The inherent complexity of Alzheimer's disease (AD) and failed clinical trials have spiked the interest in multifunctional ligands that target at least two key disease-associated macromolecules in AD pathology. Here we present a focused series of pleiotropic N-carbamoylazole prodrugs with dual mechanism of action. Pseudo-irreversible inhibition of the first therapeutic target, human butyrylcholinesterase (hBChE), enhances cholinergic transmission, and thereby provides symptomatic treatment, same as the standard therapeutics in use for AD. Simultaneously, this step also functions as a metabolic activation that liberates a nanomolar selective α ₂-adrenergic antagonist atipamezole, which blocks pathological amyloid β (Aβ)-induced and noradrenaline-dependent activation of GSK3β that ultimately leads to hyperphosphorylation of tau, thus achieving a disease-modifying effect. Lead compound 8 demonstrated long-term pseudo-irreversible hBChE inhibition, metabolic activation in human plasma, blood-brain barrier permeability, and p.o. bioavailability in mice. Multi-day in vivo treatment with 8 in an Aβ-induced AD murine model revealed a significant alleviation of cognitive deficit that was comparable to rivastigmine, the current drug of choice for AD therapy. Furthermore, decreased GSK3β activation and lowered tau phosphorylation were observed in APP/PS1 mice. This surpasses the symptomatic-only treatment with cholinesterase inhibitors, as it directly blocks an essential pathological cascade in AD. Therefore, these multifunctional α ₂-adrenergic antagonists-butyrylcholinesterase inhibitors, exemplified by lead compound 8, present an innovative, small molecule-based, disease-modifying approach to treatment of AD.

ObjectiveTo explore the efficacy and predictive indicators of stellate ganglion block （SGB） as an adjunctive intervention for chronic subjective tinnitus and accumulate experience for the application of SGB in the clinical treatment of tinnitus. MethodsA retrospective review was conducted on the data of chronic subjective tinnitus patients who received SGB intervention， with unsatisfactory outcomes otherwise. Pure tone audiometry （PTA）， tinnitus loudness evaluation and Pittsburgh sleep quality index （PSQI） were used. The tinnitus handicap inventory （THI） scores were compared before and after SGB intervention. Correlation analysis and linear regression equations were employed to identify the potential indicators predicting the effectiveness of SGB intervention. Statistical analysis was performed by SPSS 24.0 software. ResultsBy April 2023， a total of 107 patients with chronic subjective tinnitus had undergone SGB intervention， including 67 male and 40 female， with a mean age of （45.32±11.40） years old and an average tinnitus history of （20.32±24.64） months ［16 （12~20）］. Only 7 patients （6.54%） quitted the intervention for personal reasons， which demonstrated good compliance with the intervention. No patients experienced adverse reactions such as infection at the injection site， hematoma， nerve injury， local anesthetic intoxication and so on， which revealed good safety. After SGB intervention， THI scores decreased to below 36 points in 77 patients and decrease by 10 points or more in 12 of the remaining patients， with a total effective rate of 89%. A paired sample t-test showed a significant difference in THI scores before and after SGB intervention （t=15.575， P<0.001）， indicating good improvement. Pearson correlation analysis suggested that pre-intervention THI scores and subjective tinnitus loudness were significantly positively correlated with the improvement level of THI scores （P<0.05）. Further stepwise linear regression analysis found that "pre-intervention THI scores" had statistical significance （P<0.001）， with a regression coefficient of 0.308， predicting a 17.4% improvement level in THI scores. ConclusionsDue to its good and safe short-term effects， SGB intervention can be used as a supplementary option for chronic subjective tinnitus when other interventions are not ideal， especially for patients with higher THI scores. However， further research is needed to clarify the long-term efficacy and underlying mechanisms， in order to establish a more solid theoretical basis for SGB intervention in the treatment of subjective tinnitus.

Objective To investigate the predictive value of a nomogram model constructed with clinical variables and multiparametric magnetic resonance imaging(MRI)radiomics features in prognosis of patients undergoing chemotherapy for breast cancer.Methods A total of 150 breast cancer patients who received chemotherapy in Qingdao Central Hospital from January 2020 to December 2021 were enrolled for the study.All patients underwent breast MRI examination before chemotherapy,and the optimal radiomics features were extracted from the tumor regions of interest on multiparametric MRI image which were manually outlined using segmentation software.After 2-year follow-up,the patients were divided into good prognosis group and poor prognosis group according to whether there were disease recurrence,metastasis and death,and they were divided into training set(n=105)and test set(n=45)in a ratio of 7:3.The clinical data and multiparametric MRI radiomics features were collected,and the factors affecting the prognosis of breast cancer patients receiving chemotherapy were analyzed by univariate and multivariate Logistic regression analyses.Three models,namely clinical model,radiomics model,and clinical-multiparametric MRI radiomics-based nomogram model were constructed,and their predictive value in the prognosis of breast cancer patients treated with chemotherapy was analyzed with receiver operating characteristic curves.Results Out of 150 cases of breast cancer treated with chemotherapy,52 cases were included in poor prognosis group,with 33 cases of recurrence and/or metastases and 19 cases of deaths.Logistic regression analysis identified TNM stage,estrogen receptor and progesterone receptor as clinical predictors for prognosis of breast cancer patients treated with chemotherapy(P<0.05).There were 69 and 36 patients with good and poor prognoses in training set,and 29 and 16 patients with good and poor prognoses in test set.Through least absolute shrinkage and selection operator algorithm,16 radiomics features related to the prognosis of breast cancer patients treated with chemotherapy were screened out from TIRM,DWI,and DCE-MRI sequences,and their radiomics scores were calculated.Compared with patients with poor prognosis,those with good prognosis in training set and test set had higher radiomics scores(P<0.05).ROC curve analysis showed that the area under the curve of radiomics model,clinical model,and nomogram model were 0.853,0.741,0.923 in training set(P<0.05),and 0.797,0.749,0.896 in test set(P<0.05),respectively.Conclusion Based on pre-chemotherapy radiomics scores,TNM stage,estrogen receptor and progesterone receptor are identified as independent predictors for prognosis of breast cancer patients treated with chemotherapy,and the clinical-multiparametric MRI radiomics-based nomogram can effectively improve the predictive efficacy of prognosis in breast cancer patients treated with chemotherapy.

Objective:To explore the effect of music relaxation training (MRT) in patients with breast cancer after axillary lymph node dissection (ALND) .Methods:A total of 72 breast cancer patients admitted to Qingdao Central Hospital and Qingdao Cancer Hospital from January 2019 to January 2021 were selected for control experiments. All patients were treated with ALND based on their specific conditions. According to the random number table method, the patients were divided into the control group and the intervention group, each with 36 cases. The control group carried out routine nursing, and the intervention group conducted MRT on the basis of routine nursing. The Short Form 36 Health Survey (SF-36) and Visual Analog Scale (VAS) scores of the two groups before treatment and at discharge were compared.Results:After the intervention, the SF-36 score and satisfaction of the intervention group were higher than those of the control group, and the VAS score was lower than that of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The application of MRT after ALND in breast cancer patients can improve the health of patients, increase patient satisfaction, and reduce their pain.

Objective: To summarize the clinical characteristics of a patient with cerebellar ataxia, areflexia, pes cavus, optic atrophy and sensorineural hearing loss (CAPOS) syndrome, followed by relative literature review. Methods: The medical history, physical examination and results of relative auxiliary examinations were collected from a CAPOS syndrome patient, who was definitely diagnosed by gene detection. Results: The patient was a 20-year-old male, complaining of poor coordination for 19 years, impaired vision for 15 years and hearing loss for 13 years. When he was eleven months old, weakness of four limbs happened after diarrhea but recovered spontaneously a few days later. Then his poor coordination was discovered. His vision has decreased progressively since the age of five and he began to suffer from bilateral hearing loss after fever at the age of seven. Anti-infectious and immunoregulatory treatment was ineffective at that time. Physical examination showed that bilateral visual acuity decreased. Transient horizontal gaze-evoked nystagmus and bilateral hearing loss were detected. Obvious shaking was observed with closed eyes and toes together. Finger-to-nose, finger tracking, heel-knee-tibia and alternate motion tests were slightly inaccurate. Deep tendon reflexes disappeared and no pes cavus was observed. Pure tone audiometry revealed bilateral sensorineural hearing loss. Cranial magnetic resonance imaging indicated bilateral optic atrophy. ATP1A3 gene detection in the patient showed c. 2452G>A (p. Glu818Lys) heterozygous mutation while his parents were detected no such mutation in the same locus. Conclusions As for young patients who suffer from acute cerebellar ataxia after fever, disappeared tendon reflexes, atrophy of optic nerves or sensorineural hearing loss, they should be alerted to CAPOS syndrome when immunomodulating or anti-inflammatory therapy has been proved to be useless. Positive family history and ATP1A3 gene mutation would be beneficial to definite diagnosis.

ObjectiveTo study the effects of individualized scalp acupuncture based on the focus and syndrome differentiation in traditional Chinese medicine on mobility ability of children with spastic cerebral palsy. Methods55 cases were randomly divided into 2 groups: treatment group (n=29) and control group (n=26). Treatment group received individualized acupuncture combined with rehabilitation and the control group received Jin's acupuncture combined with rehabilitation for 3 month. They were assessed with the mobility domains of the Pediatric Evaluation of Disability Inventory (PEDI). ResultsThe improvement of PEDI scores was more in treatment group than in control group （P＜0.01）.ConclusionScalp acupuncture combined with rehabilitation has active effects on mobility abilities of children with spastic cerebral palsy, and the individualized acupuncture do better than the Jin's acupuncture.

Objective To investigate the reliability of Pediatric Evaluation of Disability Inventory (PEDI) in China. Methods 21 children with cerebral palsy were assessed with PEDI by 2 raters respectively. The intrarater and interrater agreement was analyzed. The internal consistency of the PEDI was also calculated. Results The ICC of intrarater and interrater was 0.919～0.993. The Cronbach α was 0.9597～0.9711 in self care, 0.9569～0.9799 in mobility, 0.9419～0.9830 in social function.Conclusion PEDI is reliable and internal consistent.
