Objective To explore syndrome and treatment law of TCM in treating pediatric pneumonia convalescence;To provide references for clinical syndrome differentiation and treatment.Methods The relevant literature about TCM in treating pediatric pneumonia convalescence was retrieved from CNKI,SinoMed,VIP,Wanfang Data and PubMed from the establishment of the databases to December 31,2024.Diagnostic basis,syndrome elements and TCM prescription information were extracted.Excel 2021 was used for frequency statistics,SPSS Modeler 18.0 was used for association rule analysis,Gephi 0.10.1 was used to complete complex network analysis,and Origin 2025 was used for data visualization.Results A total of 171 articles were included.Diagnosis basis predominantly involved symptoms,signs and course of the disease.TCM syndrome element distribution predominantly involved the lung,spleen,qi deficiency,yin deficiency,phlegm,dampness,blood stasis and heat,often presenting as a mixture of deficiency and excess patterns.A total of 98 prescriptions were collected,with high-frequency herbs primarily including tonifying and deficiency-replenishing herbs,and phlegm-resolving and antitussive herbs.The herbal properties and tastes were mainly warm,neutral,slightly cold,and sweet,bitter,pungent,primarily associated with the lung and spleen meridians.Complex network analysis identified three core patterns of syndrome-herb interactions.Conclusion The diagnosis of pediatric pneumonia convalescence relies on symptoms,signs and course of the disease as key criteria.The syndrome is often characterized by a deficiency-excess complex.Treatment should integrate the dynamic interactions among syndrome elements and adjust the balance between reinforcing healthy qi and eliminating pathogens based on the proportional weight of deficiency and excess.For deficiency patterns,it is necessary to focus on tonifying qi and nourishing yin.For concurrent syndromes or pathological factors,additional methods such as resolving phlegm,clearing heat,regulating qi,drying dampness,promoting digestion,cooling blood,and activating blood circulation may be incorporated.

Objective To compare the safety and efficacy of vancomycin in obese patients guided by trough concentration and AUC24h/MIC,and to provide data for individualized administration of vancomycin in obese patients.Methods We retrospectively collected the data of obese adult patients(BMI 30 kg/m2)who had severe infection caused by gram-positive cocci and treated with vancomycin intravenously in two Grade A tertiary hospitals in Shanghai from 2012 to 2024.The patients were assigned to trough concentration monitoring group or AUC24h/MIC monitoring group according to the therapeutic drug monitoring(TDM)method at the time of admission.Nephrotoxicity and efficacy were compared between the two groups of patients.Results A total of 22 obese patients were included in this study,including 12 in the trough concentration monitoring group and 10 in the AUC24h/MIC monitoring group.No significant difference was found between the two groups in gender,age,BMI,creatinine clearance before treatment,underlying disease,site of infection,pathogen type,or concomitant medications.The proportion of ICU admission was higher in AUC24h/MIC monitoring group.The length of ICU stay,vancomycin treatment duration,bacterial clearance rate and comprehensive efficacy rate did not show significant difference between the two groups.The average daily dose of vancomycin in trough concentration monitoring group was significantly lower than that in A UC24h/MIC monitoring group[(1.63±0.59)g vs(2.29±0.72)g,P=0.026].The average treatment duration was not significantly different between the two groups[(15.33±10.28)d vs(14.90±6.92)d,P=0.911].Compared with the trough concentration monitoring group,the initial peak concentration[(30.99±16.22)mg/L vs(19.41±5.42)mg/L,P=0.025]and overall peak concentration[(33.67±16.53)mg/L vs(22.08±3.96)mg/L,P=0.045]of vancomycin were lower in theAUC24h/MIC monitoring group,but the initial trough concentration[(11.03±8.66)mg/L vs(6.33±4.45)mg/L,P=0.139]and overall trough concentration[(13.75±9.74)mg/L vs(9.74±4.24)mg/L,P=0.218]were similar in the two groups.Vancomycin-associated nephrotoxicity did not occur in any group,but 41.7％of the patients in the trough concentration monitoring group reached the threshold of renal toxicity,i.e.trough concentration ≥15 mg/L.Conclusions Vancomycin treatment with conventional dosing regimen still have good clinical efficacy in obese adult patients.Vancomycin therapy guided by A UC24h/MIC can achieve the target value at lower concentration or exposure,which is promising for reducing vancomycin-associated nephrotoxicity.

Inflammatory bowel disease （IBD）， mainly including ulcerative colitis （UC） and Crohn's disease （CD）， is a common chronic inflammatory disease of the gastrointestinal tract， with its incidence increasing year by year. Due to its long treatment duration， difficulty in treatment， prolonged remission， and high costs， it has attracted global attention. Exploring safe， effective， and sustainable treatment regimens has become an urgent global issue. The pathogenesis of IBD is complex， involving intestinal mucosal injury，disturbances in the internal environment， and inflammatory responses. In recent years， research has found that ferroptosis is also one of the important pathogenic factors of IBD. Ferroptosis， as a new form of non-apoptotic cell death， is characterized by iron dependence， lipid peroxidation， and imbalance in the redox system. Studies have shown that inhibiting ferroptosis in intestinal epithelial cells can protect the intestinal mucosa. Targeted intervention in ferroptosis may be a new direction for the treatment of IBD. IBD is mainly treated with drugs， including corticosteroids， aminosalicylates， biologics， and immunomodulators， but drug resistance and adverse reactions are common. Traditional Chinese medicine （TCM） has unique advantages such as low cost， low drug resistance， and fewer side effects， and has accumulated rich experience in the treatment of IBD. Scholars have confirmed that TCM can inhibit ferroptosis， and recent studies have shown that TCM can not only inhibit iron-dependent lipid peroxidation in intestinal cells but also enhance the antioxidant and anti-inflammatory abilities of intestinal mucosa， thus playing a role in the treatment of IBD. Increasing evidence suggests that TCM may treat IBD by interfering with ferroptosis. This article explores the relevance of TCM intervention in ferroptosis and the treatment of IBD， discusses the possible mechanisms of ferroptosis in IBD， and aims to provide a basis for the diagnosis and treatment of IBD.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can cause acute respiratory distress syndrome, hypercoagulability, hypertension, and multiorgan dysfunction. Effective antivirals with safe clinical profile are urgently needed to improve the overall prognosis. In an analysis of a randomly collected cohort of 124 patients with COVID-19, we found that hypercoagulability as indicated by elevated concentrations of D-dimers was associated with disease severity. By virtual screening of a U.S. FDA approved drug library, we identified an anticoagulation agent dipyridamole (DIP) , which suppressed SARS-CoV-2 replication . In a proof-of-concept trial involving 31 patients with COVID-19, DIP supplementation was associated with significantly decreased concentrations of D-dimers ( < 0.05), increased lymphocyte and platelet recovery in the circulation, and markedly improved clinical outcomes in comparison to the control patients. In particular, all 8 of the DIP-treated severely ill patients showed remarkable improvement: 7 patients (87.5%) achieved clinical cure and were discharged from the hospitals while the remaining 1 patient (12.5%) was in clinical remission.

AIM: To study the chronological pharmacokinetic differences of melatonin (MEL) in non-dipper spontaneously hypertensive rats (SHR). METHODS: The HPLC detection method of MEL was established, and the specificity, precision, recovery rate and stability of the method were examined. Twelve male SD rats were divided into two groups, and a single dose of MEL (20 mg/kg) was given intragastrically at either 08:00 or 20:00, respectively. Plasma samples were collected at 0, 5, 10, 15, 20, 30, 40, 60, 90, 120, 240, 360 min after drug administration, and the plasma MEL concentration was determined by fluorescence HPLC. OBJECTIVE: To establish a method for detecting hydrazine in the air of workplace by high performance liquid chromatography with benzaldehyde derivatization. METHODS: The air samples were collected by glass fiber filter impregnated with sulfuric acid solution and desorbed by sodium dihydrogen phosphate-ethylenediamine tetraacetic acid disodium salt buffer solution. Desorption solution was derived from 1.00% benzaldehyde-acetonitrile solution, then separated by C18 liquid chromatographic column and detected by diode array detector. RESULTS: The linearity range of hydrazine mass was 12.50-150.00 μg, and the correlation coefficient was 0.999 7. The average elution efficiency of hydrazine was 89.28%-95.52%. The within-run and the between-run relative standard deviation were 1.09%-1.86% and 3.63%-4.00% respectively. The samples could be stored at 4 ℃ in a refrigerator for at least 25 days. CONCLUSION: This method has high sensitivity, precision and simple sample preparation. It can be used for detecting hydrazine in the air of workplace.
