1.Three-dimensional displacement and risk factors of midshaft clavicle fractures treated with titanium elastic intramedullary nailing

BACKGROUND:Titanium elastic intramedullary nailing for the treatment of significantly displaced midshaft clavicle fractures has the characteristics of minimally invasive and elastic fixation.The displacement of the fracture is closely related to the later function.However,there are few studies on the three-dimensional displacement analysis of the fracture ends before surgery and after intramedullary fixation such as titanium elastic intramedullary nailing.OBJECTIVE:To explore the three-dimensional displacement of fracture ends after midshaft clavicle fracture and fixation with titanium elastic intramedullary nails,and to analyze the risk factors.METHODS:A total of 91 patients with midshaft clavicle fracture(fracture end shortening ≥15 mm)admitted to Wendeng Orthopedic Hospital of Shandong Province from April 2019 to April 2024 were selected,including 57 males and 34 females,aged(51.73±10.21)years old.All patients received closed reduction and internal fixation with titanium elastic intramedullary nail.CT scans of the affected clavicle were performed before and on the first day after surgery.The CT data were imported into Mimics software for modeling.The length of the clavicle,lateral displacement of the fracture end,and rotation of the distal end of the fracture along the X,Y,and Z axes were measured and recorded before and after surgery.Pearson correlation coefficient was used for correlation analysis of various parameters,and generalized linear regression was used to evaluate risk factors.RESULTS AND CONCLUSION:(1)Preoperatively,the variable that increased the risk of lateral displacement was the number of comminuted bone fragments,the variable that increased the risk of shortening displacement was male patients,and the variable that increased the risk of Z-axis rotation was the left limb.Shortening displacement was significantly positively correlated with lateral displacement(r=0.715,P<0.001);shortening displacement was significantly positively correlated with X-axis rotation displacement and Y-axis rotation displacement(r=0.265,P=0.028;r=0.303,P=0.011);lateral displacement was significantly positively correlated with Y-axis rotation and Z-axis rotation(r=0.258,P=0.032;r=0.250,P=0.038);X-axis rotation was significantly positively correlated with Y-axis rotation(r=0.382,P=0.001),and Z-axis rotation was significantly positively correlated with Y-axis rotation(r=0.280,P=0.020).(2)Postoperatively:The number of scapula fractures and comminuted bone fragments were variables that increased the risk of postoperative shortening and lateral displacement:Preoperative X-,Y-,and Z-axis rotation displacements were risk variables that increased postoperative X-,Y-,and Z-axis rotation displacements,respectively.Postoperative lateral displacement was significantly positively correlated with postoperative shortening and displacement(r=0.584,P=0.000),and postoperative lateral displacement was also significantly positively correlated with postoperative Y axis rotation and Z axis rotation(r=0.360,P=0.002;r=0.250,P=0.038).Postoperative Y axis rotation was significantly positively correlated with postoperative Z axis rotation(r=0.248,P=0.040).(3)The results showed that the three-dimensional displacement of the clavicle end before and after surgery was affected by many factors,especially the number of comminuted bone fragments,scapula fractures,gender,and original rotation displacement.At the same time,there were complex correlations between various displacements,especially the correlation between shortening displacement and lateral displacement was the strongest.

3.Management Strategy for Full-course Chain of Gastric Precancerous Lesions Based on Core Pathogenesis of Deficiency, Stagnation, Phlegm, Stasis, and Toxin

Gastric precancerous lesions, often presenting as decreased appetite, weight loss, fatigue, epigastric fullness, and upper abdominal pain, pose challenges to treatment and considerably affect patients' quality of life. Pathologically, these lesions are characterized by alterations in the gastric mucosa accompanied by intestinal metaplasia or dysplasia, which markedly elevate the risk of developing gastric cancer. Traditional Chinese medicine (TCM) practitioners have explored various therapeutic approaches focusing on invigorating the spleen, resolving dampness, clearing heat, addressing phlegm accumulation, activating blood, and detoxifying. For contemporary populations suffering from gastric precancerous lesions, our team has innovatively proposed a core pathogenesis theory encompassing deficiency, stagnation, phlegm, stasis, and toxin. Furthermore, our team has developed a comprehensive prevention and treatment framework: Invigorating the spleen and harmonizing the stomach (foundational principles), balancing and restoring Qi movement (primary objectives), resolving phlegm and unblocking collaterals (routine practices), detoxifying and dissipating nodules (critical interventions), and comprehensive health management (supportive care). This framework provides a robust theoretical foundation along with practical guidance for preventing and treating diseases related to the spleen-stomach axis. From a pathological perspective, key interventions for managing gastric precancerous lesions include restoring integrity of the gastric mucosae while regulating its microenvironmental conditions and enhancing mucosal circulation, which are integral throughout the entire treatment process. On this basis, a full-course chain of prevention and treatment has been established within clinical practice, encompassing functional gastrointestinal disorders-superficial gastritis-atrophic gastritis-gastric precancerous lesions-prevention and treatment post-endoscopic submucosal dissection. The guiding principles of this treatment approach involve invigorating the spleen, regulating Qi movement, resolving phlegm, unblocking collaterals, and detoxifying. TCM compound formulas such as Jianpi Tongluo Jiedu prescription and Hezhong Xiaopi granules, as well as Chinese patent medicines like Weifuchun and Moluodan, can be used for treating gastric precancerous lesions at different stages. Through medicinal regulation, the state of the gastric mucosa can be improved, and thus the development of gastric precancerous lesions can be curbed from the perspective of full-course prevention and treatment.
4.Hezhong Xiaopi Granules Regulate Ncln-CHOP Axis to Inhibit Endoplasmic Reticulum Stress and Reduce Gastric Mucosal Cell Apoptosis

ObjectiveTo investigate the effect of Hezhongxiaopi granules (HZXPG) on methylnitronitrosoguanidine (MNNG)-mediated gastric mucosal cell apoptosis and the regulatory effects on the Nicalin (Ncln)-C/EBP homologous protein (CHOP) axis and endoplasmic reticulum stress (ERS). MethodsThe cell counting kit-8 (CCK-8) assay was employed to examine the inhibitory effect of MNNG on the proliferation of GES-1 cells, and the optimal effective dose of MNNG was selected. Human gastric mucosal GES-1 cells were allocated into the control, model (MNNG, 1.562 5 mmol·L-1), and low/high-dose HZXPG (5% and 10% HZXPG-containing serum) groups. Phalloidin staining was employed to detect MNNG-induced morphological changes in human gastric mucosal cells. Terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay was adopted to assess cell apoptosis. Annexin Ⅴ/propidium iodide (Annexin Ⅴ/PI) flow cytometry was utilized to measure the alterations in the apoptotic cycle of human gastric mucosal cells. Western blot was performed to determine the expression levels of proteins related to apoptosis, ERS, and the Ncln/CHOP axis. ResultsCompared with the control group, the model group exhibited a reduction in the surface area of GES-1 cells, endoplasmic reticulum (ER) dilation, increased early and late apoptosis rates of gastric mucosal cells, upregulated protein levels of truncated Bid (tBid), Bcl-2-associated X protein (Bax), cleaved-cysteinyl aspartate-specific proteinase (Caspase)-3, and cleaved-Caspase-8, and downregulated protein level of B-cell lymphoma-2 (Bcl-2) (P<0.01). In addition, the expression levels of binding immunoglobulin protein (BiP), Ncln, and CHOP were upregulated (P<0.01), with enhanced Ncln expression in the ER and elevated CHOP expression in the nucleus. Compared with the model group, the high-dose HZXPG group showed a significant increase in the surface area of gastric mucosal cells. Both dose groups exhibited inhibited ER dilation and reduced early and late apoptosis rates of gastric mucosal cells (P<0.01). Low-dose HZXPG decreased the expression of tBid and Bax (P<0.05, P<0.01) and promoted Bcl-2 expression (P<0.01). High-dose HZXPG reduced the expression levels of tBid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 (P<0.05, P<0.01) and increased the Bcl-2 expression level (P<0.05). Furthermore, high-dose HZXPG downregulated the expression levels of BiP, Ncln, and CHOP (P<0.05, P<0.01), while decreasing the nuclear translocation of CHOP. ConclusionHZXPG alleviates MNNG-induced ERR-mediated apoptosis of gastric mucosal cells by inhibiting the Ncln-CHOP axis, thereby relieving gastric mucosal cell damage caused by precancerous gastric lesions.
5.Theoretical and Practical Analyses of Traditional Chinese Medicine in Prevention and Treatment of Senile Chronic Atrophic Gastritis

Senile chronic atrophic gastritis (SCAG) is a condition characterized by high prevalence, prolonged duration, and difficulty in treatment, along with a significant risk of carcinogenesis. This severely impacts the quality of life among the elderly population in China. The prevention and management of SCAG represent one of the most urgent and prominent needs within the current health care and elderly care sectors. However, existing treatment options are limited, with insufficient mucosal repair following Helicobacter pylori eradication, as well as a lack of effective methods to reverse atrophy or intestinal metaplasia. The primary objectives in treating chronic gastritis in older adults are symptom relief and alleviation of mucosal histological abnormalities. Traditional Chinese medicine (TCM) offers unique advantages for managing patients with SCAG. Therefore, utilizing TCM methodologies underpinned by TCM theory to prevent and treat the progression of SCAG holds considerable clinical significance. Our research team has been dedicated to theoretical, clinical, and fundamental investigations into gastric precancerous lesions and SCAG for an extended period. This article systematically reviews the intricate causal relationship between the spleen-stomach system and aging while elucidating TCM pathogenesis of SCAG as internal deficiency coupled with excess in manifestations. It explores evolutionary patterns of dampness, stagnation, stasis, toxicity, and deficiency and proposes a progressive pathogenic evolution chain: Aging-deficiency of spleen and kidney-imbalance of Qi and blood-phlegm turbidity accompanied by Qi stagnation-accumulation of phlegm, stasis, and toxicity-weakness of stomach collaterals. The core treatment principle emphasizes simultaneous treatment targeting both spleen and kidney as its foundation while employing strategies such as promoting Qi movement to resolve dampness, alongside efforts aimed at removing stasis and detoxifying as primary approaches. Finally, this article summarizes current research achievements in TCM interventions for SCAG, aiming to provide new insights for clinical practice and scientific research in TCM interventions for SCAG.
6.From Spleen Deficiency with Dampness Excess to Stasis-toxin Damaging Collaterals: Dynamic Management Path in Whole-course Treatment Strategies for Ulcerative Colitis

Ulcerative colitis (UC), a chronic nonspecific inflammatory bowel disease characterized by unclear etiology, complex contributing factors, and a refractory course with malignant transformation potential, imposes a substantial burden on both individual patients and healthcare systems. There is an urgent need to explore effective therapeutic strategies and pharmacological interventions. Professor LIU Zhen, based on years of clinical practice, proposed a dynamic pathogenesis chain of UC: Spleen deficiency with dampness excess as the foundation, dampness obstruction as the nexus, stasis-toxin interaction as the key, and stasis-toxin damaging intestinal collaterals as the terminal outcome. The treatment, following the progression of deficiency (depression)-dampness-toxin-stasis (toxin), should focus on invigorating the spleen and resolving dampness. A stage-specific treatment approach is emphasized. During the susceptible stage, prevention is recommended, which relies on regulating the diet, maintaining a positive mental state, and adjusting daily routines to invigorate the spleen and replenish Qi. During the acute attack and remission stages, the treatment should be tailored according to clinical typing and disease severity, flexibly employing therapeutic strategies such as tonifying deficiency, resolving depression, eliminating dampness, clearing heat, detoxifying, arresting bleeding, and resolving stasis in order to interrupt disease progression. In the recovery stage, latent pathogenic factors should be addressed while the postnatal constitutional foundation be preserved to prevent recurrence. According to the TCM principle of treating disease before its onset, a whole-course management path of early protection-interception-stratified care-consolidation has been developed, covering all stages of UC, to inform its clinical prevention and treatment.
7.Influence of self-esteem, alexithymia and alienation on non-suicidal self-injury behaviors in adolescents

BackgroundIn recent years, the incidence of non-suicidal self-injury (NSSI) behaviors among adolescents has been increasing annually. Self-esteem and alexithymia are strongly associated with NSSI behaviors, and alienation is closely linked to both self-esteem and alexithymia. However, there is limited research on the relationship between alienation and NSSI behaviors among adolescents in China. ObjectiveTo analyze the relationship between alienation and NSSI behaviors among adolescents, and to explore the factors influencing NSSI behaviors in this population, so as to provide insights for the prevention and treatment of NSSI behaviors in adolescents. MethodsAdolescents admitted to the Department of Psychiatry and Psychology at the 923rd Hospital of the Joint Logistics Support Force of the Chinese People's Liberation Army from September 1, 2021 to March 1, 2023, who met the diagnostic criteria for NSSI in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5), were selected as the study group (n=60). Concurrently, middle school students from Nanning were recruited as the control group (n=60). Participants were assessed using Adolescent Self Harm Scale (ASHS), Rosenberg Self-Esteem Scale (RSES), Toronto Alexithymia Scale (TAS) and Adolescent Students′ Alienation Scale (ASAS). Pearson correlation analysis was employed to examine the relationships between scale scores in the study group, and Logistic regression analysis was used to identify the influencing factors of NSSI behaviors among adolescents. ResultsThe RSES score of the study group was significantly lower than that of the control group (t=-7.033, P<0.01). The TAS and ASAS scores of the study group were significantly higher than those of the control group (t=5.591, 8.124, P<0.01). The ASHS score was negatively correlated with RSES score (r=-0.410, P<0.01) and positively correlated with ASAS score (r=0.555, P<0.01). The RSES scores of the study group were negatively correlated with TAS and ASAS scores (r=-0.317, -0.590, P<0.05 or 0.01). Logistic regression analysis showed that being female (OR=0.714, 95% CI: 0.042~0.709) was a protective factor for NSSI behaviors among adolescents, while high alienation (OR=1.028, 95% CI: 1.013~1.043) and residing in rural areas (OR=6.692, 95% CI: 2.038~21.967) were risk factors for NSSI behaviors among adolescents. ConclusionAlienation was positively correlated with NSSI behaviors in adolescents. Female adolescents had a lower risk of NSSI behaviors, while those with higher levels of alienation or residing in rural areas were more prone to NSSI behaviors. 8.Mechanism of Ethoxysanguinarine in Regulating IRE1/RIDD Signaling Pathway to Inhibit Endoplasmic Reticulum Stress and Alleviate Cardiomyocyte Apoptosis

ObjectiveTo investigate the effects of ethoxysanguinarine (ETH) on angiotensin Ⅱ (Ang Ⅱ)-mediated cardiomyocyte apoptosis and its regulatory effects of inositol-requiring enzyme 1 (IRE1)/regulated IRE1-dependent decay (RIDD) signaling pathway and endoplasmic reticulum stress. MethodsWestern blot was used to detect the establishment of the H9c2 model via Ang Ⅱ stimulation, which was identified as a cardiomyocyte apoptosis model. Subsequently, the inhibitory effect of ETH on cell proliferation was assessed using the cell counting Kit-8 (CCK-8) to determine the optimal effective dose of ETH. H9c2 cardiomyocytes were divided into a blank group, a model group (Ang Ⅱ, 1 mmol·L-1), and low-, medium-, and high-dose ETH groups (1.25, 2.5, and 5 mmol·L-1). Morphological changes in cardiomyocytes induced by Ang Ⅱ were detected using phalloidin staining. Cardiomyocyte apoptosis was assessed using terminal deoxynucleotidyl transferase dUTP nick and labeling (TUNEL) staining. The apoptosis cycle was detected by Annexin V/PI flow cytometry. Western blot was used to detect the expression levels of apoptosis-related proteins, endoplasmic reticulum stress, and IRE1/RIDD pathway-related proteins. ResultsWestern blot results showed that 1 mmol/mL Ang Ⅱ stimulation significantly increased the protein expression levels of Bip, p-IRE1, and Bid in H9c2 cells (P<0.05, P<0.01), indicating the induction of endoplasmic reticulum stress, activation of the IRE1/RIDD signaling pathway, and initiation of the apoptosis process. Compared with the blank group, the model group showed a significant increase in the surface area of H9c2 cells and the apoptosis rate of cardiomyocytes, as well as in both early and late apoptosis rates (P<0.01). The expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 proteins were significantly increased, while the expression level of Bcl-2 protein was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins were significantly increased (P<0.05, P<0.01). Compared with those in the model group, the surface area of cardiomyocytes and the apoptosis rate of cardiomyocytes in all ETH groups were significantly decreased after drug intervention. Both early and late apoptosis rates were significantly decreased. The expression level of cleaved-Caspase-8 was significantly decreased in the low-dose ETH group (P<0.05). The expression levels of Bid, Bax, and cleaved-Caspase-8 were significantly decreased in the medium-dose ETH group (P<0.05, P<0.01). The high-dose ETH group significantly reduced the expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 (P<0.05, P<0.01) and significantly increased the expression level of Bcl-2 (P<0.05). The level of p-IRE1 protein in the medium-dose ETH group was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins in the high-dose ETH group were significantly decreased (P<0.05, P<0.01). ConclusionETH can alleviate Ang Ⅱ-mediated cardiomyocyte apoptosis by inhibiting the IRE1/RIDD signaling pathway and further alleviate the cardiac injury caused by hypertension.

ResultsWestern blot results showed that 1 mmol/mL Ang Ⅱ stimulation significantly increased the protein expression levels of Bip, p-IRE1, and Bid in H9c2 cells (P<0.05, P<0.01), indicating the induction of endoplasmic reticulum stress, activation of the IRE1/RIDD signaling pathway, and initiation of the apoptosis process. Compared with the blank group, the model group showed a significant increase in the surface area of H9c2 cells and the apoptosis rate of cardiomyocytes, as well as in both early and late apoptosis rates (P<0.01). The expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 proteins were significantly increased, while the expression level of Bcl-2 protein was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins were significantly increased (P<0.05, P<0.01). Compared with those in the model group, the surface area of cardiomyocytes and the apoptosis rate of cardiomyocytes in all ETH groups were significantly decreased after drug intervention. Both early and late apoptosis rates were significantly decreased. The expression level of cleaved-Caspase-8 was significantly decreased in the low-dose ETH group (P<0.05). The expression levels of Bid, Bax, and cleaved-Caspase-8 were significantly decreased in the medium-dose ETH group (P<0.05, P<0.01). The high-dose ETH group significantly reduced the expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 (P<0.05, P<0.01) and significantly increased the expression level of Bcl-2 (P<0.05). The level of p-IRE1 protein in the medium-dose ETH group was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins in the high-dose ETH group were significantly decreased (P<0.05, P<0.01). ConclusionETH can alleviate Ang Ⅱ-mediated cardiomyocyte apoptosis by inhibiting the IRE1/RIDD signaling pathway and further alleviate the cardiac injury caused by hypertension.

Hearing loss is the most prevalent disabling disease. Cochlear implantation(CI) serves as the primary intervention for severe to profound hearing loss. This consensus systematically explores the value of genetic diagnosis in the pre-operative assessment and efficacy prognosis for CI. Drawing upon domestic and international research and clinical experience, it proposes an evidence-based medicine three-tiered prognostic classification system(Favorable, Marginal, Poor). The consensus focuses on common hereditary non-syndromic hearing loss(such as that caused by mutations in genes like GJB2, SLC26A4, OTOF, LOXHD1) and syndromic hereditary hearing loss(such as Jervell & Lange-Nielsen syndrome and Waardenburg syndrome), which are closely associated with congenital hearing loss, analyzing the impact of their pathological mechanisms on CI outcomes. The consensus provides recommendations based on multiple round of expert discussion and voting. It emphasizes that genetic diagnosis can optimize patient selection, predict prognosis, guide post-operative rehabilitation, offer stratified management strategies for patients with different genotypes, and advance the application of precision medicine in the field of CI.
