Journal of Clinical Medicine in Practice 2024;28(21):142-148
One of the critical pathogenic factors contributing to non-healing chronic wounds is the formation of bacterial biofilms. In light of this, exploring effective strategies for the prevention and control of biofilms to improve the prognosis of patients with chronic wounds has become a research focus in the field of wound care and treatment. This review aimed to provide an in-depth exploration of the concept of bacterial biofilms, their complex formation mechanisms, and how they promote persistent infection states within the context of chronic wounds, offering valuable references for the detection and treatment of these biofilms in chronic wounds.

To establish a method for the determination of ketamine and MDA and their main metabolites in urine by solid phase microextraction-liquid chromatography-mass spectrometry. In a urine sample supplemented with quantitative ketamine, norketamine, MDMA and MDA control. The solution was adjusted pH 11, added solid Na2CO3, heated and stirred at 60 °C. Then, the sample was extracted by SPME with 60 μm polydimethylsiloxane-vinylbenzene copolymer(PDMS/DVB ), a middle-polar coated fiber for 15 minutes and then analyzed by HPLC-MS. The result showed good linearity in the range of 0. 03-1. 0 μg/mL, r≥0. 999 2, and LOD was 0. 01 μg/mL, the value of the average recovery rate was varying from 97. 19%-105. 44%, and RSD was within 10%. The method is simple, safe and accurate, and can be used to determine ketamine, MDMA and their main metabolites in urine.

Objective To study the risk factors of severe complications possibly associated with exchange transfusion (ET) for patients with extreme hyperbilirubinemia.Method From May 2001 to May 2018,neonates with severe complications of ET were assigned into adverse event group. The propensity score principle was used to match adverse event group/the control group with the ratio of 1 ∶ 3.The neonates in the control group received ET without any severe complications. The demographic characteristics of patients, and other clinical data were reviewed. The Logistic regression analysis was used to determine the risk factors of severe complications of ET.Result Among the 1 535 neonates who received ET during the past 17 years,71 neonates (4.6%) were identified with severe adverse events, including apnea (30.6%),necrotizing enterocolitis (18.8%),heart failure (14.1%),respiratory failure (12.9%),and shock (8.2%).The Logistic regression analysis showed that acute bilirubin encephalopathy (ABE) score ≥ 3 (OR=6.383,95%CI 2.550~15.979),ETs ≥ 2 times (OR=11.825,95%CI 2.464~56.755),cardiac murmur ≥ grade Ⅲ (OR=20.417,95%CI 4.705~40.590),and dramatic blood pressure fluctuation during ET ≥ 30 mmHg (OR=13.612,95%CI 1.795~43.342) were risk factors of ET related severe complications (all P<0.05).Conclusion The indications should be carefully assessed before ET.The patients with ABE score ≥ 3,ETs ≥ 2 times, cardiac murmur ≥ grade Ⅲ,or dramatic blood pressure fluctuation during ET ≥ 30 mmHg should be monitored carefully and ET should be stopped in time if necessary.

Objective To analyze the channels of going abroad of imported malaria patients in Jiangsu Province,so as to provide the evidence for the control of imported malaria and health intervention for high?risk population. Methods The data of imported malaria cases in Jiangsu Province in 2015 were collected and the channels of going abroad were analyzed. The Quan?tum GIS 1.7.4 software was applied to map. Results Totally 405 imported cases were reported in Jiangsu Province in 2015, and the cases increased by 14.1%compared with those in 2014. All the patients were migrant workers. The migrant workers were mostly sent abroad by construction companies[43.21%(175/405)]. The dispatching companies were mainly concentrated in Taizhou,Lianyungang,Zhenjiang and Yangzhou cities, accounting for 6.17%(25/405),3.21%(13/405),2.72%(11/405) and 2.22%(9/405),respectively. Totally 176(43.46%)patients returned to China having accompanied workers, and 1.21%(9/745)of accompanied workers were found having symptoms of malaria later. Conclusions It is difficult to monitor malaria in overseas migrant works in Jiangsu Province. The channels of going abroad of migrant workers are various and complicated ,for?mulating the main difficulty and challenge of imported malaria control.

Objective To understand the epidemic situation and epidemiological characteristics of malaria in Jiangsu Prov?ince from 2005 to 2014,so as to provide the evidence for formulating and adjusting the strategies and measures on malaria elimi?nation in this province. Method The epidemiological data of malaria in Jiangsu province from 2005 to 2014 were collected, and the prevalence situation, infection sources as well as the temporal, regional and population distribution of the cases were an?alyzed. Result A total of 5 069 malaria cases were reported in Jiangsu Province form 2005 to 2014,there were 3 422 cases (67.51%)of vivax malaria,1 497 cases(29.53%)of falciparum malaria, and 150 cases(2.96%)that were unclassified. Among all the reported cases,2 139(42.20%)were local cases,1 131 were(22.31%)imported cases from other places in Chi?na ,and 1 799(35.49%)were imported cases from abroad. There were 3 809 male cases and 1 260 female cases, with a sex ratio of 3.02∶1. The patients were mainly aged from 25 to<60 years(accounting for 67.15%),and were mainly young adult farmers (40.26%)and migrant workers(23.63%),who mainly distributed(accounting for 60.58%)in Xuzhou,Suqian,Huai’an,Su?zhou and Nantong cities. The predominant malaria type from 2005 to 2009 were vivax malaria, while after 2010,it changed to falciparum malaria. Conclusions The malaria epidemic situation has been effectively controlled in Jiangsu Province, which re?flects the initiative achievements of malaria elimination. However, there are still many imported malaria cases from abroad, and the species of the Plasmodium are diverse. Therefore, the imported malaria from abroad still remains the key of malaria control in Jiangsu Province.

This study was aimed to analyze the composition principles of prescriptions for sequela of apoplexy collected in the CNKI by the Traditional Chinese Medicine Inheritance Platform (TCMIP), in order to provide a reference for clinical selection of medication. Prescriptions for the sequela of apoplexy treatment in CNKI were collected. Based on the TCMIP, the medication rules of prescription were analyzed by data mining method after data entry. A total of 190 prescriptions involving 204 herbs for sequela of apoplexy treatment were screened and studied. The herbs for blood-activating andqi-tonifying were used with higher frequency. There were 36 commonly used herbal pairs. The herb pair of Dang-Gui(Radix Angelicae Sinensis) andHuang-Qi (Radix Astragali) was with the highest frequency. The herbal combination with three types of herbs wasDi-Long (Pheretima),Dang-Gui andHuang-Qi. The herbal combination with four types of herbs wasChuan-Xiong (Rhizoma Chuanxiong),Di-Long,Dang-Gui andHuang-Qi. The analysis on association rule showed thatChuan-Xiong,Dang-Gui,Tao-Ren (Semen Persicae),Hong-Hua(Flos carthami) andHuang-Qiwere often combined with each other. A total of 48 core combinations and 11 new prescriptions were mined. It was concluded that the prescriptions for sequela of apoplexy were mainly composed with herbs for blood-activating,qi-tonifying and collateral-dredging. According to the flavor and meridian tropism theory of Chinese medicine, the herbal medication selection in the prescription was mostly warm and hot in nature, sweet and pungent flavor, and the meridian tropism of the liver and spleen meridian.
