1.Risk factors of infectious diarrhea in older adult patients with schizophrenia and construction of a nomogram model

Objective:To investigate the prevalence of infectious diarrhea among older adult patients with schizophrenia, identify the influential factors, and construct a nomogram prediction model for infectious diarrhea.Methods:This study is a prospective cohort study that used convenience sampling to select 160 older patients with schizophrenia from three psychiatric hospitals in Wenzhou (85 cases from Wenzhou Seventh People's Hospital, 40 cases from Lucheng Third People's Hospital, and 35 cases from The Affiliated Kangning Hospital of Wenzhou Medical University) who were admitted between January 2022 and December 2023. The prevalence of infectious diarrhea and its influential factors were statistically analyzed. Regression analysis was conducted to develop a risk prediction model for infectious diarrhea, which was presented using a nomogram.Results:A total of 160 older adult patients with schizophrenia were aged between 61 and 79 years, with an average age of (67.82 ± 7.56) years. Among these patients, 53 were female (33.13%), and 107 were male (66.87%). Thirty-four patients experienced infectious diarrhea, with an incidence rate of 21.25%. Univariate analysis revealed significant differences in body mass index ( χ2 = 9.36, P = 0.009), history of diarrhea ( χ2 = 4.52, P = 0.033), disinfection of tableware ( χ2 = 4.16, P = 0.04), handwashing before and after meals ( χ2 = 6.11, P = 0.013), intestinal flora disorder ( χ2 = 19.43, P < 0.001), CD 4+/CD 8+ ratio ( t = 12.37, P < 0.001), interleukin-17 ( t = 15.86, P < 0.001), and fecal secretory IgA ( t = 20.18, P < 0.001) in older adult patients with schizophrenia. Multifactorial analysis indicated that body mass index ( OR = 0.733), disinfection of tableware ( OR = 0.733), handwashing before and after meals ( OR = 0.452), CD 4+/CD 8+ ratio ( OR = 0.359), interleukin-17 ( OR = 0.563), and fecal secretory IgA ( OR = 0.455) were protective factors against infectious diarrhea in older adult patients with schizophrenia (all P < 0.05). Conversely, a history of diarrhea ( OR = 1.774) and intestinal flora disorder ( OR = 1.528) were identified as risk factors for infectious diarrhea in this population (both P < 0.05). Conclusions:The influential factors for the occurrence of infectious diarrhea in older adult patients with schizophrenia include body mass index, history of diarrhea, disinfection of tableware, handwashing before and after meals, intestinal flora disorder, CD 4+/CD 8+ ratio, interleukin-17, and fecal secretory IgA levels. Future studies should conduct longitudinal research to optimize the prediction model and establish a dynamic nomogram model. This will provide nurses with a reliable tool for dynamically predicting the occurrence of infectious diarrhea in older adult patients with schizophrenia, with the aim of reducing the incidence of infectious diarrhea and improving the patients' quality of life.

As the location with special public health environments,funeral parlors are of paramount importance in the prevention and control of infectious diseases as well as disinfection practices.This paper analyzes the unique hy-gienic characteristics of funeral parlors,summarizes relevant laws,regulations,standards,and literatures in funeral parlors at home and abroad,and elaborates the problems relevant to infection prevention and control in funeral par-lors from perspectives of body disposal risks,microbial contamination characteristics in funeral parlors,and the current status of staff's knowledge on infectious diseases.It aims to enhance the professional prevention and control capabilities of funeral service personnel and management personnel,ensure the health and safety of service recipients and staff,optimize service quality,provide theoretical basis and practical guidance for forming a sound infectious disease prevention and control system for funeral service institutions,and clarify research directions for the preven-tion and control of infectious diseases in funeral parlors in the future.
3.Exercise-induced chronic fatigue reduces spatial recognition and memory ability and the expression of parvalbumin in the hippocampal DG region of rats

Objective:To investigate the neural regulatory mechanism of parvalbumin-positive interneurons(PV-INs)in the dentate gyrus(DG)involved in the impairment of spatial recognition memory by exercise-induced chronic fatigue.Methods:Male Sprague-Dawley(SD)rats were divided into control group and fatigue group by random num-ber method.A three-level incremental load treadmill training program was selected to establish a chronic exhaustion exercise fatigue model.The spatial recognition memory ability of rats was tested by novel object recognition test.The ac-tivation levels and quantitative changes of astrocytes(AS)and PV-INs in the DG region was observed and quantified through immunofluorescence staining and immunohistochemical staining.The phosphorylation level of calcium/calmodu-lin-dependent protein kinase Ⅱ(CaMK Ⅱ)in the hippocampus was detected by Western blot.Results:In the test of novel object recognition,the exploration time of novel object was reduced in the fatigue group,and the discrimination index was significantly lower than that in the control group(P<0.01).Immunohistochemical staining showed that PV-INs in the DG region of fatigue rats were lighter and fewer than those in the control group,the fibers were short and sparse,and the positive cell density and average optical density of cells were significantly lower than those in the control group(P<0.01).Immunofluorescence staining showed that AS was significantly activated,glial fibrillary acidic pro-tein(GFAP)was stained deeply,and the cell processes were dense and elongated in the DG region of fatigue rats.The positive cell density and mean fluorescence intensity were significantly higher than those of the control group(P<0.01).The results of Western blot showed that the phosphorylation level of CaMK Ⅱ protein in the hippocampus of the fatigue group was significantly reduced than that of the control group(P<0.01).Conclusion:Exercise-induced chro-nic fatigue inhibited PV-INs in the rat hippocampal DG region.The excessive activation of AS following exercise fatigue may be a major contributor to this PV-INs suppression.Concurrently,reduced phosphorylation levels of CaMK Ⅱ protein were observed in hippocampal tissue.These alterations ultimately impaired spatial recognition memory in the rats.

5.Multicenter survey on the current status of pediatric life support training

Objective:To investigate the current status and challenges of pediatric life support training in China and provide references for improving training quality.Methods:A cross-sectional study was conducted to collect data from pediatric life support training centers across the country,covering basic institutional information,training capacity and training faculty,training program funding,as well as existing challenges and issues.The domestic registry of training centers in 2023 was obtained through the American Heart Association's online platform.After contacting and verifying each center,an online questionnaire was distributed,and the aggregated data were statistically analyzed.Results:A total of 42 institutions participated in the survey,including 19 children's hospitals,14 general hospitals,6 maternal and child health hospitals,2 women and children’s hospitals,and 1 training institution.The distribution of training centers showed a concentration in coastal areas,with the top three provinces/municipalities being Guangdong(7/42,16.7%),Zhejiang(6/42,14.3%),and Shanghai(4/42,9.5%).As of December 31 2023,the 42 institutions had an annual basic life support(BLS)training volume of 8 587 individuals,the median was 120 (100,200),and an annual pediatric advanced life support(PALS)training volume of 2 448 individuals,the median was 30 (20,50).Among the 42 institutions,there were 598 BLS instructors and 306 PALS instructors.Among the surveyed institutions,24(24/42,57.1%)reported BLS instructor teams comprising fewer than 10 members,and 33(33/42,78.6%)reported PALS instructor teams comprising fewer than 10.Only 7 centers(7/42,16.7%)reported having dedicated funding support.The top three challenges were:training sessions occupying instructors’personal time(27/42,64.3%),low instructor compensation(16/42,38.1%),and issues with the data submission system(16/42,38.1%).Conclusion:Pediatric life support training centers in China are primarily children’s hospitals,with a geographical concentration in coastal areas,which is also reflected in the distribution of training scale and instructor resources.Most centers have relatively small training scales and limited instructor capacity,with many instructors conducting training during their personal time.These issues may hinder the implementation and effectiveness of training programs.
6.Targeting 5-HT to Alleviate Dose-Limiting Neurotoxicity in Nab-Paclitaxel-Based Chemotherapy.

Chemotherapy-induced peripheral neurotoxicity (CIPN) is a severe dose-limiting adverse event of chemotherapy. Presently, the mechanism underlying the induction of CIPN remains unclear, and no effective treatment is available. In this study, through metabolomics analyses, we found that nab-paclitaxel therapy markedly increased serum serotonin [5-hydroxtryptamine (5-HT)] levels in both cancer patients and mice compared to the respective controls. Furthermore, nab-paclitaxel-treated enterochromaffin (EC) cells showed increased 5-HT synthesis, and serotonin-treated Schwann cells showed damage, as indicated by the activation of CREB3L3/MMP3/FAS signaling. Venlafaxine, an inhibitor of serotonin and norepinephrine reuptake, was found to protect against nerve injury by suppressing the activation of CREB3L3/MMP3/FAS signaling in Schwann cells. Remarkably, venlafaxine was found to significantly alleviate nab-paclitaxel-induced CIPN in patients without affecting the clinical efficacy of chemotherapy. In summary, our study reveals that EC cell-derived 5-HT plays a critical role in nab-paclitaxel-related neurotoxic lesions, and venlafaxine co-administration represents a novel approach to treating chronic cumulative neurotoxicity commonly reported in nab-paclitaxel-based chemotherapy.

7.Exploring the construction of a digitised whole-process management platform for paediatric Investigator-Initiated Clinical Trials

Objective:To explore the construction requirements and solutions for a digital full-process management platform for Investigator-Initiated Trials (IIT) in pediatrics in China.Methods:By reviewing literature, the current status and highlights of pediatric IIT digital management systems in Europe and the United States were analyzed. The challenges faced in building a digital full-process management platform for pediatric IIT in China were summarized. The exploration and implementation achievements of our hospital in platform construction were introduced, and future construction priorities were proposed based on practical considerations.Results:The construction and application of digital full-process management platforms for pediatric IIT in Europe and the United States were relatively mature, providing comprehensive digital support for pediatric IIT research, ranging from project management to research design implementation, multi-center data interoperability, and personnel training. In China, the pediatric IIT digital management platform was still under construction, with main challenges including the formulation of construction plans based on hospital-specific conditions, data standardization, and remote recruitment and follow-up platforms.Conclusions:Considering the current status of pediatric IIT digital full-process management systems both domestically and internationally, efforts should be made to further strengthen data standardization, remote subject management, and digital pediatric IIT training from both policy and technical perspectives. This will provide one-stop services and management for projects and researchers, promoting the development of pediatric medical research.
