1.Survey of post-discharge exercise behavior and analysis of factors influencing exercise intensity in patients undergoing lung surgery

Objective To investigate the post-discharge exercise behavior and factors influencing moderate to vigorous intensity physical activity (MVPA) in patients undergoing lung surgery. Methods A total of 2874 patients from the large prospective, observational perioperative lung symptom study cohort (CN-PRO-Lung 3) in the Department of Thoracic Surgery at Sichuan Cancer Hospital between April 7, 2021, and January 31, 2024, were selected as the survey subjects. A survey was conducted using the Investigation of Exercise Behavior after Lung Surgery questionnaire and the International Physical Activity Questionnaire-Short Form (IPAQ-SF) among patients who underwent lung surgery. Binary logistic regression was used to analyze the factors influencing patients’ engagement in MVPA. Results A total of 702 patients were surveyed, including 252 males and 450 females, with an average age of (52.4±10.2) years. Patients with lung cancer accounted for 85.9%. Only 36.0% of the patients had regular exercise habits, while 42.3% did not engage in any physical activity. The three main barriers for postoperative exercise were physical discomfort (pain, coughing, shortness of breath, etc, 54.7%), lack of professional guidance (41.7%), and concerns about the surgical wound (28.9%). The proportions of patients engaging in vigorous, moderate, and low-intensity physical activity were 5.7%, 28.2%, and 66.1%, respectively. Multivariate analysis showed that patients with a personal annual income ≥50000 yuan (OR=1.52, 95%CI 1.01-2.29, P=0.044), high school education or above (OR=1.92, 95%CI 1.33-2.76, P<0.001), and lobectomy (OR=1.44, 95%CI 1.02-2.03, P=0.037) engaged in more MVPA. Conclusion Patients undergoing lung surgery have inadequate physical activity after discharge, particularly lacking in MVPA. Patients with higher income, higher educational levels, and lobectomy are more frequently engaged in MVPA. Measures such as symptom control, providing exercise guidance, and enhancing education on wound care may potentially improve the inadequate physical activity in lung surgery patients after discharge.

Health economics evidence plays an important role in linking clinical value evidence with health resource allocation decisions in the development of clinical practice guidelines. It can not only effectively balance clinical effectiveness and economic feasibility but also avoid forming "idealized" recommendations that are detached from the affordability of the healthcare system or the burden-bearing capacity of patients. To promote guideline developers to use health economics evidence more standardizedly and fully, this paper conducts an in-depth analysis of the current application status, existing challenges, access channels, and application processes of health economics evidence in current guidelines, and on this basis, puts forward considerations and suggestions for strengthening and standardizing the application of health economics evidence in China's clinical practice guidelines.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
4.Investigation and analysis ofincidence and treatment of mastitis in 79 large-scale pastures in China

This study aims to clarify the incidence and prevention status of dairy cow mastitis in 79 large-scale pastures in China in 2022,reduce the incidence of dairy cow mastitis,and provide scien-tific basis for formulating prevention and control strategies for dairy cow mastitis in large-scale pastures suitable for China's national conditions.The research team relied on the large-scale ranch of the Comprehensive Experimental Station of National Dairy Industry Technology System to car-ry out research.A total of 79 questionnaires were received.The information on the stock of large-scale farms,the proportion of cows in the herd,the level of cow yields,the incidence and cure rate of cow mastitis,the prevention and control of cow mastitis,diagnosis and treatment programs of cow mastitis,the cost of treating cow mastitis,and the culling rate of farms was collected.Chi-square test and correlation analysis were carried out on the incidence of mastitis in dairy cows and the prevention and control plan,treatment plan,yield level and elimination rate of mastitis in dairy cows by descriptive statistical cross table.The incidence of cow mastitis in some pastures was con-trolled between 5%and 10%,and the cure rate was mostly as high as 98%.According to the co-hort analysis,the importance of prevention and control measures for dairy cow mastitis was as fol-lows:DMT>DHI>milk yield>milk traits>breast apparent change>electrical conductivity>CMT.The importance of treatment measures for dairy cow mastitis is as follows:DMT>DHI>milk yield>milk traits>breast apparent changes>electrical conductivity>CMT.The importance of treatment measures for mastitis is as follows:traditional Chinese medicine>pathogen detection and drug sensitivity test>non-steroidal anti-inflammatory drugs>regular maintenance of milking equipment>normal milking without treatment>broad-spectrum antibiotics.The incidence of mas-titis in dairy cows is moderately positively correlated with the yield per unit area,that is,the in-crease in the incidence of mastitis in dairy cows has a certain impact on the yield per unit area of dairy cows.Economic analysis shows that the treatment cost of mastitis is closely related to the in-cidence rate,and the treatment cost of pasture with higher incidence rate is higher.Pastures with higher yields usually have more efficient means of disease prevention and control,and the corre-sponding treatment costs are lower.In conclusion,China's large-scale pastures should strengthen the scientific feeding management level,improve the prevention and control methods to reduce the incidence of cow mastitis,improve the cure rate,and reduce the cost of pasture treatment.
5.Characteristics of ketosis and its effect on lactation performance of dairy cows in large-scale pastures

The purpose of this study was to clarify the incidence of ketosis in dairy cows in large-scale dairy farms and the effect of ketosis on the subsequent lactation performance of dairy cows.In this experiment,79 perinatal cows in 8 stages of prenatal 21,14,7 days,delivery day,postpartum 3,7,14,21 days were selected to determine the blood biochemical indexes such as BHB,GLU,NE-FA and ALT in their blood.According to the concentration of BHB in the blood,they were divided into healthy group(CON),subclinical ketosis group(SCK)and clinical ketosis group(CK).There was a disorder of mineral elements in dairy cows in the experimental ranch,and the liver function of dairy cows was abnormal,and there was a disorder of energy metabolism.The incidence of keto-sis in dairy cows in the survey pasture was 26.14%,of which the incidence of SCK was 19.32%,and the incidence of CK was 6.82%,which was comparable to the global average incidence and lower than the average incidence in China.Compared with CON cows,SCK and CK cows had energy metabolism disorders and abnormal liver function,and CK cows were more serious.The milk yield of dairy cows with ketosis decreased significantly.The milk fat rate of dairy cows in SCK group was higher than that in CON group,while the milk fat rate of dairy cows in CK group was significantly lower than that in CON group.The fat-to-egg ratio of dairy cows in CK group was sig-nificantly lower than that in SCK group and CON group.The changes of lactation performance in different types of ketosis at 6 months after ketosis were different,and the lactation performance of dairy cows in CON group was the most stable.In the production of pastures,timely and effective treatment and management are essential for restoring the health and milk production performance of dairy cows.Ketosis can lead to disorder of energy metabolism and abnormal liver function in dairy cows,reduce milk yield,and have a continuous effect on lactation performance of dairy cows.Maintaining the health of dairy cows is the key to improving milk production and maintaining sta-ble lactation performance.For different types of ketosis cows,corresponding management and treatment measures should be taken to reduce economic losses.
6.Correlation of small dense low-density lipoprotein and lipoprotein(a)with carotid plaque stability in patients with acute cerebral infarction

Objective To analyze the relationship between small dense low-density lipoprotein(sd-LDL)and lipoprotein(a)and carotid plaque stability in patients with acute cerebral infarction(ACI).Methods A total of 160 elderly ACI patients admitted in our hospital from February 2020 to February 2024 were retrospectively recruited.All of them received cervical color Doppler ultra-sound examination,and according to carotid plaque status,they were divided into non-plaque group(43 cases),stable plaque group(56 cases)and unstable plaque group(61 cases).Another 40 healthy individuals taking physical examination in our hospital during the same period served as control group.The clinical data and sd-LDL and Lp(a)levels were compared among the 4 groups to evaluate the predictive value of sd-LDL and Lp(a)levels for unstable plaques.Results When compared with the non-plaque group,the NIHSS score and LDL-C level were significantly in-creased in the stable and unstable plaque groups,and the TC level was obviously elevated while that of HDL-C was notably reduced in the unstable plaque group,and the NIHSS score and TC,TG and LDL-C levels were remarkably declined while that of HDL-C elevated in the control group(P<0.05).The NIHSS score and LDL-C,sdLDL and Lp(a)levels were elevated while that of HDL-C was lowered in the unstable plaque group than the stable plaque group(P<0.05),and the Lp(a)level in the control group was obviously decreased than that of the stable plaque group(P<0.05).Pearson correlation analysis showed that sd-LDL and Lp(a)levels were positively cor-related with NIHSS score and TC,TG and LDL-C(P<0.05,P<0.01),and negatively with HDL-C(P<0.01).Binary logistic regression analysis revealed that NIHSS score and LDL-C,sd-LDL and Lp(a)levels were risk factors,and HDL-C was a protective factor for unstable carotid plaque in ACI patients(P<0.01).ROC curve analysis indicated that the AUC value of sd-LDL,Lp(a)and their combination in predicting carotid plaque stability was 0.830,0.847 and 0.921,respectively,and the sensitivity of combined detection was higher than that of sd-LDL or Lp(a)alone(93.44%vs 88.52%and 86.89%,P=0.000).Conclusion Plasma sd-LDL and Lp(a)levels have a certain association with carotid plaque stability in ACI patients,and they can be used as relevant reference indicators in clinical practice.
