1.Amide Proton Transfer Combined with Diffusion Kurtosis Imaging in the Differential Diagnosis of Prostate Carcinoma and Benign Prostatic Hyperplasia

Purpose To explore the value of amide proton transfer(APT)imaging combined with diffusion kurtosis imaging(DKI)in the differential diagnosis of prostate carcinoma and benign prostatic hyperplasia.Materials and Methods A retrospective analysis was conducted on 120 patients who underwent multi-parameter prostate MRI and pathological biopsy at the First Affiliated Hospital of Xinxiang Medical University from January 2020 to August 2021,including 66 cases of benign prostatic hyperplasia and 54 cases of prostate carcinoma.The parameters of APT imaging and DKI,including magnetization transfer ratio asymmetry(MTRasym),mean kurtosis(MK),mean diffusion coefficient(MD)and fraction anisotropy were measured,and the parameters between the two groups were compared and analyzed.The receiver operating characteristic curve and Delong test were used to analyze the efficacy of each parameter,APT or DKI sequence alone and their combined diagnosis,and the correlation among the parameters was analyzed.Results Compared with the benign prostatic hyperplasia group,the MTRasym,MK and fraction anisotropy of the prostate carcinoma group were significantly higher,while the MD was significantly lower,with statistical significance(t=8.23,12.53,2.20,-11.12,all P<0.05).The areas under the curve for the above parameters were 0.852,0.933,0.615 and 0.910,respectively.The diagnostic efficacy of APT combined with DKI for differentiating prostate carcinoma from benign prostatic hyperplasia is numerically higher than that of APT alone or DKI alone,with the areas under the curve being 0.994,0.988 and 0.852,respectively,as well as a sensitivity of 96.30%and a specificity of 98.48%.There was a statistically significant difference in efficacy between the APT+DKI combined approach and APT alone(Z=4.387,P<0.05),while no statistically significant difference exists between the combined approach and DKI alone(Z=1.375,P>0.05).MTRasym was positively correlated with MK(r=0.45,P<0.001).MD was negatively correlated with MTRasym and MK(r=-0.439,-0.500,both P<0.001).Conclusion The parameters MTRasym,MK and MD of APT and DKI have relatively high value in distinguishing prostate carcinoma from benign prostatic hyperplasia,and the combined diagnostic efficiency of the two sequences tends to increase,with some parameters showing correlation.

Purpose To explore the value of magnetic resonance amide proton transfer(APT)imaging combined with diffusion weighted imaging(DWI)in the differential diagnosis of prostate cancer and benign prostatic hyperplasia.Materials and Methods A retrospective analysis was made on 52 patients with prostate cancer and 60 patients with benign prostatic hyperplasia in the First Affiliated Hospital of Xinxiang Medical University from February 2020 to August 2021.The APT parameter values magnetization transfer ratio asymmetry(MTRasym)and DWI parameter values apparent diffusion coefficient(ADC)of the two groups were measured,respectively.The parameter differences between the two groups were compared and analyzed,the significant parameters and the efficacy of their joint diagnosis were evaluated.Then compared the performance of each parameter and joint diagnosis,and analyze the correlation between the two parameters.Results The MTRasym in the prostate cancer group[(3.70±0.94)%]was significantly higher than that in the prostate hyperplasia group[(2.35±0.60)%](t=8.89,P<0.05);ADC value in prostate cancer group[(0.93±0.15)×10-3 mm2/s]was significantly lower than that in the prostate hyperplasia group[(1.32±0.22)×10-3 mm2/s](t=-11.01,P<0.05).The areas under the curve for identifying prostate cancer and benign prostatic hyperplasia using MTRasym value,ADC value and their combination were 0.886,0.914 and 0.966,respectively.There was no statistically significant difference in the diagnostic efficacy between the MTRasym value and the ADC value(P>0.05),and the combined diagnostic efficacy of the two was superior to the individual diagnostic efficacy of the MTRasym value and ADC value(Z=3.125,2.776,P<0.05).The MTRasym was negatively correlated with the ADC value(r=-0.469,P<0.001).Conclusion APT and DWI have high efficacy in distinguishing between prostate cancer and benign prostatic hyperplasia.The combination of the two sequences has higher diagnostic efficacy.

5.The application of quantitative MRI in the diagnosis of early intervertebral disc degeneration

Objective To evaluate the potential clinical value of T2 mapping and mDixon Quant in the diagnosis of early interver-tebral disc degeneration.Methods A total of 79 volunteers who underwent lumbar MRI examination were enrolled.All subjects were examined for 3.0T MR with T2WI,T2 mapping,and mDixon Quant while recording the condition of low back pain.The differ-ences between T2 mapping(map)value and fat fraction(FF)values of the vertebral(V)and nucleus pulposus(NP)within the Pfir-rmann Ⅰ and Pfirrmann Ⅱ intervertebral disc(grade Ⅰ 76,grade Ⅱ 87)were statistically analyzed.Receiver operating characteristic(ROC)curve analyses were performed for meaningful parameters.Results V-FF showed a mild positive correlation with degenera-tive intervertebral disc lesions,and NP-FF and NP-map values showed a mild negative correlation with lesions.There were statistically significant differences between the two groups in V-FF(P<0.001),NP-FF(P=0.005),and NP-map(P<0.001).Some measure-ments had statistically significant differences when different intervertebral disc segments were compared.Conclusion V-FF,NP-FF,and NP-map are associated with intervertebral disc degeneration.T2 mapping and mDixon Quant are potentially valuable as diagnostic tools to quantitatively assess early intervertebral disc degeneration and help diagnose.
6.Comparison on image quality of insulinoma 5.0T and 3.0T non-contrast MRI

Objective To compare image quality of 5.0T and 3.0T non-contrast MRI for displaying insulinoma.Methods Twelve patients with insulinoma were prospectively enrolled,and non-contrast abdominal T1WI,T2WI as well as diffusion-weighted imaging(DWI)were acquired using 5.0T and 3.0T MR scanners,respectively.The subjective scores of image quality of each sequence of 5.0T and 3.0T MRI,also of tumor-pancreas parenchyma contrast scores were compared.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of insulinomas were observed,and the displayed rate of insulinoma by each sequence and overall MRI were compared.Results The subjective scores of 5.0T T1WI and DWI were higher than those of 3.0T T1WI and DWI(both P<0.05),but not significantly different between 5.0T and 3.0T T2WI(P=0.166).Furthermore,the tumor-pancreas parenchyma contrast score of 5.0T T1WI was higher than that of 3.0T T1WI(P=0.023),but not significantly different between 5.0T and 3.0T T2WI,nor between 5.0T and 3.0T DWI(both P>0.05).SNR of insulinomas on 5.0T T2WI were higher than on 3.0T T2WI(P=0.015),however,no significant difference of SNR was found between 5.0T and 3.0T T1WI,nor between 5.0T and 3.0T DWI(both P>0.05).CNR of insulinomas on all 5.0T MRI were not significantly different with those on 3.0T MRI(all P>0.05).The displayed rate of insulinoma on 5.0T T1WI,T2WI and DWI was 100%(12/12),66.67%(8/12)and 83.33%(10/12),respectively,on 3.0TT1WI,T2WI and DWI was 75.00%(9/12),58.33%(7/12),66.67%(8/12),respectively.The overall displayed rate of insulinoma on 5.0T and 3.0T MRI was 100%(12/12)and 83.33%(10/12),respectively.Conclusion Compared with 3.0T MRI,5.0T MRI was superior for displaying insulinoma,hence being helpful for diagnosis.

8.Quality control of Semen Ziziphi Spinosae standard decoction based on determination of multi-components using TOF-MS/MS and UPLC-PDA technology

A sensitive, fast and comprehensive method for the quality assessment of Semen Ziziphi Spinosae (SZS) standard decoction with characterization of its chemical components was developed and validated. UPLC-Q/TOF-MS/MS system was used to identify thirty-six chemical components of SZS standard de-coction which included nucleosides, phenolic acids, alkaloids, and flavonoids. Furthermore, a UPLC-PDA method was validated to simultaneously determine adenosine, protocatechuic acid, magnoflorine, ca-techin, protocatechin, vicenin II, spinosin, kaempferol-3-rutinoside, and 6'''-feruloylspinosin which re-present four species of characteristic compounds. The qualitative method had been validated according to Chinese Pharmacopoeia (2015 edition) in terms of lineary, repeatability, recovery and stability for all analytes, with the results showing good precision, accuracy and stability. In conclusion, the method using UPLC combined with MS and PDA provided a novel way for the standardization and identification of SZS standard decoction, and also offered a basis for qualitative analysis and quality assessment of the pre-parations for SZS standard decoction.
9.Multiple model parameters of intravoxel incoherent motion in differential diagnosis of cervical carcinoma and pre-judgement of pathological types

Objective To investigate the feasibility of multiple model parameters of intravoxel incoherent motion (IVIM) in differential diagnosis of cervical carcinoma and its pathological types.Methods IVIM images of 33 patients with cervical cancer (cervical cancer group) and 30 patients with normal cervix (normal group) were retrospectively analyzed.The monoexponential model parameters (ADC-stand),biexponential model parameters (ADC-slow,ADC-fast,f) and stretched-exponential model values (distributed diffusion coefficient [DDC],α) were measured,respectively.Then the parameters were compared among the two groups,as well as among different pathological types and different pathological grades of cervical cancer.The thresholds and diagnostic efficiency of the parameter values were evaluated with ROC curve.Results ADC-stand,ADC-slow,f,DDC and α values of cervical cancer group were lower than those of normal group (all P<0.05).The area under the ROC curve of ADC-stand,ADC-slow,DDC,f and a values in diagnosis of cervical cancer was 0.93,0.94,0.96,0.94 and 0.93 (all P<0.05),and the diagnostic thresholds were 1.03 × 10-3 mm2/s,0.99 × 10-3mm2/s,30.50%,1.21 × 10-3 mm2/s and 0.699,respectively.ADC-stand,ADC-slow and DDC values in cervical squamous cell carcinoma patients were lower than those of cervical adenocarcinoma patients (all P<0.05).The area under the ROC curve of ADC-stand,ADC-slow and DDC values in differential diagnosis of cervical squamous cell carcinoma and cervical adenocarcinoma was 0.98,0.91 and 0.98 (all P<0.05),the diagnostic thresholds were 1.11 × 10-3 mm2/s,0.81×10-3 mm2/s and 0.98 × 10-3 mm2/s,respectively.There was no significant difference of the parameters among high,moderate,low differentiation of cervical squamous cell carcinoma.Conclusion Multiple model parameters of IVIM can quantitatively reflect the histological features of cervical cancer.
10.Prostatic abscess:MRI findings and ADC value quantitative analysis

Objective To investigate the conventional MRI and DWI features of prostatic abscess.Methods 8 patients with path-ologically and clinically proved prostatic abscess who were performed MRI examination in our institution were enrolled in this study. Among them,2 patients underwent CT examination and 7 patients were performed DWI examination (b = 0 and 1 000 s/mm2 ). Their CT,conventional MRI and DWI features were retrospectively analyzed.The ADC value between prostatic abscess and normal prostate tissues were compared by using paired t test.Statistical significance was inferred at P <0.05.Results 2 patients with 3 fo-cal abscesses,2 patients with 2 focal abscesses,and 4 patients with only one focal abscess.4 abscesses perforated the prostate cap-sule and involved the fat gap in front of the rectum.Prostatic abscess showed low hypointensity on T1 WI,hyperintensity on T2 WI and DWI.2 focal abscesses with low signal areas consistent both on T1 WI and T2 WI of gas.The mean ADCs of prostatic abscess were (0.854±0.223)×10 -3 mm2/s ,which were significantly lower than those of prostate tissues (1.41 6±0.1 68 )×10 -3 mm2/s (P <0.05).Conclusion Prostatic abscess has characteristic feature on MRI,and shows restricted diffusion on DWI.MRI can clearly display their size,number and invasive condition of the circumambient organs.Thus should be considered as an optimal method in the diagnosis of prostatic abscess.
