The Guidelines for Occupational Hazard Assessment and Control of Active Pharmaceutical Ingredient in the Pharmaceutical Industry (T/WSJD 60—2024) is the first guiding standard in the field of health in China that focuses on occupational health protection for active pharmaceutical ingredient (API). It covers the general principles, work procedures, assessment methods, and control strategies for API occupational hazard assessment, providing practical guidance and recommendations for pharmaceutical enterprises to eliminate or reduce occupational health risks associated with API, improve working environment, and enhance refined management practices. This article interpreted and analyzed the background of standard establishment, formulation process, fundamental basis, and main content, to provide scientific and comprehensive technical support for occupational health managers in the pharmaceutical industry to better apply this standard.
2.miR-146a chitosan nanoparticles preparation and protective effects on acute liver cell injury induced by LPS

This study aims to investigate the characteristics of chitosan-loaded miR-146a nanoparti-cles and explore their protective effect against acute liver cell injury induced by LPS in vitro.Galac-tosylated arginine chitosan(GCA)nanoparticles were successfully synthesized,prepared and used to further prepare GCA-miR-146a nanoparticles with different mass ratios by ion cross-linking method.The binding efficiency of miR-146a by GCA nanoparticles was detected by gel retardation experiment.Particle size,morphology and potential were observed and detected by transmission e-lectron microscopy and Zetasizer NanoZS90 respectively.The cytotoxicity of GCA nanoparticles on HepG2 cells was detected by CCK-8 kit.Cellular uptake was assayed by fluorescence microscopy to select the optimum ratio for the further study.The LPS induced acute liver cell injury were evalua-ted by real-time fluorescence quantitative PCR.The results showed that the mass ratio of GCA-miR-146a was 5∶1.Encapsulation efficiency and drug loading efficiency of the nanoparticles reached 97.11%and 20.08%,respectively.The 100 nm nanoparticles with 1.15 mV surface charge showed uniform morphology.The GCA-miR-146a nanoparticles had no cytotoxicity and had high transfection efficiency on HepG2 cells.The study demonstrated that GCA-miR-146 ananoparticles could significantly increase the expression of miR-146a in HepG2 cells and reduce the mRNA ex-pression levels of IL-6 and TNF-α in vitro.The prepared GCA nanoparticles loaded with miR-146a had high loading efficiency and could protect LPS-induced acute liver cell injury in vitro.
3.Epidemiological Characteristics of Female Breast Cancer in Henan Province in 2020 and Its Temporal Trends from 2010 to 2020

[Purpose]To analyze the incidence and mortality rates of female breast cancer in Henan Province in 2020 and the trends from 2010 to 2020.[Methods]Breast cancer incidence and mor-tality data stratified by urban and rural areas and age groups were collected from Henan Provincial tumor registry,and the province's household population statistics were used.The crude incidence/mortality rate,age-standardized incidence/mortality rate by Chinese standard population(ASIRC/ASMRC)and world standard population(ASIRW/ASMRW),cumulative rate(0~74 year old)were calculated.The annual percentage change(APC),average annual percentage change(AAPC)and 95%confidence interval(CI)were calculated using Joinpoint software to analyze the trends of the incidence and mortality from 2010 to 2020.[Results]In 2020,24 744 new cases and 4 989 deaths of female breast cancer were documented in Henan Province,with a crude incidence rate of 46.96/105,ASIRC of 38.43/105 and ASIRW of 35.71/105;a crude mortality rate of 9.47/105,ASMRC of 6.80/105 and ASMRW of 6.72/105,respectively.The above indicators in urban areas were signifi-cantly higher than those in rural areas.The highest incidence was observed in the age group of 50~54 years old,while the highest mortality reached in the age group of 85 years old and above.From 2010 to 2020,the overall incidence of female breast cancer showed a slow upward trend(AAPC=2.09%,95%CI:0.62%~3.58%,P=0.010),while the mortality rate exhibited a signif-icant downward trend(AAPC=-3.49%,95%CI:-5.62%~-1.30%,P=0.005).[Conclusion]The incidence and mortality rates of female breast cancer in Henan Province are still at a high level,and corresponding preventive measures and control strategies are needed to effectively reduce the health hazards of breast cancer to women.

6.Curative effect of hyperbaric oxygen assisted butylphthalide patients with acute ischemic stroke and their influence on serum ET-1,TXB2 and VEGF levels

Objective:To explore the effects of hyperbaric oxygen-assisted butylphthalide on patients with acute ischemic stroke and the effects of serum endothelin-1(ET-1),thromboxane B2(TXB2)and vascular endothelial growth factor(VEGF)levels.Methods:A total of with acute ischemic stroke were selected,and randomly divided into two groups(according to the order of admission). The control group was treated with butylphthalide,and the combined group was treated with hyperbaric oxygen-assisted butylphthalide,the treatment time was 14 days. The National Institutes of Health Stroke Scale(NIHSS)score and Barthel index score,clinical efficacy,serum ET-1,TXB2 and VEGF levels,coagulation function,hemorheology index,and adverse reactions between the two groups of patients were compared.Results:Compared with before treatment,the NIHSS scores of the two groups of patients were significantly reduced after treatment( P<0.05),and the NIHSS scores of the combined group after treatment were significantly lower than those of the control group( P<0.05). After treatment,the Barthel index scores of the two groups were significantly higher than those before treatment( P<0.05),and after treatment,compared with the control group,the combination group had significantly higher Barthel index( P<0.05). After treatment,there was a significant difference in the distribution of curative effects between the two groups,and compared with the control group(88.03%),the total effective rate of combined group(95.77%)was significantly higher( χ 2=5.724, P<0.05). Compared with before treatment,serum ET-1 and TXB2 levels of the two groups were significantly lower after treatment,compared with the control group,the serum ET-1 and TXB2 levels of combination group were significantly lower,the VEGF levels of the two groups were significantly higher than before treatment,and compared with the control group,the combination group had significantly higher VEGF levels( P<0.05). Compared with before treatment,thrombin time(TT),prothrombin time(PT),activated partial thromboplastin time(APTT)in both groups were higher after treatment,and the TT,PT,and APTT of the combination group were significantly higher than those of control group( P<0.05). There were no statistically significant difference in incidence of adverse reactions between the two groups( P>0.05). Conclusion:Hyperbaric oxygen-assisted butylphthalide has a significant clinical effect on the treatment of patients with acute ischemic stroke,and reduce vascular endothelial damage,improve blood coagulation function and hemorheology,and it is safe and reliable.

8.Clinical analysis of suspected COVID-19 patients with anxiety and depression.

OBJECTIVE:
To explore the risk factors of anxiety and depression in patients with suspected coronavirus disease 2019 (COVID-19) so as to achieve early intervention and better clinical prognosis.
METHODS:
Seventy-six patients with suspected COVID-19 in fever isolation wards of Second Hospital of Lanzhou University were enrolled From January 31, 2020 to February 22, 2020. Their clinical baseline data were collected. The anxiety of patients was assessed by Hamilton Anxiety Scale, and the depression of patients was assessed by Hamilton Depression Scale. Multivariate Logistic regression analysis was performed to explore the risk factors of anxiety and depression in these patients.
RESULTS:
Female patients are more likely to have anxiety (=3.206, 95%: 1.073-9.583, <0.05) and depression (=9.111, 95%: 2.143-38.729, <0.01) than male patients; patients with known contact history of epidemic area and personnel in epidemic area are more likely to have depression (=3.267, 95%: 1.082-9.597, <0.05).
CONCLUSIONS
During the isolation treatment of suspected COVID-19 patients, early psychological intervention should be carried out for the female patients with known contact history of epidemic area and personnel in epidemic area, and drug treatment should be given in advance if necessary.

9.Detection of Serum CA19-9 Levels in Patients with Primary Hepatic Carcinoma by Using Chemiluminescent Protein Microarray

Objective:To develop a simple,effective,low-cost,time saving chemiluminescent protein microarray to detect the serum CA19-9 level of patients with primary hepatic carcinoma.Methods:A protein microarray was developed for detecting CA19-9 levels in the serum samples after spotting mouse-derived CA19-9 monoclonal antibody on an aldehyde-based chip.Serum from primary hepatic carcinoma (n=46) and healthy controls (n=32) were tested by using this assay.Results:The results showed that 24 out of 46 patients with primary hepatic carcinoma had serum CA19-9 levels above 37 U/mL,and 22 out of 46 patients with primary hepatic carcinoma had serum CA19-9 levels under 37 U/mL.In healthy control,30 out of 32 total cases were under 37 U/ml.Only two healthy controls were 37 U/mL.The sensitivity,specificity and AUC of protein microarray were 52.17%,93.75%,0.688 [95% CI:0.566,0.811].Conclusion:A chemiluminescent protein microarray method was established for detection of CA 19-9 in serum.
10.The urate-lowering efficacy of febuxostat and its effect on renal function in hyperuricemic patients with chronic kidney disease stages 3-5

Objective To investigate the urate-lowering efficacy and renal effect of febuxostat in hyperuricemic patients with chronic kidney disease (CKD) stages 3-5. Methods A prospective, randomized, controlled trial of CKD stages 3-5 patients with hyperuricemia was conducted from June 2015 to June 2016. Patients were randomly assigned to either febuxostat group (treatment group) or allopurinol group (control group). Patients in treatment group received febuxostat 40 mg/d after study initiation, and the dosage was changed to 20 mg/d if serum uric acid (sUA)<360 μmol/L. Patients in control group were administered a dose of 100 mg/d of allopurinol. Serum uric acid, serum creatinine and other clinical parameters were measured at baseline and 1-6 months after treatment. The rate of achieving target sUA level and the change of eGFR in two groups were performed using SPSS 21.0. Results A total of 98 patients met the inclusion criteria and completed the trial. The treatment group and the control group had 51 cases and 47 cases, respectively. There was no significant difference between the two groups in age, sex, body mass index (BMI), blood pressure, serum creatinine, eGFR, sUA and renal diseases (P>0.05). At month 1-6, there were significant differences between treatment group and control group in the rate of achieving target sUA level (P<0.01). At month 1 and month 3, no statistical difference was observed in the change of eGFR between the two groups (P=0.624, P=0.319). At month 6, the changes in eGFR were +2.23 ml·min-1·(1.73 m2)-1 and-4.36 ml·min-1·(1.73 m2)-1 in the treatment and control group, respectively, and the difference between the two groups was significant (P=0.037). In patients with CKD stages 3-5, generalized estimating equation showed that after adjusting for confounding variables, the eGFR increased 1.149 ml·min-1·(1.73 m2)-1 (P=0.003) and 24-hour urinary protein decreased 0.019 g/d (P=0.037) when per 60 μmol/L decreased in sUA. Febuxostat 20 mg/d was able to keep target sUA levels in 90.2% patients with CKD stages 3-5 within half a year and no serious adverse effects appeared. Conclusions Febuxostat performs better than allopurinol in lowering urate and delaying progression of renal function in patients with CKD stages 3-5 and HUA. Febuxostat 20 mg/d may be the effective and safe maintenance dose to maintain target sUA level in patients with CKD stages 3-5, but whether it can be used as the best long-term maintenance dose needs to be further studied.
