ObjectiveTo establish three standardized rehabilitation unit models covering acute, critical and comprehensive recovery stages for traumatic brain injury (TBI) that fit the whole-course characteristics, clarify their multidisciplinary collaborative framework, standardized procedures, and information management system, and verify the clinical effectiveness of this staged rehabilitation unit on long-term functional prognosis and in-hospital complications of TBI patients. MethodsThis study was conducted at two stages: model construction and efficacy verification. At the first stage, TBI rehabilitation units for acute, critical and recovery stages were established in the Department of Neurosurgery, Intensive Care Unit and Department of Rehabilitation Medicine, respectively, with unified multidisciplinary team configuration, standardized assessment and treatment protocols, collaborative workflows, and a dedicated information management system. At the second stage, 324 TBI patients admitted to the newly built rehabilitation units from July, 2023 to July, 2025 were enrolled as the observation group, and 167 TBI patients receiving traditional management from January, 2018 to December, 2022 were retrospectively selected as the control group. The primary outcomes were 1-year post-injury Glasgow Outcome Scale (GOS) and Disability Rating Scale (DRS) scores. Secondary outcomes included the incidence of in-hospital complications and the mean indwelling time of tracheal intubation, gastric tube and urinary catheter. ResultsThe 1-year GOS score was significantly higher (t = -7.836, P < 0.001), and the DRS score was significantly lower (t = -6.742, P < 0.001) in the observation group than in the control group. The overall incidence of complications was significantly lower in the observation group than in the control group (χ² = 18.762, P < 0.001). The incidences of pulmonary infection, urinary tract infection, pressure ulcer, lower extremity venous thrombosis and joint contracture deformity were all significantly reduced (χ² > 4.328, P < 0.05). The indwelling time of tracheal intubation, gastric tube and urinary catheter was significantly shorter in the observation group than in the control group (t > 10.276, P < 0.001). ConclusionThe acute-critical-comprehensive three-stage standardized TBI rehabilitation unit could significantly improve the long-term functional outcomes of TBI patients, reduce the incidence of complications, and shorten the indwelling time of invasive tubes through multidisciplinary collaboration, whole-course standardized management and information-based quality control. This model conforms to the pathological and functional laws of TBI, realizes seamless whole-course connection from acute treatment to recovery rehabilitation, and has good clinical feasibility and promotion value. It can provide evidence-based evidence and practical reference for the homogenized and standardized development of TBI rehabilitation in China.

Objective Based on the sampling test of national drugs,the quality of Huatan Pingchuan tablets was systematically evaluated,and the quality problems were analyzed to provide references and suggestions for the quality control of this variety and to improve its quality standard.Methods A total of 157 batches of samples were tested according to the statutory standard,and based on the testing results and prescription characteristics,microscopic identification and comprehensive analysis of its quality using thin-layer chromatography,high-performance liquid chromatography,and other methods were subsequently established or improved for the exploratory research.Results The established thin-layer chromatography identification for Radix Scutellariae,as well as the content determination methods for Radix Scutellariae,Syringae Cortex,and promethazine hydrochloride,are easy to operate and have good durability and specificity and can be applied to the quality control and evaluation of Huatan Pingchuan tablets.Conclusions The overall quality of the tablets is average;some enterprises should strengthen the quality control of raw medicinal materials(decoction pieces);individual enterprises have significant differences in the quality of samples from different batches,so they need to pay attention to the quality of raw materials and the stability of production processes;the inspection items of the current standards cannot fully reflect the key quality attributes of drugs,and standards improvement work needs to be carried out.

Objective To explore the effects of Dushu pills on cognitive ability and mitochondrial dynamics related proteins in mouse model of Alzheimer's disease(AD)based on network pharmacology.Methods The corresponding targets of Dushu pills and its related targets with AD were predicted using TCMSP database.The intersection targets of Dushu pills and AD were obtained by Venny website.The protein interaction network and drug-disease-target network were mapped using String database and Cytoscape software,respectively.GO and KEGG enrichment analysis were performed for intersection targets using David database.The mouse model of AD was established by injecting Aβ25-35 into bilateral ventricles.The learning and memory ability of the mice was detected by behavioral tests,the mitochondrial damage of neurons in the hippocampus was observed by transmission electron microscopy,and the expression of proteins related to mitochondrial dynamics was detected by Western blot.Results A total of 311 intersection targets related to drugs and diseases were screened out from the database.GO and KEGG analysis showed that the relevant targets were concentrated in mitochondria and other components,and concentrated in the pathways of ATP binding and positive regulation of MAPK activity.Compared with the normal group,the learning and memory ability of the model group was decreased(P<0.05),mitochondrial ridges appeared swelling and fracture(P<0.0001),decreased expression of MAPK,Mfn2 and OPA1 proteins in hippocampus(P<0.01),the expression of DRP1 protein increased(P<0.01).Compared with the model group,the learning and memory ability of mice in the medium and high dose groups of Dushu pills was improved(P<0.05),mitochondrial damage was significantly improved(P<0.01),increased expression of MAPK,Mfn2 and OPA1 in the hippocampus(P<0.01),DRP1 protein expression decreased(P<0.01).Conclusion Dushu pills can reduce mitochondrial damage,maintain mitochondrial homeostasis,and improve the cognitive and memory ability of AD mice.

Objective:To investigate the prevalence of work-related musculoskeletal disorders (WMSDs) in rehabilitation therapists.Methods:In September 2023, the computer searched Chinese and English databases such as CNKI database, Wanfang Data Knowledge Service Platform, VIP Chinese Science and Technology Journal Database, Chinese Biomedical Literature Database, PubMed, Cochrane Library, Web of Science, Embase database, etc. The retrieval time was from the establishment of the database to September 2023. Musculoskeletal disorders, rehabilitation therapists were selected as search terms, and the author, publication time, prevalence rate, and study type were analyzed. The Agency for Healthcare Research and Quality (AHRQ) tool and The Newcastle-Ottawa Scale (NOS) were used to evaluate the quality of the literature. Stata 15.0 software was used for meta-analysis.Results:A total of 28 literatures were included, including 8039 rehabilitation therapists. Meta-analysis showed that the overall prevalence rate of WMSDs among rehabilitation therapists was 72% (95% CI: 64%-80%). The top 3 annual prevalence rates were lower back (51%, 95% CI: 38%-64%), neck (42%, 95% CI: 30%-54%) and shoulder (32%, 95% CI: 23%-41%). The top 3 weekly prevalence rates were lower back (47%, 95% CI: 28%-65%), neck (35%, 95% CI: 14%-55%) and back (30%, 95% CI: 17%-44%) . Conclusion:The higher prevalence of WMSDs in rehabilitation therapists, especially in the lower back, neck, shoulder and back, may be related to the prolonged stouching posture and the continuous or repeated exertion of the upper limbs.

Objective:To construct a scientific, comprehensive, and reliable core competency evaluation index system for doulas based on the perspective of midwives.Methods:From September 2021 to March 2022, a preliminary core competency evaluation index system for doulas was developed by consulting domestic and foreign literature. Using the Delphi expert consultation method, two rounds of correspondence were conducted with 32 senior midwives and obstetric managers to form the final version of the core competency evaluation index system for doulas. The expert positive coefficient was expressed by the effective recovery rate of the questionnaire. The degree of expert authority was expressed by the authority coefficient, and calculated by the coefficient of expert judgment basis and the arithmetic average of experts ' familiarity with the index. The degree of expert opinion coordination was expressed by coefficient of variation and Kendall 's harmony coefficient (Kendall 's W) . Results:The response rate of the first round of questionnaire was 93.75% (30/32), and the second response rate of the first round of questionnaire was 100.00% (30/30). The expert judgment coefficient for two rounds was 0.96, the familiarity coefficient with the problem was 0.93, and the degree of authority of expert opinions was 0.95. After two rounds of correspondence, the expert opinions tended to be consistent, and the coefficient of variation of indicators at all levels was lower than 0.25. The Kendall 's harmony coefficients of the first round indexes were 0.486, 0.374 and 0.268 ( P<0.01), and the Kendall 's harmony coefficients of the second round indexes were 0.487, 0.410 and 0.287 ( P < 0.01). The core competency evaluation index system for doulas based on the perspective of midwives constructed in the study included 3 first-level indicators, such as theoretical knowledge, operational skills and comprehensive ability, 12 second-level indicators and 45 third-level indicators. Conclusions:The core competency evaluation index system for doulas based on the perspective of midwives constructed in the study is comprehensive, reliable and practical, which has guiding significance for training and assessment, the qualification certification and management of doulas.

The glymphatic system, as "waste" clearance pathway in the brain, plays a critical role in maintaining the homeostasis of the brain cell microenvironment. It has been found that changes in the glymphatic system are common in many neurological diseases. MRI is currently the only technology that can achieve human glymphatic imaging, and has the advantages of high soft tissue resolution and sensitivity to tracers. Quantitative MRI can objectively evaluate the changes of inflow and outflow of glymphatic system. Therefore, in this review, we introduce the application of quantitative MRI technology in the glymphatic system in detail, aiming to provide help for the diagnosis and treatment of diseases related to glymphatic system.

Objective To evaluate the safety and efficacy of drug-coated balloon in small branch ostial lesions of coronary arteries (Medina type 0,0,1 lesion).Methods A total of 48 patients were enrolled in the study and they were randomly divided into the Drug-Coated Balloon (DCB) angioplasty group (22 cases) and the Cutting Balloon (CB) angioplasty group (26 cases).They underwent percutaneous coronary intervention (PCI) with either DCB or with CB in small branch ostial lesions of coronary arteries respectively,The immediate outcomes and long-term efficacy were investigated.Results There were no statistical differences between the two groups in baseline clinical date before PCI.There were no coronary perforation,pericardial tamponade,acute thrombotic events in the two groups.There was no significant difference in minimal lumen diameter (MLD) immediately after PCI between the two groups.During followup angiography,the MLD in the DCB group was significantly larger than in the CB group [(1.8 ± 0.2) mm vs.(1.5 ± 0.3) mm,P =0.006].There were no death,nonfatal myocardial infarction or revascularization recorded in the groups during 6 months of follow-up.Conclusion The immediate outcomes between DCB and CB were similar in small branch ostial lesions for coronary arteries angioplasty.The long-term efficacy of DCB angioplasty is better than CB angioplasty.
