurricular ST during school day, weekend and summer and winter vacation were 10.9%, 42.1% and 51.1%, respectively, and the proportions of excessive extracurricular entertainment ST were 16.2%, 63.0% and 71.3%, respectively. The proportion of excessive extracurricular ST on weekend and summer and winter vacation was higher for middle school students than for primary school students, and the proportion of excessive extracurricular ST was higher for the students whose father’s educational level was high school and below than for those whose father’s educational level was junior college and above (both P<0.05). Multivariate logistic regression analysis after controlling confounding factors showed that the students with poor parent-child relationships (aOR=1.40‒1.96) and parental daily ST>2 h (aOR=1.83‒3.85) were more likely to have an excessive total extracurricular ST and entertainment ST. ConclusionThe proportion of excessive extracurricular ST for the primary and middle school students in Shanghai is relative high during weekend and summer and winter vacation, especially for entertainment purposes. Improving parent-child relationships and reducing parental excessive ST may be important intervention measures.

BACKGROUND: The prevalence of hypertension is high among Chinese adults, thus, identifying non-hypertensive individuals at high risk for intervention will help to improve the efficiency of primary prevention strategies. METHODS: The cross-sectional data on 9699 participants aged 20 to 80 years were collected from the China National Health Survey in Gansu and Hebei provinces in 2016 to 2017, and they were nonrandomly split into the training set and validation set based on location. Multivariable logistic regression analysis was performed to develop the diagnostic prediction model, which was presented as a nomogram and a website with risk classification. Predictive performances of the model were evaluated using discrimination and calibration, and were further compared with a previously published model. Decision curve analysis was used to calculate the standardized net benefit for assessing the clinical usefulness of the model. RESULTS: The Lasso regression analysis identified the significant predictors of hypertension in the training set, and a diagnostic model was developed using logistic regression. A nomogram with risk classification was constructed to visualize the model, and a website ( https://chris-yu.shinyapps.io/hypertension_risk_prediction/ ) was developed to calculate the exact probabilities of hypertension. The model showed good discrimination and calibration, with the C-index of 0.789 (95% confidence interval [CI]: 0.768, 0.810) through internal validation and 0.829 (95% CI: 0.816, 0.842) through external validation. Decision curve analysis demonstrated that the model was clinically useful. The model had a higher area under receiver operating characteristic curves in training and validation sets compared with a previously published diagnostic model based on Northern China population. CONCLUSION This study developed and validated a diagnostic model for hypertension prediction in Gansu Province. A nomogram and a website were developed to make the model conveniently used to facilitate the individualized prediction of hypertension in the general population of Han and Yugur.

Objective:There is no standard method for esophageal remnant gastric reconstruction for proximal gastrectomy. Reflux esophagitis caused by esophagogastrostomy remains a difficult surgical problem. To report the preliminary surgical results of novel esophagus-conical remnant gastric side overlap anastomosis (CGEO) , with particular emphasis on postoperative esophageal reflux.Methods:In June 2022, we developed a novel CGEO for laparoscopic proximal gastrectomy on two patients with Siewert type II esophagogastric junction adenocarcinoma. Surgical procedures for CGEO: (1) Laparoscopic proximal gastrectomy and preparation of conically shaped gastric remnant; (2) Determining anastomotic site of residual stomach and esophagus; (3) Side-to-side anastomosis of right esophageal wall to anterior of conical gastric remnant; (4) Valvuloplasty of esophageal stump.Results:Case 1 was a 71-year-old man with an operation time of 305 minutes and was successfully discharged from the hospital on the 9th day after surgery, and the postoperative pathology was T3N0M0. Case 2 was an 82-year-old man with an operation time of 325 minutes. He was discharged on the 10th day after surgery. In both cases, only mild esophageal mucosal changes were seen in gastroscopy, there were no obvious symptoms of esophageal reflux. There was also no significant weight change at half a year after operation.Conclusion:CGEO is moderately safe in radical surgery for proximal gastric cancer, and may have a preventive effect on the occurrence of postoperative esophageal reflux, but long-term results need to be confirmed by further studies with follow-up.
