1.Clinical observation of acupuncture and Tuina combined with Ba Duan Jin in improving motor function in patients with Parkinson disease in the early and middle stages

Objective:To observe the improving effect of acupuncture and Tuina(Chinese therapeutic massage)combined with Ba Duan Jin(Eight-brocade Exercise)on motor function in patients with Parkinson disease(PD)in the early and middle stages.Methods:Seventy patients with early-or middle-stage PD were randomly divided into a control group and an observation group,with 35 patients in each group.The control group was given conventional treatment such as oral levodopa hydrochloride tablets;the observation group was treated with acupuncture,Tuina,and Ba Duan Jin in addition to the control group's treatment.Acupuncture and Tuina were administered twice a week,and Ba Duan Jin was performed five times a week.Each course of treatment lasted 4 weeks,and a total of 3 courses were completed.The changes in the unified Parkinson disease rating scale Ⅲ(UPDRS Ⅲ)score,Berg balance scale(BBS)score,15-meter walking speed,and 6-minute walk distance were compared between the two groups.Results:The total effective rate in the observation group was 93.6%,which was significantly higher than 45.7%in the control group(P<0.01).After 4,8,and 12 weeks of treatment,the UPDRS Ⅲ score in the observation group was significantly lower than the previous score(P<0.05);the BBS score was significantly higher than the previous score(P<0.05),and the 6-minute walk distance also significantly increased compared to the previous result(P<0.05).After 8 and 12 weeks of treatment,the 15-meter walking speed in the observation group was significantly higher than that before treatment and the previous measurement(P<0.05).In comparison with the control group,the observation group showed significantly lower UPDRS Ⅲ scores after 8 and 12 weeks of treatment(P<0.05).Additionally,after 4,8,and 12 weeks of treatment,the observation group had significantly higher BBS scores,15-meter walking speed,and the 6-minute walk distance than the control group(P<0.05).There were no significant differences in the above indicators before and after treatment in the control group(P>0.05).Conclusion:The addition of acupuncture,Tuina,and Ba Duan Jin to Western medication treatment can effectively improve motor function,balance ability,and walking ability in patients with early-or middle-stage PD.
2.Curative effect of different thrombolytic regimens on patients with ultra-early acute cerebral infarction of different characteristics

Objective To explore curative effect of two thrombolytic regimens in the treatment of ultra-early acute cerebral infarction(ACI)of different age groups and different thrombolytic time windows.Methods A total of 166 patients with ACI were enrolled,with interval from onset to treatment≤4.5 h.According to different thrombolytic regiments,patients were divided into the recombinant tissue plasminogen activator(rt-PA)group(104 cases,intravenous thrombolysis with 0.9 mg/kg rt-PA)and the tenecteplase(TNK)group(62 cases,intravenous thrombolysis with 0.25 mg/kg TNK).According to different age groups and different thrombolytic time windows,patients were divided into the high age group(≥70 years),the low age group(<70 years),the short time window group(<3 h)and the long time window group(3-4.5 h).After thrombolysis,laboratory indexes[neutrophil(NEU),lymphocyte(LYM)],scores of National Institutes of Health Stroke Scale(NIHSS),modified Rankin scale(mRS)and incidence of adverse events were compared in different groups by factorial analysis method.Results There was no significant difference in any index between the rt-PA group and the TNK group(P>0.05).Compared with the high age group at 7 d after thrombolysis,NIHSS score,NEU,neutrophil to lymphocyte ratio(NLR)and high sensitive C-reactive protein(hs-CRP)were lower,while LYM and albumin(ALB)were higher in the low age group(P<0.05).Compared with the long time window group,NEU and NLR were lower,while LYM was higher in the short time window group(P<0.05).There was no significant difference in total incidence of adverse events within 14 d of thrombolysis between the different age groups and the different thrombolytic time window groups(P>0.05).Conclusion For patients with ultra-early ACI,0.9 mg/kg rt-PA and 0.25 mg/kg TNK have comparable efficacy in intravenous thrombolysis,and both have better effects when patient age is<70 years old and the thrombolysis time window is<3 hours.

5.Exploration of scientific research management based on bibliometric analysis of SCI research papers published by Beijing Anzhen Hospital, Capital Medical University from 2008 to 2017

Objective By analyzing the SCI research papers published from 2008 to 2017 by Beijing Anzhen Hospital,to evaluate the development and trend of scientific research and provide quantified evidence for policy making of scientific research management department.Methods Based on Science Citation Index Expanded (SCIE) database,SCI papers that published by Beijing Anzhen Hospital as first author and corresponding author were searched,statistic analysis was conducted by quantities,citations,periodical impact factors,subjects and fund support.Results Totally,1 731 papers were published among which 1 156 were reviews and articles.The number of the papers published was increased year by year from 2008 to 2017.All these papers were published in 375 periodicals,and each periodical recorded 4.61 papers.Periodical that impact factor is higher than 3.0 included 315 reviews and articles accounted for 27.2%.All papers were cited 8064 times in total and the average citation was decreased from 2008 to 2017.829 papers,accounted for 47.9% were authored by cardiovascular subjects that rank the first among all subjects.57.9% of reviews and articles were supported by funds.Conclusions The quantity of SCI research papers was dramatically increased while the quality was not well matched yet.The development of science research relies not only on effective scientific research management policies but also the optimal subject platform and strong talent pool.
6.A Brief Discussion on the Importance of Scientific Secretary Team Building in a Large Size Tertiary Comprehensive Hospital

Objective To analyze the status of research secretary team in Anzhen hospital and to put forward suggestions to improve the current situation of the team.Methods Factors such as the setting of research secretary duties,implementation mode,status of research secretary team building on hospital clinical departments,and existing problems were analyzed.Results The team building of research secretary reduced the department directors' burden,standardized the daily management,and partly improved work efficiency through co-ordination.But the inadequate maintenance of of scientific research strength was needed to be proved.Conclusions Improving the selection scheme of research secretary,setting up the system of rewards and penalties,Strengthening exchanges on a regular basis may sharpens research secretary professional ability and the quality,accelerate the hospital's research developments.
7.Relationship of the levels of circulating endothelial microparticles and high-sensitivity C-reactive protein with severity of chronic left heart failure in elderly patients

Objective To study the relationship of levels of circulating endothelial microparticles (EMP62E, EMP31) and high-sensitivity C-reactive protein (hs-CRP) with severity of chronic left heart failure in elderly patients. Methods According to New York Heart Association (NYHA) class and left ventricular ejection fraction (LVEF), the healthy subjects and the patients were divided into five groups: control group [LVEF: (63.97±4.65)%], classⅠ group [LVEF: (42.67±2.06)%], classⅡ group [LVEF: (34.26±3.17)%], class Ⅲ group [LVEF: (29.05±1.07)%] and class Ⅳ group[ LVEF:(25.17±1.42)%] . The levels of circulating EMP62E, EMP31 and hs-CRP of the patients and healthy subjects were measured by flow cytometry and nephelometry immunoassay, respectively. Results There were significantly differences in EMP62E, EMP31 and hs-CRP between class Ⅳ group and classⅠ group P<0.01) EMP62E [(1092.7 ± 102.8) counts/μl vs. (291.0±21.9) counts/μl], EMP31 [(1596.1±46.3) counts/μl vs. (477.8±40.3) counts/μl] and hs-CRP [(14.74±0.07) mg/L vs. (4.86 ± 0.09) mg/L]. The levels of circulating EMP62E, EMP31 and hs-CRP were gradually elevated significantly along with the increased severity of chronic left heart failure in elderly. Conclusions The upregulation of circulating EMP31, EMP62E and hs-CRP may contribute to the development of chronic heart failure in elderly.
8.Effect of changes in CD34 cell level on various degrees of chronic left ventricular failure in the elderly

Objective To observe the effect of changes in CD34+ cell level on various degrees of chronic heart failure (CHF) in the elderly. Methods The enrolled patients were divided into four CHF groups according to the New York Heare Association(NYHA) functional class: NYHA class I (n=23), Ⅱ (n=27), Ⅲ (n = 20) and IV group (n= 16) , and there were 41 healthy controls over the same period. The levels of peripheral blood CD34+ cells were measured, and the tumor necrosis factor-α (TNF-α). its soluble receptors (sTNFR-1 and sTNFR-2) and vascular endothelial growth factor (VEGF) were also measured. Results The levels of CD34+ cells were elevated in the early CHF and depressed in the advanced CHF in elderly patients. The levels of CD34+ cells were (0. 6± 0.2) 109/L in control group, (2.4±0. 4) 109/L in NYHA class I group, (1.9±0.2)×109/L in NYHA class Ⅱ group, (1.3±0.1)×109/L in NYHA class Ⅲ group and (0.5±0.2)×109/L in NYHA class Ⅳ group, respectively (all P<0.01).And TNF-α, sTNFR-1, sTNFR-2 and VEGF were increased in severe chronic left ventricular failure CNYHA class IV vs. I group:TNF-α: (61.4± 15.7) ng/L vs. (28.4±10.8)ng/L; sTNFR-1: (2820.9±1282.8)ng/L vs. (690.8±62.7) ng/L; sTNFR-2: (4113.1±1102.2) ng/L vs. (740.8± 112.3)ng/L; VEGF: (996.3±487.1)ng/L vs. (423. 3±147. 9)ng/L, all P<0. 013. Conclusions The changes of CD34+ cell level may predict various degrees of chronic heart failure in elderly patients.
