1.Risk factors analysis for severe acute kidney injury in septic patients and establishment and validation of an hour-specific prediction model

Objective:To explore the risk factors of severe acute kidney injury (AKI) in septic patients, and to establish an hour-specific prediction model.Methods:Based on the information of septic patients in the Medical Information Mart for Intensive Care-Ⅳ (MIMIC-Ⅳ) database, general information, comorbidities, vital signs, severity scoring system, laboratory indicators, invasive operations and medication use were recorded. The enrolled patients were randomized into a training set and a validation set according to a ratio of 7∶3. AKI was diagnosed according to the guidelines of Kidney Disease: Improving Global Outcome (KDIGO). Based on Lasso regression and Cox regression, the risk factors of severe AKI (AKI stage 2 and stage 3) in septic patients were analyzed and hour-specific prediction model were established. Consistency index (C-index), area under the receiver operator characteristic curve (AUC) and calibration curve were used to assess the predictive efficacy of the model.Results:A total of 20 551 septic patients were enrolled, including 14 385 patients in the training set and 6 166 patients in the validation set. Multivariate Cox regression analysis showed that atrial fibrillation [hazard ratio ( HR) = 1.266, 95% confidence interval (95% CI) was 1.150-1.393], heart failure ( HR = 1.348, 95% CI was 1.217-1.493), respiratory failure ( HR = 1.565, 95% CI was 1.428-1.715), heart rate ( HR = 1.004, 95% CI was 1.002-1.007), mean arterial pressure ( HR = 1.245, 95% CI was 1.126-1.377), lactic acid ( HR = 1.051, 95% CI was 1.025-1.077), simplified acute physiology score Ⅱ (SAPSⅡ, HR = 1.019, 95% CI was 1.016-1.023), serum creatinine ( HR = 1.171, 95% CI was 1.127-1.216), anion gap ( HR = 1.024, 95% CI was 1.010-1.038), serum potassium ( HR = 1.155, 95% CI was 1.079-1.236), white blood cell count ( HR = 1.006, 95% CI was 1.003-1.009) and furosemide use ( HR = 0.414, 95% CI was 0.368-0.467) were independently associated with severe AKI in septic patients (all P < 0.01). The above predictors were applied to construct an hour-specific prediction model for the occurrence of severe AKI in septic patients. The C-index of the prediction model was 0.723 and 0.735 in the training and validation sets, respectively. The AUC for the occurrence of severe AKI at 12, 24, and 48 hours were 0.795 (95% CI was 0.782-0.808), 0.792 (95% CI was 0.780-0.805), and 0.775 (95% CI was 0.762-0.788) in the training set, and the AUC were 0.803 (95% CI was 0.784-0.823), 0.791 (95% CI was 0.772-0.810), and 0.773 (95% CI was 0.752-0.793) in the validation set, respectively. The calibration curves of the two cohorts were in good agreement. Conclusion:The hour-specific prediction model effectively identifies high-risk septic patients for developing severe AKI within 48 hours, aiding clinicians in stratifying patients for early therapeutic interventions to improve outcomes.
2.Effects of path-based pain management combined with positive psychological suggestion on robot navigation-assisted treatment of patients with traumatic fractures

Objective:To explore the effects of path-based pain management combined with positive psychological suggestion on pain and negative emotions in patients with traumatic fractures undergoing robot navigation-assisted treatment.Methods:Totally 100 patients with traumatic fractures undergoing robot navigation-assisted treatment admitted to Beijing Jishuitan Hospital from January 2018 to March 2019 were selected by convenience sampling and divided into the observation group and control group according to the random number table, with 50 patients in each group. Patients in the control group received routine nursing intervention, while patients in the observation group underwent path-based pain management combined with positive psychological suggestion. The scores of anxiety, depression and pain were compared between the two groups after intervention.Results:From 12 to 72 hours after operation, the pain score of the observation group was lower than that of the control group ( P<0.05). After the intervention, the anxiety and depression scores of the observation group were lower than those of the control group ( P<0.05) . Conclusions:Path-based pain management combined with positive psychological suggestion can reduce postoperative pain, anxiety and depression in patients with traumatic fractures undergoing robot navigation-assisted treatment.
3.China Health and Retirement Longitudinal Study: depressive symptoms of middle-aged and elderly disabled people and its related factors

Objective:To explore the status of depression symptoms and related factors of depressive symptoms in middle-aged and elderly disabled people in China and provide references on the preventive cares of depression.Methods:Research data were obtained from the 2018 follow-up data of China Health and Retirement Longitudinal Study(CHARLS).Disabled people were selected as the research object. The scores of center for epidemiological studies depression scale(CES-D) were used to evaluate depressive symptoms. Fourteen variables derived from the 3 dimensions of demographic characteristics, health status, and health related behaviors were utilized in this study.SAS 9.4 were used for descriptive analysis, univariate analysis and Logistic regression analysis.Results:A total of 1 708 middle-aged and elderly disabled people were included, among whom 932(54.5%)were diagnosed with depressive symptoms, and the score of CES-D was (11.45±7.19).Self-rated poor health, self-rated poor memory, life dissatisfaction, physical pain were the risk factors of depressive symptoms ( OR=1.87, 4.22, 1.83, 5.30, 1.75). Male, educational level (middle-high school, high school or above), sleeping duration (6-9 h) were the protective factors of depressive symptoms( OR=0.74, 0.72, 0.10, 0.56). Conclusion:Middle-aged and elderly disabled people in China have a high detection rate of depressive symptoms. Female, lower education background, inadequate amount of sleep, self-rated unsatisfactory healthy, self-rated poor memory, dissatisfaction towards life and suffering from physical pain may be more likely to have depressive symptoms in middle-aged and elderly people with disabilities.
4.Effectiveness of cinacalcet in combination with calcitriol for secondary hyperparathyroidism in patients with maintenance hemodialysis

One hundred and twenty maintenance hemodialysis patients with secondary hyperparathyroidism were randomly assigned to receive cinacalce,calcitriol or combination of cinacalce and cacitriol for treatment,with 40 cases in each group.Patients were followed up for 12 months;and the blood tests,echocardiography,examinations for osteoporosis and soft tissue calcification were performed every month.After 3 months of treatment,the serum levels of parathyroid hormone were decreased in all three groups (P < 0.01);while the parathyroid hormone decreased more markedly with less influence on serum calcium and phosphorus levels in combination group.There were no significant changes in alkaline phosphatase,osteoporosis and cardiac valve calcification after treatment,compared with before treatment.There were no cases of hypercalcemia and hypocalcemia appearing in combination group after treatment.The study indicates that the combination of cinacalcet with calcitriol has better therapeutic effect for treatment of secondary hyperparathyroidism in maintenance dialysis patients.
5.Risk factors for progression of aortic arch calcification in patients on maintenance hemodialysis

Objective To investigate the possible risk factors for the progression of abdominal aortic calcification (AAC) in MHD patients.Methods Total of 170 patients on MHD between June 2014 and October 2014 in the dialysis center of the Second Hospital of Tianjin Medical University were included prospectively.Lateral lumbar radiography were applied to evaluate patients' AAC score (AACs) at baseline and after two-years of follow-up respectively.According to the change of AACs,the patients were divided into rapid AAC progression group and non-rapid AAC progression group.Multivariable Logistic regression models were used to determine the risk factors for the progression of AAC in MHD patients.Results At baseline,the presence of AAC (AACs≥1) was 43.5%(74/170).The mean follow-up duration was 27.6(24.7,28.0) months.AACs were available in 111 patients,and the presence of AAC was 78.4%(87/111).During the follow up,36 patients developed new AAC;rapid AAC progression was seen in 54 patients,and non-rapid AAC progression was seen in 57 patients.Multivariate Logistic regression analysis demonstrated that hyperphosphatemia (OR=4.373,95% CI 1.562-7.246,P=0.005) and high density lipoprotein (HDL) (OR=0.031,95%CI 0.003-0.338,P=0.004) were independent risk factors for AAC progression in MHD patients.Conclusions Hyperphosphatemia and low HDL may promote the progression of AAC.Well-controlled serum phosphate and lipid metabolism may slow the progression of vascular calcification,reducing cardiovascular morbidity and mortality.
6.Application of preventive care mode in prevention of pressure ulcer in elderly spinal fracture patients

Objective To investigate the application of preventive care mode in prevention of pressure ulcer in elderly spinal fracture patients.Methods A total of 80 patients with spinal fractures were randomly divided into observation group and control group according to the different nursing measures,with 40 cases in each group.The control group was given routine nursing care,while the observation group was given nutritional prescription.The incidence of pressure sores of the two groups were compared.Results The incidence of pressure sores in the observation group was significantly lower than that in the control group,and the difference was statistically significant (P < 0.05).Conclusion The preventive nursing mode in the treatment of elderly spinal fracture can effectively reduce the incidence of pressure sores in patients.

8.Efficacy and safety of neurotropin in treatment of restless legs syndrome in patients undergoing maintenance hemodialysis

Objective To investigate the efficacy and safety of neurotropin in the treatment of restless legs syndrome and sleep disorder in patients with maintenance hemodialysis. Methods Sixty eight patients who met the inclusion criteria were randomly assigned to control group (n=34) and treatment group (n=34). The trial lasted for 16 weeks, and all patients undergone thorough dialysis. 7.2 units (2 branches) neurotropin were slowly injected to the patients in the treatment group at the end of each hemodialysis and they were stopped after 8 weeks. The patients in the control group had no treatment for restless leg syndrome on the basis of adequate dialysis. All patients were assessed regularly as regards their biochemical indexes, restless legs syndrome rating scale and Pittsburgh sleep quality index. Results All the patients completed the experiment, and restless legs syndrome scores were decreased in two groups. Compared with the baseline, the restless legs syndrome scores decreased significantly in patients treated with neurotropin, and the differences between two groups were significant (P<0.01). In the Pittsburgh sleep quality index scores, the patients in the treatment group decreased in all scores and the difference was statistically significant (P<0.01). Conclusions Neurotropin can relieve the symptoms of restless legs syndrome and sleep disturbance in patients on maintenance hemodialysis and is safe, but symptoms may occur again after withdrawal.
9.Influence of age on neuromuscular block effect of cisatracurium in myasthenia gravis patients

Objective To investigate the neuromuscular block effect of intravenous injection of cisatracurium in myasthenia gravis patients with different ages.Methods Fifteen geriatric patients defined as geriatric group(≥ 65 years old)with ASA Ⅰ-Ⅱ scheduled for video-assisted thoracoscopic (VATS)thymectomy with Ⅱ b MG and fifteen young-middle aged Ⅱ b MG patients defined as youngmiddle aged group (20-50 years) with paired sex,clinical symptoms,duration of symptoms,preoperative treatment regimen,anesthesia management and surgical time were included.Neuromuscular block was monitored with TOF Watch acceleration instrument.After induction of intravenous anesthesia,0.05mg/kg Cisatracurium was administrated intravenously,followed by increments of 0.015 mg/kg until T1/T0 was less than 5% for each patient.A double-lumen bronchial tube was intubated when T1/T0 was less than5%.0.015 mg/kg cisatracurium was injected during the operation when T4/T1 was achieved to 25%.The dose of cisatracurium,the duration of blockade maintenance,duration of clinical action and neuromuscular block recovery index were obtained.Results There were no significant differences in tracheal doses of cisatracurium [(0.058 ± 0.013)mg/kg vs.(0.053±0.009)mg/kg]and the duration of blockade [(31.5±9.6)min vs.(40.0±19.8)min] between geriatric group and young-middle age group (P>0.05).The duration of clinical action of cisatracurium [(21.6±6.7)min vs.(33.7±13.4)min]and the time to achieve a TOFr of 25% were significantly shorter in geriatric MG group than those in young-middle aged MG group (P=0.045,0.037).The geriatric MG group were administrated more increments of ciastracurium than the young middle aged MG group during surgery (P=0.025).There was no significant difference in the time to achieve a TOFr of 70% [(49.3±16.4)min vs.(57.4±34.7)min] and 90% [(61.6± 19.2)min vs.(64.3±35.9)min] between geriatric MG group and young-middle aged MG group (P> 0.05).Conclusions The duration of clinical action of cisatracurium in geriatric MG patients was shorted than that in young-middle aged MG patients which was different with neuromuscular block effect in normal patients.It is suggested to do more studies to estimate the influence of age on cisatracurium and other muscle relaxants in MG patients.
10.Effects of Different Extracts of Jianpi Huogu Formula on Proliferation and Differentiation of Bone Marrow Mesenchymal Stem Cells

Objective To observe effects of different extracts of Jianpi Huogu Formula (JPHGF) on proliferation and differentiation of bone marrow mesenchymal stem cell (BMSCs). Methods Whole bone marrow adherent was used to screen, culture, and isolate BMSCs. Extracts from different parts (water, chloroform, ethyl acetate and n-butanol parts) of JPHGF were administrated for a certain time. MTS was used to detent cell proliferation;ALP staining was used to detect ALP activity;ARS staining was used to detect the formation of calcium nodules;oil red O staining was used to detect fat cell formation. Results Extracts from different parts of JPHGF could promote cell proliferation of BMSCs in different levels, followed by its strength in water, chloroform, ethyl acetate, and n-butanol parts;ALP staining results showed that the intensity of ALP expression of the order is water, acetic acid ethyl, chloroform and n-butanol parts;in promoting the formation of calcium nodules, ARS staining results showed that its intensity were water, chloroform, ethyl acetate, and n-butanol parts;oil red O staining results showed that inhibition intensity of fat cells interaction strength was formed from ethyl acetate, water, chloroform to n-butanol parts. Conclusion Extracts from different parts of JPHGF have different effects on BMSCs proliferation and differentiation. Water extraction has the strongest osteogenic differentiation and proliferation, and ethyl acetate has the best effect on the inhibition of cell formation.
