1.Analysis of monitoring data of 131I internal exposure among nuclear medicine staff in Guangdong Province, China, 2022-2024

Objective To analyze the monitoring data of 131I internal exposure in Guangdong Province over the past 3 years, study the thyroid 131I contamination levels among nuclear medicine staff, and provide a basis for optimizing the monitoring plan. Methods Using a convenience sampling method, 574 detections of nuclear medicine staff from 28 hospitals in Guangdong Province were included in the study. The thyroid 131I activity was measured directly in vivo. Differences in detection rates were analyzed, and committed effective doses were estimated. Results Over the past 3 years in Guangdong Province, 131I accumulation was found in the thyroids of staff from 14 hospitals for 62 (10.8%) detections, with an average activity of 154.8 (13.2-8730.6) Bq. The adjusted annual intake was 50.7 (4.5-820.6) kBq. The committed effective dose was 0.56 (0.05-9.03) mSv/a, and the values were ≥ 2 mSv for 8 persons. Among different positions, nurses showed the highest detection rate (19.6%), followed by cleaners (14.7%), with significant differences (P<0.05). Among different exposure types, the detection rate was highest for loading medication into automatic dispenser (23.3%), and the mean activity was highest for entering thyroid cancer patient ward (495.4 Bq), with significant differences (P<0.05). Conclusion Monitoring of 131I internal exposure in Guangdong Province found that the radiation doses of some staff members had exceeded the threshold specified in other investigations. Routine personal 131I internal exposure monitoring should be conducted, with increased monitoring frequency. Priority should be given to nurses and cleaners, with monitoring extended to cleaners and outsourced dispensing staff.
2.Epidemiological characteristics and trends of preterm births in China from 2017 to 2022

Objective:To analyze the epidemiological characteristics and trends of preterm births in China using medical institution survey data, thereby providing epidemiological data support for perinatal care.Methods:Based on a nationwide sampling survey on healthcare quality data from 2017 to 2022, this study included 3 547, 4 436, 4 513, 4 535, 5 068, and 5 790 medical institutions, with 7 039 107, 8 926 441, 9 006 420, 7 051 984, 7 311 862, and 7 354 062 parturient women, respectively. The overall rates of preterm birth (live births at 28 to 36 +6 weeks of gestation/overall live births) and early preterm birth (live births at 28 to <34 weeks of gestation/overall live births) were calculated at the national level, across diverse provinces, autonomous regions, municipalities and Xinjiang Production and Construction Corps, and for various levels of medical institutions. Generalized estimating equations were used to analyze the influence of maternal characteristics and medical institution characteristics on the rates of preterm birth and early preterm birth. Results:From 2017 to 2022, both the preterm birth rate and the early preterm birth rate in China showed a continuous increase. The preterm birth rate rose from 5.13% (363 036/7 079 454) in 2017 to 6.56% (487 150/7 424 734) in 2022, and the early preterm birth rate increased from 1.32% (118 021/8 971 870) in 2018 to 1.43% (106 157/7 424 734) in 2022. These rates showed an overall increasing trend in private, secondary public specialty, and general hospitals. In tertiary public specialty hospitals, these rates fluctuated around 8.0% and 2.0% from the year 2018, respectively, while in tertiary public general hospitals, these rates peaked in 2020 at 8.63% (205 570/2 381 523) and 2.19% (52 197/2 381 523), respectively. Compared with 2017, by 2022, the preterm birth rate had increased to varying degrees in all provinces, autonomous regions, municipalities and Xinjiang Production and Construction Corps, except for Henan Province [preterm birth rate in 2017 was 6.22% (27 173/437 187); preterm birth rate in 2022 was 5.83% (37 604/645 104)]. As for the early preterm birth rate, it showed a decline in Fujian, Guangdong, Guangxi, Hainan, Henan, Jiangsu, Shanghai, Xinjiang, Yunnan, and Zhejiang, but had increased to varying degrees in all other provinces , autonomous regions, municipalities and Xinjiang Production and Construction Corps across the country. The grade and location of medical institutions both had a significant impact on the preterm birth rate and early preterm birth rate (both P<0.05). For every 1% increase in the proportions of multiparous women, women of advanced maternal age, or twin pregnancies, the preterm birth rate increased by 0.014%, 0.042%, and 0.763%, and the early preterm birth rate increased by 0.004%, 0.013%, and 0.239%, respectively (all P<0.05). Conclusion:From 2017 to 2022, the preterm birth rate and early preterm birth rate in China have continued to rise, reflecting the dual challenges of changing characteristics in the childbearing population and the uneven distribution of medical and health resources faced by maternal and child healthcare in China.
3.Relationship of TyG index and atherogenic index of plasma with coronary artery remodeling in elderly patients with CHD and T2DM

Objective To investigate the relationship of triglyceride-glucose(TyG)index and atherogenic index of plasma(AIP)with positive remodeling of coronary arteries in elderly coro-nary heart disease(CHD)patients with concomitant type 2 diabetes mellitus(T2DM).Methods A total of 120 elderly CHD patients with concomitant T2DM admitted to our department from January 2022 to June 2023 were continuously recruited,and according to the remodeling index,they were divided into positive remodeling group(47 cases)and non-positive remodeling group(73 cases).The clinical data were compared between the two groups.Multivariate logistic analysis was used to identify the risk factors for positive remodeling of coronary artery.Spearman correla-tion analysis was employed to analyze the correlation of TyG and AIP with positive remodeling.ROC curve was plotted to analyze the predictive value of TyG and AIP for the remodeling.Results The positive remodeling group had larger ratio of smoking,higher TG and HbA1c levels,and in-creased TyG index and AIP,but lower HDL-C and blood calcium levels than the non-positive re-modeling group(P<0.05,P<0.01).Univariate logistic regression analysis showed that smoking,TG,HDL-C,HbA1c,blood calcium,TyG and AIP were risk factors for positive coronary artery remodeling in elderly patients with CHD and T2DM(P<0.05,P<0.01).Multivariate logistic re-gression analysis showed that TyG index(OR=7.253,95%CI:2.458-13.364,P=0.035)and AIP(OR=6.017,95%CI:2.205-12.025,P=0.030)were independent risk factors for positive coronary artery remodeling in elderly patients with CHD and T2DM(P<0.05).The AUC value of TyG index and AIP in predicting positive coronary artery remodeling in elderly CHD patients with T2DM was 0.783 and 0.766,respectively,and the value of their combination in the prediction was 0.868,which was significantly better than the indicator alone(P<0.05).Conclusion TyG in-dex and AIP are closely associated with positive coronary artery remodeling in elderly CHD pa-tients with T2DM,and can be used as effective indicators in the prediction of positive remodeling,which is of great significance for early clinical identification of high-risk patients and formulation of individual intervention strategies.

The female reproductive system is essential for sustaining reproductive endocrine homeostasis,however,its vulnerability to various endogenous and exogenous insults,including pathological conditions,pharmacological agents,genetic predispositions,and environmental factors,often results in compromised fertility.The existing protective approaches(including surgical interventions,hormonal replacement therapies,and assisted reproductive techniques)are constrained by several limitations,such as adverse therapeutic effects,technical complexities,and their incapacity to reverse ovarian senescence.Artemisinin and its derivatives(ARTs),characterized by their unique endoperoxide bridge configuration,have exhibited outstanding therapeutic performance across multiple domains including malaria treatment,anticancer therapy,inflammation modulation,and parasitic infection control.Emerging research has identified their novel protective capabilities against various reproductive system pathologies.This comprehensive review systematically elucidates the molecular mechanisms underlying artemisinin-based interventions in reproductive pathologies and evaluates their clinical translation prospects,thereby proposing innovative strategies for the development of next-generation fertility-protective agents with enhanced safety and efficacy profiles.
5.Prognostic analysis of laparoscopic-assisted Soave procedure for common hirschsprung's disease in infants younger than 6 months

Objective To evaluate the prognosis of infants with common Hirschsprung's disease who undergo laparoscopic-assisted Soave procedure below 6 months of age,and to explore its feasibility.Methods A retrospective analysis was conducted on the clinical data of 82 cases of common Hirschsprung's disease admitted to our department from January 2018 to August 2022.The patients were divided into two groups based on their age at the time of surgery:Young surgical age group(<6 months,50 cases)and Old surgical age group(≥ 6 months,32 cases).All patients underwent the treatment of laparoscopic-assisted Soave procedure.The surgical time,length of hospital stay,postoperative complications,and anal function were compared between the two groups.Results There were no significant differences between young surgical age group and old surgical age group in terms of surgical time[(145.04±11.21)min vs.(149.25±12.18)min,P=0.20],length of hospital stay[(13.56+0.93)d vs.(13.91±0.99)d,P=0.55],postoperative complications[22.00%(11/50)vs.28.13%(9/32),P=0.53],and Rintala score for anal function[(18.58±1.44)vs.(17.22±1.64),P=0.06].The main postoperative complications,including enterocolitis[10.00%(5/50)vs.12.50%(4/32),P=0.72],fecal/anal incontinence[6.00%(3/50)vs.6.25%(2/32),P=0.96],and constipation[4.00%(2/50)and 6.25%(2/32),P=0.65],showed no significant differences between the two groups(all P>0.05).However,there was a statistically significant difference in terms of bleeding volume[(7.86±2.02)ml vs.(9.13±2.17)ml,P<0.05].No deaths occurred in this study.Conclusion Laparoscopic-assisted Soave procedure is safe and effective for infants below 6 months of age with common congenital Hirschsprung's disease.It can achieve satisfactory anal function and reduce the incidence of adverse events during the waiting period for surgery.

7.Effect of modified TROPIS operation and open and hanging operation on high anal fistula and its effect on postoperative recovery and function

Objective:To investigate the effects of modified TROPIS surgery, incorporating both open and hanging stitches on the treatment of elevated anal fistula.Methods:For our study, 92 patients with high anal fistula, admitted between Jan. 2023 and Jun. 2024, were selected. They were divided into two categories: the observation group (45 modified TROPIS surgery) and the control group (47 cuts) ,to observe the clinical efficacy, postoperative recovery, quality of life and complications of the two groups.Results:Compared with the control group, the operation time, intraoperative blood loss and postoperative hospitalization time decreased in the observation group ( P<0.05) . Compared with the control group, the healing time of anal fistula in the observation group was (28.01±5.02) d, shorter than that in the control group (35.05±7.09) d, the healing rate was 97.78%, higher than that in the control group (80.85%) , and the recurrence rate was 4.44%, lower than that in the control group (19.14%) ( P<0.05) . The degree of postoperative pain and Wexner anal incontinence score in the observation group were lower than those in the control group ( P<0.05) . The life quality score of the observation group was higher than that of the control group ( P<0.05) . The complication rate of observation group was lower than that of control group ( P<0.05) . Conclusions:The modified TROPIS surgical method evidently provides advantages in anal fistula treatment. It can not only efficiently eliminate infections but also reduce anal movements and decrease the chances of post-surgical injuries and complications, presenting enhanced clinical options and suggesting a promising outlook for traditional anal fistula surgery.

Objective:To investigate the inflammatory indicator levels and influencing factors on middle-aged and elderly male residents at different residential distances from a uranium mine in Guangdong, so as to provide scientific data for the health risk assessment of the residents therein.Methods:With stratified sampling method, two groups of the permanent middle-aged and elderly male residents were randomly sampled within < 10 km and 10-20 km of the uranium mine, along with the basic demographic characteristics and lifestyle information collected through face-to-face questionnaire survey. Both height and weight of the respondents were measured using standard method and their body mass indexes (BMI) were calculated. Through collection of the venous blood, the levels of a wide range of inflammatory indicators were measured, such as C-reactive protein (CRP), complement C3, complement C4, leukocytes, lymphocytes, neutrophils, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and others. Multiple linear regression was used to analyze the influencing factors on the levels of inflammation indicators.Results:A total of 867 middle-aged and elderly male residents were included in this study, including one group of 431 residents within the 10-20 km at an average age of 51.54±5.37, and a second group of 436 residents within <10 km at an average age of 52.05±5.24. The result of multiple linear regression showed a positive correlation of the distance groups with complement C4 level ( β=0.014, 95% CI: 0.001-0.027) and lymphocyte number ( β=0.086, 95% CI: 0.003-0.168) ( t=2.07, 2.04, P<0.05). The ages of middle-aged and elderly male residents around the uranium mine was positively correlated ( t=2.50, P<0.05) with the levels of complement C4 ( β=0.018, 95% CI: 0.004-0.031), and negatively correlated ( t=-3.25, -2.97, P<0.05) with the levels of TNF-α ( β=-63.022, 95% CI: -101.114 to -24.929) and IL-6 ( β=-14.694, 95% CI: -24.396 to -4.992). Smoking was positively correlated ( t=4.29, 4.81, 3.19, P<0.05) with leukocytes number ( β=0.630, 95% CI: 0.341-0.918), lymphocytes number ( β=0.226, 95% CI: 0.134-0.319) and neutrophils number ( β=0.372, 95% CI: 0.143-0.601), and negatively correlated ( t=-2.07, -1.98, P<0.05) with the levels of TNF-α ( β=-43.551, 95% CI: -84.778 to -2.324) and IL-6 ( β=-10.603, 95% CI: -21.103 to -0.102). BMI was positively correlated ( t=8.60, 3.62, 3.18, 4.01, 2.10, P<0.05) with complement C3 level ( β=0.108, 95% CI: 0.084-0.133), complement C4 level ( β=0.026, 95% CI: 0.012-0.039), leukocytes number ( β=0.433, 95% CI: 0.166-0.699), lymphocyte number ( β=0.175, 95% CI: 0.089-0.261), and neutrophil number ( β=0.226, 95% CI: 0.014-0.438). Fruit consumption ( β=0.017, 95% CI: 0.001-0.034) was positively correlated with complement C4 levels ( t=2.10, P<0.05). Conclusions:This study showed no significant correlation between the inflammatory index levels of middle-aged and elderly male residents around the uranium mine and uranium mining. Age, smoking, BMI and fruit consumption were the influencing factors on the levels of inflammatory indicators of middle-aged and elderly male residents.
