1.Management and practice of ethical review for “amendment” in drug clinical trials

Driven by the growing practical need to accelerate drug development and the continuous innovation of trial design in recent years, the number of protocol amendments during clinical trials have gradually increased, and the changed contents have become more flexible and complex, which significantly heightens the difficulty of ethical review on amendments. Against this backdrop, it is of great importance to fully leverage the role and responsibilities of ethics committees, effectively control clinical trial risks, and ensure subject safety. This paper analyzed development trends of protocol amendments in recent years, sorted out requirements for protocol amendments in Chinese regulations and guiding principles, and examined difficulties of amendment ethical review in practical work. Based on these, targeted strategies and recommendations were proposed, namely, strengthening the integration with scientific review, enhancing the formal review, adjusting the scope of review according to approval notifications, and adopting appropriate review methods, with a view to providing insights and references for the management of the amendment ethical review in drug clinical trials.
2.Analysis of current situation and countermeasures of sex education in special education schools in Luzhou

Objective:
To analyze the implementation status and challenges of sex education in special education schools, so as to provide a scientific basis for formulating effective promotion strategies.
Methods:
From November 2023 to January 2024, a census survey was conducted among 120 in service teachers from 7 special education schools in Luzhou. The questionnaire covered the current status of sex education in schools, teachers attitudes and knowledge toward sex education, and their coping methods for students inappropriate sexual behaviors.
Results:
About 77.5% of teachers reported having provided sex education to students, but 93.2% indicated a lack of specialized sex education textbooks for special children, 90.4% reported no full time teachers for sex education, and the methods of sex education were relatively limited (50.0% mainly based on lecture method). Nearly 95.8% of teachers held a positive attitude toward sex education, with 98.3% supporting its implementation. Only 26.7% of teachers demonstrated a good grasp of sex education knowledge, with the best understood topic being "recognition and protection of private parts" (21.6%). When dealing with students inappropriate sexual behaviors, the active response rate of teachers was 23.9%, with the highest active response rate observed for "intentionally hugging or kissing the opposite sex" (39.7%).
Conclusions
The special education schools in Luzhou lack comprehensive sex education curricula, teaching materials and full time teachers, sufficient knowledge among teachers, and adequate proactive responses to students inappropriate sexual behaviors. Greater emphasis should be placed on sex education for special children, including the training of dedicated teachers, to provide comprehensive and high quality sex education services for special children.
3.High-throughput sequencing technology in the identification of B cell abnormalities in systemic lupus erythematosus

Systemic lupus erythematosus (SLE) is an acute or chronic autoimmune disease characterized by the presence of pathogenic autoantibodies and immune complexes, and multiorgan damage. It is a highly heterogeneous disease and commonly developed in women of childbearing age. The cause of systemic immunopathological injury in SLE is due to the production of autoantibodies by overactivated autoreactive B cells. The treatment of SLE by targeting B cells is very effective, suggesting the critical role of B cells in the development and progression of SLE. However, the current B cell depletion therapies all target the total B cell population, which are not capable of clearing specifically autoreactive B cells since the specific marker molecules and the mechanisms associated with the development of SLE remain unclear. With the development of science and technology, high-throughput sequencing technology provides new ideas for the study of B cell abnormalities in SLE. This review focuses on the progress in high-throughput sequencing to reveal new abnormalities in B cell receptors, new B cell subsets and B cell-related novel therapeutic targets, hoping to provide reference for better understanding the pathogenesis and exploring therapeutic strategies.
4.Zoledronic acid affects osteogenic differentiation and bone formation by regulating the expression of SIRT3/P53

Objective:To explore the mechanism of zoledronic acid (ZOL) affects osteogenic differentiation and bone formation through the regulation of sirtuin 3 (SIRT3) / P53 expression.Methods:Bone marrow mesenchymal stem cells (BMSCs) were induced to differentiate into osteogenic cells, the expression of SIRT3 in the cells was detected, and the targeting regulation relationship between SIRT3 and P53 was analyzed. The intracellular expressions of SIRT3 and P53 were intervened and ZOL was used to treat the cells. MTT method, Western blot method and kit were used to detect cell viability, osteogenesis-related genes Osteoprotegerin (OPG), runt-related transcription factor 2 (Runx2) expression, alkaline phosphatase (ALP) activity and alizarin red S (ARS) staining, respectively. Ovariectomy (OVX) was used to construct a rat model and explore the effect of ZOL on the progression of osteoporosis (OP) in vivo.Results:ZOL promoted osteogenic differentiation of BMSCs. The expression of SIRT3 was down-regulated in the serum of OP patients (0.78±0.23) compared with that of healthy subjects (1.00±0.26 vs. 0.78±0.23. t=3.85, P<0.001). During the osteogenic differentiation of BMSCs, the expression level of SIRT3 gradually increased with the prolonged induction of osteogenesis. Compared with the p53 protein expression and BMSCs activity in the control group, SIRT3 knockout could increase the expression level of p53 protein (0.59±0.05 vs. 1.01±0.11. t=6.02, P=0.004) but inhibited the activity of BMSCs (100.00±8.41 vs. 51.26±5.59. t=8.36, P=0.001). After ZOL treatment, the inhibitory effect of SIRT3 on cell viability (49.61±5.11 vs. 87.61±7.31. t=7.38, P=0.002) and osteogenesis was relieved, and the level of P53 was inhibited (1.10±0.10 vs. 0.69±0.04. t=6.59, P=0.003). P53 overexpression partially offseted the effects of ZOL on cell viability (84.61±6.52 vs. 66.54±5.47. t=3.68, P=0.021) and osteogenesis. Compared with the sham surgery group, the OVX group showed inhibition of osteogenesis in rats, and ZOL treatment significantly improved osteogenic inhibition. ZOL treatment increased the expression level of SIRT3 protein in bone tissue of OVX rats, but inhibited the expression level of P53. Conclusion:ZOL promoted osteogenic differentiation and bone formation of BMSCs by promoting the ubiquitination and degradation of P53 by SIRT3.
5.Ethical exploration of clinical research empowered by remote collection of digital health technology

In recent years,with the gradual maturity of achievable remote collection of digital health technology,more and more clinical research have applied this technology to improve the quality of data collection and reduce the burden on subjects.However,its technological characteristics of detachment from the diagnostic and treatment environment and real-time transmission of sensitive information also pose corresponding risks to the protection of subjects'rights and interests and data standardization management,among which personal information protection and data control permissions are more prominent issues.Based on risk analysis and regulatory review,this paper explored the responsibilities of multiple parties,including the sponsors,researchers,clinical trial institutions,and ethics committee,as well as proposed five elements of special concern for ethical review,with a view to providing a reference for promoting the standardized development of digital health technology in clinical research.
6.Effect of calaglitzin combined with Degu insulin liraglutide and pioglitazone metformin in the treatment of obese type 2 diabetes mellitus and its effect on insulin sensitivity

Objective:To evaluate the efficacy of calaglitzin combined with Degu insulin Liraglutide and pioglitazone metformin in the treatment of obese type 2 diabetes mellitus (T2DM) .Methods:Ninety-nine obese T2DM patients admitted to our hospital from Apr. 2021 to Jun. 2023 were divided into 2 groups according to different treatment methods. The control group ( n=52) were treated with Delgu insulin liraglutide and pioglitazone metformin; the observation group ( n=47) were treated with cagaglizin on the basis of the control group, and both groups were treated for 8 weeks. The therapeutic effect of the two groups was compared. Results:The effective rate of observation group was 95.74% (45/47), higher than that of control group 82.69% (43/52) ( P<0.05). After eight weeks of treatment, the levels of FPG (7.96±0.74) mmol/L, 2hPBG (8.36±1.02) mmol/L and BMI (24.63±1.51) kg/m 2 in the observation group were higher than those in the control group ( P<0.05). After 8 weeks of treatment, HOMA-IR (2.41±0.56) of observation group was lower than that of control group (2.84±0.61), GIR (5.63±0.97) mg·kg -1·min -1 was higher than that of the control group (5.17±0.95) mg·kg -1·min -1 ( P<0.05). After eight weeks of treatment, the levels of TG (4.22±0.43) mmol/L, TC (2.15±0.21) mmol/L and ApoB (1.18±0.14) mmol/L in the observation group were higher than those in the control group (2.49±0.26) mmol/L, (4.69±0.47) mmol/L and (1.45±0.19) mmo l/L ( P<0.05). The incidence of adverse reactions in the observation group was 8.51% (4/47), lower than that in the control group (5.77% (3/52) ( P>0.05) . Conclusion:Taking calaglitzin in combination with Delgu insulin liraglutide and pioglitazone metformin can significantly reduce insulin resistance, increase the sensitivity of peripheral tissues to insulin, reduce blood glucose level and BMI level, and the drug safety is reasonable.
7.The research hotspots and trends in postanesthesia care unit:Bibliometric analysis from 2003 to 2023

Objective To quantify the basic information of publications on postanesthesia care unit(PACU)and identify the most influential research,trends,and hotspots in the field of PACU.Methods The Web of Science Core Collection database was searched for research papers on PACU from 2003 to 2023.Basic metrology information,such as publication year,country,author,source title,academic organizations,subjects,and hot spots,were collected and analyzed.Results A total of 1541 articles on PACU from 2003 to 2023 were involved in the results analysis.The highest number of articles(n=814)were published in the United States,following by China(n=146)and Canada(n=97).Dexter Franklin,the most productive author,published 30 papers on PACU research.Harvard University was the most influential academic institution,publishing 52 papers with a total of 1338 citations,averaging 25.73 citations per article,and an H-index of 20.The journal Anesthesia and Analgesia has the largest papers with 232 publications.The hotspots in the field of PACU include"postoperative pain","general anesthesia","recovery","risk factors"and"postoperative nausea".Results showed that"enhanced postoperative recovery"and"pain management"are the burst keywords in the past two decades.Conclusion Based on the bibliometric research method,this study provides a comprehensive analysis and summary of the development of the PACU research field.In the future,"enhanced postoperative recovery"and"pain management"may be hot research trends in this field.
8.Comparative analysis of differentially expressed genes for biosynthesis of active ingredients in fruits of different cultivars of Lycium barbarum L. based on transcriptome sequencing.

To explore the differentially expressed genes (DEGs) related to biosynthesis of active ingredients in wolfberry fruits of different varieties of Lycium barbarum L. and reveal the molecular mechanism of the differences of active ingredients, we utilized Illumina NovaSeq 6000 high-throughput sequencing technology to conduct transcriptome sequencing on the fruits of 'Ningqi No.1' and 'Ningqi No.7' during the green fruit stage, color turning stage and maturity stage. Subsequently, we compared the profiles of related gene expression in the fruits of the two varieties at different development stages. The results showed that a total of 811 818 178 clean reads were obtained, resulting in 121.76 Gb of valid data. There were 2 827, 2 552 and 2 311 DEGs obtained during the green fruit stage, color turning stage and maturity stage of 'Ningqi No. 1' and 'Ningqi No. 7', respectively, among which 2 153, 2 050 and 1 825 genes were annotated in six databases, including gene ontology (GO), Kyoto encyclopedia of genes and genomes (KEGG) and clusters of orthologous groups of proteins (KOG). In GO database, 1 307, 865 and 624 DEGs of green fruit stage, color turning stage and maturity stage were found to be enriched in biological processes, cell components and molecular functions, respectively. In the KEGG database, the DEGs at three developmental stages were mainly concentrated in metabolic pathways, biosynthesis of secondary metabolites and plant-pathogen interaction. In KOG database, 1 775, 1 751 and 1 541 DEGs were annotated at three developmental stages, respectively. Searching the annotated genes against the PubMed database revealed 18, 26 and 24 DEGs related to the synthesis of active ingredients were mined at the green fruit stage, color turning stage and maturity stage, respectively. These genes are involved in carotenoid, flavonoid, terpenoid, alkaloid, vitamin metabolic pathways, etc. Seven DEGs were verified by RT-qPCR, which showed consistent results with transcriptome sequencing. This study provides preliminary evidences for the differences in the content of active ingredients in different Lycium barbarum L. varieties from the transcriptional level. These evidences may facilitate further exploring the key genes for active ingredients biosynthesis in Lycium barbarum L. and analyzing their expression regulation mechanism.

9.The Role and Positioning of Clinical Trial Ethics Review under Pharmaceutical Innovation Strategy

Ethical review runs through the whole process of drug clinical trials, and is a critical step to ensure the rights and interests of subjects. This paper analyzed and discussed the role and positioning of ethical review in new drug clinical trials, cleared the principles of ethical review, identified the responsibilities of ethical review, and clarified the authority of ethical review approval documents. The ethical review should primarily focus on the ethics of the clinical trials, not replace other professional institutions to review the clinical trials’ legality and scientific nature. Ethical approval is only one of the necessary conditions for conducting clinical trials, not the only factor. It is recommended to strengthen the publicity and popularization of scientific and technological ethics awareness, improve the clinical trial approval mechanism, and optimize the phrasing of ethical review approval documents. It is warranted to further optimize the quality of ethical review, improve the construction of ethical review system, ultimately achieve the unity of promoting innovation and preventing risks, so as to effectively realize the benign interaction between high-quality development of scientific and technological innovation and high-level safety.
10.Construction of a sensitive index system for nursing quality of patients with intracranial aneurysm rupture

Objective:To construct a sensitive index system for nursing quality of patients with intracranial aneurysm rupture, so as to provide a basis for evaluating the nursing quality of patients with intracranial aneurysm rupture.Methods:Based on the "structure-process-outcome" quality structure model, a sensitive index system for nursing quality of patients with intracranial aneurysm rupture was preliminarily developed through literature analysis and semi-structured interviews. From December 2021 to March 2022, the Delphi method was used to conduct two rounds of consultation with 23 experts to establish a sensitive index system for nursing quality of patients with intracranial aneurysm rupture.Results:In the two rounds of consultation, the effective response rates of the questionnaires were all 100% (23/23), with expert authority coefficients of 0.78 and 0.75, and the Kendall's W of 0.170 and 0.101 respectively ( P<0.05). The final sensitive index system for nursing quality of patients with intracranial aneurysm rupture included 3 first-level indicators, 9 second-level indicators, and 39 third-level indicators. Conclusions:The sensitive index system for nursing quality of patients with intracranial aneurysm rupture is reliable and scientific, and can be used as a basis for evaluating the nursing quality of patients with intracranial aneurysm rupture.
