1.A study of attention and interpretation therapy based on Swanson′s caring theory in children with adolescent epilepsy

Objective:To explore the application effect of the attention and interpretation therapy based on Swanson′s caring theory in adolescents with epilepsy, aiming to reduce negative emotions and improve self-management levels in children.Methods:This study was a quasi-experimental research. A convenience sampling method was used to select 78 adolescents with epilepsy who were admitted to the Department of Neurology, the Affiliated Children′s Hospital of Xiangya School of Medicine, Central South University (Hunan Children′s Hospital) from March 1 to August 31, 2023. The participants were assigned into two groups, the control group ( n=39) and the intervention group ( n=39), using a random number table. The control group received standard anti-epileptic treatment and routine nursing care, while the intervention group received the same treatment and care with the addition of a caring team providing attention and interpretation therapy based on Swanson′s caring theory. Assessments were made at three time points: before the intervention, 4 weeks after the intervention, and 12 weeks after the intervention. The Depression-Anxiety-Stress Scale (DASS-21), Epilepsy Self-Management Scale (ESMS), and Pediaric Quality of Life Inventory Measurement Methods (PedsQL TM4.0) were used to assess the participants′ scores. Results:In the control group, 23 were male and 16 were female, with aged of (13.49 ± 1.43) years. In the intervention group, 24 were male and 15 were female, with aged of (13.51 ± 1.45) years. After 4 weeks of intervention, the scores of depression, anxiety, and stress on the DASS-21 for the intervention group were (9.32 ± 2.04), (6.79 ± 2.44), and (13.63 ± 2.67), respectively, which were lower than those of the control group: (10.43 ± 2.27), (8.43 ± 2.75) and (16.05 ± 3.32); the scores of ESMS and PedsQL TM4.0 in the intervention group were (66.66 ± 10.87) and (73.63 ± 9.85), respectively, which were higher than those of the control group: (60.70 ± 10.24) and (67.27 ± 12.33), the differences were statistically significant ( t values were -2.47 to 3.49, all P<0.05). After 12 weeks of intervention, the scores on the DASS-21 for the intervention group were (5.47 ± 1.66), (3.37 ± 1.68) and (8.42 ± 2.24) for depression, anxiety and stress, respectively, which were lower than those of the control group: (8.03 ± 1.98), (6.06 ± 2.64) and (11.28 ± 3.21) ;the scores for ESMS and PedsQL TM4.0 in the intervention group were (74.66 ± 9.15) and (85.24 ± 7.00), respectively, which were higher than those of the control group: (65.36 ± 10.86) and (75.67 ± 11.39), the differences were statistically significant ( t values were -4.33 to 6.03, all P<0.05). Additionally, there were statistically significant differences in the DASS-21 depression, anxiety, and stress scores, ESMS, and PedsQL TM4.0 scores across the different time points ( F values ranged from 4.65 to 1 134.03, all P<0.05). Conclusions:Attention and interpretation therapy based on Swanson′s caring theory can effectively reduce negative emotions in adolescents with epilepsy, improve their self-management skills, and further enhance their quality of life.
2.Study on the effect of high-fidelity intelligent simulator combined with scenario simulation in emergency response training of radiology department

Objective:To explore the application effect of high-fidelity intelligent simulator combined with scenario simulation for emergency response training in the Department of Radiology, and to improve the emergency preparedness of medical, nursing, and technical staff in managing contrast agent adverse reactions.Methods:From January to July 2024, 132 medical, nursing, and technical staff from the Department of Radiology of a tertiary hospital in Chengdu City, China were selected as the training subjects. The high-fidelity intelligent simulator combined with scenario simulation teaching mode was used to conduct emergency response training for the participants. The differences in theoretical knowledge and post competence regarding contrast agent adverse reactions among the staff were compared before and after the training. A self-made questionnaire was used to investigate their needs and satisfaction of the emergency response training. SPSS 26.0 was used for data analysis. The differences in theoretical knowledge and post competence scores before and after training were compared using the paired samples t test. Results:After the training, the average score of theoretical knowledge examination increased from (84.32±10.19) points to (90.34±7.87) points, and the difference was statistically significant ( P<0.001). After the training, the scores of knowledge reserve, operational skills, situational decision-making ability, professional literacy, comprehensive literacy, and overall post competency were all significantly higher than those before the training ( P<0.05). The satisfaction score of emergency response training was (4.17±0.25) points. Conclusions:High-fidelity intelligent simulator combined with scenario simulation training improved the emergency preparedness and teamwork of radiology staff in clinical emergencies. The training received high recognition and satisfaction from the participants, which is of great significance for clinical emergency response and patient safety.
3.Pathomechanisms and Research Progress on the Radiation-Induced Cardiomyopathy

Radiotherapy of the thoracic tumors may often involve incidental exposure of the heart to ionizing radiation,which leads to radiation-induced heart disease.Radiation-induced heart disease including pericarditis,cardiomyopathy,coronary heart disease,myocardial injury,valvular heart disease,cardiac conduction abnormalities,and so on.Myocardial injury is a more serious type,which may eventually lead to cardiovascular adverse outcomes such as heart failure,affecting the quality of life and clinical prognosis of cancer patients seriously.Therefore,this article will review the mechanism and imaging research progress of radioactive myocardial injury,so as to guide clinical comprehensive management.

7.Exploration of innovation education in basic medicine: taking the construction of virtual simulation teaching system for cardiac embryonic development as an example

With the comprehensive proposal of national education digitization, virtual simulation teaching has gradually become an important component of higher education informatization. Cardiac embryonic development is an important teaching content in the basic medical course "Histoembryology", and it is also a typical representative of basic medical courses that are "abstract, complex, and lack of physical display". West China Second University Hospital of Sichuan University established a virtual simulation teaching system for cardiac embryonic development to provide experience for the development and innovation of the virtual simulation teaching model in basic medicine. The system utilizes cutting-edge technologies such as Unity 3D, BS, and CS for software architecture, creating an "immersive" teaching environment. It vividly depicts the entire developmental process of the embryonic heart, including segmentation and electrophysiological state, in a three-dimensional space. By integrating dynamic and static elements with rich visual aids, the system enables the visualization of embryonic heart structures and introduces interactive operations. This approach enhances the fusion of theory and practice, effectively improves the quality of teaching, and fosters students' practical and innovation abilities.
8.Characteristics and changes of cardiac injury with age in children of Duchenne muscular dystrophy: a prospective cohort study

Objective:To explore the characteristics and changes of cardiac injury with age in Duchenne muscular dystrophy (DMD) and its clinical significance.Methods:A prospective cohort study was conducted. The 215 patients diagnosed with DMD in West China Second Hospital from January 2019 to November 2022 and aged from 6 to 18 years were enrolled. Their clinical data, myocardial injury markers, routine electrocardiogram, cardiac magnetic resonance (CMR) and echocardiography were collected. The patients were divided into five age groups: 6-<8, 8-<10, 10-<12, 12-<14 and 14-18 years of age, and matched with healthy boys respectively. Independent sample t test or Mann-Whitney U test was used to compare the clinical data and CMR indexes between DMD patients and controls in all age subgroups, and to compare the value of left ventricular ejection fraction (LVEF) measured by echocardiography and CMR in each subgroup of DMD patitents. Pearson correlation analysis or Spearman correlation analysis was used to explore the relation between the CMR indexes and age in DMD patients. Results:A total of 215 patients with DMD (all male) and 122 healthy boys were included in the study. There were 75 DMD patients and 23 controls in 6-<8 years of age group, 77 DMD and 28 controls in 8-<10 years of age group, 39 DMD and 23 controls in 10-<12 years of age group, 10 DMD and 31 controls in the 12-<14 years of age group, and 14 DMD and 17 controls in 14-18 years of age group. In the DMD patients, the older the age, the lower the levels of creatine kinase (CK) and creatine kinase isoenzyme (CK-MB). In the 6-<8 years of age group, the CK level was 10 760 (7 800, 15 757) U/L, while in the group of 14-18 years of age, it was 2 369 (1 480, 6 944) U/L. As for CK-MB, it was (189±17) μg/L in the 6-<8 years of age group and (62±16) μg/L in the 14-18 years of age group. Cardiac troponin I remained unchanged in <12 years of age groups, but significantly increased in 12-<14 years of age group, reaching the highest value of 0.112 (0.006, 0.085) μg/L. In the DMD patients, the older the age, the higher the proportion of abnormal electrocardiogram (ECG). In the 6-<8 years of age group, the proportion is 29.3% (22/75), while in the 14-18 years of age group, it was 10/14. Correlation analysis showed that the left ventricular end-diastolic volume index was positively related with age ( r=0.18, P=0.015), and the left ventricular stroke volume index and cardiac output index were negatively related with age ( r=-0.34 and -0.31, respectively, both P<0.001). In the DMD patients, the older the age, the lower LVEF, with the LVEF decreasing to (49.3±3.1)% in the 14-18 years of age group. The LVEF of DMD cases was significantly lower than that of controls in the age subgroups of 8-<10, 10-<12, 12-<14 and 14-18 years of age groups ((57.9±5.2) % vs. (63.6±0.8)%, 60.7% (55.9%, 61.9%) vs. 63.7% (60.2%, 66.0%), 57.1% (51.8%, 63.4%) vs. 62.1 % (59.5%, 64.5)%, (49.3±3.1) % vs. (61.6±1.3)%, respectively; all P<0.01). In the DMD patients, the older the age, the higher the proportion of positive late gadolinium enhancement (LGE). In the 6-<8 years of age group, it was 22% (11/51), in the 12-<14 years of age group, it was 13/14, and in the 14-18 years of age group, all DMD showed positive LGE. The value of LVEF of DMD cases measured by echocardiography was significantly higher than that measured by CMR in 6-<8 years of age group and 8-<10 years of age group (63.2% (60.1%, 66.4%) vs. 59.1 % (55.4%, 62.9%), and (62.8±5.2) % vs. (57.9±5.2)%, all P<0.001). Conclusion:DMD patients develop cardiac injury in the early stage of the disease, and the incidence of cardiac damage gradually increases with both age and the progression of disease.
9.CircSLC8A1_005 Inhibits the Fibrotic Phenotype of Cardiac Fibroblasts by Encoding Protein

ObjectiveTo investigate the effect of circSLC8A1_005 on the fibrotic phenotype of cardiac fibroblasts and the potential mechanism involved. MethodsThe effect of adenovirus-mediated overexpression of circSLC8A1_005 on the expression of fibrosis-related genes, collagen type I alpha 1 chain (Col1a1), collagen type Ⅲ alpha 1 chain (Col3a1) and smooth muscle actin alpha 2 (Acta2), in mouse cardiac fibroblasts (mCFs) were detected. The proliferation and migration activities of mCFs were detected by EdU and wound-healing assay, respectively. Dual luciferase reporter gene assay was performed to detect the activity of potential internal ribozyme entry site (IRES) in circSLC8A1_005. CircSLC8A1_005-translated protein, SLC8A1-605aa, and its intracellular distribution was identified by Western blot assay. The effect of SLC8A1-605aa protein on transcription activity of Sod2 gene was detected by the dual luciferase reporter gene assay. RNA binding protein immunoprecipitation (RIP) was utilized to verify the interaction between SLC8A1-605aa and superoxide dismutase 2 (Sod2) mRNA. Actinomycin D treatment was used to detect the effect of SLC8A1-605aa on Sod2 mRNA stability in mCFs. ResultsAn efficient adenovirus-mediated overexpression of circSLC8A1_005 was achieved in mCFs. The enforced expression of circSLC8A1_005 suppressed proliferation and migration of mCFs, and inhibited the expression of fibrosis-related genes in mCFs. The dual luciferase reporter gene assay revealed the activities of 2 IRES in circSLC8A1_005. Results of Western blot assay showed that circSLC8A1_005 could translate protein SLC8A1-605aa with the prospected molecular weight of 70 ku, which is predominantly distributed in the nucleus. Overexpression of the circSLC8A1_005 and SLC8A1-605aa could consistently inhibit the fibrotic phenotype of mCFs. SLC8A1-605aa could up-regulate superoxide dismutase 2 (Sod2) expression, but not at the transcriptional level. RIP assay indicated that SLC8A1-605aa could specifically interact with Sod2 mRNA, and the results of actinomycin D assay showed that SLC8A1-605aa could enhance the stability of Sod2 mRNA in mCFs. ConclusionCircSLC8A1_005 inhibits the fibrotic phenotype of cardiac fibroblasts via translating SLC8A1-605aa protein, and SLC8A1-605aa may be a potential target for the treatment of myocardial fibrosis.
