Objective: To analyze the characteristics of latent classes of dietary patterns among children and adolescents in Qinghai Province, and their association with overweight and obesity, so as to provide a scientific basis for conducting targeted nutritional interventions and weight management for the local population. Methods: From May to October 2025, a multi stage stratified cluster random sampling method was employed to recruit 1 611 children and adolescents aged 6-17 years from Chengdong District of Xining City and Gonghe County of Hainan Tibetan Autonomous Prefecture in Qinghai Province. Questionnaire survey was conducted using a questionnaire developed by the Chinese Center for Disease Control and Prevention, and physical examination was conducted by using unified measurement standards. Latent class analysis (LCA) was performed on the food intake frequency of the participants. The χ 2 test and Logistic regression analysis were applied to explore the relationship between dietary patterns and overweight/obesity. Results: The detection rates of overweight, obesity, and overall overweight/obesity among the participants were 10.4%, 5.8%, and 16.2%, respectively. Latent class analysis identified three dietary patterns: modern mixed pattern ( n =399, 24.8%), low diversity pattern ( n =476, 29.5%), and traditional dietary pattern ( n =736, 45.7%). The potential category distribution differences of three dietary patterns among children and adolescents with and without overweight or obesity were statistically significant (non overweight and obesity: 22.5%, 29.1%, 48.4%; overweight and obesity: 36.4%, 31.8%, 31.8%) ( χ 2=30.69, P <0.01). Logistic regression showed that, compared with the traditional dietary pattern, the modern mixed pattern was associated with a higher risk of overweight and obesity ( OR =1.68, 95% CI =1.13-2.49); female sex was associated with a lower risk of overweight/obesity compared with male sex ( OR =0.66, 95% CI =0.50-0.86) (both P <0.05). Conclusions A positive association exists between the modern mixed dietary pattern and obesity in children and adolescents. It is recommended to reduce the intake of grains and unhealthy foods among overweight and obese children and adolescents, while increasing the consumption of legumes, fish, and shrimp, in order to cultivate healthy and rational dietary habits.

Objective To investigate the facial spectrum and color characteristics of patients with essen-tial hypertension post administering antihypertensive drugs,establish a classification and evaluation model based on the facial colors of the enrolled patients,and perform in-depth analysis on the important characteristics of their facial spectrum.Methods From September 3,2018,to March 23,2024,participants with essential hyperten-sion(receiving antihypertensive medication treatment,hypertension group)and normal blood pressure(control group)were recruited from the Cardiology Department of Shanghai Hospital of Traditional Chinese Medicine,the Coronary Care Unit of Shanghai Tenth People's Hospital,the Physical Examination Center of Shuguang Hospital Affiliated to Shanghai Uni-versity of Traditional Chinese Medicine,and the Gaohang Community Health Service Center.This study employed the propensity score matching(PSM)method to reduce study partici-pants selection bias.Spectral information in the facial visible light spectrum of the subjects was collected using a flame spectrometer,and the spectral chromaticity values were calculat-ed using the equal-interval wavelength method.The study analyzed the differences in spec-tral reflectance across various facial regions,including the entire face,forehead,glabella,nose,jaw,left and right zygomatic regions,left and right cheek regions as well as differences in parameters within the Lab color space between the two subject groups.Feature selection was conducted using least absolute shrinkage and selection operator(LASSO)regression,fol-lowed by the application of various machine learning algorithms,including logistic regres-sion(LR),support vector machine(SVM),random forest(RF),Na?ve Bayes(NB),and eX-treme Gradient Boosting(XGB).The reduced-dimensional dataset was split in a 7:3 ratio to establish a classification and assessment model for facial coloration related to primary hyper-tension.Additionally,model fusion techniques were applied to enhance the predictive power.The performance of the models was evaluated using metrics including the area under the curve(AUC)and accuracy.Shapley Additive exPlanations(SHAP)was used to interpret the outcomes of the models.Results A total of 114 participants were included in both hypertension and control groups.Reflectance analysis across the entire face and eight predefined areas revealed that the hypertensive group exhibited significantly higher reflectance of corresponding color light in the blue-violet region(P<0.05)and a lower reflectance in the red region(P<0.05)compared with control group.Analysis of Lab color space parameters across the entire face and eight predefined areas showed that hypertensive group had significantly lower a and b values than control group(P<0.05).LASSO regression analysis identified a total of 18 facial color features that were highly correlated with hypertension,including the a values of the chin and the right cheek,the reflectance at 380 nm and at 780 nm of the forehead.The results of the multi-mod-el classification showed that the RF classification model was the most effective,with an AUC of 0.74 and an accuracy of 0.77.The combined model of RF+LR+SVM outperformed a single model in their classification performance,achieving an AUC of 0.80 and an accuracy of 0.76.SHAP model visualization results indicated that the top three contributors to ideal prediction results based on the characteristics from the facial spectrum were the reflectance at 380 nm across the entire face and of the nose as well as the a value of the chin.Conclusion Within the same age group,patients with essential hypertension exhibited signif-icant and regular changes in facial color and facial spectral reflectance parameters after the administration of antihypertensive drugs.Furthermore,facial reflectance indicators,such as the overall reflectance at 380 nm and the a value of the chin,could offer valuable references for clinically assessing the drug efficacy and health status of patients with essential hyperten-sion.

OBJECTIVES: To detect the contents of Tangwei capsule main components with high performance liquid chromatography-quantitative analysis of multicomponents by single marker (HPLC-QAMS) method and to evaluate the quality with chemometrics and entropy weight-technique for order preference by similarity to an ideal solution (EW-TOPSIS). METHODS: A symmetry C₁₈ column and 0.1% formic acid-acetonitrile as mobile phase were used for HPLC of Tangwei capsule. The contents of 3'-hydroxypuerarin, puerarin, 3'-methoxypuerarin, methylnissolin-3-O-glucoside, calycosin, formononetin, rosmarinic acid, salvianolic acid B, dihydrotanshinone Ⅰ, cryptotanshinone, tanshinone Ⅰ, tanshinone ⅡA and cucurbitacin B in 15 batches of Tangwei capsule were determined simultaneously. The quality differences of 15 batches of samples were analyzed by chemometrics and EW-TOPSIS. RESULTS: The HPLC-UV showed that 13 components had good linear relationships in corresponding concentration ranges (r≥0.9991). The relative standard deviations (RSD) of precision, repeatability and stability were all less than 2.00%. The average recovery rates were between 96.86% and 100.13%, and RSD were all less than 2.00%. Cluster analysis showed that 15 batches of samples were clustered into 3 groups. Partial least squares-discriminant analysis showed that salvianolic acid B, formononetin, puerarin, 3'-methoxypuerarin and rosmarinic acid were the main potential markers affecting the quality of Tangwei capsule. EW-TOPSIS analysis showed that the quality of S12-S15 was superior. CONCLUSIONS The analytical method established in this study can be used for the comprehensive evaluation of the quality of Tangwei capsule to provide laboratory support for its quality control and overall evaluation.

Objective:To explore the application value of artificial intelligence-assisted diagnosis model based on convolutional neural network (CNN) in the differential diagnosis of benign and malignant breast masses.Methods:A total of 10 490 images of 2 098 patients with breast lumps (including 1 132 cases of benign tumor, 779 cases of malignant tumor, 32 cases of inflammation, 155 cases of adenosis) were collected from January 2016 to January 2018 in Beijing Tiantan Hospital Affiliated to the Capital University of Medical Sciences. They were divided into training set and test set and the auxiliary artificial intelligence diagnosis model was used for training and testing. Two sets of data training models were compared by two-dimensional imaging (2D) and two-dimensional and color Doppler flow imaging (2D-CDFI). The ROC curves of benign breast tumors, malignant tumors, inflammation and adenopathy were analyzed, and the area under the ROC curve (AUC) were calculated.Results:The accuracies of 2D-CDFI ultrasonic model for training group and testing group were significantly improved. ①For benign tumors, the result from training set with 2D image was: sensitivity 92%, specificity 95%, AUC 0.93; the result from training set with 2D-CDFI images was: sensitivity 93%, specificity 95%, AUC 0.93; the result for test set with 2D images was: sensitivity 91%, specificity 96%, AUC 0.94; the result for test set with 2D-CDFI images was: sensitivity 93%, specificity: 94%, AUC 0.94. ② For malignancies, the result for training set with 2D images was: sensitivity 93%, specificity 97%, AUC 0.94; the result for training set with 2D-CDFI images was: sensitivity 93%, specificity 96%, AUC 0.94; the result for test set with 2D images was: sensitivity 93%, specificity 96%, AUC 0.94; the result for test set with 2D-CDFI images was: sensitivity 93%, specificity 96%, AUC 0.94. ③For inflammation, the result for training set with 2D images was: sensitivity 81%, specificity 99%, AUC 0.91; the result for training set with 2D-CDFI images was: sensitivity 86%, specificity 99%, AUC 0.89; the result for test set with 2D images was: sensitivity 100%, specificity 98%, AUC 0.98; the result for test set with 2D-CDFI images was: sensitivity 100%, specificity 99%, AUC 0.96. ④For adenopathy, the result for training set with 2D images was: sensitivity 88%, specificity 97%, AUC 0.94; the result for training set with 2D-CDFI images was: sensitivity 93%, specificity 98%, AUC 0.94; the result for test set with 2D images was: sensitivity 94%, specificity 98%, AUC 0.93; the result for test set with 2D-CDFI images was: sensitivity 88%, specificity 99%, AUC 0.90. Its diastolic accuracy was not affected even if the maximum diameter of the tumor was less than 1 cm.Conclusions:Through the deep learning of artificial intelligence based on CNN for breast masses, it can be more finely classified and the diagnosis rate can be improved. It has potential guiding value for the treatment of breast cancer patients.

Objective: To explore the effect of Baduanjin combined with health education in patients of heart failure with preserved ejection fraction (HFPEF). Methods: According to the method of random numbers, 100 HFPEF patients were divided into the observation group and the control group, 50in each group. The control group was given routine therapy and health education, while the observation group was given Baduanjin on the basis of the therapy of control group. With 3 months follow-up, heart function, symptom distress and quality of life in HFPEF patients were observed and compared. Results: After intervention, the observation group's 6-minute walking test results were significantly higher than the control group (505.10 ± 52.08 m vs. 442.50 ± 41.35 m; t=-2.977, P=0.008). The improvement of dyspnea, edema, fatigue, palpitations, loss of appetite, chest strength and body weight in the observation group was significantly higher than that in the control group (t were 2.885, 5.422, 9.333, 5.444, 5.199, 5.020, 5.657, 5.081, all Ps<0.05), and the improvement of the observation group in the physical, emotional and other areas of life quality was significantly better than that in the control group (t were 2.764, 3.818, 3.623, all Ps<0.05). Conclusions Baduanjin combined with health education can improve heart function, symptom distress and quality of life of patients of heart failure with preserved ejection fraction.

Objective To improve the differential diagnosis between autoimmune pancreatitis (AIP) and pancreatic cancer (PC) by a contrast analysis of imageological features.Methods Thirty-six patients who had postoperative pathological with Asian AIP standards and 95 patients who had postoperative pathological consistent with PC.The imageological results of these AIP and PC patients were analyzed.Results AIP was significantly less than PC in the enhanced CT of a mass or enlargement of the pancreatic head,enlargement of the lymph nodes around the pancreas,dilation and interrupt in pancreatic and bile duct,peripheral vascular and organ involvement (11/27 vs.28/40,2/27 vs.17/40,13/27 vs.32/40,1/27 vs.10/40,8/27 vs.26/40,2/27 vs.15/40,0/27 vs.15/40,0/27 vs.10/40,P ＜ 0.05).AIP was significantly more than PC in the enhanced CT of a diffusely enlarged pancreas,calcification or pancreatic calculus,capsule-like rim or the vague peripancreatic fat interval (4/27 vs.0/40,7/27 vs.0/40,10/27 vs.6/40,P ＜ 0.05).AIP was significantly less than PC in the three-dimensional ultrasonography of dilation diameter of pancreatic duct and dilation of common bile duct [(0.421 ± 0.270) cm vs.(0.594 ± 0.270) cm,1/18 vs.16/26,P＜ 0.05].AIP was significantly less than PC in the magnetic resonance cholangiopancreatography of dilation of common bile duct and interrupt in pancreatic duct (7/13 vs.16/18,1/13 vs.10/18,P ＜ 0.05).Conclusion AIP as a unique type of chronic pancreatitis can be distinguished from PC on distinctive imageological features

Objective This study aimed to investigate the incidence and the cause for duodenalbiliary reflux and reflux cholangitis after metallic stent placement in distal common bile duct Methods After percutaneous transhepatic bile duct puncture and biliary outside drainage was performed, 16 cases with malignant distal biliary stricture underwent metallic stent placement in distal common bile duct Before stent placement, the routine laboratory studies including leukocyte, neutrophil percentage and the levels of total bilirubin and direct bilirubin in blood were performed for all patients. Two to five days [ an average of (3.3 ±0. 9) days ] after stent implantation, the above indexes were tested again, and 1 ml of water containing 185 MBq of 99Tcm-DTPA was given orally before extubation, then 99Tcm radioactivity in the bile was detected 2 hours later. For the measurement data obtained from the experiment, t test or Wilcoxon signed rank test was adopted to compare them, and P ＜ 0. 05 was considered to be statistically different Results In 14 cases, radioactivity was successfully detected in the bile 2-5 days after stent implantation. Twelve of them was detected to have radioactivity in the bile 2 hours before extubation with duodenal-biliary reflux. The technetium count in the bile accounted for 1.82% of the total intake dose. There was no radioactivity in the bile in 2 cases. In 14 patients, there were no symptoms of cholangitis such as high fever, chills, increased jaundice, and so on after stent implantation. The mean of white blood cell count was (7.59 t2. 62) × 109/L, and the median of neutrophil percentage was 0. 74. Compared with those before stent implantation, the difference did not reach statistical significance ( t = 0. 423, Z = 1. 036, P ＞ 0. 05 ).After stent implantation, the median of total bilirubin and direct bilirubin were significantly lower, which were 92. 2 and 74. 3 μmol/L. Compared with those before stenting,the difference was statistically significant (Z= -3. 170, -3. 170, P ＜0.05). Conclusions There is a high incidence of duodenal-biliary reflux after stent implantation in distal common bile duct in the early stage. However, there is no simultaneous cholangitis caused by duodenal-biliary reflux.

Objective To explore the method and value of percutanously interventional therapeusis for treatment of obstructive jaundice caused by hepatocellular carcinoma accompanied with bile duct thrombosis.Methods Sixteen cases with bile duct thrombosis proved by pathology and imaging examinations were retrospectively analyzed.According to the clinical symptoms, all the patients received percutaneous transhepatic biliary drainage (PTBD) including permanent external drainage, temporary internal drainage and implantation of covered stents.Serum total bilirubin (TBIL) after the interventional therapeusis were measured and compared with that before the treatments by t test to evaluate the efficacy of these treatments.The relief of clinical symptoms was also reviewed to evaluate the efficacy of these treatments.The patients were followed up within 2 years.Results The PTBD was successfully performed in 16 cases.Permanent external drainage, temporary internal drainage and implantation of covered stents were performed in 2 patients, 7 patients and 7 patients respectively.TBIL after the interventional therapy decreased significantly (t=7.366, P<0.01) to (161.2±80.5) μmol/L averagely from (261.9±77.2)μmol/L before the treatments.All the patients died before the end of followed-up.The average survival time was 204 days (30 to 391 d)and the median survival time was 200 days.Bleeding and infection were the main complications, which could be controlled successfully by routine treatments.Conclusion With high achievement ratio and good efficacy, percutanously interventional therapeusis are good choices for the treatments of obstructive jaundice due to bile duct thrombosis.
