1.Knowledge map and visualization analysis of pulmonary nodule/early-stage lung cancer prediction models

Objective To reveal the scientific output and trends in pulmonary nodules/early-stage lung cancer prediction models. Methods Publications on predictive models of pulmonary nodules/early lung cancer between January 1, 2002 and June 3, 2023 were retrieved and extracted from CNKI, Wanfang, VIP and Web of Science database. CiteSpace 6.1.R3 and VOSviewer 1.6.18 were used to analyze the hotspots and theme trends. Results A marked increase in the number of publications related to pulmonary nodules/early-stage lung cancer prediction models was observed. A total of 12581 authors from 2711 institutions in 64 countries/regions published 2139 documents in 566 academic journals in English. A total of 282 articles from 1256 authors were published in 176 journals in Chinese. The Chinese and English journals which published the most pulmonary nodules/early-stage lung cancer prediction model-related papers were Journal of Clinical Radiology and Frontiers in Oncology, respectively. Chest was the most frequently cited journal. China and the United States were the leading countries in the field of pulmonary nodules/early-stage lung cancer prediction models. The institutions represented by Fudan University had significant academic influence in the field. Analysis of keywords revealed that multi-omics, nomogram, machine learning and artificial intelligence were the current focus of research. Conclusion Over the last two decades, research on risk-prediction models for pulmonary nodules/early-stage lung cancer has attracted increasing attention. Prognosis, machine learning, artificial intelligence, nomogram, and multi-omics technologies are both current hotspots and future trends in this field. In the future, in-depth explorations using different omics should increase the sensitivity and accuracy of pulmonary nodules/early-stage lung cancer prediction models. More high-quality future studies should be conducted to validate the efficacy and safety of pulmonary nodules/early-stage lung cancer prediction models further and reduce the global burden of lung cancer.
2.Role of the iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance

Objective :
To pathological changes and inducible nitric oxide synthase(iNOS), phosphorylated insulin receptor substrate 1 serine 307(p-IRS1ser 307), phosphorylated protein kinase B serine 473(p-AKTser 473), glycogen synthase kinase-3β(GSK-3β), and gluconeogenic synthase(GS) proteins were observed in the liver of rats under the condition of chronic intermittent hypoxia-replicated oxygen in control. And to explore the role of iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance.
Methods :
Forty SD rats were randomly divided into a control group(NC group) and an experimental group(CIH group), with 20 rats in each group. The NC group was placed in a normoxic environment for 12 weeks, while the CIH group was first subjected to intermittent hypoxia for 8 weeks, and then resumed normoxic rearing until the 12th week. Fasting blood glucose(FBG) and fasting insulin(FINS) were measured at baseline, week 8 and week 12, and liver tissues were taken for pathology and measurement of iNOS, p-IRS1ser 307, p-AKTser 473, GSK3β and GS levels, to compare the differences between groups.
Results:
t baseline, there was no significant difference in liver pathology between the two groups, and the observed indexes were not statistically significant(P>0.05); at 8 weeks, compared with the NC group, liver pathology in the CIH group showed significant disorganization of hepatic blood sinusoids and hepatocyte cords, obvious hepatocyte edema, smaller nuclei, increased lymphocyte infiltration, and a small number of fat vacuoles, significantly higher levels of FBG, FINS, insulin resistance index(HOMA-IR), iNOS mRNA, p-IRS1ser 307 protein, GSK-3β protein levels, and decreased p-AKTser 473 protein and GS protein levels, all of which were statistically significant(allP<0.05). IRS1ser 307 protein, GSK-3β protein levels were increased, p-AKTser 473 protein and GS protein levels were decreased, and the differences were statistically significant(allP<0.05); at 12 weeks, no lymphocyte infiltration was seen in the CIH group compared with that of the NC group and fat vacuoles significantly increased, and there was no improvement in the other pathological damage that had already occurred, and the levels of p-AKTser 473 protein significantly increased. AKTser 473 protein level significantly increased, p-IRS1ser 307 protein and GS protein levels were significantly reduced, all of which were statistically significant(allP<0.05), and the rest of the observational indexes were not statistically significant. Pearson′s correlation analysis showed that HOMA-IR of CIH group was significantly positively correlated with the levels of iNOS mRNA, p-IRS1ser 307 protein, and GSK-3β protein at 8 weeks(r=0.874, 0.817,0.872;allP<0.05), and significantly negatively correlated with the levels of p-AKTser 473 protein and GS protein(r=-0.886,-0.879;allP<0.05).
Conclusion
Chronic intermittent hypoxia can lead to hepatic pathological damage that cannot be reversed even by reoxygenation interventions and may mediate the development of insulin resistance by upregulating the IRS1/AKT/GSK-3β signaling pathway through the upregulation of iNOS mRNA expression.
3.Prognostic study of 18F-FDG PET/CT combined with clinical characteristics in treated patients with esophageal squamous cell carcinoma

Objective:To evaluate the prognostic value of 18F-fluorodeoxyglucose(FDG)positron emission tomography(PET)/com-puted tomography(CT)(18F-FDG PET/CT)metabolic parameters combined with clinical characteristics in treated patients with esophageal squamous cell carcinoma(ESCC).Methods:The clinical data of 75 patients(65 males and 10 females,age of 63.41±7.75 years)with pathologically confirmed ESCC in * Hospital from January 2015 to November 2021 were retrospectively analyzed.All pa-tients underwent 18F-FDG PET/CT examination after treatment.The relevant parameters of 18F-FDG PET/CT were determined:whole body SUVmax(SUVmaxwb);SUVmean and metabolic tumor volume(MTV)were measured with 40%SUVmax as the critical value,and whole body MTV(MTVwb)and whole body total lesion glycolysis(TLGwb)were calculated.Kaplan-Meier survival curves and Cox proportional hazards model were used to evaluate the relationship between PET parameters and overall survival(OS).Results:Fifty-two(69.3%)patients died.PET-positive patients exhibited a 6.029-fold increased risk of death compared with PET-negative patients(P<0.001),with the median survival time of 22.3 months and 43.2 months,respectively.PET-positive patients were catego-rized based on median parameters of PET:SUVmaxwb=11.09,MTVwb=27.07 cm3,and TLGwb=162.34 g.Kaplan-Meier survival curves and log-rank tests revealed the correlations of pathological classification,M stage,post-PET anti-tumor treatment,MTVwb,and TLGwb with OS.M stage emerged as an independent predictor for OS(hazard ratio=5.698,95%CI=1.791-18.123,P=0.003).Patients positive for both PET imaging and serology had a 6.112-fold higher death risk compared with those negative for PET imaging(P<0.001).Conclusion:18F-FDG PET/CT volume metabolism parameters are significant prognostic predictors for treated patients with ESCC,and patients positive for PET imaging and tumor markers are associated with a poor prognosis.
4.Treatment of Recurrent Aphthous Ulcers from the Perspective of "Heart"

Based on the traditional Chinese medicine theory that "all pain, itching, and sores are related to the heart", this paper proposes treating recurrent aphthous ulcers from the perspective of the heart. It suggests that excessive heart fire and tissue erosion due to flaming fire in the heart meridian constitute the core pathogenesis of this condition. Hyperactive heart fire is identified as the key pathogenic factor, while heart yin deficiency, obstruction of the heart collaterals, and malnourishment of the heart spirit are considered significant contributing factors. Clinically, the treatment follows the principle of clearing heart fire as the main strategy, supplemented by nourishing yin, activating collaterals, and calming the spirit. The self-formulated Qingxin Yuchuang Formulation (清心愈疮方) serves as the base prescription, with flexible modifications incorporating the Yuyin Formulation (育阴方), Huoxue Formulation (活血方), and Yu'an Formulation (郁安方) to address specific syndromes involving heart yin deficiency, collateral blockage, and emotional disturbance.
5.Analysis of transcriptomic comorbidity mechanism of nonalcoholic fatty liver disease and colorectal cancer based on bioinformatics

Objective To screen and analyze biomarkers of comorbidity in nonalcoholic fatty liver disease(NAFLD)and colorectal cancer(CRC)based on bioinformatics so as to explore the mechanism of comorbidity.Methods After data related to NAFLD and CRC were downloaded from the public database(GEO),gene set enrichment analysis(GSEA)was performed for all genes to screen the expression pathways,and the common differentially expressed genes of both genes were annotated by GO and KEGG functions.Subsequently,PPI protein interaction network was constructed using STRING online database to screen key genes,and transcription factor-gene regulatory network was constructed based on key genes,and biomarker genes were identified using LASSO regression.Finally,the transcription factor prediction analysis of key genes and SCD was conducted through Network Analyst database,and the transcription factor network was constructed.Results GSEA analysis revealed that signaling pathways such as TNF-α and IL-6-JAK-STAT3 were activated in NAFLD patients.The 17 common differentially expressed genes were mainly enriched in biological processes such as bivalent inorganic cation homeostasis and neutrophil chemotaxis in GO analysis.KEGG pathway analysis showed that common differentially expressed genes were mainly concentrated in IL-17 signaling pathway.The protein interaction network screened out five key genes:TREM1,FPR1,IL1RN,S100A9,and S100A8.IL1RN and SCD,identified by LASSO regression,were validated as biomarker genes using external datasets and immunohistochemistry data from the human protein atlas(HPA)database,showing high expression in CRC tissues.A transcription factor-key comorbidity gene regulatory network was constructed using Network Analyst.Conclusion The genes for comorbidities between NAFLD and colorectal cancer discovered by bioinformatics reveal the potential pathogenesis of comorbidities,and provide ideas for cancer screening and early diagnosis of colorectal cancer in NAFLD patients.
6.Preliminary evaluation of the protective effects of semicircular canal occlusion under continuous irrigating mode of endoscopic ear surgery on hearing function

Objective:To introduce a new technique of precise exposure and preservation of membranous semicircular canal under endoscope, and to evaluate the protective effect of this method on hearing preservation of semicircular canal occlusion.Methods:The study included three patients with unilateral refractory Meniere′s disease, including 1 male and 2 females, with hearing stage 4 in 2 cases and stage 3 in 1 case, the average hearing thresholds were 72.5, 82.5 and 48.8 dBHL, respectively. All of them were treated with triple semicircular canal occlusion under continuous irrigating mode of endoscopic ear surgery (CIM-EES) in RenMin Hospital of WuHan University. The surgical procedure involved the following key steps: the bony walls of the three semicircular canals near the ampullae were gradually thinned to expose the blue line. The endoscope was positioned as close as possible to the operating area to obtain a clear, magnified view. A micro hook needle was used to create a window on the bony semicircular canals. The hook needle progressively removed the bony material while preserving the membranous canals, which were clearly visible during the procedure. The bone dust was removed out of the surgical area by the continuous exchanged insuline without the help of suction that can avoid the damage of the membranous semicircular canals caused by aspiration and the open bony canals were precisely filled with bone wax and covered with bone powder to ensure complete occlusion of the three semicircular canals. Postoperative follow-up included regular assessments of vertigo control and hearing preservation.Results:All the 3 patients obtained complete hearing preservation at 6 months after surgery, and the hearing preservation rate was 100%. The average hearing thresholds at 6 months postoperatively were 66.3, 81.3, and 50.0 dBHL, respectively. Postoperatively, all patients experienced horizontal spontaneous nystagmus toward the healthy side on the day of surgery and the first day after surgery. One patient reported complete disappearance of tinnitus, while the other two patients showed no change in tinnitus severity. Postoperative MRI revealed complete occlusion of the three semicircular canals with normal vestibular imaging. At 6 months postoperatively, none of the patients experienced any vertigo attacks, suggesting effective control of vertigo symptoms.Conclusions:Precise Semicircular canal occlusion under CIM-EES is a promising technique for the treatment of intractable vertigo in patients with refractory Meniere′s disease. This method provides clear visualization of the membranous canals, reducing the risk of inner ear damage and preserving hearing function of the patients.
7.Analysis of clinical features and prognostic factors of focal cerebral arteriopathy in children

Objective:To summarize the clinical characteristics of focal cerebral arteriopathy (FCA) in children, and to analyze its influencing factor of prognosis.Methods:A retrospective cohort study was conducted. Clinical data from 40 children with FCA who were hospitalized at the Department of Neurology, Beijing Children′s Hospital, Capital Medical University, from September 2015 to August 2024 were collected. A centralized follow-up was conducted in October 2024 via outpatient clinics or the internet. The pediatric stroke outcome measure (PSOM) was used to evaluate their outcomes. Based on the PSOM, the children were further divided into a group with normal neurological function and another group with abnormal neurological function. Differences between groups were analyzed using the Mann-Whitney U test and Fisher exact test. Univariate Logistic regression analysis was performed to identify the influencing factors for neurological outcomes in children with FCA. Results:A total of 40 children were included, with 20 males and 20 females, and the onset age of 9.2 (6.8, 12.5) years. Among them, 12 cases (30%) had a history of varicella within 1 year before onset. There were 23 cases (58%) presenting with transient ischemic attack (TIA) or recurrent fluctuating symptoms of onset, while 3 cases (8%) developed progressive stroke within the first month of onset. The M1 segment of the middle cerebral artery was the most commonly affected vascular site, with a total of 16 cases (40%). Arterial occlusion occurred in 8 cases (20%). Lumbar puncture was completed in 36 children, and white blood cell counts in cerebrospinal fluid was increased in 6 cases. All 23 patients who completed magnetic resonance vessel wall imaging (VWI) showed circular enhancement of the arterial wall. A total of 28 patients (70%) received antiplatelet or anticoagulation therapy, and 16 patients (40%) received hormone therapy. At admission, the pediatric National Institute of Health Stroke Scale (PedNIHSS) score was 6.0 (2.0, 8.8) points, which decreased to 0.5 (0, 3.0) points at discharge. The follow-up duration was 1.6 (0.8, 4.9) years, with 1 case lost to follow-up. There was 1 case presenting with recurrence course manifesting as TIA. Among the 39 cases who completed the follow-up, 23 cases (59%) were assessed as neurologically normal by PSOM, while 16 cases (41%) were assessed as neurologically abnormal. Among the 29 cases who completed the imaging review, magnetic resonance angiography (MRA) review in 23 cases indicated stability or improvement in the original arterial stenosis, with 6 cases experiencing transient worsening of arterial stenosis early in the disease course (within 2 months), which later improved. Arterial stenosis progression occurred in 6 cases at the final review of 29 cases who completed the imaging review, with 1 case developing progressive cerebral arteriopathy. The proportion of patients with headache, altered consciousness, and aphasia in the abnormal neurological function group, as well as the PedNISS scores at admission and discharge, were all higher than those in the normal neurological function group (all P<0.05). Univariate Logistic regression analysis revealed that only a PedNISS score>6 points at onset was an influencing factor for abnormal neurological function ( OR=20.58, 95% CI 3.93-107.70, P<0.001). Conclusions:Childhood FCA often presents with fluctuating onset, and the proximal segment of the middle cerebral artery is frequently affected. Progression of arterial stenosis is common within 2 months of the disease course, but clinical progression and new ischemic lesions are uncommon. Most patients have a favorable long-term prognosis. PedNIHSS score>6 points at admission is related to abnormal neurological function outcomes.
8.Investigation of 90Sr radioactivity levels in drinking water and food around the Qinshan Nuclear Power Plant from 2012 to 2022

Objective:To investigate the radioactivity levels and variation trends of 90Sr in drinking water and food around the Qinshan Nuclear Power Plant in operation. Methods:From 2012 to 2022, the source, factory, and tap water was collected within 30 km around the Qinshan Nuclear Power Plant during the wet and dry seasons (i.e., May and October, respectively) each year to determine the 90Sr concentration in water. According to the dietary habits of local residents, four kinds of food, including rice, cabbage, mullet, and crucian carp, were collected to determine and analyze the 90Sr radioactivity concentration in food using the bis-(2-ethylhexyl) phosphoric acid extraction chromatographic method. Results:From 2012 to 2022, the 90Sr radioactivity concentrations in the source, factory, and tap water around the Qinshan Nuclear Power Plant ranged from 3.73 to 11.89 mBq/L, 2.95 to 9.83 mBq/L, and 3.12 to 8.70 mBq/L, respectively, showing nonsignificant fluctuations over the years. The 90Sr radioactivity concentrations in rice, cabbage, mullet, and crucian carp ranged from 0.02 to 0.46 Bq/kg (dry weight), 0.26 to 1.07 Bq/kg (fresh weight), 0.38 to 1.05 Bq/kg (fresh weight), and 0.08 to 1.32 Bq/kg (fresh weight), respectively, all below the national standard limits. Conclusions:From 2012 to 2022, the 90Sr radioactivity levels in drinking water and food around the Qinshan Nuclear Power Plant were at the background level and remained stable.
9.High-quality development of radiological health service in Zhejiang province by means of a multi-dimensional closed-loop quality control system

To promote the development of radiological health by means of high quality and efficient quality control measures is worthy of further discussion and practice. There are 34 radiological health technical service institutions (including 11 Grade A ones), 36 occupational health examination institutions for radiation workers, and 4 radiation disease diagnosis institutions in Zhejiang province. Zhejiang Provincial Health Commission approved the establishment of Zhejiang Provincial Radiological Health Technical Service Quality Control Center and Zhejiang Provincial Occupational Health Technical Service Quality Control Center in 2012 and 2021, respectively, both under the leadship of Zhejiang Provincial Center for Disease Prevention and Control. Through constructing a multi-dimensional closed-loop quality control system, a great progress has been made in efforts in radiological health of Zhejiang province.
10.Progress in clinical diagnosis and treatment of Haglund syndrome

Haglund syndrome (HS) is a common cause for posterior heel pain in ankle surgery, but the etiology of heel pain is so complicated that its pathogenic factors are currently unclear. For such diseases as posterior heel pain, conservative treatment should be carried out first. However, as their cause is not eliminated their symptoms are likely to recur. With the rapid development of biotechnology, imaging technology, and arthroscopy technology, biological therapy and minimally invasive surgery have gradually become the main treatments for HS. This review expounds on the factors, mechanisms, imaging diagnostic methods, options of conservative and surgical treatments concerning HS, hoping to help the clinical treatment of HS.
