1.Progress on the clinical application of echocardiography in evaluation of right heart dysfunction

With the development of modern medicine,the evaluation of right heart function of patients with acquired heart disease,congenital heart disease,cardiomyopathy and pulmonary hypertension(PH)has become a research hotspot in clinical diagnosis and treatment strategies and disease prognosis judgment.The new technology of echocardiography is also constantly advancing and developing.New echocardiographic parameters are gradually applied in clinical practice.Early detection of right heart dysfunction is very important for the outcome of heart disease.In order to explore the current clinical application of these indicators,this article provides a brief review of the clinical application progress of echocardiography in evaluating right heart function.
2.Surveillance of antifungal resistance in clinical isolates of Candida spp.in East China Invasive Fungal Infection Group from 2018 to 2022

Objective To monitor the antifungal resistance of clinical isolates of Candida spp.in the East China region.Methods MALDI-TOF MS or molecular methods were used to re-identify the strains collected from January 2018 to December 2022.Antifungal susceptibility testing was performed using the broth microdilution method.The susceptibility test results were interpreted according to the breakpoints of 2022 Clinical and Laboratory Standards Institute(CLSI)documents M27 M44s-Ed3 and M57s-Ed4.Results A total of 3 026 strains of Candida were collected,65.33%of which were isolated from sterile body sites,mainly from blood(38.86%)and pleural effusion/ascites(10.21%).The predominant species of Candida were Candida albicans(44.51%),followed by Candida parapsilosis complex(19.46%),Candida tropicalis(13.98%),Candida glabrata(10.34%),and other Candida species(0.79%).Candida albicans showed overall high susceptibility rates to the 10 antifungal drugs tested(the lowest rate being 93.62%).Only 2.97%of the strains showed dose-dependent susceptibility(SDD)to fluconazole.Candida parapsilosis complex had a SDD rate of 2.61%and a resistance rate of 9.42%to fluconazole,and susceptibility rates above 90%to other drugs.Candida glabrata had a SDD rate of 92.01%and a resistance rate of 7.99%to fluconazole,resistance rates of 32.27%and 48.24%to posaconazole and voriconazole non-wild-type strains(NWT),respectively,and susceptibility rates above 90%to other drugs.Candida tropicalis had resistance rates of 29.55%and 26.24%to fluconazole and voriconazole,respectively,resistance rates of 76.60%and 21.99%to posaconazole and echinocandins non-wild-type strains(NWT),and a resistance rate of 2.36%to echinocandins.Conclusions The prevalence and species distribution of Candida spp.in the East China region are consistent with previous domestic and international reports.Candida glabrata exhibits certain degree of resistance to fluconazole,while Candida tropicalis demonstrates higher resistance to triazole drugs.Additionally,echinocandins resistance has emerged in Candida albicans,Candida glabrata,Candida tropicalis,and Candida parapsilosis.
3.Serological and Molecular Biological Analysis of Rare Cases of p-Blood Type

Objective:To clarify the specificity of patient blood type and blood type antibody through patient blood type serological tests and molecular biology results,and to search for matching blood for patients.Methods:Blood type serological methods were used for the identification of red blood cell ABO,RhD,and p blood type.Salt water method,microcolumn gel method and IAT method were used for irregular antibody screening and antibody specificity identification.Forward and reverse sanger sequencing was used to amplify specifically the third exon(CDS sequence)of the A4GALT gene,identify the mutation site and genotype of the gene.Results:The patient's serological blood type was positive for type B and D,indicating a p phenotype.At the same time,anti-Tja was detected in the serum,and further sequencing of the A4GALT gene exon was performed,the homozygous mutation with G>C occurred at base 559,and the genotype ISBT was named A4GALT*01N.10.Molecular biology verification confirmed it to be p blood group.Serological methods confirm that the patient's serum contains anti-Tja.Conclusion:The joint identification of the rare blood type—p blood type by serological and molecular biological methods is of great significance for safe clinical blood transfusion.
4.The Value of REG3α,sST2,and TNFR1 in Risk Stratification and Prognostic Evaluation of Acute Graft-versus-Host Disease after Allogeneic Hematopoietic Stem Cell Transplantation in Children

Objective:To explore the value of REG3α,sST2 and TNFR1 in peripheral blood for risk stratification and prognostic evaluation of acute graft-versus-host disease(aGVHD)after allogeneic hematopoietic stem cell transplantation(allo-HSCT)in children.Methods:From January 2020 to March 2022,70 children with aGVHD after allo-HSCT in the First Affiliated Hospital of Zhengzhou University were selected as the research objects,of which 50 cases were mild aGVHD(grade Ⅰ-Ⅱ)and 20 cases were severe aGVHD(grade Ⅲ-Ⅳ).30 healthy children who underwent physical examinations in our hospital during the same period were selected as the control group.Luminex platform was used to detect the protein expression levels of REG3α,sST2 and TNFR1 during aGVHD occurrence,and the differences between the three groups were analyzed by one-way ANOVA.According to the outcome of aGVHD treatment within 28 days,the patients were divided into a good prognosis group of 58 cases and a poor prognosis group of 12 cases.The ROC curve was used to analyze the value of REG3α,sST2 and TNFR1 in predicting the prognosis of children with aGVHD.Results:The peripheral blood levels of REG3α,sST2 and TNFR1 in the mild aGVHD and severe aGVHD groups were significantly higher than those in the control group(P<0.05),and those in the severe aGVHD group were significantly higher than those in the mild aGVHD group(P<0.05).Compared with the good prognosis group,the peripheral blood levels of REG3α,sST2 and TNFR1 in the poor prognosis group were significantly higher(t=9.27,3.33,2.97;P<0.01).ROC curve analysis showed that the area under the curve(AUC),sensitivity and specificity of the combined detection of REG3α,sST2 and TNFR1 in predicting the prognosis of children with aGVHD were higher than those of the above indicators detected alone or in pairs.Conclusion:The expression levels of REG3α,sST2 and TNFR1 were related to the severity of aGVHD.The combination of REG3α,sST2 and TNFR1 has a high clinical value in predicting the prognosis of children with aGVHD,which is expected to provide a reliable reference for clinical evaluation of the prognosis of children with aGVHD.
5.Association of Triglyceride Glucose-Derived Indices with Recurrent Events Following Atherosclerotic Cardiovascular Disease

Background:
Triglyceride glucose (TyG) and TyG-body mass index (TyG-BMI) are reliable surrogate indices of insulin resistance and used for risk stratification and outcome prediction in patients with atherosclerotic cardiovascular disease (ASCVD). Here, we inserted estimated average glucose (eAG) into the TyG (TyAG) and TyG-BMI (TyAG-BMI) as derived parameters and explored their clinical significance in cardiovascular risk prediction.
Methods:
This was a population-based cohort study of 9,944 Chinese patients with ASCVD. The baseline admission fasting glucose and A1C-derived eAG values were recorded. Cardiovascular events (CVEs) that occurred during an average of 38.5 months of follow-up were recorded. We stratified the patients into four groups by quartiles of the parameters. Baseline data and outcomes were analyzed.
Results:
Distribution of the TyAG and TyAG-BMI indices shifted slightly toward higher values (the right side) compared with TyG and TyG-BMI, respectively. The baseline levels of cardiovascular risk factors and coronary severity increased with quartile of TyG, TyAG, TyG-BMI, and TyAG-BMI (all P<0.001). The multivariate-adjusted hazard ratios for CVEs when the highest and lowest quartiles were compared from low to high were 1.02 (95% confidence interval [CI], 0.77 to 1.36; TyG), 1.29 (95% CI, 0.97 to 1.73; TyAG), 1.59 (95% CI, 1.01 to 2.58; TyG-BMI), and 1.91 (95% CI, 1.16 to 3.15; TyAG-BMI). The latter two showed statistical significance.
Conclusion
This study suggests that TyAG and TyAG-BMI exhibit more information than TyG and TyG-BMI in disease progression among patients with ASCVD. The TyAG-BMI index provided better predictive performance for CVEs than other parameters.
6.Correlation analysis of functional constipation and its related factors with acute appendicitis in children

Objective:To explore the correlation between functional constipation (FC)and its related factors with acute appendicitis(AA)in children,so as to provide a reference for the prevention and clinical diagnosis and treatment of children with AA. Methods:A case-control study was conducted on 170 children diagnosed with AA in the Department of Pediatrics,the Second Affiliated Hospital of Air Force Military Medical University,from August 2022 to March 2023,and 170 non-AA children during the same period were selected as control objects.The clinical data,incidence of FC,symptoms related to FC,Bristol stool classification,past constipation history and other information were compared between two groups.Results:The incidence of FC in 170 children with AA was 22.9%(39/170),which was significantly higher than 10.6%(18/170)in the non-AA group( P<0.01);For children under four years old,the proportion of faecal retention in AA group was higher than that in non-AA group(25.6% vs. 9.3%, P<0.05);For children ≥ 4 years old,the proportion of faecal retention and dyschezia in AA group were higher than those in non-AA group(28.2% vs. 6.9%,29.0% vs. 16.4%,respectively,all P<0.05).The proportion of past constipation history in AA group was higher than that in non-AA group(29.4% vs.14.1%).The duration of constipation in AA group was longer than that in non-AA group [0.00(0.00,1.25)month vs. 0.00(0.00,0.00)month,all P<0.01].The proportion of children with low Bristol stool classification in AA group was higher than that in non-AA group( P<0.01).Multivariate Logistic regression analysis showed that faecal retention[ OR=6.186(95% CI 2.336~16.380)] and long constipation time [ OR=1.310(95% CI 1.095~1.567)]were independent risk factors for AA in children(all P<0.05). Conclusion:The incidence of faecal retention in children with AA is higher than that in children without AA,and the median duration of constipation is longer than that in children without AA.Fecal retention and long-term constipation are independent risk factors for AA in children.
7.Impact of rehabilitation exercise intervention mode based on cardiac function classification on clinical effect and quality of life in CHF patients

Objective:To explore the impact of rehabilitation exercise intervention mode based on cardiac function classification on clinical effect and quality of life(QOL)in patients with chronic heart failure(CHF).Methods:A total of 160 CHF patients who visited our hospital from Dec 2021 to Jan 2023 were selected,and 154 cases were fi-nally enrolled.According to the random number table method,patients were divided into study group and control group with 77 cases in each group.Control group received routine nursing program,while the study group received rehabilitation exercise intervention based on cardiac function classification on the basis of control group,both groups were intervened for three months.Clinical total effective rate,and cardiopulmonary function,serum oxidative stress indicators and MLHFQ score before and after intervention were compared between two groups.Results:Total effective rates of study subgroups of class Ⅱ and Ⅲ were significantly higher than those of control group(class Ⅱ:100.00%vs.83.78%;class Ⅲ:97.37%vs.80.00%)(P<0.05 both).Compared with control subgroup of classⅢ after intervention,there were significant rise in peak VO2[(16.98±2.03)ml·min-1·kg-1 vs.(18.61±2.41)ml·min-1·kg-1],LVEF[(41.73±4.53)%vs.(48.03±5.22)%]and 6MWD[(351.34±61.00)m vs.(391.53±64.42)m](P<0.01 all);and significant reductions in LVEDd[(57.55±3.91)mm vs.(53.18±3.07)mm],LVESd[(35.90±2.91)mm vs.(30.50±2.67)mm],levels of LPO[(6.00±0.99)mg/L vs.(3.95±0.61)mg/L],MPO[(3.83±0.58)mg/L vs.(2.03±0.28)mg/L],and MLHFQ total score[(57.05±4.57)points vs.(45.29±3.94)points]in study subgroup of class Ⅲ(P=0.001 all).Compared with control subgroup of class Ⅱ after intervention,there were significant rise in peak VO2,LVEF and 6MWD,and significant reductions in LVEDd,LVESd,levels of LPO,MPO and MLHFQ score in study subgroup of class Ⅱ,P<0.05 or<0.01.There was no significant difference in the incidence rate of adverse events during follow-up between two groups(3.90%vs.6.49%,P=0.717).Conclusion:Rehabilitation exercise intervention based on cardiac function classifi-cation can significantly improve cardiopulmonary function,inhibit oxidative stress response in vivo and improve quality of life in CHF patients,which is worthy of promotion and application in clinical practice.
8.Exploration and practice of the training model for applied talents in Traditional Chinese Medicine based on the"Four Combinations"

Objective:To explore the practical effect of applied talent training model based on the"Four Combinations"among students majoring in Traditional Chinese Medicine in a Medical College.Methods:The undergraduate students majoring in Traditional Chinese Medicine at a medical college from grade 2021 and 2022 were selected as the research subjects and divided into two groups.Experimental group were students who had participated in various extracurricular competitions and innovation and entrepreneurship projects,while control group were students who had not participated in any competitions or projects.The final intellectual education scores of the two groups of students were compared(excluding the scores of Marxist basic principles).A questionnaire survey was conducted on the satisfaction of talent cultivation models among the above undergraduate students,and the validity and satisfaction analysis of the survey results were conducted.Results:The experimental group had significantly higher intellectual education scores than the control group(P<0.05).The results of factor analysis indicated that the sample suitability and the questionnaire validity seem plausible(KMO=0.874).The overall satisfaction score of students towards the"Four Combinations"talent cultivation model was 4.16±0.2(out of 5),reflecting their high recognition of the model.Conclusion:The construction of a"Four Combinations"characteristic Traditional Chinese Medicine applied talent cultivation model is beneficial for improving the quality of cultivating high-quality and composite applied talents in Traditional Chinese Medicine,so as to provide high-quality applied talents with solid theoretical foundation and practical innovation capabilities for the Liaoshen region.

Objective To investigate the incidence rate,clinical characteristics,and prognosis of neonatal stroke in Shenzhen,China.Methods Led by Shenzhen Children's Hospital,the Shenzhen Neonatal Data Collaboration Network organized 21 institutions to collect 36 cases of neonatal stroke from January 2020 to December 2022.The incidence,clinical characteristics,treatment,and prognosis of neonatal stroke in Shenzhen were analyzed.Results The incidence rate of neonatal stroke in 21 hospitals from 2020 to 2022 was 1/15 137,1/6 060,and 1/7 704,respectively.Ischemic stroke accounted for 75%(27/36);boys accounted for 64%(23/36).Among the 36 neonates,31(86%)had disease onset within 3 days after birth,and 19(53%)had convulsion as the initial presentation.Cerebral MRI showed that 22 neonates(61%)had left cerebral infarction and 13(36%)had basal ganglia infarction.Magnetic resonance angiography was performed for 12 neonates,among whom 9(75%)had involvement of the middle cerebral artery.Electroencephalography was performed for 29 neonates,with sharp waves in 21 neonates(72%)and seizures in 10 neonates(34%).Symptomatic/supportive treatment varied across different hospitals.Neonatal Behavioral Neurological Assessment was performed for 12 neonates(33%,12/36),with a mean score of(32±4)points.The prognosis of 27 neonates was followed up to around 12 months of age,with 44%(12/27)of the neonates having a good prognosis.Conclusions Ischemic stroke is the main type of neonatal stroke,often with convulsions as the initial presentation,involvement of the middle cerebral artery,sharp waves on electroencephalography,and a relatively low neurodevelopment score.Symptomatic/supportive treatment is the main treatment method,and some neonates tend to have a poor prognosis.
10.Risk factors for embolism in children with refractory Mycoplasma pneumoniae pneumonia and construction of a nomogram model for prediction of embolism

Objective To study the risk factors for embolism in children with refractory Mycoplasma pneumoniae pneumonia(RMPP)and to construct a nomogram model for prediction of embolism.Methods This retrospective study included 175 children diagnosed with RMPP at Children's Hospital Affiliated to Zhengzhou University from January 2019 to October 2023.They were divided into two groups based on the presence of embolism:the embolism group(n=62)and the non-embolism group(n=113).Multivariate logistic regression analysis was used to screen for risk factors of embolism in children with RMPP,and the R software was applied to construct the nomogram model for prediction of embolism.Results Multivariate logistic regression analysis indicated that higher levels of D-dimer,interleukin-6(IL-6)and neutrophil to lymphocyte ratio(NLR),lung necrosis,and pleural effusion were risk factors for embolism in children with RMPP(P<0.05).The area under the curve of the nomogram model for prediction of embolism constructed based on the aforementioned risk factors was 0.912(95%CI:0.871-0.952,P<0.05).The Hosmer-Lemeshow goodness-of-fit test showed that the model had a good fit with the actual situation(P<0.05).Calibration and decision curve analysis indicated that the model had high predictive efficacy and clinical applicability.Conclusions Higher levels of D-dimer,IL-6 and NLR,lung necrosis,and pleural effusion are risk factors for embolism in children with RMPP.The nomogram model based on these risk factors has high clinical value for predicting embolism in children with RMPP.
