1.Vessel wall magnetic resonance imaging for central nervous system vasculitis

Central nervous system vasculitis (CNSV) is a type of inflammatory disease involving the blood vessels of the brain, spinal cord and meninges, and its clinical diagnosis is quite challenging. The sensitivity and specificity of traditional imaging examinations are very low, while high-resolution vessel wall imaging (HRVWI) can clearly show vascular wall and perivascular lesions. CNSV on HRVWI may manifested as concentric thickening and enhancement of the vascular wall, and perivascular enhancement with or without vascular stenosis. HRVWI has high value in the diagnosis and differential diagnosis of CNSV and related diseases. This article reviews the HRVWI characteristics of CNSV and the differential diagnosis of related diseases.
2.The impact of the recovery-oriented family psychoeducation on social support and family resilience in patients with schizophrenia in remission stage

Objective:Exploring the effect of recovery-oriented family psychoeducation on the level of social support and family resilience of patients with schizophrenia in remission stage, providing theoretical basis for clinical application.Methods:Using a randomized controlled trial method and convenience sampling method, 60 patients with schizophrenia in remission who visited the outpatient department of the Affiliated Mental Health Center of Shanghai Jiao Tong University School of Medicine from April 2020 to March 2022 were selected as the study subjects, and they were divided into experimental group and control group, with 30 patients in each group according to their residence places. The control group was given routine health education; the experimental group was given the rehabilitation oriented family psychoeducation, before intervention and 3 months after intervention, Social Support Rating Scale and Family Resilience Questionnaire were used to evaluate the intervention effect.Results:A total of 60 patients were included in the control group, including 13 males and 17 females, aged (27.0 ± 6.5)years.In the experimental group, there were 14 males and 16 females, aged (28.0 ± 5.6) years. After intervention, the scores of subjective support, objective support and social support utilization in Social Support Rating Scale of the experimental group were 14.33 ± 3.36, 8.43 ± 1.63 and 9.60 ± 2.49, respectively, the control group were 16.20 ± 3.83, 10.00 ± 2.59 and 8.33 ± 2.28, respectively, and the differences were statistically significant ( t=2.00, 2.80, -2.05, all P<0.05). After intervention, the total score of Family Resilience Questionnaire and the scores of perseverance, support and harmony were 78.77 ± 9.61, 24.90 ± 3.07, 15.10 ± 2.23 and 23.63 ± 3.04, respectively. Higher than the control group 72.67 ± 15.96, 23.43 ± 4.92, 14.03 ± 3.60, 21.13 ± 5.13, the differences were statistically significant ( t values were -2.29--1.37, all P<0.05). Conclusions:Recovery-oriented family psychoeducation can improve the level of social support and family resilience of schizophrenia patients in remission.

OBJECTIVE:
To investigate the characteristics of growth and metabolism and the toxicity of under different conditions.
METHODS:
We observed the growth of and under routine culture conditions and in different pH and salt concentrations, and compared their activities of sugar fermentation using microbiochemical reaction tubes. Four-week-old nude mice were randomized into infection group (=5), infection group (=5) and control group (=5) for intragastric administration of 0.3 mL suspension the two (5×10 cfu/mL) or 0.3 mL normal saline. Samples of the liver, kidney, intestine, feces and blood were taken for analysis of the distribution and toxicity of by fungal culture and histopathological examination.
RESULTS:
exhibited logarithmic growth at 8-24 h after inoculation and showed stable growth after 24 h. showed optimal growth within the pH value range of 5-7 with a growth pattern identical to that of . grew better than in media containing 5% and 10% NaCl, and could ferment glucose, sucrose, trehalose and sorbitol. could be isolated from the feces, blood, liver and kidney of infected nude mice, and the liver had the highest fungal load (5.7 log cfu/g). could cause pathological changes in the liver and intestine of the mice, but with a lesser severity as compared with .
CONCLUSIONS
exhibits optimal growth in mildly acidic or neutral conditions with a high salt tolerance, and can potentially penetrate the intestinal barrier into blood and lead to tissue injuries in hosts with immunosuppression.

5.Analysis of genetic and clinical characteristics of nine cases of myelodysplastic syndrome with near tetraploid/tetraploidy karyotype.

OBJECTIVE:
To explore the genetic and clinical characteristics of near-tetraploidy/tetraploidy karyotype (NT/T) in patients with myelodysplastic syndrome (MDS).
METHODS:
Cytogenetic findings of 1576 inpatients with primary MDS were retrospective analyzed, among which 9 were diagnosed with NT/T. Clinical data including gender, age, morphology, genetic feature and prognosis were analyzed.
RESULTS:
The prevalence of MDS patients with NT/T (NT/T-MDS) among all cases was 0.57%. Karyotyping analysis suggested that eight MDS patients had sole NT/T, while the remainder one had a complex karyotype. In addition to the typical morphology of MDS, NT/T-MDS had unique morphology including huge blast, double-nuclear cell and irregular nuclear membrane. One NT/T-MDS patient gave up therapy, and the remaining eight underwent the first course of treatment, albeit with poor prognosis. Only one patient had complete remission, one had partial remission, three had no remission; and three had converted to acute myeloid leukemia.
CONCLUSION
NT/T-MDS is rare and has unique morphology. Generally, NT/T-MDS patients have poor prognosis. However, NT/T cannot be simply classified as high-risk group, but with consideration whether they have affected particular chromosomal structures as well as other clinical data.

6.Clinical significance of liquid-based cytology and cell paraffin blocks combined with immunocytochemistry in cytopathologic diagnosis of hydrothorax and ascites

Objective To investigate the clinical value of liquid-based cytology and cell paraffin blocks combined with immu-nocytochemical examination in cytopathologic diagnosis of hydrothorax and ascites.Methods One hundred and fourteen cases of hydrothorax and ascites sample were collected.The detection positive rates were compared between the liquid-based cytology and cell paraffin blocks combined with immunocyochemical examination.The suspected positive and positive cases were performed the immunocytochemical examination for further judging benign and malignancy and histological source.Results 114 cases of hydro-thorax and ascites samples,36 cases(31.58%)of positive were diagnosed by liquid-based cytologyamong and 55 cases(48.24%)of positive were diagnosed by cell paraffin blocks combined with immunocytochemical examination,the difference was statistically sig-nificant(P<0.01).There were 55 cases of malignant hydrothorax and ascites,45 cases were hydrothorax and 10 cases were ascites, in hydrothorax,there were 32 cases of lung adenocarcinoma,3 cases of small cell lung cancer,4 cases of breast cancer,1 case of en-dometrial carcinoma,1 case of lymphatic hematopoietic system tumor and 4 cases of unknown origin;in ascites,there were 2 cases of ovarian cancer,1 case of digestive system tumor,1 case of endometrial cancer,1 case of lymphatic hematopoietic system tumor and 5 cases of unknown origin.Conclusion The cell paraffin blocks combined with immunocytochemistry can significantly improve the positive detection rate of hydrothorax and ascites,and helps to judge the source of tissue.
7.Variation in serum visfatin levels 24 hours after coronary stent implantation

BACKGROUND:Coronary stent implantation can cause blood vessel damage and wal reconstruction, leading to vascular stent restenosis. Studies have found that visfatin is associated with inflammatory reaction, and exhibits an increased expression at the site of plaque rupture in acute myocardial infarction. OBJECTIVE:To investigate the influence of percutaneous coronary intervention on the levels of visfatin in patients with coronary heart disease. METHODS:Thirty patients with acute myocardial infarction within 12 hours after the onset of the chest pain, 30 patients with unstable pectoris and 30 patients with stable angina pectoris were included. Al patients were successfuly treated by percutaneous coronary intervention. Meanwhile, 30 patients only undergoing coronary angiography but not stenting treatment were selected, and another 30 patients without any treatment served as normal control group. RESULTS AND CONCLUSION:According to enzyme-linked immunosorbent method, the visfatin levels of acute myocardial infarction, unstable angina, stable angina and coronary angiography groups continue to rise at pre-operation, 30 minutes, 6 hours, 12 hours, 24 hours after operation, al of which were higher than that in the normal control group (P < 0.05). The results confirmed that within 24 hours after coronary stent implantation the visfatin levels continue to rise.
8.Protection of L-glutamine against myocardial cell injury induced by endotoxin

AIM:To observe protective effect of L-glutamine on myocardial cell injury induced by endotoxin. METHODS:22 male SD rats,weighting(250?30)g,were randomly divided into three groups:control group( n= 7),endotoxin group( n= 7),glutamine/endotoxin group( n= 8). Heart were isolated from rats and perfused on Langendorff apparatus with Krebs- Henseleit(K-H)buffer(Saturation 95%O 2+5%CO 2)at a constant pressure(8.33 kPa)and temperature(37℃). The activity of superoxide dismutase (SOD)and malondialdehyde (MDA)content of efflux from coronary artery, monophasic action potential(MAP)and contractile force were measured at certain timepoints (0 min,20 min,50 min,80 min). RESULTS: Monophasic action potential and contractile force were markedly improved in Gln/ET group. The activity of SOD and MDA level in Gln/ET group restored closely to that in control group. CONCLUSION:L-gluta mine protects myocardial cells from injure induced by endotoxin.
