1.Effects of short-term insemination and early rescue ICSI on pregnancy and neonatal outcomes

Objective This study aims to explore the impacts of short-term insemination and early rescue intracytoplasmic sperm injection (E-RICSI) on clinical and neonatal outcomes for IVF patients. Methods A retrospective analysis was conducted on the clinical data from the patients who underwent fresh embryo transfer at the Reproductive Center from January 2019 to December 2023. Patients were divided into four groups based on fertilization method:short-term IVF group (n=204),conventional IVF group (n=208),E-RICSI group (n=13) and conventional ICSI group (n=92). The fertilization rates,embryo development,pregnancy outcomes,and neonatal outcomes were compared between the short-term IVF and conventional IVF groups,and between the E-RICSI and conventional ICSI groups. Results There were no statistically significant differences in embryo development,clinical pregnancy,miscarriage,ectopic pregnancy,live birth rates,neonatal sex,and birth weight between the short-term IVF group and conventional IVF group. Similarly,no significant differences were observed in the E-RICSI group compared to the conventional ICSI group (P>0.05). However,the fertilization rate (79.11% vs. 84.39%,P<0.001) and the rate of 2PN zygotes (63.98% vs. 70.83%,P<0.001) were significantly lower in the short-term IVF group compared to the conventional IVF group;The fertilization rate (65.49% vs. 91.68%,P<0.001) and the rate of 2PN zygotes (57.75% vs. 88.35%,P<0.001) were significantly lower in the E-RICSI group compared to the conventional ICSI group. Conclusions Although the fertilization rate of short-term insemination and E-RICSI is lower than that of conventional IVF and ICSI,it has no effect on embryonic development,preg-nancy outcome and neonatal outcome. Short-term insemination combined with early rescue ICSI is an effective and safe technology to prevent complete fertilization failure.
2.Clinical characteristics of 22 patients with neurobrucellosis

Objective:To learn about the clinical characteristics of patients with neurobrucellosis (NB) and provide references for clinical diagnosis and treatment of NB.Methods:The clinical data of 22 NB patients diagnosed and treated at Beidahuang Industry Group General Hospital from January 2018 to November 2024 and 178 healthy individuals undergoing physical examinations during the same period were retrospectively collected. The epidemiological characteristics, clinical manifestations, laboratory tests, treatment and prognosis of NB patients were analyzed.Results:Among the 22 NB patients, 12 were males (54.55%) and 10 were females (45.45%). The age was (51.77 ± 12.75) years old, ranging from 27 to 80 years old. Most of the patients were farmers (95.45%, 21/22), and 16 cases (72.73%) had contacted with cattle/sheep. The onset seasons were mainly in summer (40.91%, 9/22) and spring (31.82%, 7/22). Among all NB patients, there were 10 cases of encephalitis/meningoencephalitis, 9 cases of myelitis, and 3 cases of meningitis. The general symptoms were mainly fever (68.18%, 15/22), the neurological symptoms were mainly nausea and vomiting (36.36%, 8/22), and the physical signs were mainly muscle weakness (50.00%, 11/22) and pathological signs (45.45%, 10/22). The laboratory test results showed that the levels of hemoglobin, hematocrit, C-reactive protein, total protein, albumin, γ-glutamyl transpeptidase, α-hydroxybutyric acid dehydrogenase, lactate dehydrogenase, and glutathione reductase in NB patients were significantly different from those in healthy individuals ( P < 0.001). Brain magnetic resonance imaging revealed ischemic changes in 5 cases (22.73%), abnormal brain signals in 2 cases (9.09%), and demyelinating lesions in white matter in 1 case (4.55%). After treatment, 18 NB patients were followed up and showed good prognosis, with only 2 cases exhibiting varying degrees of sequelae (walking disorders or memory impairment). Conclusions:The clinical manifestations of NB patients are diverse. A comprehensive judgment should be made by combining epidemiological characteristics, clinical manifestations, laboratory tests and imaging examinations.

5.Mediating role of spirituality between social support and benefit finding in patients with advanced gastric cancer

Objective:To examine the effect of social support on benefit finding in patients with advanced gastrointestinal cancer and the mediating role of spirituality in this relationship, providing insights for promoting psychological well-being in gastrointestinal cancer patients.Methods:A convenience sampling method was used to select 300 inpatients with advanced gastrointestinal cancer admitted to the Department of Integrated Traditional Chinese Medicine, Sun Yat-sen University Cancer Center, from February 2022 to December 2023. Patients were surveyed using the General Information Questionnaire, Benefit Finding Scale (BFS), Perceived Social Support Scale (PSSS), and Spiritual Attitude and Involvement List (SAIL). Pearson correlation analysis was used to assess correlations between social support, spirituality, and benefit finding. Mediation analysis was conducted using model 4 in the PROCESS macro in SPSS 24.0.Results:A total of 300 questionnaires were distributed, with 295 valid responses (effective response rate of 98.33%). The mean scores for BFS, PSSS, and SAIL among 295 patients were (72.62±6.30), (62.31±10.41), and (100.17±17.34), respectively. Both social support and spirituality were positively correlated with benefit finding ( r=0.540 and 0.529, P<0.05), and social support was positively correlated with spirituality ( r=0.611, P<0.05). Social support positively predicted benefit finding directly, and the mediating effect of spirituality between social support and benefit finding was 0.194, accounting for 35.79% of the total effect. Conclusions:Providing strong social support while fostering patients' spirituality can significantly enhance their benefit finding.

7.Effects of nursing service under the Internet platform on self-efficacy and adherence of newly diagnosed tumor patients

Objective:To explore the effect of nursing services under the Internet platform in cancer patients.Methods:From July 2019 to July 2021, convenience sampling was used to select patients who were diagnosed with cancer for the first time in the Department of Comprehensive Diseases of the Sun Yat-Sen University Cancer Center. The patients were randomly divided into the intervention group and the control group by random number table. The control group was given routine nursing during hospitalization. On the basis of routine nursing, the intervention group implemented the intervention using the hospital's WeChat public platform, mobile APP and WeChat group as the main media. Adherence and self-efficacy were compared between the two groups.Results:There was no significant difference in the adherence between the two groups one month after discharge ( P>0.05) . Three and six months after discharge, the adherence of the intervention group was higher than that of the control group, and the difference was statistically significant ( P<0.05) . There was no significant difference in the scores of total self-efficacy, self-decompression, self-decision-making and positive attitude between the two groups before intervention ( P>0.05) . After the intervention, the total score and each dimension score of self-efficacy in the intervention group were higher than those in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Nursing services based on the Internet platform can improve the adherence of newly diagnosed tumor patients and enhance patients' self-efficacy, which is worthy of clinical promotion.
8.Sex ratio at birth of different assisted reproductive technologies and embryo transfer at different developmental stages in frozen-thawed embryo transfer cycles

Objective:To investigate the effect of different assisted reproductive technologies (ART) and transplanted embryos at different developmental stages on live birth sex ratio in frozen-thawed embryo transfer (FET) cycles.Methods:From April 2010 to October 2018, the clinical data of patients and neonatal underwent in vitro fertilization-frozen-thawed embryo transfer (IVF-FET), intracytoplasmic sperm injection-frozen-thawed embryo transfer (ICSI-FET) and preimplantation genetic testing-frozen-thawed embryo transfer (PGT-FET) cycles that led to birth in the Center of Reproductive Medicine, Guangzhou Women and Children's Medical Center were retrospectively analyzed. The relationship of the sex ratio of the live births to different ART and transplanted embryos at different developmental stages was studied. Results:1) IVF-FET cycles included 898 couples (920 FET cycles) and 1166 babies, with a general sex ratio of 106.7. The live birth sex ratio increased gradually at pre-morula stage, morula stage and blastocyst stage embryo transfer (99.0, 135.4, 142.6), but no statistical difference was observed between the groups ( P>0.05). 2) ICSI-FET cycles included 415 couples (433 FET cycles) and 555 babies, with a general live birth sex ratio of 111.0. After the transfer of embyos at different stages the proportion of male babies displayed no significant difference ( P>0.05). 3) PGT-FET cycles included 33 couples (33 FET cycles) and 33 babies, with a general live birth sex ratio of 153.8. Transplantation of blastocysts in IVF, ICSI or PGT had no significant effect on the proportion of male babies ( P>0.05). Conclusion:In FET cycles, compared with embryo transfer at early stage, late stage embryo transfer may be associated with a higher live birth sex ratio. The transfer of ICSI-FET embryos at different developmental stages and the transfer of blastocysts by IVF, ICSI or PGT technique did not affect the sex ratio of live birth.

10.Analysis of the related factors of spontaneous clearance of hepatitis B virus DNA in patients with chronic hepatitis B virus infection

Objective To observe the incidence of spontaneous clearance of hepatitis B virus (HBV) DNA in chronic hepatitis B (CHB) patients ,and to investigate the related factors of the spontaneous clearance of HBV DNA and to determine the time to start antiviral therapy .Methods Patients who met the inclusion criteria were recruited from the follow-up cohort of chronic HBV infection from January 2008 to August 2017 for observation .The liver function including alanine aminotransferase (ALT) levels ,HBV DNA load and serum markers of HBV were measured at baseline ,month 1 ,month 3 and month 6 of follow-up . Evaluation index included cumulative HBV DNA negative conversion rate and cumulative HBeAg negative conversion rate .Multivariable analysis was used to analyze the factors associated with the spontaneous clearance of HBV DNA .Results A total of 116 patients were recruited in this study .All the patients showed ALT level elevation at baseline .Without antiviral treatment ,the cumulative HBV DNA negative conversion rate was 12 .9% after 6-month observation .HBeAg negative conversion rate was 22 .5% .Multivariable analysis showed that patients without a family history of HBV infection ,baseline ALT level >3 times the upper limit of normal (ULN) and HBV DNA level <6 lg copies/mL had higher cumulative HBV DNA spontaneous clearance rate .HBV DNA negative conversion rate in patients whomet all the above three conditions was up to 75% .Conclusions In CHB patients and ALT level elevation for the first time , some patients could achieve spontaneous clearance of HBV DNA without antiviral therapy .Patients without a family history of HBV infection ,baseline ALT level >3 ULN and HBV DNA level <6 lg copies/mL have higher rate of cumulative HBV DNA spontaneous clearance .
