1.Flavonoid extract from Dracocephalum rupestre hance in improving gouty arthritis:study based on network pharmacology,molecular docking and animal experiment

AIM:To investigate the mechanism of flavonoid extract from Dracocephalum rupestre hance(DRHF)in the treatment of gouty arthritis through network pharmacology,molecular docking and animal experiment.METHODS:Literature re-trieval was used to explore the main active chemi-cal components and targets of DRHF.Gouty arthri-tis disease targets were obtained using Gene Cards and OMIM databases,and drug-disease intersect-ing targets were obtained using Wayne online tools.protein-protein interactions(PPI)and other related network diagrams were constructed using Cytoscape software.GO and KEGG enrichment analyses were performed on the shared intersect-ing targets using Metascape database.A rat model of gouty arthritis was established by Coderre meth-od;the swelling degree of ankle joint,gait behav-iour scores of rats were observed,and hematoxylin-eosin(HE)staining was performed.ELISA and real-time PCR were used to detect the key targets pre-dicted by the network pharmacology,and the ef-fects of DRHF on the molecular mechanism and key targets of gouty arthritis were observed.RESULTS:A total of 7 active compounds and 129 candidate targets for the treatment of GA were obtained,in-cluding IL-6,IL-1β,RELA,TNF,PPARG,etc.and the KEGG enrichment results suggested that DRHF may be involved in PI3K-Akt,TNF,IL-17 and other signal transduction pathways.Animal results:HE staining showed that the thickening of synovial tissue was not obvious in each administered group,and syno-vial cell proliferation and inflammatory cell infiltra-tion were significantly improved;compared with the normal group,the serum levels of TNF,IL-6,and IL-1β in the model group were significantly higher(P<0.05),and the mRNA of PPARG,IL-6,and RELA in the synovial tissues were significantly high-er;compared with the model group,the levels of TNF,IL-6,and IL-1β were significantly lower(P<0.05)in the low group of DRHF(0.45 g/kg)and high group of DRHF(0.9 g/kg),TNF,IL-6,IL-1β lev-els were significantly reduced(P<0.05);PPARG,IL-6,RELA mRNA in synovial tissue were significantly reduced.CONCLUSION:DRHF inhibits IL-17/PI-3K/TNF signaling pathway by down-regulating the ex-pression of IL-6,PPARG and RELA mRNA,decreas-ing the levels of IL-6,IL-1β and TNF,and then treat-ing gouty arthritis.

Chronic disease patients often experience negative emotions such as anxiety and depression,and the accurate assessment of these emotions is crucial for developing effective care plans and interventions.Traditional assessments of negative emotions are limited by subjectivity,error and accessibility issues,making precise identification and evaluation difficult.In recent years,vision technology has demonstrated unique advantages in emotion recognition due to its accuracy,speed,consistency,objectivity nature.This paper reviews the development process of computer vision technology,the content and effects of its application in negative emotion assessment for chronic disease patients,and the current challenges,in order to provide references for the evaluation of chronic disease patients'negative emotions.
3.Diagnostic value of targeted next-generation sequencing for community-acquired respiratory virus infections in patients with hematological diseases

Objective:To evaluate the diagnostic value of targeted next-generation sequencing (tNGS) of throat swab samples for detecting community-acquired respiratory viruses (CARV) in patients with hematological diseases.Methods:Clinical and laboratory data from 64 episodes involving patients with hematological diseases and suspected infections—who underwent both pharyngeal swab tNGS and CARV polymerase chain reaction (PCR) testing concurrently—were retrospectively analyzed. The cases were drawn from the Department of Hematology, Peking University People’s Hospital, between September 2023 and April 2024. Concordance between tNGS and CARV PCR results, as well as the diagnostic performance of tNGS in detecting CARV, were evaluated.Results:Among the 64 episodes, 29 were clinically diagnosed with respiratory tract infections, including one case of cytomegalovirus pneumonia and 28 CARV-positive cases. The remaining 35 episodes involved patients with fever or respiratory symptoms attributed to other causes, including 14 with extrapulmonary infections and 21 with noninfectious etiologies. The median follow-up duration was 215.5 days (range: 7-271 days). PCR detected 24 strains of seven CARV types, whereas tNGS detected 25 strains of eight CARV types. Using PCR results as the reference standard, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of tNGS were 85.0%, 88.6%, 77.3%, 92.9%, and 87.5%, respectively. The two methods showed good concordance (Kappa=0.717, P<0.001) . Conclusion:Pharyngeal swab tNGS may serve as a viable alternative to PCR for diagnosing CARV infections in patients with hematological diseases.

5.Diagnosis of infertility and clinical analysis of fallopian tube patency by three-dimensional ultrasonic hysterosalpingography

Objective To explore the diagnosis of infertility and clinical analysis of tubal patency by three-dimensional ultrasonic hysterosalpingography.Methods A total of 102 infertility patients admitted to Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medicine University from December 2022 to December 2024 were selected as the study objects to analyze the diagnosis of infertility and the clinical application of three-dimensional ultrasound hysterosalpingography on tubal patency.Results Of 102 infertility patients,the detection rate was 97.06%,the misdiagnosis rate was 0,and the missed diagnosis rate was 2.94%.The sensitivity,specificity and area under the curve of three-dimensional ultrasound hysterosalpingography for infertility were 72.65%,84.83%and 0.819.A total of 204 fallopian tubes were examined in 102 patients.Hysteroscopic fallopian tube staining showed patency(30.39%),obstruction(31.37%),and three-dimensional ultrasonic hysterosalpingography showed patency(31.86%),obstruction(32.35%).Compared with inflammation,the injection pressure,flow time and reverse flow of contrast agent in tumor and cyst lesions and congenital dysplasia were decreased(P<0.05),but there was no significant difference between tumor and cyst lesions and congenital dysplasia(P<0.05).In 102 patients,there was no hypersensitivity or other adverse reactions in the examination of three-dimensional ultrasound hysterosalpingography,and the satisfaction rate was 96.08%.Conclusion Three-dimensional ultrasound hysterosalpingography has a high diagnostic value for infertility,and can effectively evaluate the patency of fallopian tube,with high safety and patient satisfaction.

10.Rituximab based treatment in pediatric Epsstain Bar Virus associated lymphocyte proliferative diseases after aplastic anemia with haplo-identical transplantation:a prospective single centre study

Epstein-Barr virus (EBV) associated post-transplant lymphoproliferative disorders (PTLD) are one of the most severe complications after hematopoietic stem cell transplantation (HSCT). This study includes 31 cases of aplastic anemia (AA) patients who developed PTLD after haploidentical transplantation, summarizing their clinical characteristics and categorizing them into either rituximab monotherapy group or combination therapy group based on whether their condition improved by 1 log after a single dose of rituximab. The incidence of PTLD after HSCT in children with AA was 10.16%, and the incidence of PTLD in patients with age >10 years was significantly increased ( χ2=11.336, P=0.010). Of the 31 patients, 27 were clinically diagnosed and 4 were pathologically confirmed. Finally, 15 patients were classified into the rituximab treatment group and 15 patients into the combination treatment groups. Finally three patients died, and the 2-year overall survival rate was (89.7±5.6) %. Standard pre-treatment protocols and EBV reactivation are risk factors affecting the prognosis of PTLD. There was no statistically significant difference in the impact of the two treatment schemes on prognosis.
