1.Relationship between peripheral blood NLR and CAR and early postoperative anastomotic leakage in esophageal cancer patients

Objective To explore the relationship between peripheral blood neutrophil/lymphocyte ratio(NLR),C-reactive protein/albumin ratio(CAR)and early postoperative anastomotic leakage in patients with esophageal cancer.Methods A total of 116 esophageal cancer patients who were treated in Handan Central Hospital and underwent esophagectomy from December 2016 to January 2024 were retrospectively included as research objects,and the patients were divided into AL group(21 cases)and non-AL group(95 cases)based on whether AL occurred within 7 days after surgery.General information of patients was collected,peripheral blood neutrophil,lymphocyte,C-reactive protein,and albumin levels of all patients were measured,and NLR and CAR were calculated.Multivariate Logistic regression was used to analyze the factors affecting early post-operative AL in patients with esophageal cancer.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of NLR,CAR and their combination for early postoperative AL in patients with esophageal cancer.Results The peripheral blood NLR and CAR in the AL group were higher than those in the non-AL group,and the operation time was shorter than that in the non-AL group(P<0.05).Multivariate Logistic regression analysis results showed that NLR and CAR were risk factors for early postoperative AL in esophageal cancer patients(P<0.05).The ROC curve results showed that the area under the curve(AUC)of NLR,CAR and combined prediction of early postoperative AL in patients were 0.875,0.871,and 0.958,and the AUC of combined prediction was greater than that of individual prediction(Z=2.274,2.927,P<0.05).Conclusion Peripheral blood NLR and CAR in patients with esophageal cancer are closely related to early postoperative AL.
2.Relationship Between Empathy and Depression in Adolescents:The Mediating Role of Family Functioning

Objective To investigate the effect of empathy on depressive symptoms in adolescents and to explore the potential mediating role of family functioning in the effect of empathy on depressive symptoms.Methods The 2022 cross-sectional data from the Chengdu Positive Child Development(CPCD)cohort were analyzed in the study.A survey was conducted in Chengdu in June 2022,involving 3 020 students in grades 5-8 from three randomly selected stratified schools.The Interpersonal Reactivity Index(IRI-C),the Chinese Family Assessment Instrument(C-FAI),and the Center for Epidemiologic Studies Depression Scale for Children(CES-DC)were used in the survey.Chi-square test or one-way analysis of variance was performed to examine the differences in various demographic characteristics(sex,grade,region,and total monthly household income)between groups of respondents,as well as the differences in family functioning,empathy,and depression.Pearson correlation coefficient was used to examine the correlation between family functioning,empathy,and depressive symptoms.Structural equation modeling and SPSS PROCESS component Model 4 were used to analyze whether family functioning played a mediating role in the effect of empathy on depressive symptoms in adolescents.Results The detection rate of depressive symptoms among survey respondents was 25.40%.The results of the difference analysis revealed significant differences in the detection rates of depressive symptoms among respondents of different grades,regions,and monthly household incomes(P<0.05).There was no significant difference in the detection rates of depressive symptoms between male and female students.There was a significant difference in the detection rate of depressive symptoms between respondents with different scores for family dysfunction and empathy ability(P<0.001).Correlation analysis results showed that empathy scores were negatively correlated with depression(r=-0.11,P<0.001),that family dysfunction was positively correlated with depression(r=0.29,P<0.001),and that empathy scores were negatively correlated with family functioning(r=-0.37,P<0.001).The mediating role of family dysfunction in the relationship between empathy and depressive symptoms was established,with the direct effect being 0.039(95% confidence interval[CI]:0.010-0.069,P<0.001)and the indirect effect value being-0.096(95% CI:-0.115--0.079,P<0.001).The direct effect value accounted for 28.89% of the total effect value,while the mediation effect value accounted for 71.11% of the total effect value.Conclusion The empathy ability of adolescents is correlated to depressive symptoms,and family functioning plays a mediating role between empathy and depressive symptoms in adolescents.It is suggested that adolescents'empathy ability and family functioning should be enhanced through multiple channels to reduce the occurrence of depressive symptoms.

Oridonin is one of the main anti-tumor active ingredients of Rabdosiae Rubescentis Herba. In the development process of a variety of malignant tumors, such as liver cancer, breast cancer, cervical cancer, pancreatic cancer, lung cancer, gastric cancer, esophageal cancer, colon cancer, etc, it has a significant inhibitory effect. Oridonin plays the rold of anti-tumor effect mainly by inhibiting tumor cell proliferation, inducing tumor cell apoptosis, inhibiting tumor cell invasion and migration, inducing tumor cell autophagy, reducing telomerase activity and reversing the drug resistance of the tumor cell.
4.Clinical Study on Acupuncture at Zusanli, Taichong and Hegu Points Combined with Three-step Analgesic Ladder for Treatment of Gastric Cancer Pain

Objective To investigate the clinical effectiveness and safety of acupuncture at Zusanli (ST36),Taichong(LR3) and Hegu(LI4) points combined with three-step analgesic ladder for the treatment of gastric cancer pain.Methods Sixty patients with moderate and severe pain induced by gastric cancer were randomly divided into study group and control group,30 cases in each group.The study group was treated with acupuncture at Zusanli,Taichong and Hegu points combined with three-step analgesic ladder for gastric cancer pain management,and the control group was treated with three-step analgesic ladder only.After treatment for 7 days,the clinical effectiveness was evaluated with the average pain scores of Numerical Rating Scale(NRS),Quality of Life Scale(QOL-BREF by WHO),Self-rating Depression Scale(SDS).And the incidence of adverse reaction was also observed.Results (1) After treatment,the markedly-effective analgesic rate of the study group was higher than that of the control group (P < 0.05).(2) After treatment,QOL scores of both groups were much improved as compared with those before treatment(P < 0.05),and the improvement of the study group was superior to that of the control group(P <0.05).(3) The depression scores of both groups were much improved after treatment (P < 0.05 compared with those before treatment),but the difference between the two groups was insignificant(P > 0.05).(4) The incidence of adverse reaction of the study group was lower than that of the control group(P < 0.05).Conclusion The effect of acupuncture at Zusanli,Taichong and Hegu points combined with three-step analgesic ladder is better than three step analgesic ladder alone for the treatment of gastric cancer pain.
5.Clinical Observation of Modified Liujunzi Decoction in Adjuvant Chemotherapy for Advanced Non-small Cell Lung Cancer

OBJECTIVE:To observe the therapeutic efficacy and safety of modified Liujunzi decoction in adjuvant chemotherapy for advanced non-small cell lung cancer (NSCLC).METHODS:A total of 118 patients with advanced NSCLC were randomly divided into observation group and control group,with 59 cases in each group.Both groups were given Dexamethasone acetate tablets 15 mg orally 12,6 h before chemotherapy,Diphenhydramine hydrochloride injection 20 mg intramuscularly 30 min before chemotherapy,and also given routine treatment as acid suppression,liver protection,antiemetic,hydration and dieresis.Control group was additionally given Paclitaxel injection 175 mg/m2 intravenously within 3 h,d1,and 30 min later given Cisplatin for injection 25 mg/m2 intreavenously for 2 h,d1-3.Observation group was additionally given modified Liujunzi decoction,decocted to 200 mL,one dose a day,morning and night,on the basis of control group.A treatment course of 2 groups lasted for 21 d,and both received 4 courses.The short-term efficacies of 2 groups were observed.The levels of T lymphocyte subset and tumor marker (CEA,SCC-Ag,CYFRS21-1),the occurrence of toxic reaction were observed before and after chemotherapy.RESULTS:Total response rate of observation group (49.15%) was significantly higher than that of control group(32.20%);the incidence of thrombocytopenia,leucopenia,abnormal liver function,digestive tract reaction and peripheral nerve injury was significantly lower than control group,with statistical significance (P< 0.05).After treatment,CD4+ and CD4+/CD8+ of observation group were significantly higher than before chemotherapy,while CD8+ was significantly lower than before chemotherapy;CD4+ and CD4+/CD8+ of control group were significantly lower than before chemotherapy,while CD8+ was significantly higher than before chemotherapy;CD4+ and CD4+/ CD8 + of observation group were significantly higher than those control group,while CD8 + was significantly lower than control group;The levels of tumor markers in 2 groups were significantly lower than before chemotherapy,and the observation group was significantly lower than the control group,with statistical significance (P<0.05).CONCLUSIONS:Based on routine treatment,modified Liujunzi decoction shows significant therapeutic efficacy for NSCLC,improves immune function and relieve toxic reaction.
6.Association of serum levels of HDL-C,LDL-C with lymphatic metastasis and clinical stage in gastric cancer

Objective To investigate the correlation of serum levels of HDL-C,LDL-C with clinical stage and lynlphatic metastasis in gastric malignant tumors. Methods Preoperative serum levels of HDL-C and LDL-C were retrospectively analyzed in 114 gastric cancer patients who were diagnosed by pathology.Results Serum, levels of HDL-C showed a remarkable correlation with tumor differentiation,lymphatic metastasis and the diameter of tumor in gastric cancer (P<0.05).The lower serum levels of HDL-C were significantly observed in gastric cancer patients with TNM Ⅲ and IV stage,compared with those with TNM Ⅰ and Ⅱ stage (P<0.05). Conclusion Preoperative serum levels of HDL-C might be a useful biomarker of lymphatic metastasis and advanced stage of gastric cancer.

Biejiajian Pills was created by ZHANG Zhong-jing of Han Dynasty. Its feature was harmonizing qi-blood, appllying drugs with nature of cold and heat, and simultaneous application of purging-tonifying therapy, it was a distinguished prescription of treating malarial nodule. Today, the prescription was applied in treating liver fibrosis and had obvious effect. According to the TCM theory, Professor CHEN Zhi-qiang identified the theory of treating kidney by Biejiajian Pills on basis of analyzing the etiopathogenisis and pathogenesis of liver fibrosis and kidney fibrosis. It has made certain progress in the treatment kidney fibrosis by ancient medical prescription.
8.Protective effects of Chailing Guiqi Decoction combined with lumbrukinase on renal function in rats with adriamycin nephropathy

OBJECTIVE: To study the protective efffects of Chailing Guiqi Decoction (CLGQD) combined with lumbrukinase on renal function in rats with adriamycin nephropathy. METHODS: Thirty-six SD rats were randomly divided into four groups: normal control group, untreated group, simvastatin-treated group and CLGQD -treated group. Adriamycin nephropathy was induced by intravenous injection with 5 mg/kg adriamycin. After seven-day treatment, quantitative measurement of 24-h urine protein was determined with trichloroacetic acid, and serum total protein (TP), albumin (Alb), total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL), creatinine (Cr) and blood urea nitrogen (BUN) were assessed using automatic biochemistry analyzer. The pathomorphological changes of renal tissues were observed with light and electron microscopes. RESULTS: In the untreated group, the 24-h urine protein excretion, serum TC, TG, LDL, Cr and BUN were significantly higher than those in the normal control group (P<0.05 or P<0.01), while the serum TP, Alb, HDL were significantly lower than those in the normal control group (P<0.01). In the CLGQD-treated group, the 24-h urine protein excretion, serum TC, TG, LDL, Cr and BUN were significantly lower as compared with those in the untreated group (P<0.05 or P<0.01), while the serum TP, Alb and HDL were significantly higher as compared with those in the untreated group (P<0.05 or P<0.01). The pathomorphological findings of the renal tissues under the light microscope in the untreated group showed focal segmental glomerulosclerosis in a few of glomerulus, degenerated and swelled proximal tubular epithelial cells, proteins in cast formation in some renal tubules and scattered fibrosis in interstitial tissues of the kidney, while the electron microscope images showed the fusion of foot processes in glomerular epithelial cells. The pathomorphological changes in the CLGQD-treated group were slighter than those in the untreated group. CONCLUSION: CLGQD combined with lumbrukinase can reduce proteinuria, regulate lipid metabolism, protect renal function, and delay progressive renal damage in rats.
