This study aimed to investigate the effect of atractylenolide I (Atr-I) on myocardial mitochondrial function in mice with dilated cardiomyopathy (DCM) by regulating the cyclic GMP-AMP synthase (cGAS)/stimulator of interferon genes (STING) pathway. Sixty SPF-grade male cTnT R141W transgenic DCM mice were randomly divided into the DCM group, Atr-I low-dose group (60 mg/kg), Atr-I high-dose group (240 mg/kg), captopril group (0.01 g/kg), and Atr-I high-dose+cGAS/STING pathway activator 5,6-dimethylxanthenone-4-acetic acid (DMXAA) group, with 12 mice in each group. Additionally, 12 male C57BL/6J mice were used as the control group. All mice were administered via oral gavage once daily for 8 weeks. Cardiac function was assessed using the Vevo 770 ultrasound system; myocardial pathology was examined via HE staining; mitochondrial ultrastructure in cardiomyocytes was observed using transmission electron microscopy; the proportion of cardiomyocytes without reduced mitochondrial membrane potential was detected using JC-1 staining; reactive oxygen species (ROS) content in myocardial tissue was measured using 2',7'-dichlorodihydrofluorescein diacetate (DCFH-DA) staining; adenosine triphosphate (ATP) content in myocardial tissue was determined using a commercial kit; and Western blot was performed to detect the protein expression levels of mitofusin-2 (MFN2), dynamin-related protein 1 (DRP1), cGAS, STING, interferon-β (IFN-β), CXC chemokine ligand 10 (CXCL10), and interleukin-6 (IL-6) in myocardial tissue. The aim was to observe the effect of Atr-I on myocardial mitochondrial function in DCM mice. The results showed that low- and high-dose Atr-I (60 mg/kg, 240 mg/kg) intervention improved cardiac function, alleviated cardiomyocyte hypertrophy and disordered muscle fiber arrangement, ameliorated mitochondrial ultrastructure in cardiomyocytes, reduced ROS content and the protein expression levels of DRP1, cGAS, STING, IFN-β, CXCL10, and IL-6 in myocardial tissue, and increased the proportion of cardiomyocytes without reduced mitochondrial membrane potential, as well as ATP content and MFN2 protein expression in myocardial tissue. However, DMXAA attenuated the beneficial effects of high-dose Atr-I on myocardial mitochondrial function in DCM mice. In conclusion, Atr-I may improve myocardial mitochondrial function in DCM mice by inhibiting the cGAS/STING pathway.

OBJECTIVE To analyze the current status and associated factors of human rabies immunoglobulin (HRIG) use in children with rabies-exposure, so as to provide a reference for the rational use of HRIG.METHODS Clinical data of children with rabies-exposure who were admitted to the outpatient and emergency departments of Xiamen Children’s Hospital from January to December 2024 were retrospectively collected. HRIG use in these children was analyzed, and univariate and multivariate Logistic regression analyses was used to identify independent risk factors for non-use of HRIG among children indicated for it.A questionnaire survey was conducted to assess 42 surgeons’ knowledge of rabies-post-exposure prophylaxis,self-rated factors influencing HRIG use,and reasons for non-use in high-risk exposure scenarios.RESULTS A total of 564 pediatric patients were enrolled, of whom 122 met the indications for HRIG use; however, only 24 received HRIG, yielding an actual utilization rate of 19.67%. Multivariate Logistic regression analysis showed that head and facial exposure was an independent risk factor for non-use of HRIG [OR=7.77, 95%CI (1.11, 54.27), P =0.039]. The questionnaire survey revealed that surgeons had the lowest awareness rate regarding the time to antibody production after rabies vaccination (35.71%), followed by the appropriate timing of HRIG administration (50.00%). Among the self-rated factors influencing HRIG use by surgeons, wound depth and bleeding status had the highest mean scores (2.95 points). The primary reasons for non-use HRIG in special cases and grade Ⅲ exposure pediatric patients were the judgment that the offending animal was healthy or posed low risk, accounting for 45.24% and 38.10%, respectively; followed by parental refusal due to cost, pain, or other concerns, accounting for 26.19% and 33.33%, respectively.CONCLUSIONS The actual utilization rate of HRIG among children meeting the indications for HRIG use in our hospital was relatively low. Head and facial exposure was an independent risk factor for non-use of HRIG. Some surgeons had insufficient understanding of the time to antibody production after rabies vaccination and the appropriate timing of HRIG administration. Animal-related risk assessment and parental preferences may influence the use of HRIG.

ObjectiveTo investigate the mechanism by which Kangxian Yixin prescription regulates the circular RNA 0006719 (circ0006719)/microRNA 6852-5p (miR-6852-5p)/transforming growth factor-β1 (TGF-β1)/Smad axis to ameliorate myocardial fibrosis in rats with dilated cardiomyopathy (DCM) and angiotensin Ⅱ (Ang Ⅱ)-induced activation of cardiac fibroblasts (CFs). MethodsA DCM rat model was established by administration of furazolidone. The rats were divided into normal group, model group, captopril (CAP, 10.1 mg·kg-1·d-1) group, and low-, medium-, and high-dose Kangxian Yixin prescription groups (0.9, 1.8, 3.6 g·kg-1·d-1), with 15 rats in each group. After modeling, the treatment groups received corresponding drug solutions by gavage, whereas the normal and model groups received an equal volume of normal saline for 60 consecutive days. Echocardiography was used to measure left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), and left ventricular fractional shortening (LVFS). Transmission electron microscopy (TEM), hematoxylin-eosin (HE) staining, and Masson staining were used to observe myocardial ultrastructure, pathological changes, and collagen deposition. Immunohistochemistry (IHC) was used to detect the protein expression of collagen type Ⅰ alpha 1 (Col1A1) and collagen type Ⅲ(Col Ⅲ) in myocardial tissue. Real-time quantitative polymerase chain reaction (Real-time PCR), Western blot, and fluorescence in situ hybridization (FISH) were used to detect the expression and localization of circ0006719, miR-6852-5p, fibrosis-related molecules, and TGF-β1/Smad pathway-related molecules in myocardial tissue and CFs. CFs from neonatal rats were isolated and cultured in vitro. Cell Counting Kit-8 (CCK-8) assay was used to determine the optimal concentration of Kangxian Yixin prescription for subsequent intervention. An Ang Ⅱ-induced CF activation model was established, and the cells were divided into normal group, Ang Ⅱ group, Ang Ⅱ+CAP group, Ang Ⅱ+Kangxian Yixin prescription group, and Ang Ⅱ+Kangxian Yixin prescription+miR-6852-5p inhibitor group. CF viability and fibrosis-related molecular expression were detected. RNA immunoprecipitation (RNA IP) and dual-luciferase reporter assays were used to verify the targeting relationship between circ0006719 and miR-6852-5p. ResultsCompared with the normal group, the model group showed significantly increased LVESD and LVEDD, significantly decreased LVEF and LVFS, aggravated myocardial ultrastructural and histopathological injury, and significantly increased collagen volume fraction (CVF) in myocardial tissue (P<0.01). The expression of circ0006719, Col1A1 mRNA, Col ⅢmRNA, and Col1A1, Col Ⅲ, TGF-β1, Smad3, and Smad4 proteins in myocardial tissue was significantly upregulated, whereas miR-6852-5p expression and Smad7 protein expression were significantly downregulated (P<0.01). Compared with the model group, the CAP group and the high- and medium-dose Kangxian Yixin prescription groups showed varying degrees of improvement in cardiac function, myocardial fibrosis, circ0006719/miR-6852-5p expression, and TGF-β1/Smad pathway-related indicators, with more pronounced improvements in the CAP group and the high-dose Kangxian Yixin prescription group (P<0.05, P<0.01). In in vitro experiments, 2.0 g·L-1 was determined as the optimal concentration of Kangxian Yixin prescription for subsequent intervention. Compared with the normal group, the Ang Ⅱ group showed significantly increased CF viability, significantly upregulated expression of circ0006719, Col1A1 mRNA, α-smooth muscle actin (α-SMA) mRNA, and Col1A1, α-SMA, TGF-β1, Smad3, and Smad4 proteins in CFs, and significantly downregulated miR-6852-5p expression and Smad7 protein expression (P<0.01). Compared with the Ang Ⅱ group, the Ang Ⅱ+CAP group and the Ang Ⅱ+Kangxian Yixin prescription group showed markedly decreased CF viability and fibrosis-related mRNA and protein expression, markedly increased miR-6852-5p expression, and markedly attenuated TGF-β1/Smad pathway activation (P<0.05, P<0.01). Compared with the Ang Ⅱ+Kangxian Yixin prescription group, the Ang Ⅱ+Kangxian Yixin prescription+miR-6852-5p inhibitor group showed significantly increased CF viability (P<0.01), markedly increased expression of circ0006719, Col1A1 mRNA, and Col1A1, TGF-β1, and Smad4 proteins in CFs, and markedly decreased miR-6852-5p expression (P<0.05, P<0.01). RNA IP and dual-luciferase reporter assays showed a targeting binding relationship between circ0006719 and miR-6852-5p (P<0.01). ConclusionKangxian Yixin prescription may alleviate myocardial collagen deposition and myocardial fibrosis, improve cardiac function in DCM rats, and inhibit Ang Ⅱ-induced abnormal activation of CFs. Its mechanism may be associated with downregulation of circ0006719, restoration of miR-6852-5p expression, and inhibition of the TGF-β1/Smad pathway.

Venous system diseases mainly include varicose veins and venous malformations of lower limbs and the genital system. Most of them are chronic diseases that cause serious clinical symptoms to patients and affect their health and quality of life. Sclerotherapy has become the first-line therapy for venous system diseases. However, there are problems such as incomplete fibrosis and vascular recanalization after sclerotherapy, and improper operation will cause serious adverse consequences. Therefore, exploring a safe and effective sclerotherapy strategy is essential for developing clinically successful sclerotherapy. To solve the above problems, we proposed a new sclerotherapy strategy with a dual mechanism of "vascular damage and plasmin (PLA) system inhibition." We intended to construct a novel cationic surfactant (AEOx-TA) by reacting tranexamic acid (TA), a parent structure, with fatty alcohol polyoxyethylene ether (AEOx) by ester bonds. AEOx-TA could damage vascular endothelium and initiate a coagulation cascade effect to induce thrombus. Furthermore, AEOx-TA could be degraded by esterase and release the parent drug, TA, which could inhibit the PLA system to inhibit the degradation of thrombus and extracellular matrix and promote the process of vascular fibrosis. In addition, such surfactant-based sclerosants have foam-forming properties, and they can be blended with polyvinyl alcohol (PVA) to prepare a highly stable foam formulation (AEOx-TA/P), which can achieve a precise drug delivery and prolonged drug retention time, thereby improving drug efficacy and reducing the risk of ectopic embolism. Overall, the novel cationic surfactant AEOx-TA provides a new avenue to resolve the bottleneck: surfactant sclerosants' efficiency is relatively low in the current sclerotherapy.
