1.Exploration on the Influence of Jiedu Huayu Granules on the Clinical Efficacy of Patients with Liver Failure from the Perspective of Endoplasmic Reticulum Stress and Inflammatory Response

Objective To investigate the clinical efficacy of Jiedu Huayu Granules(mainly composed of the Chinese herbal medicines of Artemisiae Scopariae Herba,Rhei Radix et Rhizoma,Hedyotis Diffusae Herba,Paeoniae Radix Rubra,Curcumae Radix and Acori Tatarinowii Rhizoma)in the treatment of liver failure(LF)and to observe their effects on endoplasmic reticulum stress and inflammatory response.Methods From May 2022 to May 2024,a total of 102 patients with LF of toxin-heat-stasis stagnation syndrome were collected from Hepatology Department of Yulin Traditional Chinese Medicine Hospital,and were divided into the control group and the study group by using a table of random numbers,with 51 cases in each group.The two groups were given comprehensive western medicine treatment for inhibiting inflammatory response and protecting liver,regulating intestinal microecology,counteracting viral infection,decreasing transaminase level and treating jaundice.Additionally,the study group was treated with Jiedu Huayu Granules.The course of treatment for the two groups covered eight weeks.Before and after treatment,the two groups were observed in the changes of traditional Chinese medicine(TCM)syndrome scores,biochemical indicators,inflammatory factors and endoplasmic reticulum stress indicators.After treatment,the clinical efficacy and the incidence of adverse reactions of the two groups of patients were compared.Results(1)After eight weeks of treatment,the total effective rate of the study group was 86.27%(44/51)and that of the control group was 68.63%(35/51),and the intergroup comparison(tested by chi-square test)showed that the efficacy of the study group was significantly superior to that of the control group(P<0.05).(2)After treatment,the TCM syndrome scores in both groups were significantly decreased compared with those before treatment(P<0.01),and the decrease in the study group was significantly superior to that in the control group(P<0.05).(3)After treatment,the serum levels of biochemical indicators of total bilirubin(TBIL),total bile acid(TBA),alanine transaminase(ALT),and aspartate transaminase(AST)in the two groups were significantly decreased compared with those before treatment(P<0.05),and the decrease in the study group was significantly superior to that in the control group(P<0.05 or P<0.01).(4)After treatment,the serum levels of inflammatory factors of interleukin 6(IL-6),tumor necrosis factor α(TNF-α),transforming growth factor β(TGF-β),and C-reactive protein(CRP)in the two groups were significantly decreased compared with those before treatment(P<0.05),and the decrease in the study group was significantly superior to that in the control group(P<0.01).(5)After treatment,the levels of endoplasmic reticulum response indicators such as glucose-regulated protein 78(GRP78),CCAAT enhancer-binding protein homologous protein(CHOP),and cysteine aspartic acid specific protease 12(caspase-12)in the two groups were significantly decreased compared with those before treatment(P<0.05),and the decrease in the study group were significantly superior to those in the control group(P<0.01).(6)The incidence of adverse reactions in the study group was 15.69%(8/51)and that in the control group was 17.65%(9/51),and the comparison between the two groups showed that the difference was not statistically significant(P>0.05).Conclusion The combined use of Jiedu Huayu Granules with comprehensive western medicine treatment exerts certain efficacy in treating patients with LF of toxin-heat-stasis stagnation syndrome.The combined therapy is effective on relieving clinical symptoms and improving liver function,and is helpful for relieving endoplasmic reticulum stress and inflammatory response.
2.Classification prediction of exercise perception in elderly hip arthroplasty patients and nursing implications

Objective To explore the potential categories of exercise perception in elderly hip arthroplasty(HA)patients,analyze the prediction factors of different categories,and provide references for clinical nursing.Methods A convenience sampling method was used to select elderly HA patients treated in 6 orthopedic wards of 2 tertiary A hospitals in Guilin from October 2024 to February 2025.The general data questionnaire,Exercise Benefits and Barriers Perception Scale for Hip/Knee Throplasty Patients,Tampa Scale of Kinesiophobia,the 5-Factor Modified Frailty Index,and Self-Efficacy for Rehabilitation Outcome Scale were used for investigation.Latent profile analysis was used to identify the potential categories of exercise perception of elderly HA patients,and decision tree model was used to explore the core predictive factors of different profile.Results 222 valid questionnaires were collected,with an effective response rate of 96.52%.A total of 222 elderly HA patients were divided into 3 profiles:high benefit-low barrier group(19.82%),low benefit-high barrier group(22.07%),and mild barrier group(58.11%).Frailty,kinesophobia,rehabilitation self-efficacy,residence,educational level and daily exercise were significant predictors of latent profiles(P<0.05),and the frailty was located in the root node of the decision tree model.Conclusion The level of exercise perception in elderly HA patients needs to be improved,and shows population heterogeneity.Medical staff should prioritize interventions for low benefit-high barrier and mild-barrier groups,implementing targeted strategies based on profile characteristics and predictive factors to improve their exercise perception.
3.Study on apoptosis of canine mammary tumor cells induced by oleuropein mediated PI3K/AKT signaling pathway

Canine mammary tumors are one of the most common causes of cancer-related death in female dogs,challenging traditional surgical therapies due to their high recurrence and metastasis.The natural compound Ole is strongly associated with a variety of phenomena,such as increased longevity,reduced disease incidence and mortality.However,its effect on canine mammary tumors is unclear.Therefore,the following experiments were conducted to investigate whether Ole has an effect on the development of canine mammary tumors:The effects of Ole at different concentra-tions on CHMm cell viability,migration ability,apoptosis,expression levels of corresponding pro-teins Bax and BCL-2 and mRNA expression of tumor-related genes were detected by scratch test,flow cytometry,RT-qPCR and protein western blot,and the mechanism of action was also ex-plored.The results showed that Ole activated the PI3K/AKT signaling pathway,induced phospho-rylation of related proteins,and significantly inhibited the expression levels of p-PI3K and p-AKT after applying OLE to CHMm of canine mammary tumor cells.At the same time,Ole significantly inhibited the survival,migration and proliferation of canine mammary tumor cells.In addition,the expressions of proteins Bax and BCL-2,which are closely related to apoptosis,and mRNA expres-sions of some tumor-related genes were significantly regulated,suggesting that Ole mediated PI3K/AKT signaling pathway effectively induced apoptosis of canine mammary tumor cells.This study suggests that Ole has the potential to become a key drug in the treatment of mammary canc-er,and provides a key target for the development of effective therapies against canine mammary tumors.
4.Research on the trinity path of integrating narrative education into the cultivation of humanistic care a-bility for higher vocational nursing students

Objective To explore the effectiveness and practical pathways of integrating narrative education into higher vocational nursing courses for cultivating students' humanistic care ability.Methods A total of 156 nursing students from a higher vocational college were randomly divided into an observation group(narrative education intervention)and a control group(traditional teaching).Quantitative evaluations were conducted using the Caring Ability Inventory(CAI)and a self-developed Humanistic Care Quality Scale,supplemented by semi-structured interviews for qualitative thematic analysis.Results The ob-servation group demonstrated significantly higher total caring ability scores(180.1±10.9 vs 145.2±14.3),humanistic care quality(97.5±3.8 vs 77.7±5.9),and course satisfaction(9.5±0.6 vs 7.0±1.3)compared to the control group(P<0.01).Qualitative findings revealed that narrative education enhanced professional beliefs and practical skills through empathy training,reflective writing,and team collaboration.Conclusion Narrative education systematically improves the humanistic care ability and professional of higher vocational nursing students,providing empirical support for nursing education reform.

8.Study on metabolites derived from Zhideke granules in rats in vivo

OBJECTIVE To analyze the metabolites of Zhideke granules and speculate its metabolic pathway in rats in vivo. METHODS Male SD rats were randomly divided into blank group and administration group (Zhideke granules, 9.45 g/kg); they were given ultrapure water or relevant medicine, twice a day, every 6-8 h, for 3 consecutive days. Serum, urine and feces samples of rats were collected, and their metabolites were identified by UPLC-Q-Exactive-MS technique after intragastric administration of Zhideke granules; their metabolic pathways were speculated. RESULTS After intragastric administration of Zhideke granules, 16 prototype components (i.g. irisflorentin, baicalin, chlorogenic acid) and 11 metabolites (i.g. hydration products of kaempferol or luteolin, methylation products of chlorogenic acid, and hydroxylation products of baicalin) were identified in serum, urine and feces of rats. Among them, 8 prototype components and 4 metabolites were identified in serum samples; 10 prototype components and 7 metabolites were identified in urine samples; 8 prototype components and 5 metabolites were identified in the fecal samples. CONCLUSIONS The metabolites of Zhideke granules in rats mainly include baicalin, irisflorentin,chlorogenic acid, and the main metabolic pathways included methylation, hydroxylation, glucuronidation.

We conducted a prospective study to assess the non-inferiority of adjuvant chemotherapy alone versus adjuvant concurrent chemoradiotherapy (CCRT) as an alternative strategy for patients with early-stage (FIGO 2009 stage IB-IIA) cervical cancer having risk factors after surgery. The condition was assessed in terms of prognosis, adverse effects, and quality of life. This randomized trial involved nine centers across China. Eligible patients were randomized to receive adjuvant chemotherapy or CCRT after surgery. The primary end-point was progression-free survival (PFS). From December 2012 to December 2014, 337 patients were subjected to randomization. Final analysis included 329 patients, including 165 in the adjuvant chemotherapy group and 164 in the adjuvant CCRT group. The median follow-up was 72.1 months. The three-year PFS rates were both 91.9%, and the five-year OS was 90.6% versus 90.0% in adjuvant chemotherapy and CCRT groups, respectively. No significant differences were observed in the PFS or OS between groups. The adjusted HR for PFS was 0.854 (95% confidence interval 0.415-1.757; P = 0.667) favoring adjuvant chemotherapy, excluding the predefined non-inferiority boundary of 1.9. The chemotherapy group showed a tendency toward good quality of life. In comparison with post-operative adjuvant CCRT, adjuvant chemotherapy treatment showed non-inferior efficacy in patients with early-stage cervical cancer having pathological risk factors. Adjuvant chemotherapy alone is a favorable alternative post-operative treatment.

10.Optimization of the Extraction Technology of Zhideke Granules

OBJECTIVE:To optimize the extraction technology of Zhideke granules. ME THODS:The extraction technology (water extraction ,alcohol extraction ,water extraction and ethanol precipitation )of Zhideke granules was initially screened by ammonia-induced cough experiment and xylene-induced ear swelling experiment in mice. Based on its preparation route ,the immersion time of medicinal materials containing volatile oil was investigated with water absorption as index firstly. The single factor test was adopted to investigate the amount of water added and the extraction time taking the volatile oil yield as index to optimize the extraction technology of medicinal materials containing volatile oil. Taking the contents of irisflorentin and total flavonoids as indicators ,on the basis of single factor investigation ,orthogonal test was adopted to examine the influence of three factors including the amount of water added ,extraction time and extraction frequency ,so as to optimize the water extraction technology of Zhideke granules and the validation tests were conducted. RESULTS :The results of pharmacodynamics experiment showed that the cough latency of mice in water extract low-dose and high-dose groups (6.34,12.68 g/kg,by crude drug )and water-extraction alcohol-precipitation extract high-dose group (12.68 g/kg,by crude drug )were significantly longer than those inmodel group ,and the number of cough within 2 minutes was significantly reduced (P<0.05 or P<0.01). Compared with model group , the ear swelling of mice in water extract low-dose and high-dose groups (6.34,12.68 g/kg,by crude drug),ethanol extract high-dose group (12.68 g/kg,by crude drug) and water-extraction alcohol-precipitation extract hig dose group (12.68 g/kg,by crude drug ) were decreased significantly (P<0.05 or P<0.01). The swelling inhibition rates were 42.26%,55.08%,33.49%,51.56%,39.57% and 44.36% in low-dose and high-dose groups of water extract ,alcohol extract , water-extraction and alcohol-precipitation extract respectively ,indicating that the water extract had better antitussive and anti-inflammatory effects. The optimal extraction technology of volatile oil was adding 5-fold water ,soaking for 30 minutes,and extracting for 3 hours. The optimal water extraction technology was adding 12-fold water ,extracting for 3 times after soaked for 50 min,lasting for 1 h each time. Results of 3 times of validation tests showed that average content of irisflorentin in the extract obtained by optimal technology was 76.47 μg/g(RSD= 2.15%,n=3)and the average content of total flavonoids was 92.45 mg/g(RSD=0.48%,n=3). CONCLUSIONS :The optimal extraction technology of Zhideke granules is stable and feasible.
